1.Expression and Clinical Significance of ki-67 in Gastric Adenocarcinoma

		                        			Objective To detect the expression level of ki-67 in gastric adenocarcinoma tissues,and to analyze the correlation be-tween ki-67 and clinicopathological parameters of tumor and its clinical significance.Methods A total of 113 patients with gastric ade-nocarcinoma undergoing surgery in the Affiliated Hospital of Xuzhou Medical University from January 2016 to December 2019 were select-ed.The expression level of ki-67 in gastric adenocarcinoma and paracancerous tissues were detected by immunohistochemical method.The positive expression rates of ki-67 in gastric adenocarcinoma tissues and paracancerous tissues were compared and the correlation be-tween the positive expression of ki-67 in gastric adenocarcinoma tissues and the clinicopathological characteristics of gastric adenocarcino-ma was analyzed;Kaplan-Meier survival curve was used to analyze the correlation between ki-67 levels and the prognosis of patients.COX regression model was established for multivariate prognostic analysis.Results The positive expression rate of ki-67 in gastric ade-nocarcinoma tissues was higher than that in the paracancerous tissues(74.34%vs 38.94%),and the difference was statistically signifi-cant(P<0.05).Tumor diameter≥5cm(P=0.046),vascular invasion(P=0.028)both increase the positive expression rate of ki-67 in gastric adenocarcinoma tissues.Median survival time of patients with high ki-67 expression level(≥50%)was significantly lower than that of patients with low ki-67 expression level(<50%),and the difference was statistically significant(P=0.0361).Multivari-ate COX regression analysis showed that TNM stage(P=0.029)and high ki-67 leve(≥50%,P=0.026)were independent risk factosr affecting the prognosis of gastric adenocarcinoma patients.Conclusion The positive expression of ki-67 is closely related to the occur-rence and development of gastric adenocarcinoma,which has important reference value to predict the progression and prognosis of gastric adenocarcinoma.
		                        		
		                        		
		                        		
		                        	
2.Study on the mechanism of Taohong siwu decoction improving peripheral nerve injury induced by paclitaxel in rats

		                        			OBJECTIVE To explore the mechanism of Taohong siwu decoction (THD) improving peripheral nerve injury induced by paclitaxel (PTX) in rats. METHODS The effects of THD (1 g/mL drug-containing serum) and PTX (0.1 μmol/L) alone or in combination on the proliferation rate of Schwann cells line RSC96 as well as the expressions of lysosomal-associated membrane protein-2 (LAMP2), autophagy marker protein yeast Atg 6 homolog (Beclin1), phosphatidylinositol 3-kinase (PI3K), protein kinase B (Akt), and mammalian target of rapamycin (mTOR) were investigated, and then compared with autophagy promoter rapamycin and autophagy inhibitor 3-methyladenine (3-MA). The effects of high-dose and low-dose THD on the expressions of myelin basic protein (MBP) and myelin protein zero (MPZ), S100 calcium-binding protein (S100), LAMP2, Beclin1, PI3K, Akt and mTOR were tested at the end of the experiment. RESULTS After treatment of THD+PTX, the proliferation rate of RSC96 cells was significantly higher than those treated with PTX alone. After treatment of THD+PTX or THD+ 3-MA, the protein expressions of LAMP2 and Beclin1 in RSC96 cells were significantly higher than those treated with PTX or 3- MA alone, while the protein expressions of PI3K, Akt and mTOR were significantly lower than those treated with PTX or 3-MA alone (P<0.05). Compared with model group, the protein expressions of MBP, MPZ, S100, LAMP2 and Beclin1 in sciatic nerve of rats were increased significantly in THD high-dose and low-dose groups, while the protein expressions of PI3K, Akt and mTOR were significantly decreased (P<0.05). CONCLUSIONS THD may activate Schwann cell autophagy by inhibiting the PI3K/Akt/ mTOR signaling pathway, thereby improving peripheral nerve injury caused by PTX.

		                        			Traumatic cerebrospinal fluid leakage commonly presents in traumatic brain injury patients, and it may lead to complications such as meningitis, ventriculitis, brain abscess, subdural hematoma or tension pneumocephalus. When misdiagnosed or inappropriately treated, traumatic cerebrospinal fluid leakage may result in severe complications and may be life-threatening. Some traumatic cerebrospinal fluid leakage has concealed manifestations and is prone to misdiagnosis. Due to different sites and mechanisms of trauma and degree of cerebrospinal fluid leak, treatments for traumatic cerebrospinal fluid leakage varies greatly. Hence, the Craniocerebral Trauma Professional Group of Neurosurgery Branch of Chinese Medical Association and the Neurological Injury Professional Group of Trauma Branch of Chinese Medical Association organized relevant experts to formulate the " Chinese expert consensus on the diagnosis and treatment of traumatic cerebrospinal fluid leakage in adults ( version 2023)" based on existing clinical evidence and experience. The consensus consisted of 16 recommendations, covering the leakage diagnosis, localization, treatments, and intracranial infection prevention, so as to standardize the diagnosis and treatment of traumatic cerebrospinal fluid leakage and improve the overall prognosis of the patients.
		                        		
		                        		
		                        		
		                        	
4.Construction of evaluation system for clinical effectiveness of Drug Selection Guideline for Medical Institutions

		                        			OBJECTIVE To establish an evaluation syste m of clinical effec tiveness of Drug Selection Guideline for Medical Institutions,and to provide reference for drug selection in medical institution. METHODS Retrieved from relevant Chinese government websites ,PubMed,Embase,CBM and CNKI ,etc.,from the inception to Sept. 14th 2021,related contents of clinical effectiveness related to three secondary indicators ,such as “recommended level and strength of guideline ”“clinical pathway ”and “evidence and level of efficacy ”were extracted respectively ;evaluation system was construction for the clinical effectiveness. RESULTS A total of 5,4 and 17 policy documents or literatures were included according to “recommended level and strength of guideline”“clinical pathway ”and“evidence and level of efficacy ”,respectively.“The recommended level and strength of drug guideline”could reflect the clinical effectiveness of drugs ,and the evaluation content referred to the recommended level and strength of the selected drugs in the guidelines for corresponding indications. “Clinical pathway ”was the embodiment of drug effectiveness, and the evaluation content referred to the clinical path of whether the selected drugs were included in the corresponding indications. The evaluation contents of “evidence and level of efficacy ”were different between chemical medicine/ biological agent and Chinese patent medicine ;evidence and quality level of efficacy research for chemical medicine/biological agent referred to GRADE system ,while those for Chinese patent medicine referred to classic works or clinical experience inheritance. Therefore,the evaluation contents of this index system were the evidence and quality level of the efficacy research related to selected drugs. CONCLUSIONS The evaluation system of clinical effectiveness of drugs constructed from the perspective of drug selection in medical institutions can lay the foundation of evaluation system for the construction of Drug Selection Guideline for Medical Institutions ,and also provide reference for drug selection in medical institutions.
		                        		
		                        		
		                        		
		                        	
5.Effectiveness and safety of programmed cell death-1 inhibitor in the treatment of advanced non-HBV non-HCV-related hepatocellular carcinoma

		                        			 Objective To investigate the clinical effectiveness and adverse events of domestic programmed cell death -1 (PD-1) inhibitor in the treatment of advanced non-HBV non-HCV-related hepatocellular carcinoma (NBNC-HCC). Methods A totals of 31 patients with advanced NBNC-HCC who received domestic PD-1 inhibitor in the Affiliated Hospital of Xuzhou Medical University from June 2019 to February 2022 were retrospectively enrolled and their clinicopathological data were retrieved from their medical records and analyzed, i.e., the time to disease progression (TTP), disease control rate (DCR), objective response rate (ORR), and adverse events were recorded and statistically analyzed. The Kaplan-Meier method was used for survival analysis. Results Among these 31 patients, only one achieved the complete response and four achieved the partial response, and six had the stable disease, but 20 showed a disease progression, resulting in an ORR of 16.1% and a (DCR of 35.5%. The median TTP was 7.2 months [95% confidence interval: 6.4-8.0) months]. The incidence of adverse events was 61.30% and the common adverse events were skin rash (29.03%) and hypertension (22.58%). However, there was no grade 4 adverse reactions or related death in these patients. Conclusion Advanced NBNC-HCC patients had a relative weak response to the PD-1 inhibitor although the adverse events were controllable. 6.Chemotherapeutic drugs affect methylation of ER-α in breast cancer cells by down-regulating miR-148b

		                        			Objective To investigate the effects of chemotherapeutic drugs on ER-α expression and methylation in breast cancer cells.Methods Human breast cancer cells MCF-7(ER+,Luminal A) were induced by paclitaxel(PTX) and epirubicin(EPI) for more than 6 months,with an incremental dose,respectively.The expression and methylation status of ER-α in MCF-7 cells were detected before and after drug treatment.miRNAs with consistent expression changes in MCF-7 cells after two drugs' treatment were screened by microarray,and verified by quantitative PCR (qPCR).Targets of the most significantly down-regulated miRNA were analyzed by bioinformatics.miRNA inhibitor was transfected into MCF-7 cells,miRNA mimic was transfected into MCF-7/PTX and MCF-7/EPI cells,then ER-α and DNA methyltransferase 1 (DNMT1) expression were detected by Western blot,and ER-α methylation was detected by quantitative methylation-specific PCR (qMSP).Results PTX resistant MCF-7/PTX cell line and EPI resistant MCF-7/EPI cell line were established.Both drug treatments caused a decrease in ER-α protein expression and an increase in methylation levels,with up-regulation of DNMT1 and his tone deacetylase 1 (HDAC 1) expression.miRNAs with consistent expression changes in MCF-7 cells after drug treatments were screened and verified by qPCR,the most significant down-regulation among which was miR-148b.Bioinformatics analysis,and further confirmed by luciferase reporter gene assay (Luciferas) that DNMT1 was a direct target of miR-148b.miR-148b inhibitor induced decreased expression of ER-α and increased methylation level in MCF-7 cells,accompanied by increased expression of DNMT1;whereas miR-148b mimic caused an increased expression of ER-α and decreased methylation level in MCF-7/PTX and MCF-7/EPI cells,with a decreased expression of DNMT1.Conclusion Chemotherapeutic drugs (represented by PTX and EPI) induce aberrant miRNA expression in breast cancer MCF-7 cells,and down-regulate miR-148b further to attenuate the inhibition of DNMT1 expression,which promote,hypermethylation and down-regulation of ER-α.
		                        		
		                        		
		                        		
		                        	
7.Effect Bservation of the Pain Education Performed by Clinical Pharmacists in the Patients with Fracture

		                        			Objective:To observe the effect of clinical pharmacists on the pain education in the patients with fracture. Methods:A total of 122 fracture patients with ASAⅠ/Ⅱaged 18-80 years were randomly divided into the intervention group (n=61) and the control group(n=61). The control group received the conventional orthopedic treatment..Flurbiprofen was injected for the postopera-tive analgesia,and if the pain VAS score was above 6,pethidine solution was given at the dose of 50 mg immediately. Based on the conventional treatment,the patients in the intervention group were educated by clinical pharmacist one day before the surgery,and then strengthened education was given after the surgery.The pain VAS score,and sleeping quality at 6h,24h,48h and 72h and satisfaction of pain management were compared between the groups.Results:The VAS score of the intervention group was significantly lower than that of the control group at 6 h,24 h,48 h and 72 h after the operation (P<0.05). The scores of sleeping quality in the intervention group at 6 h and 24 h after the operation were higher than those in the control group (P<0.05). The scores of patients' satisfaction were significantly higher in the intervention group than those of the control group (P<0.05). Conclusion: The clinical pharmacist's pain education can improve the degree of pain control and sleeping quality,increase the patients' satisfaction with pain control to a cer-tain extent.
		                        		
		                        		
		                        		
		                        	
8. Effects of bone marrow stromal cells on the chemotherapeutic sensitivity of acute lymphoblastic leukemia cells

		                        			To investigate the influences of bone marrow stromal cells, components of extracellular matrix and cytokine secreted by stromal cells on the chemotherapeutic sensitivity of acute lymphoblastic leukemia cells to cytosine arabinoside (Ara-C).
		                        		
		                        			Methods:
		                        			The co-culture model of acute lymphoblastic leukemia cell Sup-B15 and bone marrow stromal cell OP9 was constructed. Sup-B15 cells were cultured alone or co-cultured with OP9 cells, inactivated OP9 cells, the conditional medium (CM) of co-cultured OP9 cells and Sup-B15 cells, the CM of OP9 cells alone or Sup-B15 cells alone, respectively. The effects of different concentrations of Ara-C on the proliferation of each Sup-B1 cell group mentioned above were detected by cell counting kit-8 (CCK-8) method. The effects of different concentrations of Ara-C on the apoptosis of each group were detected by flow cytometry (FCM). The expressions of Bcl-2 protein in each group were detected by western blot.
		                        		
		                        			Results:
		                        			The results of CCK-8 test showed that the inhibitory efficiency of Ara-C was in a dose-dependent manner. With different concentrations of Ara-C treatment for 48 hours, the half maximal inhibitory concentrations (IC50) of Sup-B15 and OP9 co-cultured group, Sup-B15 and inactivated OP9 co-cultured group were 0.510 and 0.339 μg/ml, respectively, significantly higher than 0.091 μg/ml of Sup-B15 cultured alone group (
		                        		
		                        	
9.Preliminary study of the differences of related gene expression in cancer stem cells of human breast cancer cell lines MCF-7 and MDA-MB-231

		                        			Objective To enrich breast cancer stem cells of breast cancer cell lines MCF-7 and MDA-MB-231 through culturing mammospheres,and to detect the expression differences of gene of breast cancer stem cells makers.Methods The MCF-7 and MDA-MB-231 cell lines were cultured by serum and serum-free medium.The proportion of CD44+/CD24-and CD133+ cancer stem cells was measured in cells derived from mammosphere cells or monolayer culture cells by flow cytometry,and the expression of CD44,CD24,CD133,ALDH3A1,ABCG2 and CXCR4 mRNA were detected by RT-PCR.Results Flow cytometry analysis indicated that the CD44+/CD24-low proportion of the MCF-7 mammosphere cells was higher than its adherent culture cells (P < 0.05),and CD133+ proportion had no difference between them (P > 0.05).However,the CD44+/CD24-/low proportion of the MDA-MB-231 mammosphere cells was lower than its adherent culture cells (P < 0.05),while CD133+ proportion was significantly higher than its adherent cultured cells (P < 0.05).RT-PCR analysis suggested that the expression of CD44 and ABCG2 increased obviously in MCF-7 microspheres (P< 0.05),and the expression of CD24,CD133,ALDH3A1 and CXCR4 had no significant difference between the mammosphere cells and adherent culture cells (P > 0.05).The CD24,CD133,ABCG2,ALDH3A1 and CXCR4 increased obviously in MDA-MB-231 microspheres.On the contrary,the CD44 decreased (P < 0.05).The expression of CD44 and CD24,CD133 and ALDH3A1 had significant differences in the microspheres of MCF-7 and MDA-MB-231 cells (P < 0.05).Conclusion The related cancer stem cells gene expression is different in the microspheres of human breast cancer cell line,which indicates that the different subtypes of breast cancer may be derived from different origins.
		                        		
		                        		
		                        		
		                        	
10.Analysis of clinical effect of recombinant human granulocyte-macrophage colony-stimulating factor combined with recombinant bovine basic fibroblast growth factor in the treatment of oral ulcer caused by chemotherapy

		                        			Objective To investigate the effect of recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF) combined with recombinant bovine basic fibroblast growth factor (rb-bFGF) in the treatment of oral ulcer caused by chemotherapy.Methods 108 patients of oral ulcer caused by chemotherapy were randomly divided into two groups.54 cases in the control group were treated with cydiodine buccal tablets at first,then received the aerosol treatment which was prepared by mixing gentamicin,dexamethasone,2 % lidocaine and physiologic saline,three times per day.54 cases in the treatment group firstly received the gargle which was prepared by mixing rhGM-CSF,dexamethasone and physiologic saline,then were treated with rb-bFGF by spraying on the oral ulcer surface,three times per day.Results The effective rate of the treatment group was 96.30 % (52/54),which was significantly higher than that of the control group [64.81% (35/54)],there was a significant difference between the two groups (x2 =17.08,P < 0.05).Conclusion The effect of rhGM-CSF combined with rb-bFGF in the treatment of oral ulcer caused by chemotherapy is very significant.
