1.Analysis and research of online teaching supervision based on the characteristics of medical disciplines

		                        			Given the systematic, rigorous, and practical characteristics of medical disciplines, ensuring the teaching quality of online courses has become a significant focus. In traditional teaching models, teaching supervision is an important method to guarantee instructional quality, and introducing teaching supervision into online teaching activities is of great significance. This article systematically reviews and summarizes the domestic and international experience of conducting online medical courses. 2.Analysis of the burden of diabetes attributed to metabolic factors from 1990 to 2019

		                        			ObjectiveTo analyze the long-term trends of the disease burden of diabetes attributed to metabolic factors in China from 1990 to 2019, and provide scientific recommendations for diabetes prevention and control in China. MethodsDescriptive analysis was conducted on the disease burden data of diabetes attributed to metabolic factors in China from 1990 to 2019, obtained from GBD 2019, encompassing death form diabetes, disability-adjusted life years (DALY), years of life lost (YLL), and years lived with disability (YLD). Joinpoint regression models were employed to analyze the long-term trends in mortality and DALY rates. Furthermore, the study examined the impact of two metabolic risk factors, high fasting plasma glucose (FPG) levels and high body mass index (BMI) levels, on the disease burden of diabetes. ResultsFrom 1990 to 2019, the overall standardized mortality and DALY rates attributed to metabolic factors for diabetes in the general population in China showed an upward trend, with both average annual percent changes (AAPCs) of 0.1% in the total population. The trend was upward in males with AAPCs of 0.9% and 0.6%, while it was downward in females with AAPCs of -0.4% and -0.3%. As age increased, the disease burden of diabetes attributed to metabolic factors showed an upward trend, with high FPG and high BMI ranking as the top two attributing risk factors. The disease burden of diabetes attributed to metabolic factors was higher in Chinese males than females. ConclusionThe disease burden of diabetes attributed to metabolic factors is increasing among the overall population and particularly among males, while the burden for female is declining. There is a need to increase intervention efforts for males aged 65 and above, provide scientific guidance on residents’ diet and lifestyle habits, and control blood glucose and body weight. 
		                        		
		                        		
		                        		
		                        	
3.Self-rated health status and its influencing factors of medical staff in Lanzhou City

		                        			Background Because of high working intensity, high responsibility, and unexpected situations, health care workers may suffer great work pressure, which may lead to health damage. Objective To explore the self-rated health status and its influencing factors such as demographic and occupational-related factors of medical staff in Lanzhou. Methods In-service medical staff were selected by using cluster random sampling method from 18 public hospitals in Lanzhou City and were investigated with a self-made questionnaire and the Self-rated Health Measurement Scale (SRHMS). SRHMS includes 48 items in 10 dimensions, which are divided into three sub-scales of physical health, mental health, and social health, and another independent dimension is overall health. The scores were converted into a percentage scale and expressed as the percentage of measured score to full score; a higher score indicated better health, and >70% was considered good health status. t test, Kruskal-Wallis H test, and Spearman correlation were used to analyze the scores of SRHMS and the demographic and occupational-related factors affecting the scores of physical, mental, and social health sub-scales. Results A total of 2989 valid questionnaires were recovered. There were statistically significant differences in total score and the scores of physical, mental, and social health among medical staff of different age, educational background, length of service, and weekly working hours groups (P < 0.05). The percentage of total score to full score in the medical staff was 71.41%, but the scores of physical, mental, and social health sub-scales and total scale of selected participants were all lower than the corresponding domestic norms (t=−3.323, −12.283, −7.157, −9.659, P < 0.05); the percentage of psychological symptoms and negative emotions in mental health scale to full score was the lowest, only 58.39%. Educational background, length of service, and weekly working hours were negatively correlated with physical health score (r=−0.061, −0.060, −0.165, P < 0.05); professional title was positively correlated with mental health score (r=0.045, P < 0.05), while educational background and weekly working hours were negatively correlated with it (r=−0.051, −0.172, P < 0.05). Monthly income, professional title, and length of service were positively correlated with social health score (r=0.040, 0.049, 0.071, P < 0.05), while educational background and weekly working hours were negatively correlated with it (r=−0.038, −0.110, P < 0.05). Conclusion The self-rated health status of selected medical staff in Lanzhou is generally good, but lower than that of the norm, especially the mental health score is the lowest. 4.Effect comparison of lateral-rectus approach and ilioinguinal approach in the treatment of toddlers with Torode-Zieg type IV pelvic fracture

		                        			Objective:To compare the clinical efficacy of lateral-rectus approach and ilioinguinal approach in the treatment of Torode-Zieg type IV pelvic fracture in toddlers.Methods:A retrospective cohort study was used to analyze the clinical data of 12 toddlers with Torode-Zieg type IV pelvic fracture admitted to Affiliated Hospital of Youjiang Medical College for Nationalities and Third Affiliated Hospital of Southern Medical University from June 2012 to June 2019. There were 6 males and 6 females, aged 13-36 months [(23.9±7.4)months]. Treatment via ilioinguinal approach was performed for 5 patients (ilioinguinal group), and via lateral-rectus approach for 7 patients (lateral-rectus group). Operation duration, intraoperative blood loss and postoperative complications were compared between the two groups. At the last follow-up, pelvic function was evaluated by Majeed function score, and fracture reduction was evaluated according to Mears-Velyvis radiological evaluation criterion.Results:All patients were followed for 24-72 months [(46.0±18.5)months]. In ilioinguinal group and lateral-rectus group, the operation time was (295.0±95.3)minutes and (165.1±52.2)minutes, respectively ( P<0.05), and the intraoperative blood loss was (190.0±65.2)ml and (225.7±92.0)ml, respectively ( P>0.05). In ilioinguinal group, the development of bilateral iliac wings was asymmetric in 3 patients. In lateral-rectus group, fracture-dislocation of epiphyseal plate together with abnormal development of iliac wing occurred in 1 patient, but had no effects on the function of lower limbs. According to Majeed function score at the last follow-up, the pelvic function in ilioinguinal approach group was rated as excellent in 1 patient, good in 2, fair in 1 and poor in 1, with the excellent and good rate of 60%, while the pelvic function in lateral-rectus group was excellent in 4 patients, good in 1, fair in 2 and poor in none, with the excellent and good rate of 71% ( P>0.05). According to Mears-Velyvis radiological evaluation criterion at the last follow-up, the fracture reduction in ilioinguinal group was rated as satisfied in 3 patients and dissatisfied in 2 patients, with the satisfaction rate of 60%, while the fracture reduction in lateral-rectus group was satisfied in 7 patients and dissatisfied in none, with the satisfaction rate of 100% ( P>0.05). Conclusions:For Torode-Zieg type IV pelvic fracture in toddles, both surgical approaches can complete the reduction and fixation. 5.Closed reduction and minimally invasive fixation for the treatment of pelvic fractures of type C2 and C3

		                        			Objective:To explore the surgical indications, techniques and methods of closed reduction and minimally invasive fixation for the treatment of pelvic fractures of Tile C2 and C3, and evaluate the clinical efficacy.Methods:A retrospective analysis of the data of 20 cases with Tile C2 and C3 pelvic fractures treated with closed reduction and minimally invasive fixation from January 2016 to July 2019. There were 7 males and 13 female, with an average age of 35.6±14.6 years (range 12-60 years). The time from injury to operation was 5-30 d, with an average of 19.3±7.1 d. Tile classification of pelvic fracture: 13 cases of C2 type and 7 cases of C3 Type. 2 cases were complicated with ipsilateral or bilateral lumbosacral nerve injury. Classification of nerve injury: 2 cases were partial injury, British Medical Research Council (BMRC) Grade M3. The operation is treated with closed reduction and minimally invasive fixation. First, the side with obvious displacement is fixed on the operating table with a pelvic reduction frame, and the side with less displacement is traction. After reduction, insert S 1 and S 2 sacroiliac screw guide-pin on this side to the contralateral sacral fracture. And then change the traction, fix the reset side on the operating table, change the side with obvious traction displacement, after the reset is ideal, pass the inserted guide-pin through the contralateral sacroiliac joint to the outer iliactable. Then insert the sacroiliac screw. The patients complicated with acetabular fracture were reduced and fixed by the corresponding approach, and the anterior ring was fixed by INFIX. The operation time, intraoperative bleeding volume and postoperative complications were recorded. The quality of fracture reduction was evaluated by Matta's criteria, and the clinical effect was evaluated by Majeed score. Results:All the 20 patients successfully completed the operation. The operation time was 105-210 min, with an average of 167.00±31.21 min. The intraoperative bleeding volume was 30-100 ml, with an average of 82.00±5.36 ml. Postoperative X-ray and CT showed that the fracture was reduced and fixed. According to the Matta's criteria, the reduction quality was rated as excellent in 14 cases, good in 4 case, fair in 2 case, with an excellent and good rate of 90%. Two patients showed symptoms of lateral femoral cutaneous nerve injury without other complications related to surgery. Follow-up for 1 to 4 years, the fractures healed, and the healing time was 6 to 12 weeks. According to the Majeed score, the result was rated as excellent in 18 cases, good in 2 case, with an excellent and good rate of 100%.Conclusion:Closed reduction and minimally invasive fixation for the treatment of pelvic fractures of type C2 and C3, with the characteristics of less damage and good results, will become a trend in the treatment of pelvic fractures.
		                        		
		                        		
		                        		
		                        	
6.To evaluate clinical efficacy and surgical methods of sacroiliac joint dislocation with ipsilateral sacrosacral wing fracture

		                        			Objective:To investigate the surgical methods of sacroiliac joint dislocation with ipsilateral sacrosacral wing fracture through the lateral-rectus approach (LRA) for exposure, fracture reduction and fixation, and nerve exploration and decompression, and to evaluate its clinical efficacy.Methods:Data of 12 patients with sacroiliac joint dislocation and ipsilateral sacral wing fracture treated with LRA for exposure, reduction and fixation of sacroiliac wing fracture and sacroiliac joint dislocation, lumbosacral trunk nerve exploration and decompression (combined with lumbosacral trunk nerve injury) were retrospectively analyzed from January 2016 to July 2019. They were 3 males and 9 females, aged 34.35±16.32 years (13-58 years). The time from injury to operation was 29.25±25.49 d (7-96 d). By the Tile classification, there were 7 cases of type C1.3, 1 case of type C2, 4 cases of type C3. Among them, 8 cases were combined with ipsilateral or bilateral lumbosacral nerve injury. The grade of nerve injury: 6 cases of complete injury and 2 cases of partial injury. Interval time from injury to surgery: less than 1 week: 1 case, 1-2 weeks: 2 cases, 2-3 weeks: 4 cases, >3 weeks: 5 cases. Surgery was performed through LRA, the sacroiliac joint was exposed outside the peritoneum, and the sacral fracture and sacroiliac joint dislocation were reduced. At the same time, the lumbosacral nerve was decompressed and loosened for patients with lumbosacral nerve injury. Then the posterior ring was fixed with a sacroiliac screw or a transsacroiliac joint plate attached to the bony surface.Results:All the 12 cases underwent the operation successfully. The average surgical time was 172.08±36.8 min (range, 105-230 min) and the mean blood loss was 981.67±369.44 ml (range, 400-1 700 ml). Postoperative X-ray and CT indicated an excellent reduction of fracture. One patient with bladder dysplasia had wound fat liquefaction after operation, and no other surgery-related complications. During the follow-up period of 12-72 months, all sacral fractures healed, and the healing time was 7.7±3.38 weeks (6-12 weeks) without complications such as loss of fracture reduction and internal fixation failure. At the 1-year follow-up, 6 of the 8 patients with lumbosacral nerve injury recovered completely, one recovered partially, and the other one had no recovery without nerve exploration.Conclusion:LRA is an ideal surgical approach for treatment of sacroiliac joint dislocation complicated with ipsilateral sacral wing fracture and lumbosacral nerve injury, because it can well expose the medial pelvic joint from the sacroiliac joint to the symphysis pubis, allow direct release of the lumbosacral plexus nerve compressed and stretched, and together with traction of the lower limbs, lead to satisfactory fracture reduction.
		                        		
		                        		
		                        		
		                        	
7.Clinical epidemiological analysis of 2 403 cases of lung cancer

		                        			Objective To understand preliminaryly the epidemiological trend of lung cancer in recent years by retrospective analysis of 2 403 cases of lung cancer in Zhongnan Hospital of Wuhan University from 2013 to 2017.Methods The clinical data of patients with primary bronchogenic cancer diagnosed in Zhongnan Hospital of Wuhan University from 2013 to 2017 were collected.The clinical data such as gender,age,history of tobacco and alcohol,operation,pathological type,clinical stage and epidermal growth factor receptor (EGFR) gene mutation were analyzed statistically.Results A total of 2 403 cases of lung cancer were collected,including 1 766 males and 637 females.There was no significant difference in gender ratio between male and female in five years (x2 =8.481,P =0.075).There were 2 398 cases with age information,the male-to-female ratios of lung cancer patients aged less than 40,40-49,50-59,60-69,70-79,80 and over were 0.9 ∶ 1.0,1.4 ∶ 1.0,2.4 ∶ 1.0,3.6 ∶ 1.0,3.4 ∶ 1.0 and 3.3 ∶ 1.0 respectively,and the difference was statistically significant (x2 =59.004,P < 0.001).The composition difference of adenocarcinoma was not statistically significant in five years (x2 =2.165,P =0.705).There was no statistically significant difference in the composition ratio of squamous cell carcinoma (x2 =4.788,P =0.310).Adenocarcinoma accounted for 43.15% (762/1 766) and 81.95% (522/637) of the pathological types of male and female patients respectively,and the difference was statistically significant (P < 0.001).Squamous cell carcinoma accounted for 39.01% (689/1 766) and 7.28% (47/637) respectively,and the difference was statistically significant (P < 0.001).The proportion of squamous cell carcinoma in smoking patients was 42.99% (583/1 356),which was significantly higher than that in non-smoking patients (14.61%,153/1 047);the proportion of squamous cell carcinoma in drinking patients was 40.56% (277/683),which was higher than that in nondrinking patients (26.69%,459/1 720),and the differences were statistically significant (both P < 0.001).A total of 1 252 patients underwent surgery,accounting for 52.10% (1 252/2 403) of the total cases.The surgical rate of small cell carcinoma was 21.72% (48/221),and that of non-small cell carcinoma was 55.18% (1 204/2 182).In five years,the surgical rates of lung cancer patients were 55.11% (221/401),51.53% (252/489),58.23% (244/419),53.18% (276/519) and 45.04% (259/575) respectively,and there was significant difference in the proportion of surgical and non-surgical patients in each year (x2 =19.553,P =0.001).A total of 483 patients were tested for E GFR mutation,the EGFR mutation rate was 58.8% (251/ 427) in adenocarcinoma patients and 15.6% (5/32) in squamous cell carcinoma patients.Among lung cancer patients aged less than 40,40-49,50-59,60-69,70-79,80 and over,the proportions of adenocarcinoma were 76.74% (33/43),62.39% (136/218),57.73% (381/660),47.95% (455/949),52.22% (235/450) and 52.56% (41/78) respectively,and the difference was statistically significant (x2 =33.078,P < 0.001);the proportions of squamous cell carcinoma were 9.30% (4/43),21.56% (47/218),28.03 % (185/660),34.14% (324/949),32.44% (146/450) and 35.90% (28/78) respectively,and the difference was statistically significant (x2 =26.977,P < 0.001).The difference of composition ratio of TNM staging was statistically significant in five years (x2 =21.003,P =0.034).Conclusion There has been no significant change of male-to-female ratio in patients with lung cancer in the past five years.With the increase of age,the male-to-female ratio increases first and then decreases.The proportion of adenocarcinoma and squamous cell carcinoma has not increased or decreased significantly in the past five years.Adenocarcinoma and squamous cell carcinoma are both common in male lung cancer patients,while the pathological type of female patients is mainly adenocarcinoma.Squamous cell carcinoma is highly prevalent in smokers and drinkers.The surgical rate of squamous cell carcinoma is higher than that of adenocarcinoma,and the surgical rate of non-small cell lung cancer is higher than that of small cell lung cancer.The EGFR mutation rate is higher in adenocarcinoma.With the increase of age,the proportion of adenocarcinoma in all pathological types tends to decrease,while that of squamous cell carcinoma tends to increase.The patients' TNM staging has a downward trend,and the operation rate decreases slightly.
		                        		
		                        		
		                        		
		                        	
8.Study on Micronization Technology of Alitretinoin Crude Drug

		                        			OBJECTIVE:To screen the optimal micronization technology of Alitretinoin crude drug. METHODS:Using characteristic value of particle size distribution [d(0.9)] and the content of impurity A of crude drug after crushed as indexes, crushing method(universal pulverizer,ball crusher,airslide disintegrating mill),crushing gas source(compressed air,high pressure nitrogen)and crushing pressure(0.2,0.4,0.6 MPa)of Alitretinoin crude drug were screened,and validation test was also conducted. RESULTS:The optimal technology was as follows as airslide disintegrating mill,high pressure nitrogen as gas source,at 0.4 MPa.In validation test,particle sizes for 3 batches of crude drug after crushed were 8.57,8.55,8.54 μm(<10 μm, RSD=0.15%,n=3). The content of impurity A was not increased compared with before crushed(0.07%). 9.The diagnosis value of DWI in duodenal papillary adenocarcinoma

		                        			Objective To investigate the MRI findings of duodenal papillary adenocarcinoma(DPA),and to evaluate the importance of diffusion-weighted imaging(DWI)in diagnosis of DPA.Methods A complete data of 52 patients with DPA were prospectively collected.All patients underwent surgery within 72 hours after conventional MRI,DWI and MRCP scans.Before surgery,four different MRI findings were used to calculate the sensitivity,specificity and the probability of correctness.Two experienced radiologists who were blind to the pathologic diagnosis handled the MRI findings.Thirty eight patients were pathologically diagnosed for DPA.Based on the pathological diagnosis,the detection rate of DPA by the MR sequence was recorded and the chi square test was used to do the statistical analysis. Results The accuracy rate in diagnosis of DPA with MRI was 78.8% in our study.The findings of DPA consist of thickening wall of duodenal,duodenal papilla node imaging,DWI showing high signal of duodenal papilla and"beak"sign of dilated bile duct.Corresponding sensitivities were 70.5%,66.7%,86.3% and 87.9%,and specificities were 75.0%,30.0%,50.0% and 63.1% respectively.The incidences of positive on T2WI and T1WI,MRCP,DWI scans were 60.5%,76.3% and 92.1% respectively.The detection rate of each sequence has significant difference(χ2=10.48,P<0.005).Conclusion The MRI manifestations of DPA consist of thickening wall of duodenal,duodenal papilla node imaging,DWI showing high signal of duodenal papilla and"beak"sign of dilated bile duct.The detection rate of DWI sepuence on DPA lesions is significantly higher than that of other sequences.
		                        		
		                        		
		                        		
		                        	
10.BRAF/ KRAS and PIK3CA mutation characteristics and clinical significance in primary small cell lung caner

		                        			Objective To detect the frequency of BRAF/ KRAS and PIK3CA mutations in the small cell lung cancer (SCLC) specimens from a large population of Chinese patients and to analyze the gene mutation and clinical characteristics. Methods A total of 557 samples were collected from SCLC patients from 2009 to 2014.BRAF,KRAS,PIK3CA,NRAS and MEK1 gene mutations were detected by the dideoxy sequencing. Chi-square test was adopted to analyze the correlation between clinical factors and gene mutation. Kaplan-Meier method was utilized for survival analysis. Cox model was used for multivariate prognostic analysis. Results BRAF mutations were detected in 13 out of 557 specimens. The mutation types included V600E (n= 5) ,V600A (n= 2) ,V600M (n= 1) ,D594G (n= 1),G464E (n= 1),K601R (n= 2) and S605N (n= 1).KRAS mutation was detected in 6 cases including G12C (n= 3),G12A (n= 1),G12D (n=1) andG13D (n= 1).PIK3CA mutation was observed in 4 samples including E545G (n= 2) and H1047R (n= 2).Besides,NRAS mutation (Q61R) was detected in 1 case and MEK1 mutation (D61Y) was noted in 1 case. These gene mutations were not significantly correlated with the age, gender, smoking status and clinical staging of the patients. Univariate survival analysis demonstrated the median survival time of patients with gene mutation was (10.30±0. 751) months (95%CI:8. 829-11. 771 months),significantly shorter than (12.80±0. 543) months (95%CI:11. 736-13. 864 months) of their counterparts without gene mutation (P=0. 011). Conclusions BRAF/ KRAS and PIK3CA gene mutation is detected in a small proportion of SCLC patients. These gene mutations are not significantly correlated with the clinical characteristics.
