1.Application of virtual reality technology combined with case-based learning in forward surgical team skill teaching for undergraduates

Objective:To investigate the application of virtual reality technology combined with case-based learning in forward surgical team (FST) basic skill teaching for undergraduates.Methods:A total of 42 undergraduates who received clinical practice in The Second Affiliated Hospital of Navy Medical University from January 2020 to January 2021 were selected as research subjects, and they were randomly divided into experimental group (virtual reality technology combined with case-based learning for FST basic skill teaching) and control group (traditional teaching methods for FST basic skill teaching). A questionnaire survey and assessments were performed to evaluate the effectiveness of teaching, and SPSS 23.0 was used to perform the t-test, the chi-square test, or the Fisher's exact test. Results:The questionnaire survey showed that there were no significant differences between the two groups in the degree of overall satisfaction with teaching, comprehension and practice in learning, and post-learning memory, and compared with the control group, the experimental group had significantly higher scores of improvement in theoretical knowledge (4.33±0.26 vs. 4.17±0.21, P<0.05), improvement in skill operation (4.32±0.22 vs. 4.12±0.27, P<0.05), improvement in the ability to analyze and solve practical problems (4.04±0.37 vs. 3.69±0.38, P<0.05), learning interest and enthusiasm (4.34±0.28 vs. 3.92±0.43, P<0.05), learning attention (4.21±0.35 vs. 3.81±0.34, P<0.05), and learning interaction (4.18±0.29 vs. 4.01±0.21, P<0.05). The results of assessments showed that compared with the control group, the experimental group had a significantly higher total score (85.96±5.35 vs. 77.03±7.29, P<0.05) and significantly better scores of theoretical knowledge (28.25±4.74 vs. 25.01±5.37, P<0.05) and skill operation (57.47±4.96 vs. 51.99±8.03, P<0.05). Conclusions:Virtual reality technology combined with case-based learning has unique advantages in FST basic skill teaching for undergraduates, and related studies and application research can be conducted in the future.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
3.Analysis of microbiota characteristics in tumor tissues sampled under gastroscope

Objective:To investigate the characteristics of bacterial community in upper gastrointestinal tumors.Methods:The study population was patients with upper gastrointestinal tumors (esophageal cancer and gastric cancer). Gastroscopy was performed on the enrolled patients ( n=17), and the specimens were taken from the tumor sites. At the same time, non-tumor tissues more than 4 cm away from the tumor tissues were taken as the control. After total DNA was extracted and purified, high-throughput 16S DNA gene sequencing was used to detect the microbiota in tumor tissues and control tissues. Bioinformatics analysis was carried out and the differences between groups were compared. Results:16S DNA PCR showed that there was no significant difference in bacterial load between tumor tissues and control tissues. The α-diversity and β-diversity indexes showed that the community composition of the two groups was similar; the samples were discrete and the colony composition was different, but there was no significant difference between the two groups. The results of Venn diagram showed that there were more operational taxonomic units (OTUs) in non-tumor tissues than in tumor tissues (2 068 vs 1 358), indicating that the bacterial species in normal tissues were more abundant than those in tumor tissues. Compared with the control tissues, the percentages of Prevotellaceae ( Prevotella), Lactobacaceae ( Lactobacillus) and Fusobacteriaceae ( Fusobacterium) in tumor tissues were relatively higher (the average percentage was more than twice that of the control). Further paired comparison of the top ten bacteria in the family and genus abundance of the two groups of samples showed that Pseudomonas decreased significantly in tumor tissues at the family ( P=0.041) and genus ( P=0.041) levels, while Prevotella was significantly enriched in tumor tissues at the family ( P=0.031) and genus ( P=0.007) levels. Conclusions:The bacterial community in the tumor microenvironment of the upper gastrointestinal tumor changed, and the species enriched in the tumor site were mainly oral common anaerobic bacteria, such as Prevotellaceae ( Prevotella), Lactobacaceae ( Lactobacillus) and Fusobacteriaceae ( Fusobacterium), especially Prevotellaceae ( Prevotella).
4. Final examination paper analysis of Surgery and Field Surgery in five-year students from comprehensive class of Grade 2014

Objective:
To analyze the quality of final exam papers of Surgery and Field Surgery for five-year medical students from comprehensive class of Grade 2014, so as to check the mastery of different students, perfect the construction of item banking, verify the quality of teaching and learning, and thus provide references to further teaching reform.
Methods:
SPSS 21.0 was used for statistical analysis of 133 Surgery and Field Surgery papers of students from comprehensive class of Grade 2014.
Results:
The highest score was 93.5, the lowest score was 52.0, and the average score was (70.60±9.00). Among those 133 students, six students were failed, occupying 4.51%. The overall difficulty coefficient of examination paper (P) was 0.71, and P value of multiple choice questions and blank-filling questions was 0.4-0.7. Difference degrees (D) of question types were more than 0.600 and the overall reliability was 0.764. Scores of examination paper were generally in normal distribution. The average scores of students form anesthesiology major were higher than those of students from navigation medicine major, but the average scores of students from above two majors were higher than those of students from psychology major, with statistically significant differences (
5.The review of Qili-Qiangxin capsule's effect on the cardiovascular diseases

Qili-Qiangxin capsules have been wildly used for the cardiovascular diseases. Qili-Qiangxin capsules have good effect on the myocardial infarction, dilated cardiomyopathy, congestive heart failure, diastolic heart failure and HFPEF patients. Patients' cardiac function and quality of life can be improved after treatment. In the basic researches, we concluded the mechanisms of Qili-Qiangxin capsules'effect on the heart failure that Qili-Qiangxin capsules could effectively restrain the cardiac remodeling, improve cardiac function.Although the Qili-Qiangxin capsules were wild used in the clinic, its precise molecular mechanisms for cardiovascular diseases remained unknown.

The widespread application of next generation sequencing (NGS) in clinical settings has enabled testing, diagnosis, treatment and prevention of genetic diseases. However, many issues have arisen in the meanwhile. One of the most pressing issues is the lack of standards for reporting genetic test results across different service providers. The First Forum on Standards and Specifications for Clinical Genetic Testing was held to address the issue in Shenzhen, China, on October 28, 2017. Participants, including geneticists, clinicians, and representatives of genetic testing service providers, discussed problems of clinical genetic testing services across in China and shared opinions on principles, challenges, and standards for reporting clinical genetic test results. Here we summarize expert opinions presented at the seminar and report the consensus, which will serve as a basis for the development of standards and guidelines for reporting of clinical genetic testing results, in order to promote the standardization and regulation of genetic testing services in China.
7.Reflections on improving the teaching quality of basic surgery

The Surgery foundation is a course that emphasizes both theory and practice. It is an important part of surgical study and a compulsory basic course for medical students. The teaching quality of this course is directly related to the future level of clinical operation of medical students, and the good teaching methods and means are one of the key factors to determine the quality of the course. In this paper, the reform of teaching means and methods of this course are discussed, so as to further improve the teaching quality and summarize the experience and advice.

Influenced by a variety of pathologic factors, cardiac fibroblasts can transdifferentiate into myofibroblasts and pro-liferate excessively, followed by an overdue secretion of extracellular matrix ( ECM) .This process involves complicated signaling path-ways and electrophysiological mechanisms and may result in atrial fibrosis and contribute to atrial remodeling in structure, function and electrophysiological signaling, which is considered as a most important phase of atrial fibrillation.This review focuses on the role of fi-broblasts in atrial fibrosis, atrial remodeling, and the development of atrial fibrillation.
10.A tentative study of the reform in teaching methods in the time of new media

With the development of wireless networks and mobile intelligent terminal the new media technology is profoundly changing people's lifestyle and study mode. We applied new media technologies to the teaching practice of eight-year medical program to stimulate students' learning en-thusiasm and productivity, which greatly expanded the knowledge of students, enrich teaching methods, and achieved good teaching effect.
