1.Effect of Buyang Huanwu Decoction on the Expressions of Hippocampal Neuroplasticity Proteins in Rats with Focal Cerebral Ischemia

Objective To observe the effect of Buyang Huanwu Decoction on hippocampal tissue structure and blood perfusion in rats with focal cerebral ischemia using multi-modal MRI;To analyze the mechanism of Buyang Huanwu Decoction on hippocampal neuroplasticity combined with the expression changes of neuroplasticity proteins and glucose metabolism related transporters.Methods Focal cerebral ischemia rat model induced by right middle cerebral artery occlusion were established.The rats were divided into sham-operation group,sham-operation+Buyang Huanwu Decoction group,model group and model+Buyang Huanwu Decoction group.The rats in the sham-operation group and model group were given normal saline for 30 days,and those in the sham-operation+Buyang Huanwu Decoction group and model+Buyang Huanwu Decoction group were given Buyang Huanwu Decoction for 30 days.T2 mapping imaging was used to detect the changes in hippocampal tissue structure,the changes of cerebral blood perfusion in hippocampus were detected by arterial spin labeling imaging,Western blot was used to detect the protein expressions of SYN,GAP-43,glucose transporters and MCT.Results Compared with the sham-operation group,the T2 relaxation time of the right and left hippocampus in the model group rats significantly increased(P<0.01,P<0.001),the blood flow in the right hippocampus was significantly reduced(P<0.05),the protein expressions of SYN,GAP-43,MCT4 and MCT2 in the right hippocampus were significantly decreased(P<0.01,P<0.001),the protein expressions of GLUT1 and GLUT3 in bilateral hippocappal tissue significantly decreased(P<0.05,P<0.01,P<0.001).Compared with the model group,the T2 relaxation time in the right hippocampus of rats in model+Buyang Huanwu Decoction group was significantly reduced(P<0.05),the blood flow in the right hippocampus significantly increased(P<0.05),the protein expressions of SYN,GAP-43,GLUT1 and GLUT3 in bilateral hippocampal tissues significantly increased(P<0.01,P<0.001),and the protein expressions of MCT4 and MCT2 in right hippocampal tissue significantly increased(P<0.01,P<0.001).Conclusion Buyang Huanwu Decoction can alleviate hippocampal injury in rats with focal cerebral ischemia,which may be related to improving blood perfusion,up-regulating neuroplasticity-related proteins,promoting hippocampal axon regeneration and synaptic remodeling,regulating energy metabolism of nerve cells.

3.Analysis of the current situation and trend of infectious disease public health emergencies in Hainan Province from 2013 to 2022

Abstract:Objective To evaluate the basic development status of public health emergencies of infectious diseases in Hainan Province from 2013 to 2022, analyze the distribution and characteristics of these emergencies, as to provide important scientific basis for the study and formulation of economic health development planning policies, for the health security policies and infectious disease prevention and control strategies in Hainan Province. Methods The relevant statistical data of public health emergencies involving infectious diseases reported in Hainan Province from 2013 to 2022 were collected, and the status quo of these emergencies was analyzed by using descriptive data statistical analysis method. The ARIMA model was used to predict the number of public health emergencies reported by infectious diseases in Hainan Province from 2023 to 2025. Results From 2013 to 2022, a total of 482 public health emergencies were reported in Hainan Province, of which 426 were infectious disease public health emergencies, accounting for 88.38%, with a total of 8 876 cases, an incidence rate of 0.61%, and eight deaths, a case fatality rate of 0.09%. The major public health emergencies of infectious diseases were unclassified events and general events, accounting for 69.25% and 29.58%, respectively. The main diseases were novel coronavirus infection, chickenpox and hand, foot and mouth disease, accounting for 45.07%, 15.49% and 11.97%, respectively; in terms of time distribution, the number of reported incidents showed an upward trend over time, with a decline in 2021, and mainly had three peak periods, April, August and December, with a total of 220 cases reported; in terms of regional distribution, the major cities and counties reported were Haikou, Wanning and Wenchang, accounting for 27.46%, 19.25% and 9.86%, respectively; in terms of location distribution, the events mainly occurred in schools, accounting for 42.02% of the total number of incidents. The ARIMA(1, 1, 0) model was used to predict infectious disease public health emergencies, and the results showed that the forecast state would be stable from 2023 to 2025, with no obvious upward trend, that is, the fluctuation range of public health emergencies related to infectious diseases in Hainan Province would be stable in the next three years. Conclusion The number of infectious disease public health emergencies reported in Hainan Province from 2013 to 2022 has increased year by year, with a decline in 2021. Although the forecast shows that public health emergencies such as infectious diseases are relatively stable with no obvious upward trend, the prevention and control of the epidemic should not be taken lightly. Hainan Province should continue to increase the investment in monitoring public health emergencies, improve the information system of public disease prevention and control, and carry out real-time monitoring of public health emergencies of infectious diseases.
4.A clinical study on the efficacy and safety of camrelizumab combined with stereotactic body radiation therapy in the treatment of advancedoligometastaticnon-small cell lung cancer

Objective:To investigate the efficacy and safety of camrelizumab combined with stereotactic body radiation therapy (SBRT) in the treatment of advanced oligometastaticnon-small cell lung cancer (NSCLC).Methods:Eighty-six patients with advanced oligometastatic NSCLC who met the inclusion and exclusion criteria from March 2020 to August 2021 in the Second People′s Hospital of Yibin were divided into the control group (43 cases) and the treatment group (43 cases) according to the random number table method, the control group was given camrelizumab combined with conventional radiotherapy, and the treatment group was given camrelizumab combined with SBRT. After 8 weeks of treatment, the efficacy of the two groups was evaluated, the occurrence of side effects in the two groups was counted, the serum tumor markers [carcinoembryonic antigen (CEA), squamous cell carcinoma antigen (SCC-Ag), cytokeratin 19 fragment anti-21-1 (CYFRA21-1)] levels were detected.Results:The objective effective rate of the treatment group was significantly higher than that of the control group:: 72.09% (31/43) vs. 51.16%(22/43), the difference was statistically significant ( P<0.05); the incidence of radiation pneumonia in the treatment group was significantly lower than that in the control group: 4.65% (2/43) vs. 18.60% (8/43), the difference was statistically significant ( P<0.05), and there was no significant difference in the incidences of other side effects such as cutaneous capillary endothelial proliferation (CCEP), liver damage, hypothyroidism, and radiation esophagitisbetween the treatment group and the control group ( P>0.05); the levels of serum CEA, SCC-Ag, CYFRA21-1after treatmentin the two groups were significantly lower than those before treatment, treatment group: treatmentgroup: (8.81 ± 4.82) ng/L vs. (81.67 ± 50.88) ng/L, (1.13 ± 0.55) ng/L vs. (1.56 ± 1.03) ng/L and (2.92 ± 0.99) ng/L vs. (4.63 ± 1.39) ng/L, controlgroup: (30.49 ± 19.44) ng/L vs. (89.91 ± 50.10) ng/L, (1.56 ± 1.23) ng/L vs. (1.86 ± 1.33) ng/L and (4.01 ± 2.10) ng/L vs. (5.03 ± 3.44) ng/L. and the levels after treatment in the treatment group were significantly lower than those in the control group, and there were statistical differences ( P<0.05). Conclusions:Camrelizumab combined with SBRT treatment for patients with advanced oligometastatic NSCLC can effectively reduce the levels of serum CEA, SCC-Ag, CYFRA21-1, and significantly improve the short-term efficacy, with relatively low incidence of toxic side effects.
5.Research Progress on Prognostic Survival Assessment of Inflammatory Response Parameters in Ampullary Carcinoma

Ampullary carcinoma is a rare malignant tumor of the digestive tract. Compared with other malignant tumors around the ampulla, its prognosis is significantly better, but the prognosis of some patients with ampulla cancer is unsatisfactory. Studies have confirmed that the systemic inflammatory response is closely related to the prognosis and efficacy of tumors, and the tumor microenvironment plays a vital role in tumor progression and prognosis. In recent years, inflammatory parameters and related prediction models have become a hot spot in predicting the prognosis and survival of ampullary cancer, and they have been confirmed to have certain predictive value. This article reviews the correlation between inflammatory parameters and prognosis of ampullary carcinoma and presents the possible progress for future research.

Background:
Clinical dexamethasone (DEX) treatment or stress in bovines results in extensive physiological changes with prominent hyperglycemia and neutrophils dysfunction.
Objectives:
To elucidate the effects of DEX treatment in vivo on cellular energy status and the underlying mechanism in circulating neutrophils.
Methods:
We selected eight-month-old male bovines and injected DEX for 3 consecutive days (1 time/d). The levels of glucose, total protein (TP), total cholesterol (TC), and the proinflammatory cytokines interleukin (IL)-1β, IL-6 and tumor necrosis factor (TNF)-α in blood were examined, and we then detected glycogen and adenosine triphosphate (ATP) content, phosphofructosekinase-1 (PFK1) and glucose-6-phosphate dehydrogenase (G6PDH) activity, glucose transporter (GLUT)1, GLUT4, sodium/glucose cotransporter (SGLT)1 and citrate synthase (CS) protein expression and autophagy levels in circulating neutrophils.
Results:
DEX injection markedly increased blood glucose, TP and TC levels, the Ca 2+ /P 5+ ratio and the neutrophil/lymphocyte ratio and significantly decreased blood IL-1β, IL-6 and TNF-α levels. Particularly in neutrophils, DEX injection inhibited p65-NFκB activation and elevated glycogen and ATP contents and SGLT1, GLUT1 and GR expression while inhibiting PFK1 activity, enhancing G6PDH activity and CS expression and lowering cell autophagy levels.
Conclusions
DEX induced neutrophils glucose uptake by enhancing SGLT1 and GLUT1 expression and the transformation of energy metabolism from glycolysis to pentose phosphate pathway (PPP)-tricarboxylic acid (TCA) cycle. This finding gives us a new perspective on deeper understanding of clinical anti-inflammatory effects of DEX on bovine.
7.Effect and mechanism of circ-EIF3I on the growth and metastasis of hepatocellular carcinoma

Objective:To investigate the function of circ-EIF3I on the growth and metastasis of hepatocellular carcinoma (HCC) and its possible mechanism.Methods:A total of 39 HCC patients admitted to the Fourth Hospital of Hebei Medical University from May 2014 to October 2015 were selected as the study subjects, including 29 males and 10 females, aged (62.2±5.6) years old. Part of HCC tissues and adjacent tissues were obtained during the surgical operation. Fluorescence quantitative PCR was used to detect the expression of circ-EIF3I, and Western blotting was used to detect phosphoglycerate kinase 1 (PGK1) in HCC and adjacent tissues, respectively. Small molecule RNA interference and gene overexpression experiments were used to adjust its expression in Hep3B and Huh-7 HCC cell lines, and then MTT cell viability test was used to detect cell proliferation ability. Transwell assaya was used to detect cell migration and invasion ability. Finally, the dual luciferase report experiment was used to detect the targeting relationship between circ-EIF3I and miR-149-5p/miR-1271-5p, and the targeting relationship between miR-149-5p/miR-1271-5p and PGK1.Results:Compared with adjacent tissues, the relative expression of circ-EIF3I [(4.32±0.62) vs. (1.24±0.59)] and PGK1 [(2.69±0.19) vs. (1.00±0.07)] in HCC tissues from 39 cases were increased, and the differences were statistically significant (all P<0.05). Compared with the negative control group, the cell viability of the circ-EIF3I small molecule interfering RNA group was reduced [Hep3B: (55.3±7.5)% vs. (100.0±9.2)%; Huh-7: (42.7±6.0)% vs. (100.0±5.6)%] , the number of migrating cells was decreased [Hep3B: (71.0±10.0) vs. (130.0±15.0); Huh-7: (50.0±8.5) vs. (125.0±10.0)], the number of invasive cells also was decreased [Hep3B: (52.0±7.0) vs. (105.0±13.0); Huh-7: (60.0±8.0) vs. (144.0±11.0)], the difference was statistically significant (all P<0.05). The dual luciferase report experiment confirmed that circ-EIF3I could target miR-149-5p/miR-1271-5p, and miR-149-5p/miR-1271-5p can target PGK1. Over expression of PGK1 could significantly reverse the effects of knockdown of circ-EIF3I on the proliferation, migration and invasion of HCC cells. Conclusion:Knockdown of circ-EIF3I could inhibit the proliferation, migration, invasion of HCC cells by regulating the miR-149-5p/miR-1271-5p/PGK1 molecular axis.
8.Effects of different iodine levels on thyroid function in rats after pregnancy

Objective:To investigate the effects of different levels of iodine intake on thyroid function in Wistar rats after pregnancy, and to provide experimental basis for scientific supplementation of iodine and thyroid function screening during pregnancy.Methods:One hundred and fifty female SPF Wistar rats weaned for 2 weeks were selected. Female Wistar rats were intervened with iodine nutrition by drinking deionized water containing potassium iodide (KI). According to the random number table, female Wistar rats were divided into 5 groups [severe iodine deficiency (SID) group, mild iodine deficiency (MID) group, control group (NI), mild iodine excess (MIE) group, and severe iodine excess (SIE) group, 30 rats per group]. The iodine doses of the five groups were 0.0, 1.5, 5.5, 70.0 and 350.0 μg/d, respectively. The animal model was established and intervened for 3 months to detect the 24 h urinary iodine content of rats, and compared with the NI group to determine the success of the model or not. After successful modeling, the tested female Wistar rats were mated with male Wistar rats (female : male=2-3 : 1). There were about 15 pregnant rats in each group, and continued to intervene the pregnant rats for 21 d with the same doses as the modeling conditions. The abdominal aortic blood of non-pregnant and pregnant rats was taken. After serum separation, 5 items [free thyroxine (FT 4), free triiodothyronine (FT 3), thyroid stimulating hormone (TSH), anti-thyroglobulin antibodies (TgAb), thyroid peroxidase antibody (TPOAb)] of serum thyroid function were detected in each group. Results:There was significant difference in urinary iodine content between the five groups (the medians urinary iodine were 3.540, 51.410, 286.801, 644.192 and 2 368.701, respectively, H = 94.791, P < 0.01). Rats with different iodine nutrition levels were successfully established. There were no significant differences in TSH level, TPOAb and double antibodies positive rates of non-pregnant rats among groups ( P > 0.05). There were statistically significant differences in the levels of FT 4, FT 3 and the positive rate of TgAb among groups ( P < 0.05). The level of FT 4 in SID group was lower than that in NI group ( P < 0.05). The level of FT 3 in SID group was higher than that in NI group ( P < 0.05). The positive rate of TgAb in SIE group was higher than that in NI group ( P < 0.05). There were no significant differences in TSH, FT 4 and FT 3 levels of pregnant rats among groups ( P > 0.05). There were significant differences in the positive rates of TgAb, TPOAb and double antibodies among the groups ( P < 0.05). The positive rates of TgAb, TPOAb and double antibodies in MIE and SIE groups were higher than those in NI group ( P < 0.05). The positive rate of TPOAb in MIE group was higher than that in NI group ( P < 0.05), and the positive rates of double antibodies in MID and MIE groups were higher than that in NI group ( P < 0.05). Conclusions:Iodine deficiency can lead to the change of thyroid hormone level in non pregnant rats, while iodine excess can increase the positive rate of related antibodies in non pregnant and pregnant rats.
9.Influencing factors of thyroid volume in school-age children aged 8 - 10 years in Xinjiang Uygur Autonomous Region in 2020

Objective:To investigate the influencing factors of thyroid volume in school-age children aged 8 - 10 years in Xinjiang Uygur Autonomous Region (Xinjiang for short).Methods:In 2020, counties (cities, districts) were taken as the units in the whole region of Xinjiang. Each county (city, district) was divided into 5 sampling areas according to the orientation of east, west, south, north, and middle, one township/street was selected from each area, and one primary school was selected from each township/street, 40 non-boarding children aged 8 - 10 years were selected from each primary school as the investigation subjects. Height and weight of children were measured, and body mass index (BMI) and body surface area were calculated; 24 h mixed urine samples of children and household edible salt samples were collected to detect the contents of urinary iodine and salt iodine; thyroid volume of children was measured by B-ultrasonography. Pearson correlation analysis was used to analyze the correlation between thyroid volume and age, height, weight, body surface area, BMI, urinary iodine content, and salt iodine content. Univariate and multiple linear regression analyses were used to evaluate the correlation variables affecting thyroid volume.Results:A total of 18 334 children aged 8 - 10 years were investigated. The median urinary iodine was 237.88 μg/L. There were 132 children with goiter, and the rate of goiter was 0.72%. Of these, 9 249 (50.45%) were girls and 9 085 (49.55%) were boys. Girls' thyroid volume was positively correlated with age, height, weight, body surface area, BMI, urinary iodine content, and salt iodine content ( r = 0.125, 0.135, 0.167, 0.167, 0.154, 0.031, 0.019, P < 0.05); boys' thyroid volume was positively correlated with age, height, weight, body surface area, and BMI ( r = 0.132, 0.326, 0.156, 0.149, 0.146, P < 0.05), and there was no correlation with urinary iodine content and salt iodine content ( r = 0.019, 0.017, P > 0.05). Univariate linear regression analysis showed that age, height, weight, BMI, body surface area and urinary iodine content were the influencing factors of thyroid volume ( t = 14.92, 12.54, 20.98, 17.98, 20.25, 4.28, P < 0.01). Further multiple linear regression analysis showed that age, BMI, body surface area and urinary iodine content had significant independent effects on thyroid volume ( t = 9.61, 8.57, 7.76, 4.89, P < 0.01), the coefficient of determination ( R2) of the model was 0.278 2. According to the regression coefficient (β), the body surface area (β = 0.522 6) had the greatest influence on thyroid volume. Conclusions:The iodine nutrition of children aged 8 - 10 years in Xinjiang is sufficient. Thyroid volume is affected by age, BMI, body surface area and urinary iodine content.
10.Analysis of iodine nutritional status of pregnant women in the early, middle and late pregnancy in different regions of Xinjiang Uyghur Autoromous Region in 2019

Objective:To understand the iodine nutritional status of pregnant women in the early, middle and late pregnancy in historical iodine deficiency areas (South Xinjiang) and non historical iodine deficiency areas (North Xinjiang) in Xinjiang, so as to provide scientific basis for the prevention and control of iodine deficiency disorders in pregnant women and the formulation of appropriate prevention and control measures.Methods:From March to June 2019, using cluster yandom sampling, each county (city, district, county for short) in the whole region was divided into five sampling areas according to the east, west, south, north and middle direction. Twenty pregnant women were selected from each area, and the iodine content was determined by taking household salt samples and random urine samples.Results:A total of 9 461 salt samples were collected from pregnant women's families in 96 counties, of which 9 099 were qualified iodized salts, 22 were non iodized salts, the rate of non iodized salt was 0.23%, the coverage rate of iodized salts was 99.77% (9 439/9 461), the consumption rate of qualified iodized salts was 96.17% (9 099/9 461), and the median of salt iodine was 27.42 mg/kg. A total of 9 456 urine samples of pregnant women were tested. The median of urinary iodine was 187.30 μg/L, ranging from 0.30 to 1 300.00 μg/L. The median of urinary iodine of pregnant women in 12 counties (North Xinjiang) was < 150 μg/L. The medians of urinary iodine of pregnant women in the early, middle and late pregnancy in historical iodine deficiency areas were 215.85, 208.10 and 196.60 μg/L, respectively, while that in the non historical iodine deficiency areas were 179.10, 180.70 and 179.15 μg/L, respectively, the differences were statistically significant ( P < 0.01). There was a significant difference in the urinary iodine content of pregnant women in the early, middle and late pregnancy in historical iodine deficiency areas ( H = 8.85, P < 0.05), but there was no significant difference in the non historical iodine deficiency areas ( H = 0.28, P > 0.05). Conclusions:Some pregnant women in North Xinjiang are in iodine deficiency. The contents of urinary iodine in the early, middle and late pregnancy in historical iodine deficiency areas are higher than those in non historical iodine deficiency areas. The distribution of urinary iodine in the early, middle and late pregnancy of pregnant women in the historical iodine deficiency areas is different.
