1.Dose response relationship between daily steps counts and fat loss in obese children and adolescents

Objective:
To investigate whether there is a nonlinear relationship and threshold effect between the number of daily steps counts and fat loss in obese children and adolescents.
Methods:
Forty eight obese children and adolescents were randomly recruited to undergo 4 week closed exercise training during July to August 2021. Actigraphy GT3X+ was used to monitor the daily steps, and fat loss effect was evaluated by changes of body composition indicators before and after the intervention. Paired samples t test was used to compare the indicators before and after the intervention; the dose response relationship between daily steps counts and fat loss was analyzed by linear regression and piecewise regression, and the nonlinear relationship was analyzed by restricted cubic spline (RCS).
Results:
Body fat mass, percentage of body fat, lean body mass and skeletal muscle mass decreased (27.50± 7.33 )kg, (36.17±5.59)%, (47.55±6.48)kg, (26.14±3.84)kg, respectively compared with those before intervention[(31.97± 7.82 )kg,(39.06±5.12)%,(49.08±6.93)kg,(27.08±4.15)kg] ( t=21.04, 13.32, 7.65, 8.35, P <0.05). There was a significant nonlinear doseresponse relationship between daily steps counts and the change of these indicators ( P <0.05). After adjusting for age, sex and baseline BMI, each 1 000 step increase in daily walking number, BMI increased by 0.44(95% CI =0.03-0.84)kg and body fat percentage by 0.61%(95% CI =0.17%-1.04%). After adjusting for age, sex, and baseline BMI, each 1 000 step increased in daily number of steps occurred when the daily number of steps ranged from 8 300 to 11 400, lean body weight increased by 0.58(95% CI =0.11-1.04)kg, skeletal muscle mass increased by 0.29(95% CI =0.03-0.54)kg.
Conclusion
There is a non linear dose response relationship between daily steps and fat loss in obese children and adolescents, for optimal fat loss efficiency, daily steps to 8 300-11 400 appropriate.
2.Visualization analysis of research on executive function in overweight or obesity

ObjectiveTo study the current status and development trend of research on the executive function in overweight or obesity, and to grasp the research hotspots in this field. MethodsA total of 1 321 literatures relevant to the executive function in overweight or obesity collected in Web of Science Core Collection database from 2010 to 2021 were selected. CiteSpace and VOSviewer were used to generate knowledge graphs for visualization analysis, then the number of articles issued, countries/regions, institutions and the co-occurrence, clustering and burst of keywords were analyzed. Results①In terms of the trend of the number of articles issued from 2010 to 2021, the annual quantity of published articles about the executive function in overweight or obesity presented a rapid growth over 2010-2014, then entered into a slow growth stage over 2014-2017, and emerged a rapid growth over 2017-2021. ②From the perspective of countries / regions, a total of 64 countries and regions participated in the research. The largest number of articles issued in this field was the USA, which had the most frequent cooperation and exchanges with other countries and regions, and exerted the greatest academic influence, occupying the core position of this research field. ③From the perspective of research institutions, a total of 1 627 institutions participated in this research field, among which the Illinois University published the most papers and closely cooperated with many productive institutions, forming a research group with a certain scale. ④From the perspective of keywords, the research content mainly involved dietary behavior research, prevention and intervention research, risk factor assessment, and analysis of the characteristics of different groups. ConclusionResearches on executive function in overweight or obesity have been developing rapidly, attracting international attention and covering a wide range of areas.
3.Evaluation criteria of moderate to vigorous physical activity in overweight or obese adolescents

Objective:
To verify the current cut off points of physical activity intensity for adolescents to assess moderate to vigorous physical activity (MVPA) among overweight or obese adolescents.
Methods:
The total activity counts, heart rate and steps indicators most commonly used to reflect physical activity intensity were adopted, and a total of 15 MVPA cut off points standards for adolescents were included. Ninety four overweight or obese adolescents were tested for walking and running at 3-7 km/h in a free state, while simultaneously wearing MetaMax 3B gas metabolism analyzer, polar belt and actigraph w-GT3x BT triaxial accelerometer to collect energy consumption and activities count, heart rate and steps. Kappa consistency test and paired χ 2 test were used for statistical analysis.
Results:
Kappa consistency coefficients (0.27-0.53) <0.60 between all cut off points standards and the "gold standard" and the P <0.01, indicating that the consistency is varied and not strong. In the standard diagnosis of each cut points, low sensitivity (49.11-67.59), high specificity (92.50-97.65), high - LR (0.14-0.52, >0.1) and low DOR (8.26-25.19, <30) indicated high rate of misdiagnosis. Low specificity (36.75-69.41), high sensitivity (84.82-96.36) and low + LR (1.52- 9.83 , <10) indicated a high rate of misdiagnosis; AUC of 0.67-0.80 suggested lower diagnostic performance.
Conclusion
Existing physical activity intensity cut off points for overweight or obese adolescents were not consistent with MVPA and have low diagnostic capabilities. The following criteria of MVPA for overweight or obese adolescents are supposed.
4. Efficacy and safety of low dose sublingual nifedipine dripping pills (5 mg) in the acute treatment of moderate and severe hypertension: a randomized, double-blind, positive-drug parallel-controlled, multi-center clinical study

Objective:
To evaluate the efficacy and safety of low dose sublingual nifedipine dripping pills (5 mg) in treating moderate and severe hypertension in comparison with normal dose (10 mg) of sublingual nifedipine dripping pills.
Methods:
This study was designed as a randomized, double-blind, positive drug parallel controlled, multi-center, non-inferiority clinical trial. Patients with moderate and severe hypertension were enrolled by 14 clinical trial centers, randomly divided into the trial group (sublingual 5 mg nifedipine dripping pills) and the control group (sublingual 10 mg nifedipine dripping pills). The changes in blood pressure were monitored continuously within 2 hours after the initial administration, repeated the dose in 20 minutes interval after the initial administration for up to additional 3 doses (maximum 4 doses) if the antihypertensive efficacy was not satisfactory. The efficacy of antihypertensive therapy between the two groups was evaluated by repeated administration rates and blood pressure changes at 60 minutes post the initial administration, and the safety of treatment was evaluated by recording adverse event rate of the two groups.
Results:
The anti-hypertensive effective rates at 60 minutes after sublingual administration were 83.5% (202/242) and 86.7% (208/240) respectively between the trial group and control group (χ2=1.307,
5.Impact of smoking and platelet distribution width on clopidogrel resistance in patients with coronary ar-tery disease LI

Objective To explore the influence of smoking on clopidogrel resistance in patients with coro-nary artery disease. Methods A total of 216 patients with coronary artery disease who accept selective percutane-ous coronary intervention in our hospital from May 2015 to December 2015 were selected as study subjects;their average age was 63 years old ,146 were male and 70 were female. All the patients were divided into clopidogrel resistance group (CR group) and normal clopidogrel response group (NCR group) according to the results of platelet aggregation test. History of smoking ,alcohol drinking and diabetes mellitus ,baseline medication uses , level of platelet counts,platelet distribution width,mean platelet volume,HbA1c,and results of echocardiogram and coronary angiography were compared between the two groups. Results The rate of smoking was significantly higher in CR group than in NCR group (P < 0.05). The changed value of PDW before and after treatment with clopidogrel was smaller in CR group than in NCR group ,the difference was statistically significant (P < 0.05). Conclusions Smoking may be a protective factor for clopidogrel resistance. The changed value of PDW before and after treatment with clopidogrel can reflect the level of clopidogrel resistance.
6.Comparison of the two different systems for contrast injection in coronary angiography

Objective To study whether the contrast volume and radiation dose can be reduced by automated contrast injection system(ACIS) in coronary angiography compared with manual contrast injection system(MCIS).Methods 200 patients undergoing coronary angiography with transradial approach in the People′s Hospital of Liaoning Province were enrolled in the study from January 2016 to June 2016.They were divided into the ACIS group (n=100) and the MCIS group (n=100).The clinical data, the net amount of contrast the total amount of contrast media consumed, number of angiographic views performed, fluoroscopy time, air kerma (AK) and dose area product (DAP) of the two groups were statistically analyzed.Results There were no statistical differences in the clinical data, the net amount of contrast used, number of angiographic views performed and fluoroscopy time between the two groups (all P>0.05).The total amount of contrast media used, AK, and DAP were less in the ACIS group than in the MCIS group (all P<0.05).Conclusions The volume of contrast consumption and radiation dose can be reduced by ACIS during coronary angiography with transradial approach compared to MCIS.
7.Effects of asymmetric dimethylarginine on apoptosis and expression of c-Jun N-terminal kinase in endothelial outgrowth cells

Objective To investigate the effects of asymmetric dimethylarginine (ADMA) on apoptosis and phosphorylation of c-jun N-terminal kinase (JNK) in endothelial outgrowth cells (EOCs). Methods The mononuclear cells were harvested from umbilical cord blood by ficoll density gradient centrifugation, and induced into EOCs and then expanded in vitro. The identified EOCs were treated with different concentrations of ADMA (0, 1, 5, 10, 20 μmol/L) for 48 h. The adherent cells were treated with 10 μmol/L ADMA,then different concentrations of JNK specific inhibitor SP600125 (0, 5,10,20 and 40 μmol/L) were added and incubated for 48 hours. Caspase-3 activity was measured by microplate reader. Apoptotic incidences of EOCs were quantitatively determined by flow cytometry. The expression of Caspase- 3 and phosphorylase-JNK (p-JNK) were detected by Western blot assay. Results The treatment of ADMA (1-20 μmol/L) significantly induced apoptosis in EOCs by enhancing Caspase-3 express and also induced phosphorylation of JNK (P<0.05). Meantime, the JNK specific inhibitor SP600125 could attenuate the apoptosis induced by ADMA during this process (F=6.733,P<0.05) and inhibit the expression of Caspase-3 and p-JNK. Conclusion ADMA can induce apoptosis in EOC, which may be achieved by activating JNK signal transduction pathway.
8.Investigation on the Cognition and Attitude of TCM Practitioners to TCM Formula Granules in Our University

OBJECTIVE:To provide reference for exploring the strategy of promoting TCM formula granules.METHODS:The questionnaire was designed to investigate the cognition and attitude of TCM practitioners from different grades to TCM formula granules in our university,and the investigation data were analyzed statistically.RESULTS:A total of 312 questionnaires were collected and 306 questionnaires were collected with recovery rate of 98.08%.Among them,305 questionnaires were effective with effective rate of 99.67%.46.89% of interviewed practitioners were found to know and understand about TCM formula granule,52.46% knew but did not understand;the higher the grade,the higher the proportion of the known and the better understanding (P<0.05);the cognitive channels included diagnosis and treatment course (69.64%),took by others (65.02%),took by oneself (51.49%).TCM formula granules were easy to take without decoction (95.71%) and easy to carry (87.13%),the advantage of which had high popularity,but there was not much knowledge about other advantages.7.54% of interviewed practitioners fully appreciated TCM formula granules and 69.18% recognized them;the higher the grade,the higher the degree of recognition (P< 0.05).89.51% were willing to learn more about TCM formula granules.60.98% said they would prescribed TCM formula granules in the furure.Among the influential factors for the attitude of the practitioners to TCM formula granules,the three factors with the highest influence were the influence of the teachers,the influence of the patients and their own practice.CONCLUSIONS:The practice of TCM practitioners in our university in Beijing is not ideal for the cognition of TCM formula granules.There is still a great room for improvement in the attitude to TCM formula granules.The teachers and patients are key influential factors for the attitude to TCM formula granules.
9.Meta-analysis of efficacies of dasatinib and imatinib for patients with chronic myeloid leukemia-chronic phase

Objective To compare the efficiencies of imatinib and dasatinib in patients with chronic myeloid leukemia-chronic phase (CML-CP).Methods The databases were retrieved,including Cochrane Library,OVID,Embase,PubMed,China National Knowledge Infrastructure (CNKI),WanFang database and VIP database,besides,references of articles were further to search.The quality of randomized controlled trials (RCTs) was assessed by the Cochrane collaboration' s risk tool.Meta-analysis was performed by RevMan 5.1 software.Results A total of 5 articles involved 2 031 patients with CML-CP were included.Meta-analysis showed that the rate of complete cytogenetic response (CCyR) at the 12th month in dasatinib group was higher than that in imatinib group [83.6 % (478/572) vs 70.6 % (406/575),OR =2.11,95 % CI 1.59-2.80,P< 0.05],and the rate of major molecular response (MMR) at the 12th month in dasatinib group was higher than that in imatinib group [49.3 % (296/600) vs 30.6 % (185/605),OR =2.22,95 % CI 1.75-2.82,P < 0.05].Conclusion Dasatinib can improve CCyR and MMR rate at the 12th month in CML-CP patients.
10.Clinical application of Voriconazole treatment in invasive fungal infection for malignant hematology patients: a meta analysis

Objective To estimate the efficacy and safety of Voriconazole as antifungal prophylaxis of invasive fungal infection ( IFI) for malignant hematology patients. Methods The randomized controlled trials of Voriconazole treatment in invasive fungal infection for malignant hematology patients (ended in September 2014) were searched from Cochrane library, Medline, Embase, Pubmed, CBM, CNKI, Blood database. The meta analysis were performed by RevMan5.0. Results Ten literatures reported in 1 773 cases, in which there was significantly difference in effective rate between Voriconazole and other antifungal agents such as Amphotericin-B, Itraconazole, Micafungin and Fluconazole(P < 0.000 01); Four literatures indicated that there was significantly difference in adverse event rate between Voriconazole and amphotericin-B (P < 0.00 001); no significantly difference in adverse event rate between Voriconazole and amphotericin-B(P = 0.57); no significantly difference in adverse event rate between Voriconazole and Micafungin (P = 0.69); no significantly difference in adverse event rate between Voriconazole and Fluconazole (P = 0.70); Subgroup analysis indicated that adverse event rate between Voriconazole and Itraconazole is P=0.001, P = 0.17 respectively. Conclusion Voriconazole showed relative high efficient and low toxicity characteristics in treatment of malignant hematology accompanied by invasive fungal infection. But with its widely clinical application, the clinical value of Voriconazole needs to be further tested.
