1.Effect of basic treatment combined with orthodontic treatment on periodontal pocket depth and bleeding index in patients with anterior tooth displacement caused by periodontal disease

Objective:To analyze the effects of basic treatment of periodontal diseases combined with orthodontic treatment on periodontal pocket depth and bleeding index (BI) in patients with anterior tooth displacement caused by periodontal diseases.Methods:A total of 136 patients with anterior tooth displacement due to periodontal disease treated in the Second People′s Hospital of Yueqing from October 2020 to October 2022 were enrolled and they were divided into two groups by random number table method, and each group with 68 cases. The control group was given basic treatment for periodontal disease, while the experimental group was combined with orthodontic treatment on the basis of the control group. The clinical efficacy, masticatory efficiency, periodontal pocket depth (PD), probing BI, gingival sulcus inflammatory factors and the total incidence of complications were compared between the two groups.Results:The total effective rate in the experimental group was higher than that in the control group: 95.59%(65/68) vs. 79.41%(54/68), there was statistical difference ( P<0.05). After treatment for 3 months, the masticatory efficiency in the experimental group was higher than that in the control group, and PD, BI in the experimental group were lower than that in the control group: (70.52 ± 6.88) % vs.(61.52 ± 4.04) %, (2.12 ± 0.62) mm vs. (3.18 ± 0.85) mm, (1.26 ± 0.26) scores vs. (2.42 ± 0.33) scores, there were statistical differences ( P<0.05). After treatment, the levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6 and IL-8 in the experimental group were lower than those in the control group: (11.16 ± 1.85) μg/L vs. (16.92 ± 2.84) μg/L, (8.16 ± 1.34) ng/L vs. (15.49 ± 2.74) ng/L, (6.18 ± 1.46) ng/L vs. (10.33 ± 1.75) ng/L, there were statistical differences ( P<0.05). The total complication rate in the experimental group was lower than that in the control group: 1.47%(1/68) vs. 11.76%(8/68), there was statistical difference ( P<0.05). Conclusions:Basic treatment of periodontal diseases combined with orthodontics can effectively improve the masticatory efficiency of patients with anterior tooth displacement caused by periodontal diseases, reduce PD,BI and alleviate gingival sulcus inflammation, and reduce the incidence complications.
2.Analysis of laboratory characteristics and evaluation of prognostic value of patients with NPM1 mutated acute myeloid leukemia

Objective:To analyze the clinical and laboratory characteristics of acute myeloid leukemia (AML) patients with NPM1 mutation, and to explore the prognostic factors.Methods:A total of 77 AML patients with NPM1 gene mutation admitted to Hebei Yanda Ludaopei Hospital from May 1st 2012 to December 31st 2021 were enrolled in the study, including 34 male and 43 female patients. The median age was 40 (3, 68) years old. Patients were selected and divided into 4 groups according to the morphological FAB classification. There were 29 cases (37.7%) of M1 type, 13 cases (16.9%) of M2 type, 23 cases (29.9%) of M4 type, and 12 cases (15.5%) of M5 type. The clinical characteristics, bone marrow/peripheral blood cell morphology, immunophenotype, cytogenetics, molecular biology and overall survival of different groups were retrospectively analyzed, and the risk factors affecting the prognosis of AML were also explored. Cox multivariate regression was used to analyze the clinical influencing factors of survival and prognosis.Results:The white blood cell counts were highest in M4 and M5 patients and lowest in M2 patients, while no significant difference in the red blood cell, hemoglobin, and platelet counts( P>0.05). Morphologically, there were significant differences in the percentage of blasts and blasts with cup-like nuclei on bone marrow (BM) and peripheral blood (PB). The proportion of blasts in BM and PB was the highest in M1 and the lowest in M2 ( P<0.001). The positive rate of blasts with cup-like nuclei was the highest in M1 and the lowest in M5 of BM ( P<0.001), while the highest in M2 and the lowest in M5 of PB ( P=0.006). The scores of myeloperoxidase and chloroacetate esterase were all the highest in M1 and the lowest in M5 ( P<0.001, 0.001, respectively). In terms of molecular biology, the occurence rate of blasts combined with DNMT3A mutation was the highest in M4 and the lowest in M2 ( P=0.044), while those combined with FLT3-ITD mutation was the highest in M4 and the lowest in M5 ( P=0.002). In immunophenotype, there were significant differences in the expression positivities of seven antigens including HLA-DR, CD56, CD11c, CD15, CD14, CD96 and cMPO ( P<0.05). Multivariate COX regression analysis showed that no recurrence after treatment ( P<0.001), complete remission after treatment ( P=0.015) and transplantation ( P<0.001) were correlated with overall survival (OS). No recurrence after treatment ( P=0.033), transplantation ( P=0.027), no mutation of FLT3-ITD ( P=0.040), and hemoglobin concentration ( P=0.023) were associated with relapse-free survival (RFS). Survival analysis by Kaplan-Meier curve showed that there was no significant difference in survival time between the M1, M2, M4 and M5 groups in OS and RFS. Conclusion:There were significant differences in the white blood count, the percentage of blasts and blasts with cup-like nuclear morphology, cytochemical staining (MPO integration, CE integration and percentage of NAS-DCE), gene mutation (DNMT3A and FLT3-ITD) and immunophenotypes (HLA-DR, CD56, CD11c, CD15, CD14, CD96 and cMPO) between the four groups. The multivariate analysis revealed that no recurrence after treatment and transplantation were independent prognostic factors in NPM1 mut AML patients. On the other hand, FLT3-ITD mutation and hemoglobin concentration were associated with RFS and complete remission after treatment was associated with OS in the entire NPM1 mut cohort.
3.Regulating effects of Neiguan(PC6)and Gongsun(SP4)on hypothalamic-pituitary-adrenal axis in rats with functional dyspepsia

Objective:To investigate the effects and the possible mechanisms of Neiguan(PC6)and Gongsun(SP4)on the hypothalamic-pituitary-adrenal(HPA)axis in rats with functional dyspepsia(FD),thus to provide a theoretical basis for the clinical application of the Eight Confluent Points.Methods:Forty specific-pathogen-free Sprague-Dawley rats were divided into a blank group,a model group,an electroacupuncture(EA)group,and a Western medicine group by the random number table method,with 10 rats in each group.Rats in the blank group did not receive modeling or intervention.Rats in the other three groups were subjected to the FD with mood disorder model using the compound etiology modeling method.After the successful modeling,rats in the model group did not receive any interventions,rats in the Western medicine group received deanxit and mosaprid intervention,and those in the EA group received EA intervention on the ipsilateral Neiguan(PC6)and Gongsun(SP4)for 21 d.The sugar-water consumption rate was measured before the experiment and before and after interventions to assess the emotional status.The gastric emptying rate was measured after interventions to assess the gastrointestinal dynamics.The expression levels of hypothalamic corticotropin-releasing hormone(CRH),pituitary adrenocorticotropic hormone(ACTH),and adrenal corticosterone(CORT)were measured by enzyme-linked immunosorbent assay.Results:Compared with the blank group,the sugar-water consumption rate and the gastric emptying rate were decreased(P<0.01),and the hypothalamic CRH,pituitary ACTH,and adrenal CORT expression levels were increased(P<0.01)in the model group.Compared with the model group,the sugar-water consumption rate and the gastric emptying rate were significantly increased(P<0.01),while the expression levels of hypothalamic CRH,pituitary ACTH,and adrenal CORT were significantly decreased(P<0.01)in the EA group and the Western medicine group.The differences between the EA group and the Western medicine group were not statistically significant(P>0.05).Conclusion:The Eight Confluent Points Neiguan(PC6)and Gongsun(SP4)can improve the mood and gastrointestinal dynamics in FD rats,which may be achieved by down-regulating the hypothalamic CRH,pituitary ACTH,and adrenal CORT,as well as by correcting the HPA axis hyperfunction.
4.Establishment and evaluation of pyroptosis model of rat chondrocytes induced by TNF-α

Objective:
To establish and evaluate the pyroptosis model of rat chondrocytes induced by tumor necrosis factor-α(TNF-α).
Methods:
A two-step enzymatic digestion method was used to obtain rat articular chondrocytes, inverted phase contrast microscope was used to observe the morphological structure of chondrocytes. Toluidine blue staining and type Ⅱ collagen(Col Ⅱ) staining were used to identify chondrocytes. Different mass concentrations of TNF-α(5, 10, 20, 40 ng/ml) were used to establish the pyroptosis model with TNF-α(0 ng/ml) as the control group. Cell viability was detected by CCK-8 method and proteins of pyroptosis signal were detected by Western blot. The levels of IL-1β and IL-18 in cultured supernatants were examined by ELISA kits. The expression of gasdermin D(GSDMD) in chondrocytes was detected by immunofluorescence. Scanning electron microscope was used to observe the morphological changes of chondrocyte.
Results:
Compared with the control group, the cell viability of rat chondrocytes gradually decreased with the increase of TNF-α concentration and the expression of nucleotide-binding and oligomerization domain-like receptor containing protein 3(NLRP3), caspase-1, GSDMD and Phospho-NF-κB p65(P-p65) proteins increased. Furthermore, TNF-α(20 ng/ml) could up-regulate the expression of matrix metalloproteinase-13(MMP-13), the fluorescence expression of GSDMD and the levels of IL-1β and IL-18 while the expression of Col Ⅱ was distinctly reduced. What′s more, the articular chondrocytes were swollen,and the microstructure was destroyed.
Conclusion
TNF-α(20 ng/ml) can cause the swelling and death of rat chondrocytes, degradation of cartilage matrix and activation of pyroptosis signaling pathway. The pyroptosis model of rat chondrocytes was successfully established.
5.Study on the incidence of adult herpes zoster in Yichang city and its association with early-life famine exposure

Based on Yichang health big data platform, 850 608 patients from September 2018 to September 2019 were included in this study. According to the date of birth, the participants were divided into early childhood famine exposure group, fetal famine exposure group and non-famine exposure group. The incidence of adult herpes zoster (HZ) in Yichang city was analyzed, and the correlation between early life famine exposure and adult HZ was analyzed. In 2019, the crude incidence rate of adult HZ in Yichang was 6.83‰. The crude incidence rate of adult HZ in females (7.26‰) was higher than that in males (6.40‰). Compared with the non-famine exposure group, fetal famine exposure was associated with the incidence of adult HZ ( OR=1.21; 95% CI: 1.01-1.45, P=0.041). After stratification by sex, fetal famine exposure was only found to be associated with the onset of adult HZ in females ( OR=1.28, 95% CI:1.02-1.61, P=0.034).

8.Change of fractional amplitude of low frequency fluctuation in untreated primary insomnia patients: a resting state functional magnetic resonance imaging

Objective The aim of this study is to understand the impairment and compensation mechanism of brain function in untreated primary insomnia (PI) patients.The approach of fractional amplitude of low frequency fluctuation (fALFF) is used to analyze raw data between the PI patients and the normal control group in resting state using functional magnetic resonance imaging (fMRI).Methods Fifty-nine PI patients,admitted to our hospital from November 2015 to November 2016,and 47 age-,education-,and gender-matched normal healthy subjects were chosen in our study.Pittsburg sleep quality index (PSQI),insomnia severity index (ISI) were employed to evaluate the sleep quality.Self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were employed to evaluate the emotion.Resting state fMRI and fALFF analyses were used to compare the functional regional activities.The correlations of fALFF data with PSQI,SAS and SDS scores were analyzed.Results In PI patients,2 had mild to moderate insomnia,41 had moderate insomnia,and 16 had serious insomnia.ISI scores in the normal healthy subjects were less than 7.The PSQI,SAS,SDS and ISI scores in the PI patients were significantly higher than those in the normal healthy subjects (P<0.05).As compared with the control group,the PI group had significantly increased fALFF value in the right hippocampus (HIP),right parahippocampa gyms,right amygdala,and bilateral thalamus.The fALFF value was positively correlated PSQI,SASandSDSscores (r=0.582,P=0.000;r=0.617,P=0.000;r=0.653,P=0.000).Conclusion Some brain regions in the PI patients are abnormal in the resting state,which can reflex functional regional activities of PI patients.
9.Study on nursing service needs among elderly people lived in nursing institutions in Fuzhou

Objective To investigate nursing care needs of elderly nursing homes residents in Fuzhou, to help to improve nursing care quality provided by elderly nursing homes. Methods A total of 120 elderly residents from 10 elderly nursing homes in Fuzhou were investigated with a self-developed questionnaire focusing on nursing care needs of elderly nursing home residents. The questionnaire included the following two parts: the general information of the elderly residents and the needs of disease care, basic nursing care, daily life care as well as psychological care. Results The retrieving rate was 95.8%(115/120). The overall nursing care needs of elderly nursing home residents was at the middle level (2.59 ±0.80), the highest demand for nursing care was disease care (3.34±1.07), followed by psychological care (2.56±0.93), basic nursing care (2.20±1.15) and daily life care (2.23±0.97). The single living elderly residents got lower scores than the couple living elderly residents in disease care, basic nursing care 3.19±1.07, 2.06±1.10 vs. 3.78±0.96, 2.62±1.20 (t=2.656, t=2.311, P<0.05). Over 90 years old age groups got lower scores than the other age groups in the disease care, 2.25 ±0.69, 3.80 ±0.72, 3.35 ±1.09, which showed significant difference (F=7.10, P < 0.05). The elderly suffering from hypertension, diabetes, cardiovascular disease varied in the demand for different care. Conclusions The overall nursing care needs of elderly nursing home residents was at the middle level. Staying with spouse, age, disease state are the influencing factors of nursing care needs of elderly nursing home residents.
10.Application of dilution regression method in the coagulation detection of fat blood samples

Objective To explore the dilution regression method of coagulation detection(PT ,APTT) in fat blood samples . Methods We collected 40 normal blood coagulation specimens (no fat blood ,no jaundice ,no hemolysis) in Yan′an hospital of Kun-ming ,then we detected the PT and APTT of the original plasma and 3-fold diluted plasma and 5-fold diluted plasma ,the we used the data both of before dilution and diluted to do the linear regression analysis ,and finally we got the regression equations of each index .we also collected 33 fat blood samples in Yanan hospital of Kunming ,which be divide into three groups through the severity of triglycerides :mild fat blood group(1 .7 mmol/L≤TG<11 .0 mmol/L) and moderate fat blood group(11 .0 mmol/L≤TG<20 .0 mmol/L)and severe fat blood group(TG≥20 .0 mmol/L) ,then we detected the PT and APTT after 3-fold diluted plasma and high-speed centrifugation plasma ,and then we brought diluted results into the normal regression equations and the results were compared with the high-speed centrifugation results .Results Because most of the 5-fold diluted plasma can not get the effective results ,so we use 3-fold diluted plasma to get the regression equations .The results of 3-fold diluted plasma was calculated by the regression equa-tions ,which then compared with high-speed centrifugation results ,after the analysis of statistical software ,two results had not sta-tistically significant (P>0 .05) .Conclusion Dilution regression method can be used to detect the fat blood samples in the clinical coagulation detection .
