1.Epidemiological Characteristics of Lung Cancer Incidence in the Tumor Registration Area of Gansu Province from 2010 to 2019

Background and objective Lung cancer is the malignant tumor with the highest incidence rate and the heaviest disease burden in China.In recent years,lung cancer has shown a high incidence trend,seriously affecting the health of the population.In this paper,we analyze the characteristics of lung cancer incidence in 2019 and the trend of incidence rate from 2010-2019 in the tumor registration area of Gansu province,in order to provide a reference basis for the development of lung cancer prevention and control strategies in Gansu province.Methods By analyzing the cases of lung cancer incidence in the tumor registration area of Gansu province in 2019,we calculated the incidence rate,medium incidence rate,world in-cidence rate and other related indexes;we used Joinpoint to calculate the annual percentage change(APC)for trend analysis.Results In 2019,a total of 3757 new cases of lung cancer were reported in Gansu province,accounting for 14.96%of all new malignant tumors.The incidence rate,medium incidence rate and world incidence rate and world rate of lung cancer were 40.52/105,25.78/105,25.86/105;and the cumulative rate of 0-74 years old,and the truncation rate of 35-64 years old were 3.23%,40.03/105,respectively.The incidence of lung cancer rises with age,and is high in the age group of 40 years and above,and the incidence peaks in the male and female populations in the group of 75 years and above,and the group of 80 years and above,respectively.The crude incidence rate of lung cancer in the tumor registration area of Gansu province from 2010-2019 showed an overall increasing trend,and the rate of increase was relatively fast,with an APC 5.39%(P<0.05);Separately,accord-ing to gender,urban and rural areas,the incidence of lung cancer in all populations showed an increasing trend,and the APC of male,female,urban and rural populations were 4.98%,6.39%,6.26%,and 4.64%,respectively(all P<0.05).According to the trend analysis of lung cancer incidence rate by age group,only lung cancer incidence in the age group of 65 years and above increased at an annual average rate of 4.15%(P<0.05).Conclusion The incidence rate of lung cancer in the tumor registration area of Gansu province from 2010 to 2019 shows a rising trend year by year,and there are differences in the incidence of lung cancer in people of different genders,regions and age groups,so comprehensive prevention and control work should be carried out for the key populations of lung cancer incidence.
2. Survey on standardized general practice residency training program in Shanxi province

A Survey on the current general practice residency training was carried out in January 2018 among 1 114 general practitioners (GPs) who attended the standardized residency training program during 2014-2017. Basic information was obtained from the management department, other information was obtained from questionnaire. Total 1089 valid questionnaires were received with a retrieval rate of 97.8%. The survey showed that the ratios of male trainees, trainees aged 20-30 years and with bachelor degree were increasing during 2014-2017. The participants were more satisfied with the teaching quality in general hospital bases than that in community practice bases. Participants suggested to improve teaching quality, add teaching contents of humanity courses and TCM knowledge. Only14.4%(157/1 089) participants choose GPs as their career because liked this specialty. And 77.7%(846/1 089) GPs understood the main work of GP. There were 81.4%(887/1 089) participants wanted to work in; 77.7%(846/1 089) participants thought that the prospect of general practice would be excellent or good. Increasing income, professional promotion and a sense of social recognition were their wishes. Standardized residency training is crucial for general practitioners to upgrade their quality and competence, to improve training quality and to develop incentive policies are necessary.
3.Clinical and genetic analysis of a patient with Perrault syndrome and additional neurological features.

OBJECTIVE:
To explore the clinical, neuropathological and genetic characteristics of a patient with Perrault syndrome caused by TWNK mutation.
METHODS:
Potential variation of the TWNK gene was detected by next-generation sequencing (NGS) and verified by Sanger sequencing.
RESULTS:
The patient has featured primary amenorrhoea and progressive sensorineural hearing loss since childhood. She also had gait anormaly, distal limb atrophy and weakness, and nystagmus. Further study confirmed sensory neuronopathy accompanied with upper and lower motor neuron involvement as well as cerebellum atrophy. NGS has identified two heterozygous variants of the TWNK gene, namely c.794G>A (p.Arg265His) and c.1181G>A (p.Arg394His). Sanger sequencing confirmed that c.1181G>A (p.Arg394His), a known pathogenic variant, was derived from her farther, while c.794G>A(p.Arg265His), a novel variant, was derived from her mother and likely pathogenic according to the ACMG guidelines.
CONCLUSION
Perrault syndrome is a group of disorders with a high phenotypic heterogeneity. The compound heterozygous variation of c.794G>A (p.Arg265His) and c.1181G>A(p.Arg394His) of the TWNK gene may underlie Perrault syndrome in the patient.

Objective To investigate the antimicrobial resistance profile of the clinical isolates collected from selected hospitals across China. Methods Twenty-nine general hospitals and five children's hospitals were involved in this program. Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems. Results were interpreted according to CLSI 2017 breakpoints. Results A total of 190 610 clinical isolates were collected from January to December 2017, of which gram negative organisms accounted for 70.8% (134 951/190 610) and gram positive cocci 29.2% (55 649/190 610). The prevalence of methicillin-resistant strains was 35.3% in S. aureus (MRSA) and 80.3% in coagulase negative Staphylococcus (MRCNS) on average. MR strains showed much higher resistance rates to most of the other antimicrobial agents than MS strains. However, 91.6% of MRSA strains were still susceptible to trimethoprim-sulfamethoxazole, while 86.2% of MRCNS strains were susceptible to rifampin. No staphylococcal strains were found resistant to vancomycin. E. faecalis strains showed much lower resistance rates to most of the drugs tested (except chloramphenicol) than E. faecium. Vancomycin-resistant Enterococcus (VRE) was identified in both E. faecalis and E. faecium. The identified VRE strains were mainly vanA, vanB or vanM type based on phenotype or genotype. The proportion of PSSP or PRSP strains in the non-meningitis S.pneumoniae strains isolated from children decreased but the proportion of PISP strains increased when compared to the data of 2016. Enterobacteriaceae strains were still highly susceptible to carbapenems. Overall, less than 10% of these strains (excluding Klebsiella spp.) were resistant to carbapenems. The prevalence of imipenem-resistant K. pneumoniae increased from 3.0% in 2005 to 20.9% in 2017, and meropenem-resistant K. pneumoniae increased from 2.9% in 2005 to 24.0% in 2017, more than 8-fold increase. About 66.7% and 69.3% of Acinetobacter (A. baumannii accounts for 91.5%) strains were resistant to imipenem and meropenem, respectively. Compared with the data of year 2016, P. aeruginosa strains showed decreasing resistance rate to carbapenems. Conclusions Bacterial resistance is still on the rise. It is necessary to strengthen hospital infection control and stewardship of antimicrobial agents. The communication between laboratorians and clinicians should be further improved in addition to surveillance of bacterial resistance.
6.Five Chinese patients with mitochondrial diseases caused by POLG gene mutations

Objective To report the clinical features, myopathological changes, and gene mutations in five Chinese patients with mitochondrial diseases caused by POLG gene mutations. Methods Clinical materials of five unrelated patients who were referred to Department of Neurology, Peking University First Hospital from April 2012 to January 2018, carrying POLG gene mutations, were retrospectively analyzed. Muscle/nerve biopsies and targeted second-generation gene sequencing were performed on the patients. Results Among the five patients, three were male and two were female. Two cases were dominant inheritance and three were sporadic or recessive inheritance. The ages of onset were from 15 to 40 years with disease course of one to 26 years. One of them showed atypical SANDO (sensory ataxic neuropathy, dysarthria, and ophthalmoparesis) syndrome accompanied by cardiac preexcitation syndrome. There were two cases with autosomal dominant and one case with recessive progressive external ophthalmoplegia plus syndrome. One case presented with cognitive delay and sensory neuropathy. The pathological changes of mitochondrial myopathy were observed in all four patients with muscle involvement. Sural nerve biopsy in the patient with cognitive delay and sensory ataxia revealed chronic axonal pathological changes. POLG gene mutations were found in all five patients by targeted next generation sequencing, including single heterozygous mutations in two dominant inherited patients (c. 914 G>A and c. 2864A>G, respectively), and compound heterozygous POLG gene mutations in the other three sporadic/recessive inherited patients (c. 2591 A>G/c. 1790 G>A, c. 924G>T/c. 3002delG and c. 1613A>T/c. 1612 G>T, respectively). There were six novel mutations not reported before, i.e., c.914G>A(p.S305N), c.924G>T(p.Q308H), c.1613A>T(p.E538V), c.1612G>T(p.E538*), c.1790 G>A(p.R597Q) and c.3002delG. Conclusions POLG gene mutations can lead to different clinical spectrums. Progressive external ophthalmoplegia, limb weakness and axonal sensory neuropathy are common presentations in this group of patients with POLG gene related mitochondrial neuromuscular diseases. Novel mutations found in this study expand the mutational spectrum of POLG gene.
7.Clinical effect of minimally invasive vitreoretinal surgery combined with a modified suprachoroidal drainage surgery for retinal detachment associated with choroidal detachment

Objective To observe the clinical effect of minimally invasive vitreoretinal (MIV) surgery combined with a modified suprachoroidal drainage surgery for retinal detachment associated with choroidal detachment (RRDCD).Methods A prospective clinical study.A total of 27 patients (27eyes) diagnosed as RRDCD were recruited in this study.There were 16 males and 11 females,with an average of (53.67± 14.82) years.The mean intraocular pressure (IOP) was (8.2± 2.1) mmHg (1 mmHg=0.133 kPa) and best corrected visual acuity (BCVA) of minimum resolution angle logarithm (logMAR) was 1.87±0.58.All subjects underwent 23G MIV combined a modified suprachoroidal drainage surgery,which 23G stab knife and 1 ml syringe needle were used for surgery.The visual outcome,IOP,rate of retinal reattachment and complications were comparatively analyzed preoperatively and postoperatively.Results At 1 day,10 days,1 month and 3 months after surgery,the average of logMAR BCVA were 1.62 ± 0.67,1.51 ± 0.63,1.39 ± 0.54,1.32± 0.56 and the mean of IOP were (13.47 ± 5.06),(14.43 ± 4.09),(14.89 ± 4.30),(15.38 ± 3.37) mmHg,respectively.There were significant differences of logMAR BCVA and IOP between before and after surgery (F=6.19,15.21;P<0.05).Retinal reattachments were achieved in 27 eyes (100%) at 1 day and 10 days after surgery.At 1 month and 3 months after surgery,the rate of retinal reattachment were 88.89% (24 eyes) and 85.19% (23 eyes),respectively.No severe complications such as endophthalmitis and choroidal hemorrhage were found at follow-up visits.Conclusion MIV combined with a modified suprachoroidal drainage surgery is an effective and safe treatment for RRDCD,which can promote retina tear closure,improve visual acuity.
8.Tibial nerve stimulation to inhibit the micturition reflex by an implantable wireless drivermicrostimulator in cats

Objective Objective To evaluate the effects of a new type of tibial nerve microstimulator on the micturition reflex in cats.Methods From March to May in 2017,the implantable wireless driver micro-stimulator was implanted around the tibial nerve in 9 α-chloralose anesthetized domestic shorthairs cats (2.5-3.5 kg,6-12 months old).The stimulator was placed near the neurovascular bundle parallel to the tibial nerve and its cathode perpendicular to the cushion.The intensity which can induce toe movement was defined as threshold (T).The ureters were isolated via an abdominal incision.The ureters were cut and drained externally.The bladder was inserted via a double lumen catheter through the urethra.The catheter was then secured by a ligature around the urethra.One lumen of the catheter was used to infuse the bladder with either 0.9% normal saline (NS) or 0.25% AA at a rate of 1 to 2 ml/min after connecting to a pump.The other lumen was connected to a pressure transducer to measure the bladder pressure.The bladder capacity was used to test the inhibitory effect of the stimulator.After the appearance of the first large-amplitude (> 30 cmH2O) bladder contraction,the bladder infusion was stopped.First,after emptying the bladder,2 or 3 cystometrograms with NS were performed without stimulation to obtain the control bladder capacity.After the bladder was stabilized,TNS (6 Hz,1-2 T) was applied during 2 sequential cystometrograms.Second,after emptying the bladder,0.25 % AA was infused into the bladder to irritate and induce bladder overactivity.After the bladder stabilized,TNS (6 Hz,1-2 T) was applied again during 2-3 sequential cystometrograms.If bladder capacity increased significantly,the stimulationhad an inhibitory effect on the micturition reflex.Results During normal saline infusion,the bladder baseline was (17.03 ± 4.10) ml.TNS at 1T did not change the bladder capacity [(18.56 ±0.81)ml] (P >0.05).TNS at 2T significantly increased the bladder capacity [(25.05 ± 1.19) ml] (P < 0.05).Compared to normal saline infusion,bladder overactivity was irritated by the intravesical infusion of 0.25% acetic acid,which significantly reduced the bladder capacity [(9.34 ± 0.75) ml] (P < 0.05).Compared to acetic acid infusion,TNS at 1T did not change the bladder capacity [(11.32 ± 0.82) ml] (P > 0.05).TNS at 2T significantly increased the bladder capacity [(14.82 ± 1.15) ml] (P < 0.05).Conclusions The implantable wireless driver tibial nerve micro-stimulator appears to be effective in inhibiting the micturition reflex during physiologic and pathologic conditions.The implantable wireless driver tibial nerve microstimulator was excepted to be used to treat overactive bladder (OAB).
9.Development and application of a calculation software for paternity index (PI) in parentage testing and likelihood ;ratio (LR) in individual recognition

Objective To establish a calculation software to determine paternity index(PI) in parentage testing and likelihood ratio (LR) in individual recognition.Methods Based on relevant industry standards and literature, using Visual Basic 6.0 to write the program.Results We successfully developed the calculation software for paternity index (PI) in parentage testing and likelihood ratio (LR) in individual recognition.Conclusion The calculation software can help staff to improve the calculation efifciency, and serve the forensic evidence.
10.An Emerging Simulation Method Used in System Simulation of Flow Chamber of Hematology Analyzer

Infl uidfl owfi eld analysis, the common simulation method is 1-D simulation or 3-D simulation, In order to analyze bloodfl uid system more quickly and accurately and choose the appropriate sample tube at the beginning of design for the system, this paper adopts a recently emerging 1D-3D simulation method to make simulations offl ow chamber subsystem. Respectively using Flowmaster and ANSYS system modeling, use MpCCI connects the two parameter coupling, realize the sheathfl uid velocity of research. The software can meet the needs of design and analysis of the Hematology Analyzerfl uid system. In this paper, the method of co-simulation for medical apparatus and instruments of Hematology Analyzerfl uid system development provides a new method, has important signifi cance on the subsequent simulation.
