Objective:To summarize the surgical treatment of malignant carotid body tumor (MCBT).Methods:A retrospective analysis of 14 MCBT patients admitted at our hospital from Mar 2005 to Nov 2019 was made, and the imaging data, surgical records, perioperative complications and follow-up data were collected. Results:There were 8 males and 6 females, with an average age of (40.8±11.3) years. 10 patients underwent surgical resection of CBT, with one case undergoing tumor enucleation only, nine cases underwent internal carotid artery reconstruction, and all patients underwent intraoperative lymph node biopsy.Tumors were completely removed in all 10 patients. No perioperative death or cerebral infarction occurred. The intraoperative blood loss was (955±658.5) ml. Four patients had permanent nerve injury after surgery. The follow-up time ranged 1-132 months. There were no cases of cerebral infarction or death, and the reconstructed graft remained patent. Apart from the 2 patients who developed tumor metastasis after surgery, other patients recovered uneventfully with no disease progression. Conclusions:Surgery is still the main treatment for MCBT, but MCBT is large and Shamblin class is more advanced. Therefore, complete tumor removal and reconstruction of the carotid arteries are difficult. Surgery should seek to completely remove the tumor and neck lymph node biopsy should be performed to determine the lymph node metastasis.

Objective:To analyze effects of shared outpatient management model on healthy dietary behavior and stability of blood glucose control in patients with diabetes mellitus. Methods:Using the convenient sampling method, 98 patients with type 2 diabetes mellitus who were followed up in Endocrinology Clinic of Yangzhou First People's Hospital from July 2018 to April 2019 were selected. According to the ending numbers of patients' outpatient medical records, patients with odd numbers ( n=44) were included in the routine outpatient management group, while patients with even numbers ( n=54) were included in the shared outpatient management group. The routine outpatient management group was given routine outpatient follow-up schemes. On the basis of routine outpatient management, the shared outpatient management group was given shared outpatient management model, that was, a single outpatient service activity equipped with professional guidance from diabetes specialists, diabetes education nurses, nutritionists, rehabilitation therapists, psychological counselors and pharmacists in various aspects. After 6 months of intervention, the Healthy Dietary Behavior and Attitude and Social Impact Scale were used to evaluate the healthy dietary behavior of patients and the dynamic blood glucose monitoring system was used to evaluate the stability of patients' blood glucose control. Results:After 6 months of intervention, the scores of attitude scale (24.57±4.64) and Social Impact Scale (27.74±5.47) in the shared outpatient management group were higher than those in the routine outpatient management group, and the differences were statistically significant ( t=-8.632, -16.192; P< 0.01) . Mean amplitude of glycemic excursions (MAGE) , largest amplitude of glycemic excursions (LAGE) , coefficient of variation (CV) of blood glucose and coefficient of variation of fasting blood glucose (CV-FPG) of patients in the shared outpatient management group were respectively (2.23±0.46) mmol/L, (3.24±0.51) mmol/L, (17.32±5.87) and (9.81±4.17) , all lower than those of the routine outpatient management group, and the differences were statistically significant ( t=16.344, 15.056, 3.292, 2.834; P<0.01) . Conclusions:Compared with routine outpatient management, shared outpatient management can standardize the healthy dietary behavior of patients with diabetes mellitus and improve the stability of blood glucose control.

Objective To observe the influence of low or middle power microwave radiation on tissues adja-cent to titanium alloy implants. Methods Titanium alloy internal fixation plates were implanted into the upper femur of the right side of New Zealand white rabbits, the left femurs served as the control. Then the femurs were exposed to 20, 40, 60, or 80 W of microwave radiation at a frequency of 2450 MHz for 30 minutes, and real-time temperature of the implants and its adjacent muscles were recorded. The nerve conduction study of the sciatic nerve was performed by stimulating the nerve and recording from gastrocnemius in both groups. Finally, all the rabbits were sacrificed and histo-logical studies were performed on nerve and muscle sections. Results Compared to the control limbs, temperatures of limbs with titanium alloy implants increased significantly when exposed to microwave radiation at 60 and 80 W ( P<0.05) but no significant differences were revealed when exposed to radiation at 20 and 40 W ( P<0.05) . For the sciatic nerve conduction, significantly prolonged distal latency of CMAP ( P<0.05) and slower nerve conduction velocity ( P<0.05) were observed in the experimental limbs when exposed to microwave radiation at 60 and 80 W, but the decrease of CMAP amplitude was not significant (P>0.05). Pathological examination showed obvious congestion and bleeding in the intercellular spaces of skeletal muscle cells in the experiment group when exposed to radiation at 60 W, with no ob-vious pathological changes found in the sciatic nerve. However, when exposed to radiation at 80 W, vacuoles appeared in myocytes, the cellular structure was destroyed and the myelin sheath of the sciatic nerve swelled. Conclusion Ex-posure to microwave irradiation at 20 and 40 W, but not 60 and 80 W exerts no adverse effects on nerves and muscles adjacent to titanium alloy internal fixation plates. Consequently, continuous wave microwave irradiation at low powers (20~40 W) is a safer auxiliary treatment for limbs with titanium alloy implants.
