Objective :
Mendelian randomization analysis was used to explore the causal relationship of T.gondii in- fection and the cyst distribution and inflammation in brain tissue by immunohistochemistry.
Methods :
Genome- wide association analysis data of T.gondii infection and encephalitis were obtained ,single nucleotide polymor- phisms (SNPs) were selected,Mendelian randomization analysis was conducted by inverse variance weighting, and the causal relationship between T.gondii infection and encephalitis was evaluated by OR value and 95% CI.Quality control was carried out by using heterogeneity test, horizontal multi-efficiency test and leave-one-out sensitivity test. Immunohistochemical staining was performed using brain sections of mice infected with tissue cysts of Wh6 strain for image analysis using Image J software.
Results :
A total of 29 SNPs were associated with toxoplasmic encephalitis. The results of IVW method suggested that T.gondii infection made encephalitis risk 0. 98 times higher ( OR = 0. 98, 95% CI = 0. 76 to 1. 27) ,indicating no causal relationship between the two.The quality control results suggested that the selected SNPs were stable and reliable.Toxoplasma cysts were distributed in various parts of the brain tis- sue.
Conclusion
T.gondii infection and encephalitis are related, but there is no sufficient evidence to prove the causal relationship between the two.

ObjectiveTo analyze the efficacy and safety of Buzhong Yiqi Decoction (补中益气汤, BYD) in the treatment of mechanically ventilated patients of intensive care unit acquired weakness (ICU-AW) with spleen qi deficiency syndrome. MethodsSeventy-two patients eligible for mechanical ventilation ICU-AW and spleen qi deficiency were selected and randomly divided into routine ICU basic treatment and nursing combined with nasal feeding or oral administration of normal saline 3 times a day, 50 ml per time as control group, and routine ICU basic treatment and nursing combined with BYD 3 times a day, 50 ml per time as treatment group, with 36 cases in each group. The treatment cycle was 7 days. Traditional Chinese medicine (TCM) syndrome score, Medical Research Council (MRC) score, diaphragm mobility, diaphragm thickness at the end of inspiration and expiration, diaphragm thickness rate, time to successfully remove the tracheal tube, superoxide dismutase (SOD) and malondialdehyde (MDA) levels of patients were measured before and after treatment, and the efficacy of TCM syndrome was evaluated. ResultsThe total effective rate regarding TCM syndrome was 88.89% in the treatment group, which was higher than 66.67% in the control group (P = 0.033). After treatment, the main symptom score, secondary symptom score and total score of TCM syndrome, diaphragm mobility and MDA level significantly decreased in both groups, while MRC score, diaphragm thickness rate and SOD level increased (P<0.01), with more improvement in the treatment group than in the control group (P<0.05 or P<0.01). The number of successful tracheal intubation removal within 7 days after treatment in the treatment group (28/36 cases, 77.78%) was higher than that in the control group (19/36 cases, 52.78%). ConclusionBasic treatment combined with oral administration of BYD can improve the MRC score, general condition, diaphragm contraction, and antioxidant capacity of patients with mechanical ventilation in ICU-AW with spleen qi deficiency, thereby improving the success rate of extubation with sound safety.

Objective To study the mechanical response of tibial prostheses and the distal tibial cancellous bone after implantation of radial and axial functionally graded materials(FGM)into the ankle joint. Methods Three FGM were used:titanium alloy-bioactive glass composite FGM(FGM-Ⅰ),titanium(Ti)alloy-ideal bone elastic composite FGM(FGM-Ⅱ),and Ti alloy-hydroxylapatite composite FGM(FGM-Ⅲ).A three-dimensional finite element model of total ankle arthroplasty(TAA)was established, and the simulation software ABAQUS was used for secondary development based on Fortra.By changing the volume fraction, the mechanical properties of the FGM tibial prosthesis can be adjusted both axially and radially. The stress distributions of the tibial prosthesis and cancellous bone after FGM axial and radial tibial component implantation in the standing position were analyzed. Results Compared with Ti alloy tibial prosthesis, three kinds of FGM could effectively reduce the stress concentration on the tibial prosthesis, and the overall effect of FGM-Ⅲ tibial prosthesis was better than that of FGM-Ⅰ and FGM-Ⅱ tibial prosthesis;the radial distribution of FGM could effectively reduce the maximum von Mises stress of the prosthesis. For the tibial cancellous bone, the three types of FGM radial tibial prostheses and FGM-Ⅲ axial tibial prostheses could effectively increase the distal stress, thus, relieving the stress shielding on the cancellous bone;the FGM-Ⅲ radial tibial prosthesis was the most effective in improving the stress level of the cancellous bone. Conclusions FGM-Ⅲ radial ankle prosthesis can effectively reduce the stress concentration phenomenon and the stress shielding effect on the prosthesis to prolong its life, with potential application prospects.

Objective:The aim of this retrospective study is to evaluate the safety and efficacy of tislelizumab in patients with recurrent/metastatic head and neck squamous cell carcinoma. Methods:Six patients with recurrent/metastatic head and neck squamous cell carcinoma who received tislelizumab monotherapy in our hospital from 2018 to 2020 were retrospectively analyzed. The information of sex, age, TNM stage, efficacy, and adverse reactions were collected. All patients were recruited from the RATIONALE 102 study. The primary end point was the objective response rate, and other end points included progression-free survival and overall survival. We performed tumor immune-related gene sequencing and transcriptome sequencing analysis on the tumor tissues of the patient, and used bioinformatics methods to enrich immune cells and analyze signaling pathways. All analyses were performed using R 4.1. 0 software, SPSS Statistics 24.0 software and GraphPad Prism 8. Results:As of May 31, 2020, the median follow-up time was 26.35 months. The objective response rate with tislelizumab was 50.0%, the median progression-free survival was 6.44 months, and the estimated median survival was 20.07 months. The incidence of grade 3 or higher adverse reactions was 66.7%, including hyponatremia, hypokalemia, hypercalcemia, etc. The expression of macrophage, Treg and neutrophil-related genes are higher in immune-sensitive patients, and the signaling pathways of the intestinal immune network for IgA production, graft versus host disease and autoimmune thyroid disease are significantly activated. Conclusion:Tislelizumab was found to be controllable and tolerable in patients with recurrent/metastatic head and neck squamous cell carcinoma. The response to tislelizumab is related to immune cell infiltration and activation of immune-related signaling pathways.

【Objective】 To evaluate the safety and efficacy of the collection of peripheral blood stem cells (PBSCs) in pediatric patients with thalassemia major (TM) weighing 20 kg or less. 【Methods】 PBSCs collection data of 170 pediatric patients with TM weighing 20 kg or less from January 2013 to December 2020 in our center were reviewed. Safety was assessed by the occurrence of adverse events during apheresis procedures, and efficacy was evaluated by the number of CD34+ cells collected. 【Results】 A total of 171 PBSCs procedures were performed on 170 patients with TM weighing 20 kg or less, with a median age of (4.98±1.53) years and a median weight of (17.30±2.18) kg. The probability of collecting at least 1×106 CD34+ cells/kg during a single course of apheresis was 99.41% (169/170), with a median (5.88±4.23) ×106 CD34+ cells collected per kg of weight of the recipient. A minimum pre-apheresis hemoglobin (Hb) of 60 g/L in patients with TM weighing 20 kg or less was safe and feasible. The most common adverse event of G-CSF mobilization in TM patients is bone pain, with the incidence of 7.65% (13/170), which was higher than that of healthy children donors in our center. The most common adverse events during the collection were pain at the puncture site of the femoral vein (6.47%, 11/170) and low pressure of the fluid (2.92%, 5/170). And no serious complications related to PBSCs mobilization, central venous catheter(CVC)placement or the apheresis procedure occurred. 【Conclusion】 PBSCs collection by COM.TEC blood cell separator in children weighing 20 kg or less is safe and efficacious.
