1.The value of spectral CT combined with metal artifact reduction algorithms in improving the CT image quality for patients with 125I seeds implantation in the chest and abdomen

Objective:To investigate the value of the virtual monoenergetic image (VMI) obtained by a new dual-layer detector spectral CT combined with metal artifact reduction algorithms(O-MAR) in reduction of different types of artifacts caused by 125I seeds implantation and in improvement of the post-operative CT image quality. Methods:This was a cross-sectional study. Thirty-five patients who underwent dual-layer detector spectral CT scanning of the chest and abdomen after 125I seeds implantation were retrospectively included at the First Affiliated Hospital of Zhengzhou University from March to September 2022. The spectral data were collected and reconstructed into conventional CT image (CI), VMI image (50-150 keV, 20 keV/level), CI+O-MAR image, and VMI+O-MAR image (50-150 keV, 20 keV/level). The artifacts′ removal effects and image quality improvement in each group were evaluated. Two slices with the strongest artifacts were selected for analysis for each patient, resulting in a total of 70 slices. Objective indicators including artifact index (AI), signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of soft tissue regions affected by artifacts were measured and calculated. Subjective indicators including the overcorrected artifacts and new artifacts, the different forms of artifacts, the diagnosis of artifacts, and the image quality were assessed. One-way analysis of variance was used for comparisons among multiple groups. Paired t test was used to compare the quantitative indicators between the combined O-MAR group and the non-O-MAR group. Kappa statistics was used to evaluate the consistency between observers. Results:In high/low-density artifacts (ROI H/L), the AI values in all groups showed decrease with increasing VMI keV. In artifact-affected tissue (ROI T), SNR of the CI/VMI (70-150 keV)+O-MAR group were significantly higher than those of the CI/VMI group ( P<0.05), CNR of the CI/VMI(50-150 keV)+O-MAR group were significantly higher than those of the CI/VMI group ( P<0.05). Both overcorrection and new artifacts mainly presented in VMI 50 keV and VMI 70 keV groups; Compared with VMI (50-70 keV) group, significantly less numbers of overcorrection and new artifacts were found in VMI (50-70 keV)+O-MAR group ( P<0.05); regarding the comparison of artifact types, with the VMI keV increasing, the number of a-type banded artifacts gradually decreased on images with high-density artifacts, reaching a minimum of 3 in the VMI 150 keV+O-MAR group; while the number of e-type artifacts with little or no artifacts increased, with the highest number of 23 in the VMI 150 keV+O-MAR group. The total number of high-density artifacts in each type decreased with increasing VMI keV. As VMI keV increased, the diagnostic and image quality scores of high-density artifacts in each group were significantly higher than those of low-density artifacts in the VMI+O-MAR group ( P<0.05). Conclusions:VMI combined with O-MAR can significantly improve the objective and subjective image quality of follow-up CT imaging after 125I seed implantation, enhancing lesion visibility and diagnostic confidence. Additionally, VMI+O-MAR showed more pronounced correction effect on high-density artifacts.
2.Celoside I enhances mitophagy of retinal ganglion cells in optic nerve in-jury model by regulating ROS-mediated JNK/c-Jun signaling pathway

AIM:This study aimed to investigate the mechanism by which celoside I enhances mitophagy in a model of optic nerve injury through regulation of reactive oxygen species(ROS)-mediated c-Jun N-terminal kinase(JNK)/c-Jun signaling pathway.METHODS:Twenty-four New Zealand white rabbits were randomly divided into four groups:sham surgery,model,mecobalamin,and experimental group.Optic nerve injury was induced in the model,mecobala-min,and experimental groups,while the sham surgery group underwent a sham procedure.The mecobalamin group re-ceived mecobalamin,the experimental group received celoside I,and the sham surgery and model groups received saline.Interventions were administered daily for 28 d.Various techniques including endoscopy,hematoxylin-eosin(HE)stain-ing,TUNEL method,immunofluorescence staining and Western blot were used to assess fundus condition,retinal mor-phology,apoptosis,ROS expression,and protein levels in the retina.RESULTS:Fundus examination revealed im-proved blood flow in the mecobalamin and experimental groups compared to the model group.Retinal morphology showed enhanced retinal ganglion cells(RGCs)in the mecobalamin and experimental groups.Apoptosis index was lower in the mecobalamin group compared to the experimental group.Immunofluorescence staining indicated reduced ROS and P62 ex-pression and increased parkin and microtubule-associated protein light chain 3(LC3)expression in the experimental group compared to the mecobalamin group.Protein analysis showed decreased JNK,c-Jun,and P62 levels,and increased parkin and LC3 levels in the mecobalamin and experimental groups compared to the model group.CONCLUSION:Celo-side I reduces ROS expression,inhibits the JNK/c-Jun pathway,enhances mitophagy,reduces apoptosis,and protects RGCs in optic nerve injury models.
3.Clinical Study on the Analgesic Effect of Electroacupuncture Combined with Indomethacin Suppository in Transvaginal Ultra-sound-Guided Oocyte Retrieval

OBJECTIVE To observe the auxiliary analgesic effect of electroacupuncture at Hegu and Neiguan combined with in-domethacin suppository in patients undergoing transvaginal ultrasound-guided oocyte retrieval(TUGOR and its effect on the outcome of in vitro fertilization(IVF.METHODS 64 IVF-ET patients undergoing TUGOR were randomly divided into a treatment group and a control group,32 cases in each group.One case dropped out of the treatment group during the treatment.The control group was given indomethacin suppository rectal administration,and the treatment group was given electroacupuncture at Hegu and Neiguan in addition to the treatment of the control group.The patients'tenderness threshold,VAS score,pain grade score,respiratory rate,and pulse rate were evaluated before and after TUGOR operation.The number of oocytes obtained,the rate of two pronuclei(2PN,embryo utiliza-tion rate,and high-quality embryo rate were evaluated after TUGOR operation.The adverse reactions of the two groups were monitored during and after operation.RESULTS After TUGOR operation,the VAS score and pain grade of the treatment group were signifi-cantly lower than those of the control group(P<0.01;the tenderness threshold of the two groups was significantly reduced after opera-tion(P<0.05,P<0.01,and the treatment group was better than the control group(P<0.05;the incidence of nausea during opera-tion,abdominal distension and nausea 48 h after operation in the treatment group were better than those in the control group(P<0.05;the high-quality embryo rate in the treatment group was better than that in the control group(P<0.05.CONCLUSION Electroacupuncture at Hegu and Neiguan combined with indomethacin suppository can effectively assist in analgesia,and can reduce the incidence of adverse reactions during and after TUGOR to varying degrees,and may have certain advantages in improving the rate of high-quality embryos.

The human gut is populated by a complex community of microbiota that coexists with the host to maintain homeostasis.Macrogenomic and metabonomics analysis shows that the imbalance in gut microbiota composition and diversity, some bacterial components, metabolites and the signaling pathways mediated by them are involved in the occurrence and development of a variety of diseases, especially in the elderly.Intestinal microorganisms are one of the important factors affecting age-related diseases and can provide insights on treatment strategies for the elderly.This review focuses on the composition of the intestinal microbiota, interaction between the microbiota and the host, features of the gut microbiota in common geriatric diseases and pathogenic mechanisms.
5.Roles and mechanisms of microRNA-124 in regulating the expression of APLN in epileptic neuron injury

ObjectiveTo study the regulatory effect of microRNA-124(miR-124) on the expression of Apelin(APLN) in epileptic neurons and its effect on neuronal injury. Methods Epileptic model of neurons was established by low magnesium extracellular medium. Dual-luciferase reporter gene system was used to verify the regulatory effect of miR-124 on APLN. Quantitative real-time PCR(qRT-PCR) was used to examine mRNA expression of miR-124 and APLN gene. Western blotting was employed to detect the expression of APLN,Bax,Bcl-2,and Caspase 3 proteins. Flow cytometry was utilized to determine the apoptosis of neurons. 〖WTHZ〗Results〖WTBZ〗Although no significant difference was reached,the luciferase activity was lower in miR-124+APLN gene wide-type group than that in control group. Both qRT-PCR and western blotting showed that up-regulation of miR-124 could promote the expression of APLN,and down-regulating miR-124 expression could inhibit APLN mRNA expression. Furthermore,up-regulating miR-124 expression could promote the apoptosis of epileptic neurons. The underlying mechanisms involved up-regulation of pro-apoptotic protein Bax and caspase 3,and down-regulation of anti-apoptotic protein Bcl-2. Conclusion The results of this study suggested that miR-124 might positively regulate APLN expression,and miR-124 might lead to neurons deaths by regulating other gene expression other than APLN gene.
6.Sensitivity and specificity of nucleic acid testing in close contacts of COVID-19 cases in Guangzhou

Objective:To analyze the sensitivity and specificity of SARS-CoV-2 nucleic acid testing in 20 348 close contacts of COVID-19 cases in different prevention and control stages in Guangzhou and to provide scientific evidence for optimizing epidemic response strategies.Methods:A total of 20 348 close contacts of COVID-19 cases in Guangzhou were traced between February 21 and September 22,2020. All the close contacts were tested for the nucleic acid of SARS-CoV-2. The sensitivity and specificity of nucleic acid testing and diagnosis in the different prevention and control stages were compared.Results:In 20 348 close contacts, 12 462 were males (61.24%), the median ( P 25, P 75) of age of them was 31.0 years (23.0,43.0), the median number ( P 25, P 75) of nucleic acid testing for them was 2.0 (1.0,3.0), and the median ( P 25, P 75) of their quarantine days was 12.0 (8.0,13.0) days, respectively. A total of 256 COVID-19 cases were confirmed in the close contacts after seven nucleic acid tests. In the 1 st, 2 nd, 3 rd and 7 th nucleic acid testing, the sensitivity and specificity were 69.14% and 99.99% (177 cases confirmed), 89.84% and 99.99% (230 cases confirmed), 97.27% and 99.99% (249 cases confirmed), and 100.00% and 99.98%, respectively. In the three stages of COVID-19 prevention and control in China: domestic case stage, imported case stage, and imported case associated local epidemic stage, the sensitivity of the 1 st nucleic acid testing was 70.68%, 68.00% and 67.35%, and the specificity was 99.98%, 100.00% and 100.00%, respectively. Conclusions:The sensitivity of nucleic acid testing in the close contacts at the different stages were consistent with slight decrease, which might be related to the increased proportion of asymptomatic infections in the late stage of epidemic prevention and control with COVID-19 in Guangzhou. It is suggested to give three nucleic acid tests to improve the sensitivity and reduce false negative risk.
7.The effect of periosteum deficiency on the treatment of lower extremity segmental bone defect with Ilizarov bone transport technique

Objective To investigate the effect of periosteum deficiency on the treatment of segmental bone defect of lower extremity by Ilizarov bone transport Technique.Methods From April 2009 to April 2016,data of 54 cases with lower limb segmental bone defect who were treated by Ilizarov bone transport technique were retrospectively analyzed.According to intraoperative observation of the distribution of periosteum at the osteotomy site,all cases were divided into two groups.There were 10 cases in absent periosteum group,including 6 males and 4 females,aged from 23 to 57 years old (mean,38.20±9.90 years old).There were 3 cases locating at femur,and 7 cases at tibia.4 cases had non-infective bone defect,while the other 6 had infective bone defect.Segmental bone defect ranged from 4.0 to 14.0 cm (mean,6.85±2.87 cm).There were 44 cases in normal periosteum group,including 32 males and 12 females,aged from 22 to 65 years old (mean,38.90±10.10 years old).There were 9 cases locating at femur and 35 cases at tibia.12 cases had non-infective bone defect,while 32 cases had infective bone defect.Segmental bone defect ranged from 4.0 to 9.0 cm (mean,6.09±1.54 cm).Visual analogue scale (VAS) was used to estimate the pain during bone transport.The distraction index,bone healing index,external fixator index was collected to evaluate the distraction osteogenesis and mineralization.The Paley method was utilized to appraisal the function of proximal joints in bone transport.Results All the 54 cases were followed up for 24-48 months,with an average of 28.50±5.70 months.All skin and soft tissue defects were satisfied healed.All limbs were restored to their original lengths,and bone healing was eventually achieved.There were no statistically significant differences in the distraction index,bone healing index,external fixator index,VAS between absent periosteum group (12.29± 1.04 d/cm,39.00±3.96 d/cm,51.25±3.69 d/cm,3.30± 1.77 score) and normal periosteum group (11.67±0.96 d/cm,38.07±4.22 d/cm,49.74±4.25 d/cm,3.36± 1.66 score)(P > 0.05).Paley adjacent joint function evaluation showed no statistically difference between absent periosteum group [good rate 100%(10/10)] and normal periosteum group [good rate 97.7% (43/44)] (P=0.901).The overall incidence of complications was 20.0% (2/10) in the absence of periosteum group,with 1 case of mild infection and 1 cases of tissue contracture incarcerated at the docking site.The overall incidence of complications was 27.3% (12/44) in the normal of periosteum group,with 2 cases of mild infection,3 cases of poor limb strength alignment,6 cases of tissue contracture incarcerated at the docking site and 1 case of severe foot drop deformity.There was no statistically significant difference in the overall incidence of complications between the two groups (P=0.636).Conclusion The absence of periosteum at the epiphyseal osteotomy site hasno significant effect on Ilizarov bone transport technique in the treatment of segmental bone defects of lower limbs in bone regeneration and healing,mineralization and bone remodeling,and does not increase the incidence of complications.
8.Effect of club health education model on psychosomatic status of type 2 diabetes mellitus and anxiety

Objective To explore the effect of club health education on metabolic parameters and mental state in patients with type 2 diabetes mellitus and anxiety.Methods From January 1 to December 31, 2016, 60 patients with type 2 diabetes mellitus and anxiety who were admitted to the Beijing First Hospital of Integrated Chinese and Western Medicine were divided into the intervention group(30 patients) and control group(30 patients).In the intervention group,a club health education model was used based on conventional diabetes treatment. The control group only received conventional diabetes treatment and routine education.After 12 weeks,fasting plasma glucose(FPG)levels,2-hour postprandial plasma glucose (2-h PG) levels, total cholesterol (TC), triglycerides (TGs), body mass index (BMI), Hamilton anxiety scale(HAMA)scores,and symptom checklist-90(SCL-90)scores were compared before and after the intervention. The main statistical methodology included the χ2test, t-test for independent samples, paired t-test, and nonparametric test.Results FPG levels,2-h PG levels,TC,TGs,and BMI in the intervention group after 12 weeks were (6.11 ± 0.94) mmol/L, (7.75 ± 1.96) mmol/L, (4.85 ± 0.87) mmol/L, (1.45 ± 0.26) mmol/L, and (27.13±3.38)kg/m2,respectively.These metabolic parameters before intervention were(7.42±1.18)mmol/L, (9.38±1.17)mmol/L,(5.58±1.48)mmol/L,(1.74±0.41)mmol/L,and(29.11±3.36)kg/m2,respectively.The differences were statistically significant(t=5.110,5.080,2.807,4.283,and 2.3387,respectively;P<0.05). Indicators were lower than in the control group, and the differences between the groups were statistically significant (all P< 0.05). Regarding mental and psychological status, after 12 weeks in the intervention group, the HAMA scores (14.24 ± 1.68) were lower than those before intervention (27.77 ± 1.24), and the difference was statistically significant(t=36.062,P=0.000).The scores were lower than in the control group after intervention(26.48±1.21),and the difference was statistically significant(t=-31.152,P=0.000).After intervention, the SCL-90 scores in the six dimensions, including symptom total score, somatization, compulsion,interpersonal relationships,depression,and anxiety and horror,decreased compared with those before intervention, and the differences were statistically significant (all P<0.05). After intervention, the differences between the intervention and control groups were statistically significant (P<0.05) in five dimensions, including symptom score, somatization, compulsion, depression, and anxiety and horror. Conclusion Based on the routine intervention for type 2 diabetes, a club health education model can significantly improve the metabolic index of patients with type 2 diabetes mellitus complicated with anxiety. In addition,anxiety,interpersonal sensitivity,and other adverse mental states may be significantly alleviated.
9.Study on the teaching of geriatric diseases from the perspective of discipline cross

The teaching of geriatrics requires the combination of basic medicine,clinical medicine,nursing,rehabilitation medicine and pharmacy,and the intersection of medical science and humanities.In order to meet the problems of coping with much disease coexistance in geriatrics,poor efficacy,long course of disease,slow recovery and adverse drug reactions,the article puts forward the combination of blackboard and multimedia,the combination of teacher teaching and student lectures,the combination of PBL and CBL,and the combination of theoretical knowledge and clinical practice.It also puts forward carrying out reforms from such measures as the classroom teaching,teaching rounds,case discussion,literature reading,academic research,humanistic influence,going to community pension center for social practice and so on,to improve the quality of gerontology teaching.
10.Analysis of the concentrations of calcium, phosphorus and magnesium in beef cattle which in seven different areas of Heilongjiang province

The objective of this study was to determine the concentrations of calcium,phosphorus and magnesium in blood and compare the differences in seven areas of Heilongjiang province,and then estimate calcuim-phosphorus metabolism of beef cattle in seven beef cattle farms to provi detheoretical foundation for the prevention of calcium phosphorus metabolism diseases of beef cattle.Seven beef cattle farms of Daqing,Shuangyashan,Jiusan and Mudanjiang in Heilongjiang province were selected as the survey sites,which were recorded as group A (both grazing and stall-feeding in Shuangyashan),group B (stall-feeding mode in Shuangyashan),group C (stall-feeding mode in Daqing),group D (grazing mode in Jiusan),group E (both grazing and stall-feeding in Daqing),group F (both grazing and stall-feeding in Daqing) and group G (stall-feeding mode in Mudanjiang).Then the concentrations of Ca,Mg,P,free fatty acid (NEFA),glucose (Glc) and β-hydroxybutyric acid (BHBA) in the blood were compared to estimate the calcuim-phosphorus metabolic states.Results showed that the concentrations of calcium,magnesium and phosphorus in the plasma of 65 beef cattle in seven survey sites were within the normal range,and there was no significant difference in calcium concentration among seven sites.The P contents in group C and G were significantly higher those that in group A and B(P<0.01),which in group C was significantly higher than those in group D,E and F (P<0.01),which in group G was significantly higher than those in group D,E and F(P<0.01).NEFA content in group B was significantly higher than that in group D (P<0.05),and there was no difference among other groups.The concentration of Glc in group A was significantly higher than that in group E (P<0.05),which in group B was significantly higher than those in group A and D,and was very significantly higher than those in group E,F and G (P<0.01),which in group C was very significantly higher than those in group A,D,E,F and G (P<0.01),which in group C was significantly higher than those in group A,D,E,F and G (P<0.01),which in group D was significantly higher than that in group E (P<0.05),and which in group F was significantly higher than that in group E (P<0.05).The concentration of BHBA in group C,D and E were significantly higher than that in group A (P<0.01),which in group C was significantly higher than that in group B (P<0.05),which in group D and E was significantly higher than that in group B (P<0.01),which in group C was significantly higher than that in group G (P<0.01),which in group D was significantly higher than those in group F and G (P<0.01),and in group E was highly significantly higher than that in group F and G (P<0.01).Overall,there were not calcuim-phosphorus metabolic disorders within the seven beef cattle farms which were selected,but it is also necessary to strengthen feeding management and health care to prevent the occurrence of nutrition and metabolic diseases.
