[Objective] To analyze the mediating pathways of autonomy perception on the intrinsic motivation for blood donation and the moderating mechanism of satisfaction, and to explore the factors that enhance the intrinsic motivation of blood donation in order to promote sustained donor engagement and effectiveness. [Methods] This study, grounded in Self-Determination Theory (SDT), recruited 1 293 employees from private enterprises in Zhejiang Province with prior blood donation experience as the research subjects. Data on demographic characteristics, intrinsic motivation for blood donation, and influencing factors were collected through a voluntary blood donation questionnaire. Spearman correlation analysis and the PROCESS macro were employed to examine the relationships among variables and their underlying mechanisms. [Results] In the multiple linear regression analysis with autonomy perception, life balance perception, and satisfaction as independent variables, 60% of the variance in intrinsic motivation for blood donation was explained by these predictors (adjusted R2=0.60). In the moderated mediation model test, life balance perception mediated the effect of autonomy perception on intrinsic motivation for blood donation. Furthermore, blood donation satisfaction positively moderated the direct path (β=0.06, P<0.01) while negatively moderating the second half of the mediation path (β=-0.04, P<0.01), revealing a dual moderating effect of satisfaction. [Conclusion] Autonomy perception and blood donation satisfaction jointly drive intrinsic motivation for blood donation. Strategies for optimizing blood donation satisfaction, which take into account both the direct path effect and the indirect mediation effect, can offer valuable insights for addressing the discrepancy between high first-time donation rates and low donor retention rates.

ObjectiveTo explore the clinical safety of Feilike Mixture (FLK) in the real world. MethodsThe safety of all children who received FLK from 29 institutions in 12 provinces between January 21,2021 and December 25,2021 was evaluated through prospective centralized surveillance and a nested case control study. ResultsA total of 3 035 juveniles were included. There were 29 research centers involved, which are distributed across 12 provinces, including one traditional Chinese medicine (TCM) hospital and 28 general hospitals. The average age among the juveniles was (4.77±3.56) years old,and the average weight was (21.81±12.97) kg. Among them,119 cases (3.92%) of juveniles had a history of allergies. Acute bronchitis was the main diagnosis for juveniles, with 1 656 cases (54.46%). FLK was first used in 2 016 cases (66.43%),and 142 juvenile patients had special dosages, accounting for 4.68%. Among them,92 adverse drug reactions (ADRs) occurred, including 73 cases of gastrointestinal system disorders,10 cases of metabolic and nutritional disorders, eight cases of skin and subcutaneous tissue diseases, two cases of vascular and lymphatic disorders, and one case of systemic diseases and various reactions at the administration site. The manifestations of ADRs were mainly diarrhea, stool discoloration, and vomiting, and no serious ADRs occurred. The results of multi-factor analysis indicated that special dosages (the use of FLK)[odds ratio (OR) of 2.642, 95% confidence interval (CI) of 1.105-6.323],combined administration: spleen aminopeptide (OR of 4.978, 95%CI of 1.200-20.655),and reason for combined administration: anti-infection (OR of 1.814, 95%CI of 1.071-3.075) were the risk factors for ADRs caused by FLK. Conclusion92 ADRs occurred among 3 035 juveniles using FLK. The incidence of ADRs caused by FLK was 3.03%,and the severity was mainly mild or moderate. Generally, the prognosis was favorable after symptomatic treatment such as drug withdrawal or dosage reduction, suggesting that FLK has good clinical safety.

Objective: To analyze the demographic characteristics of patients with red blood cell transfusion reactions, the usage of red blood cell preparations, and the differences in the composition ratio of adverse reactions based on multi-center data from the Haemovigilance Network, in order to reveal the clinical characteristics of red blood cell transfusion and its underlying issues. Methods: Clinical data of patients who experienced transfusion reactions after red blood cell transfusion in the Haemovigilance Network from 2018 to 2023 were collected. The demographic characteristics of patients who experienced transfusion reactions with different types of red blood cell preparations, the utilization of these preparations, and the differences of the composition ratios of transfusion reactions were analyzed. Count data were expressed as numbers (n) or percentages (%), and comparisons between groups were performed using the Chi-square test. Results: Red blood cell transfusion reactions were more common in females (53.56%), with the majority of patients aged 50-69 years (35.54%). The Han polulation accounted for the vast majority of patients (92.77%), and patients in the hematology and obstetrics/gynecology departments had a relatively high proportion of transfusion reactions (13.26% and 14.26%, respectively). Leukocyte-reduced red blood cells and suspended red blood cells were the most common types of transfusion reactions reported among red blood cell preparations. Allergic reactions and non-hemolytic febrile reactions were the most common transfusion reactions, and there were significant differences in the composition ratios of allergic reactions (χ
=869.89, P<0.05) and non-hemolytic febrile reactions (χ
=812.75, P<0.05) across various types of red blood cell preparations. Conclusion: There are differences in the demographic characteristics and composition ratio of transfusion reactions among different red blood cell preparations. The management of red blood cell transfusion reactions should be tailored to patient characteristics and conditions, and the selection and use of blood products should be optimized to reduce or avoid the occurrence of transfusion reactions, such as considering the use of washed red blood cells for patients with a history of transfusion allergies or those prone to allergies.

Objective:To clarify the application effect of information-guided enteral nutrition-associated diarrhea (ENAD) management process in patients with chronic obstructive pulmonary disease (COPD) undergoing non-invasive assisted ventilation. Methods:A mixed cohort study of pre- and post-control was conducted. Thirty-nine patients with COPD who were admitted to the emergency intensive care unit (ICU) of Huzhou First People's Hospital from July 1, 2021 to July 31, 2022 were enrolled. Taking the completion of the software development of ENAD management software for critically ill patients on January 28, 2022 as the time node, 20 patients admitted from July 1, 2021 to January 28, 2022 were set as the control group, and 19 patients admitted from January 29 to July 31, 2022 were set as the observation group. The two groups of patients received the same enteral nutrition support treatment, and the control group implemented the conventional ENAD treatment process with enteral nutrition intolerance disposal process as the core. On the basis of the control group, the observation group implemented the information-guided ENAD treatment process, and the system software actively captured the information of ENAD patients and reminded the medical team to improve the patient's diarrhea-related examination and provide alternative treatment plans. The duration of antidiarrhea, feeding interruption rate, and energy and protein intake, blood biochemical indexes, incidence of abnormal blood electrolyte metabolism, daily continuous non-invasive assisted ventilation and endotracheal intubation after 7 days of targeted diarrhea intervention were compared between the two groups. Results:Except for the basal pulse rate, there were no significant differences in gender distribution, age, and vital signs, basic nutritional status, arterial blood gas analysis and blood biochemistry at admission between the two groups, indicating comparability between the two groups. When ENAD occurred, the patients in the observation group obtained earlier cessation of diarrhea than those in the control group [days: 3.00 (2.00, 3.25) vs. 4.00 (3.00, 5.00), P < 0.01], and the feeding interruption rate was significantly lower than that in the control group [10.53% (2/19) vs. 65.00% (13/20), P < 0.01]. After 7 days of diarrhea intervention, the energy intake of the observation group was significantly higher than that of the control group [kJ·kg -1·d -1: 66.28 (43.34, 70.36) vs. 47.88 (34.60, 52.32), P < 0.01], the levels of hemoglobin (Hb), albumin (Alb) and serum prealbumin (PAB) were significantly higher than those in the control group [Hb (g/L): 119.79±10.04 vs. 110.20±7.75, Alb (g/L): 36.00 (33.75, 37.25) vs. 31.00 (30.00, 33.00), PAB (mg/L): 155.79±25.78 vs. 140.95±14.97, all P < 0.05], the daily continuous non-invasive assisted ventilation duration was significantly shorter than that of the control group [hours: 14 (12, 16) vs. 16 (14, 18), P < 0.01], and the arterial partial pressure of carbon dioxide (PaCO 2) was significantly lower than that of the control group [mmHg (1 mmHg ≈ 0.133 kPa): 66.00 (62.00, 70.00) vs. 68.00 (67.50, 70.05), P < 0.05]. However, there were no significant differences in protein intake, incidence of abnormal electrolyte metabolism, and incidence of endotracheal intubation due to acute respiratory failure between the two groups. Conclusion:The information-guided ENAD treatment process can enable the COPD patients undergoing continuous non-invasive assisted ventilation who experience ENAD to receive earlier cessation of diarrhea, and improve the protein energy metabolism and respiratory function of the patients.

Journal of Chinese Physician 2024;26(2):176-179
Objective:To explore the relationship between peripheral blood lymphocyte subsets, interleukin-6 (IL-6) levels, and prognosis in patients with viral pneumonia. Methods:A total of 100 patients with viral pneumonia admitted to the Emergency Department of the Chuiyangliu Hospital Affiliated to Tsinghua University from December 2021 to November 2023 were selected and divided into a survival group and a death group. 20 healthy individuals who underwent physical examinations during the same period were selected as the control group. General information of the patients was collected, and peripheral blood lymphocyte subsets and related cytokine IL-6 were detected. Differences in peripheral blood lymphocyte subsets and IL-6 levels among different groups were analyzed, And multiple logistic regression was used to analyze the influencing factors of viral pneumonia disease and death. Results:The proportion of death group with diabetes was significantly higher than that of survival group ( P<0.05). The mortality of patients with diabetes was higher than that of patients without diabetes [60.7%(17/28) vs 12.5%(9/72), P<0.05]. The total number of peripheral blood T lymphocytes, helper/inducible T lymphocytes (Th), CD4 + /CD8 + ratio, B lymphocyte count, and natural killer (NK) cells in patients with viral pneumonia were significantly lower than those in the healthy control group (all P<0.05), and the level of IL-6 was significantly higher than that in the healthy control group ( P<0.05); The total number of peripheral blood T lymphocytes, Th cells, CD4 + /CD8 + ratio, B lymphocyte count, and NK cells in the death group were significantly lower than those in the survival group (all P<0.05), and the level of IL-6 was significantly higher than that in the survival group ( P<0.05). The results of multivariate logistic regression analysis showed that T lymphocyte subsets and IL-6 were all influencing factors for the disease and death of viral pneumonia (all P<0.05). Conclusions:The mortality of viral pneumonia patients with diabetes increased; The absolute number of lymphocyte subsets decreases and the level of IL6 increases in patients with viral pneumonia; The difference in changes between the death group and the survival group is more significant; Early detection of peripheral blood lymphocyte subsets and IL6 levels is beneficial for evaluating the prognosis of patients with viral pneumonia and has certain guiding value for clinical practice.
