1.Finite element analysis of osteoporosis in proximal femur after cannulated screw fixation for femoral neck fracture

BACKGROUND:After the internal fixation of cannulated screws in femoral neck fractures,because the affected limb is often unable to bear weight in the short term and the implants with high stiffness have a stress shielding effect on the fracture end,it is easy to cause osteoporosis of the affected limb and changes in the biomechanical distribution of the proximal femur,the incidence of osteonecrosis of the femoral head is high after surgery.At present,few studies have been conducted on the biomechanical effects of osteoporosis at the proximal end of the femur occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws. OBJECTIVE:Using finite element analysis,to investigate the biomechanical effects of osteoporosis occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws and explore the role of biomechanical factors in osteonecrosis of the femoral head. METHODS:Based on the obtained CT scan data of the femur in a patient with a femoral neck fracture,a proximal femoral model for internal fixation for femoral neck fracture was established by Mimics 19.0,3-Matic,UG 11.0,Hypermesh 14.0,and Abaqus software.One finite element model of the proximal femur without osteoporosis and three finite element models of the proximal femur with osteoporosis were analyzed using Abaqus software.The stress,contact pressure,displacement peak and cloud map under different components of the four models were measured and analyzed,and the internal stress changes and distribution of the femoral head were compared and analyzed. RESULTS AND CONCLUSION:The stresses and contact pressures of the femoral head and lower anterior cannulated screws varied more with the degree of osteoporosis.The peak displacement of the four models increased slowly with the degree of osteoporosis.By one-way analysis of variance,there was no significant effect of the degree of osteoporosis on the peak stress,contact pressure,and displacement of the different components.The internal stress distribution of the femoral head changed with the degree of osteoporosis.Changes in the biomechanical environment of the proximal femur have an important impact on osteonecrosis of the femoral head.
2.Clinical characteristics and prognosis analysis of acute myeloid leukemia patients with PTPN11 gene mutation

Objective To investigate the clinical characteristics and prognosis of acute myeloid leukemia(AML)patients with PTPN11 gene mutation.Methods Total 115 adult AML patients who underwent initial diagnosis,treatment,and second-generation sequencing(NGS)detecting at hospital were recruited in this study.Clinical da-ta included disease characteristics,treatment efficacy,long-term prognosis,immune cell subpopulations,and leu-kemia stem cells were collected to analyze the clinical characteristics and prognosis of AML patients with PTPN11 gene mutation.Results PTPN11 gene mutation rate in newly diagnosed adult AML was 9.57%,and the mutation site mainly occurred in exon 3 region with all mutation type being point mutation.Compared with PTPN11 wild-type group,PTPN11 gene mutation group had a higher early mortality rate(18.18%vs 4.00%,P=0.048),a lower complete response rate(33.33%vs 67.71%,P=0.039),a higher recurrence rate(83.33%vs 42.31%,P=0.043),a shorter median overall survival time(9 months vs 20 months,P=0.026),a lower proportion of ef-fector T cells[(1.39±0.12)%vs(3.56±0.46)%,P=0.038],and a higher proportion of leukemia stem cells[(13.82±3.66)%vs(3.87±1.40)%,P=0.021].Conclusion PTPN11 gene mutation is a poor prognostic marker for AML.Those patients have a high early mortality rate,low complete remission rate,high recurrence rate,short median overall survival time,a low proportion of effector T cells,and a high proportion of leukemia stem cells.
3.Meta-analysis on the effects of exercise training-based respiratory rehabilitation therapy in pneumoconiosis patients

Objective To comprehensively analyze the effectiveness of exercise training-based respiratory rehabilitation therapy on patients with occupational pneumoconiosis (hereinafter referred to as "pneumoconiosis"). MethodsLiterature on randomized controlled trials of exercise training-based respiratory rehabilitation therapy for pneumoconiosis patients published from the establishment of the database to July 2023 was retrieved from academic systems such as the China National Knowledge Infrastructure, Wanfang Data Knowledge Service Platform, VIP Database, and China Biology Medicine using bibliometrics method. The RevMan 5.4 software was used for meta-analysis of the selected literatures. Subgroup analysis was conducted to explore the source of study heterogeneity. The funnel plot method was used to test publication bias. Results A total of 55 articles were included, involving 2 436 pneumoconiosis patients in the experimental group and 2 405 pneumoconiosis patients in the control group. The result of random or fixed effect model showed that the six minutes walking distance, the total score of Short from Health Survey-36, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, arterial partial pressure of oxygen of pneumoconiosis patients increased after respiratory rehabilitation therapy (all P<0.05), while the total score of the St. George's Respiratory Questionnaire and arterial partial pressure of carbon dioxide decreased compared with the conventional treatment (all P<0.05). BACKGROUND:
Textbook outcome (TO) can guide decision-making among patients and clinicians during preoperative patient selection and postoperative quality improvement. We explored the factors associated with achieving a TO for gallbladder carcinoma (GBC) after curative-intent resection and analyzed the effect of adjuvant chemotherapy (ACT) on TO and non-TO patients.
METHODS:
A total of 540 patients who underwent curative-intent resection for GBC at the Department of Hepatobiliary Surgery of the First Affiliated Hospital of Xi'an Jiaotong University from January 2011 to December 2020 were retrospectively analyzed. Multivariable logistic regression was used to investigate the factors associated with TO.
RESULTS:
Among 540 patients with GBC who underwent curative-intent resection, 223 patients (41.3%) achieved a TO. The incidence of TO ranged from 19.0% to 51.0% across the study period, with a slightly increasing trend over the study period. The multivariate analysis showed that non-TO was an independent risk factor for prognosis among GBC patients after resection ( P = 0.003). Age ≤60 years ( P = 0.016), total bilirubin (TBIL) level ≤34.1 μmol/L ( P <0.001), well-differentiated tumor ( P = 0.008), no liver involvement ( P <0.001), and T1-2 stage disease ( P = 0.006) were independently associated with achieving a TO for GBC after resection. Before and after propensity score matching (PSM), the overall survival outcomes of non-TO GBC patients who received ACT and those who did not were statistically significant; ACT improved the prognosis of patients in the non-TO group ( P <0.05).
CONCLUSION
Achieving a TO is associated with a better long-term prognosis among GBC patients after curative-intent resection, and ACT can improve the prognosis of those with non-TO.

5.Effect of the transcription factor SOX4 on the proliferation, migration and apoptosis of ovarian granulosa cells

Objective :
To investigate the impact of SOX4 on ovarian granulosa cells,stable overexpression of SOX4 was achieved in human KGN cell line,followed by analysis of its effects on proliferation,migration and apoptosis.
Methods :
The recombinant lentiviral plasmid pLV-EF1a-GFP / Puro-SOX4 was generated through homologous recombination with linearized pLV-EF1a-GFP / Puro vector.Human ovarian granulosa cells ( KGN cell line ) were transduced with Lentiviral expression vectors.KGN cells infected with pLV-EF1a-GFP / Puro-NC were served as the LV-CON group,while those infected with pLV-EF1a-GFP / Puro-SOX4 were designated as the LV-SOX4 group.Following transfection,puromycin selection was employed to establish stable SOX4-expressing KGN cells.The expres- sion levels of SOX4 m RNA and protein in KGN cells from the LV-CON and LV-SOX4 groups were assessed using RT-qPCR and Western blot analysis.Cell proliferation was assessed using the CCK-8 assay in both LV-CON and LV-SOX4 groups.Cell migration ability was evaluated by means of a cell scratch test in these two groups.The proportion of apoptotic cells was determined via flow cytometry analysis in both LV-CON and LV-SOX4 groups.
Results:
The sequencing results of pLV-EF1a-GFP / Puro-SOX4 indicated a complete match between the inserted gene se- quence and the SOX4 mRNA sequence.The lentiviral titers were 7 × 108 TU / ml in the LV-CON group and 1 × 108 TU / ml in the LV-SOX4 group.The recombinant plasmid was successfully transfected into KGN cells with a transfection efficiency of over 90% under fluorescence inverted microscopy.The results of RT-qPCR and Western blot tests demonstrated a significant increase in the expression level of SOX4 in KGN cells of LV-SOX4 group compared to that of LV-CON group (t = 3. 10,P <0. 05 ; t = 14. 20,P <0. 05) .The CCK-8 assay results demonstrated that the LV-SOX4 group exhibited a significant increase in cell proliferation (24 h : t = 45. 92,P<0. 01 ; 72 h : t = 25. 60,P <0. 01) compared to the LV-CON group.The cell scratch assay indicated that the migratory capacity of KGN cells in the LV-SOX4 group was significantly enhanced (t = 7. 65,P <0. 01) compared to that in the LV-CON group. The LV-SOX4 group exhibited a significant reduction in apoptosis ratio (t = 25. 84,P<0. 01) compared to the LV- CON group.
Conclusion
SOX4-overexpressing KGN cell line was successfully established,and the overexpression of SOX4 facilitated proliferation and migration while inhibiting apoptosis in human ovarian granulosa cells.

Objective:To investigate the knowledge, attitudes, and practices (KAP) of pulse oximetry among pediatric healthcare providers in China and analyze the factor influencing the KAP.Methods:A self-developed questionnaire was used for an online research on the KAP of 11 849 pediatric healthcare providers from 31 provinces, autonomous regions, and municipalities of China from March 11 to 14, 2022.The factors influencing the KAP of pulse oximetry among pediatric healthcare providers were examined by Logistic regression. Results:The scores of KAP, of pulse oximetry were 5.57±0.96, 11.24±1.25 and 11.19±4.54, respectively.The corresponding scoring rates were 69.61%, 74.95%, and 55.99%, respectively. Logistic regression results showed that the gender and working years of pediatric healthcare providers, the region they were located, and whether their medical institution was equipped with pulse oximeters were the main factors affecting the knowledge score (all P<0.05). Main factors influencing the attitude score of pediatric healthcare providers included their knowledge score, gender, educational background, working years, region, medical institution level, and whether the medical institution was equipped with pulse oximeters (all P<0.05). For the practice score, the main influencing factors were the knowledge score, gender, age, and whether the medi-cal institution was equipped with pulse oximeters (all P<0.05). Conclusions:Chinese pediatric healthcare providers need to further improve their knowledge about and attitudes towards pulse oximetry.Pulse oximeters are evidently under-used.It is urgent to formulate policies or guidelines, strengthen education and training, improve knowledge and attitudes, equip more institutions with pulse oximeters, and popularize their application in medical institutions.

Objective To evaluate the safety and clinical efficacy of resection of caudate lobe tumor via devascularization and reconstruction of right hepatic vein.Methods Clinical data of a patient who underwent resection of caudate lobe tumor in the First Affiliated Hospital of Xi'an Jiaotong University in November 2017 were retrospectively analyzed.The patient was female,54-year-old,was diagnosed with hepatocellular carcinoma in the liver caudate lobe before operation.The informed consent of the patient was obtained and the local ethical committee approval was received.Resection of caudate lobe tumor was performed via a new approach,devascularization and reconstruction of the right hepatic vein.The short hepatic vein was treated preferentially.Devascularization of the right hepatic vein was performed,followed by resection of caudate lobe tumors and subsequently the right hepatic vein was reconstructed.Results The operation was successfully accomplished.Blood flow of the right lobe and the inferior hepatic vena cava was excluded for 20 min.Total hepatic flow was occluded once for 15 min.The intraoperative blood loss was 800 ml,and 4 U of red blood cell transfusion was performed.Postoperative treatments anti-infection,acid suppression,lowering portal vein pressure,liver protection and nutritional support were routinely delivered.CT scan of the upper abdomen at postoperative 7 d indicated patchy low-density shadows in the right lobe near the hepatic porta,no expansion of the portal vein and intrahepatic bile duct,and a slight pneumatosis and hydrops in the gallbladder fossa.Pathological examination showed a bulky moderately-differentiated hepatocellular carcinoma combined with local cystic lesions.The patient recovered well and discharged at postoperative 15 d.Conclusions Compared with the conventional caudate lobectomy,devascularization and reconstruction of the right hepatic vein is a novel approach for caudate lobectomy with slighter trauma and less bleeding,which is suitable for the resection of caudate lobe tumor involving the vena cava and caudate process.
9. Relationship between noise-induced hearing loss,hypertension and abnormal electrocardiogram in noise-exposed workers

OBJECTIVE: To explore the relationship between noise-induced hearing loss, hypertension and abnormal electrocardiogram(ECG) in noise-exposed workers. METHODS: A judgment sampling method was adopted to select 555 male workers with hearing loss as study group and 555 male workers with normal hearing as control group in the similar environment in an automobile manufacturing enterprise in Guangzhou. Pure tone audiometry,blood pressure measurement and ECG examination were performed in both groups to analyze the relationship between hearing loss and hypertension and abnormal ECG. RESULTS: The prevalence rate of hypertension and abnormal ECG was higher in the hearing loss group than the control group(P < 0. 05). The multiple logistic regression analysis showed that after adjusting confounding factors such as age,seniority,body mass index,drinking et al,the risk of hypertension in hearing loss group was higher than the control group(P < 0. 05) and the odds ratio(OR) was 2. 255 [95% confidence interval(CI) : 1. 093-4. 655 ],while adjusting the confounding factor of drinking,the risk of ECG abnormalities in hearing loss group was higher than the control group(P < 0. 05) and the OR was 1. 408(95% CI: 1. 027-1. 930). CONCLUSION: Workers exposed to noise with hearing loss increase the risk of hypertension and abnormal ECG.
10.Promoting the Safety Management of Heparin Sodium Injection Used for Children by HFMEA Method

OBJECTIVE:To promote the safe use of Heparin sodium injection for children. METHODS:According to litera-ture retrieve,questionnaire survey and field investigation,Health care failure mode and effect analysis method(HFMEA)was ad-opted to search for the failure modes of Heparin sodium injection for children,and preventive strategies were established and imple-mented based on these failure modes. Its effects were evaluated by comparing the error rate and risk priority number rating (RPN value)before and after the implementation. RESULTS:Totally 5 risk points of Heparin sodium injection requiring immediate inter-vention were obtained from RPN value,including wrong dose of Heparin sodium injection in prescribing,pharmacists'unclearness for the calculation method of the dose of Heparin sodium injection in dispensing,and nurses'no performing of double check sys-tem before infusion in administration,etc. After implementing related countermeasures,related error of Heparin sodium injection was decreased from 8 cases to 1 case,and error rate was declined from 3.76% to 0.51%(P<0.01). The RPN values of 5 failure modes were decreased from 112.08,91.56,115.78,94.52,99.23 to 28.02,23.91,27.71,23.63,20.55,respectively. CONCLU-SIONS:HFMEA can prospectively conduct systematical analysis for Heparin sodium injection for children. Implementing related countermeasures based on the current situation of our hospital can reduce the related error of Heparin sodium injection and promote the medication safety of children.
