Space Medicine & Medical Engineering 2024;35(2):84-90
Objective To explore the influence of continuous thermal exposure on human cognitive performance. It provides solutions for the reduction of cognitive performance caused by high temperature emergency, and reduces the misoperation problems or even safety accidents of astronauts in high temperature emergency. Methods 14 right-handed male subjects were recruited and continuously exposed to four thermal environments of 32℃,35℃,38℃,and 41℃for 150 minutes, during which 5 different cognitive tasks(Posner Cueing Task,Deary-liewald Task,N-back Task,Wisconsin Card Task,Visual Search Task)were continuously performed to obtain relative cognitive performance. At the same time,a portable 2-lead holter was used to continuously capture ECG R signals during the cognitive test.Thereafter, the heart rate variability(HRV)analysis was conducted based on the ECG data,to explore the correlations between nonlinear indices(SD1,Shannon entropy,α2,D2)and the cognitive performance. Results Tresults showed that all nonlinear indexes changed with the exposure time and exposure temperature with certain patterns. In addition, through the correlation analysis between relative cognitive performance and nonlinear indicators, the most relevant indicator is α2(correlation coefficient 0.7).The correlation analysis results showed that the nonlinear index α2 had a U-shaped relationship with relative cognitive performance(correlation coefficient was 0.7).Based on that,a quantitative relationship between α2 and relative cognitive performance was obtained. It is found that when α2 was equal to 0.98,the worst relative cognitive performance was observed. Conclusion This study showed that the HRV nonlinear index α2 could serve as a reliable indicator of the cognitive performance of human during hot exposure, which provided a potential solution for the productivity and health evaluation of astronauts.

Chinese Journal of General Practitioners 2021;20(10):1066-1071
Objective:To analyze the etiology of biliary fungal infection and risk factors of case fatality. Methods:Clinical and laboratory data of 91 biliary fungal infection patients admitted in Li Huili Hospital of Ningbo Medical Center from January 2013 to June 2019 were retrospectively reviewed, including 14 patients (16.4%) with fungal infection and 77 patients (84.6%) with fungal and bacterial mixed infection. There were 79 survivors and 12 deaths, the risk factors of fatality were analyzed by binary Logistic regression analysis. Results:The fungal strain Candida albicans was detected in 61 cases, Candida glabrata in 19 cases and Candida tropicalis in 6 cases. Drug sensitivity test showed that the fungal strains were highly sensitivity to amphotericin B and 5-fluorouracil [100.0%(91/91),97.8%(89/91)]. In 77 mixed infection cases Gram-negative bacteria was the more common (34 cases,44.2%). The average age of patients was 70.7 years old. Benign diseases were found in 66 cases (72.5%) and 61(67.0%)of them were cholelithiasis. Patients with a history of repeated biliary operation were more likely to have mixed infection of biliary fungi and bacteria (χ 2= 4.56, P=0.03). The mean albumin level in the fatal group was significantly lower than that in the survival group [(28.1±5.2)g/L vs. (33.3±5.3)g/L; t=2.77, P=0.01]. The median length of hospital stay in the survival group was significantly shorter than that in the fatal group [12.0(9.0, 18.0)d vs. 29.5 (13.0, 42.7)d; Z=-2.37, P=0.02]. Multiple logistic regression analysis showed that the history of repeated biliary operation ( OR=4.46, 95% CI: 1.06—4.97) and mixed infection of fungi with bacteria ( OR=10.20, 95% CI: 1.48—70.27) were the risk factors of case fatality. Conclusion:Candida albicans is the main fungus in biliary fugal infection which is often complicated with bacterial infection. Repeated biliary operations and mixed infection of fungi with bacteria are the risk factors of death in patients with biliary infection.

Objective To analyze the secular trends of pancreatic cancer incidence and mortality in Changning district of Shanghai from 1974 to 2013. Methods We calculated the age-standardized rates of incidence and mortality and the average annual percent changes for pancreatic cancer using Segi's world standard population and the data from Shanghai Cancer Registry. Age-period-cohort model was constructed to further assess the effect of age, diagnosis period and birth cohort on the secular trends of pancreatic cancer incidence and mortality. Results During 1974-2013, the age-standardized incidence and mortality rates were 6.49/105 and 6.01/105 in male, 4.83/105 and 4.57/105 in female, respectively. The age-standardized incidence was increased by 0.8% per year in male during past 40 years, while there was no change in mortality. The age-standardized incidence and mortality rates were increased by 1.6% and 1.3% per year in female. After adjusting the effects of diagnosis period and birth cohort, the incidence and mortality rates of pancreatic cancer increased by about 11% every 5 years older in both male and female. Diagnosis period and birth cohort had no statistical impact on the incidence and mortality of pancreatic cancer. Conclusion The age-standardized incidence of pancreatic cancer shows significantly rising trends during 1974-2013 in both male and female in Changning district of Shanghai, as well as the age-standardized mortality in female. The incidence and mortality rates also increase with age.

Objective:To analyze the correlation between the changes of lung function and serum proinflammatory cytokines in workers occupationally exposed to toluene diisocyanate (TDI), and to explore the evaluation index of respiratory toxicity of TDI.Methods:In October 2014, 61 male workers engaged in TDI synthesis process, purification process, packaging process and the above production process in a TDI factory in western China were selected as TDI exposure group; 62 male enterprise managers who were not exposed to TDI and other known allergenic chemicals were selected as control group, which were matched at the age of workers in exposure group. The questionnaire survey obtained information such as gender, length of service, age, occupational history, exposed length of service and so on. The lung function indexes [forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), FEV1/FVC] and serum levels of interleukin (IL)-1 β, IL-6, IL-8, tumor necrosis factor (TNF)-α, macrophage inflammatory factor-1 β, monocyte chemoattractant factor-1 and vascular endothelial growth factor were measured. The urine was collected after the weekend shift, and the concentration of (TDA), the metabolite of TDI, was determined as the index of internal exposure. Spearman rank correlation was used to analyze the correlation between cytokines and lung function indexes, and multivariate linear regression was used to analyze the changes of lung function indexes and cytokines with TDI exposure concentration and time. Results:The median age ( P5- P95) of the exposed group and the control group was 36.5 (24.0-51.0) and 38.0 (24.0-50.0) years, respectively. In the exposed group, the median length of service ( P5- P95) was 6.94 (0.97-26.33) years, and the median concentration of TDA in urine was 15.56 (2.28-112.16) ng/ml. The three indexes of lung function, FVC, FEV1, FEV1/FVC and the levels of serum IL-8 and TNF-α were significantly lower than those in the control group ( P<0.01). With the increase of exposure concentration and exposure time, the level of serum TNF-α, FVC and FEV1 decreased, and showed a good dose-effect and time-effect relationship (all Ptrend values< 0.05). Serum IL-8 and TNF-α were positively correlated with FVC, FEV1 and FEV1/FVC (all P values<0.01). Conclusion:The levels of serum inflammatory factors IL-8 and TNF-α in worker exposed to TDI are related to lung function indexes, which can be used as early evaluation indexes of respiratory toxicity induced by TDI.
