1.Epidemiological trends and disease burden of lower gastrointestinal bleeding in children within 10 years

Objective To retrospectively analyze the epidemiological trend of children with lower gastrointestinal bleeding in recent 10 years,and investigate the change of their disease burden,so as to provide a theoretical basis for the accurate prevention and control of children's lower gastrointestinal bleeding. Methods A total of 671 children with "lower gastrointestinal bleeding" who were diagnosed in our hospital from 2012 to 2021 were collected as research subjects. To analyze the microscopic examination rate and common etiology of lower gastrointestinal bleeding in children in the past 10 years,as well as the epidemiological characteristics of different age groups, different regions and different basic diseases; Calculate and compare the rate of disability life lost (YLD), early death life lost (YLL) and disability adjusted life year (DALY) of children with lower gastrointestinal bleeding within 10 years, and calculate the annual change percentage (AAPC) to analyze the change trend of disease burden. Results The microscopic examination rate of children with lower gastrointestinal bleeding showed a trend of increasing in the past 10 years (P<0.001). Among them, the most common causes are Crohn's disease, ulcerative colitis and chronic colitis. The proportion of children with lower gastrointestinal bleeding was higher in boys, >18 years old, hypertension and gastroenteritis. The DALY rate, YLL rate and YLD rate caused by lower gastrointestinal bleeding in the past 10 years showed an upward trend (P<0.05). Conclusion The microscopic examination rate of lower gastrointestinal bleeding in children was graduallyincreasing,and the prevalence rate of basic diseases such as boys,hypertension and gastroenteritis was increasing;in addition,the disease burden caused by children's lower gastrointestinal bleeding was also increasing year by year and should be protected.
2.The early use of iNO in extremely premature neonates with refractory hypoxic respiratory failure

Objective:To study the early use of inhaled nitric oxide (iNO) as a rescue therapy in extremely premature infants (EPIs) with refractory hypoxic respiratory failure (HRF).Methods:Between January 2021 and December 2021, EPIs with refractory HRF receiving iNO within the first week of life in our NICU were enrolled. Their clinical characteristics and outcomes were retrospectively analyzed.Results:A total of 11 EPIs were included with 5 males and 6 females. The median gestational age (GA) was 24(22.6, 25.2) weeks. The median birth weight (BW) was 580(490, 770) g. The most common primary diagnoses were moderate/severe respiratory distress syndrome (RDS) (5/11) and early-onset sepsis (3/11). The median age starting iNO therapy was 6.5(4.5, 34.0)h and the median duration of iNO was 24(12, 36)h. The median iNO starting dose was 5(5, 8) ppm and the therapeutic range was 5-20 ppm. Therapeutic efficacy was defined as ≥30% FiO 2 reduction after 6 h of iNO treatment. The treatment was effective in 8 cases. The oxygenation index (OI) decreased more than 10% from baseline 1 h after initiation in 9 patients and in all 11 patients after 12 h of iNO. The reduction of OI was more prominent in EPIs with a higher OI at baseline. Of the 11 patients, 8 survived, 1 died and 2 abandoned further treatments. Conclusions:As an early rescue therapy for EPIs with refractory HRF, iNO can improve oxygenation without obvious short-term adverse effects.
3.Value of extrahepatic bile duct and main pancreatic duct segment patterns on MRCP to differentiate the periampullary carcinoma

Objective:To investigate the value of the extrahepatic bile duct and main pancreatic duct segment patterns on magnetic resonance cholangiopancreatography (MRCP) for differentiating the periampullary carcinoma (PAC).Methods:The clinicopathologic data of 125 patients with PAC who were admitted to Wuxi No.2 People’s Hospital from June 2013 to December 2021 were retrospectively analyzed, including 72 males and 53 females, aged (64.9±8.6) years. According to its anatomy, the extrahepatic bile duct (B) was divided into suprapancreatic and intrapancreatic (including ampullary) segments, and the main pancreatic duct (P) was divided into tail-body and head segments. MRCP patterns: i. the extrahepatic bile duct or main pancreatic duct visible without dilatation, ii. cutoff of the distal extrahepatic bile duct or main pancreatic duct with upstream dilatation, iii. cutoff of the intrapancreatic or head segment with upstream dilatation and remnant intrapancreatic or head segments invisible, iv. cutoff of the intrapancreatic or head segment with upstream dilatation and nondilated remnant intrapancreatic or head segments, were represented as 0, 1, 2, and 3, respectively. Segment patterns of B1/P0+ B1/P1, B0/P2+ B0/P3+ B2/P2+ B2/P3+ B3/P3, B3/P0, and B0/P0+ B2/P0 on MRCP were compared in PAC patients.Results:Of the 125 patients, there were 57 (45.6%) with pancreatic head carcinoma, 36 (28.8%) with ampullary carcinoma, 20 (16.0%) with distal cholangiocarcinoma, and 12 (9.6%) with periampullary duodenal carcinoma. Segment patterns of B0/P2+ B0/P3+ B2/P2+ B2/P3+ B3/P3 were found in 52 patients with pancreatic head carcinoma (91.2%, 52/57), with a significant difference between PAC (χ 2=110.66, P<0.001). Segment patterns of B1/P0+ B1/P1were found in 36 patients with ampullary carcinoma (100.0%, 36/36), fallowed by 11 (91.7%, 11/12) with periampullary duodenal carcinoma, with a significant difference between PAC (χ 2=129.95, P<0.001). Segment pattern of B3/P0 presented in 16 patients with distal cholangiocarcinoma (80.0%, 16/20), with a significant difference between PAC (χ 2=62.45, P<0.001). The segment patterns of B0/P0+ B2/P0 were only seen in 3 of 57(5.3%) patients with pancreatic head carcinoma. Conclusion:On MRCP, cutoff of the head segment with upstream dilatation and remnant head segment invisible or nondilated indicates the pancreatic head carcinoma. Cutoff of the intrapancreatic segment with upstream dilatation, remnant intrapancreatic segment visible, and main pancreatic duct nondilated, indicates the distal cholangiocarcinoma. And cutoff of the distal extrahepatic segment with upstream dilatation and main pancreatic duct dilatation or not, indicates the ampullary or periampullary duodenal carcinoma.
4.Reliability and validity analysis of the Chinese version of cyberbullying inventory for college students

Objective:To test the reliability and validity of the Chinese version of cyberbullying inventory for college students(CICS-CV)in the context of Chinese culture and make preliminary application.Methods:A total of 1 528 college students completed the CICS-CV, and 208 students completed CICS-CV after six months.Trait anger subscale of state-trait anger expression inventory-2, and Buss-Perry aggressive qusetionnaire were used as the criterion of cyberbullying aggression, and depression anxiety stress scales were used as the criterion for cyberbullying victimization to test its validity. Item analysis, exploratory factor analysis, reliability test, calibration validity test and corss-lag analysis were conducted by SPSS 24.0 software, and confirmatory factor analysis was conducted by Mplus 8.3.Results:The results of exploratory factor analysis showed that two factors were obtained named as cyberbullying aggression and cyberbullying victimization, and the factor loadings were 0.59-0.82, 0.59-0.80.Confirmatory factor analysis showed that the fitting index of the double factor model was fitting well ( χ2/ df=2.61, CFI=0.90, TLI=0.89, RMSEA=0.04, SRMR=0.05). The internal consistency coefficient of the two subscales were 0.88, 0.89, and test-retest reliability were both 0.97. There was a significant positive correlation between cyberbullying aggression(victimization) and the criterions( r=0.23-0.42, all P<0.05). Cross-lag analysis of moral disengagement and cyberbullying aggression showed that the first measurement of moral disengagement significantly predicted the second measurement of cyberbullying aggression( β=0.16, t=2.16, P<0.05). Conclusion:The CICS-CV has good reliability and validity, which can be used as an effective tool for evaluating college students' cyberbullying in China.

The composition of serum is extremely complex,which complicates the discovery of new pharmaco-dynamic biomarkers via serum proteome for disease prediction and diagnosis.Recently,nanoparticles have been reported to efficiently reduce the proportion of high-abundance proteins and enrich low-abundance proteins in serum.Here,we synthesized a silica-coated iron oxide nanoparticle and devel-oped a highly efficient and reproducible protein corona(PC)-based proteomic analysis strategy to improve the range of serum proteomic analysis.We identified 1,070 proteins with a median coefficient of variation of 12.56%using PC-based proteomic analysis,which was twice the number of proteins iden-tified by direct digestion.There were also more biological processes enriched with these proteins.We applied this strategy to identify more pharmacodynamic biomarkers on collagen-induced arthritis(CIA)rat model treated with methotrexate(MTX).The bioinformatic results indicated that 485 differentially expressed proteins(DEPs)were found in CIA rats,of which 323 DEPs recovered to near normal levels after treatment with MTX.This strategy can not only help enhance our understanding of the mechanisms of disease and drug action through serum proteomics studies,but also provide more pharmacodynamic biomarkers for disease prediction,diagnosis,and treatment.
6.Application of defect scenario simulation combined with workshop teaching method in pre-job skills training of newly recruited nurses

Objective:To explore the application effect of defect scenario simulation combined with workshop teaching method in pre-job skills training of newly recruited nurses.Methods:Using the cluster sampling method, the newly recruited nurses of Yantaishan Hospital in Yantai City from 2019 to 2020 were selected as the research objects. The newly recruited nurses in 2019 were set as the control group ( n=72) and the newly recruited nurses in 2020 were set as the observation group ( n=61) . Nurses in the control group implemented the traditional situational simulation pre-job skills training method, while the nurses in the observation group were trained using the defect scenario simulation combined with workshop teaching method. The results of skill assessment, clinical teaching effect evaluation and clinical comprehensive ability of newly recruited nurses were compared between the two groups. Results:At the second, third and fourth week after training, the skill assessment scores of newly recruited nurses in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . After the training, the newly recruited nurses in the observation group had higher scores on the teaching effect evaluation than the control group, and the differences were statistically significant ( P<0.05) . After the training, the observation group were better than the control group in terms of examination and assessment, communication ability, practical ability, ability to rectify existing problems and comprehensive evaluation, and the differences were statistically significant ( P<0.05) . Conclusions:The training method based on defect scenario simulation combined with workshop is helpful to improve the operational skills of newly recruited nurses, their satisfaction with the teaching effect and clinical comprehensive ability, which is worthy of promotion and application.

Objective:
Comparing the benefit of Abidor, lopinavir/ritonavir and recombinant interferon α-2b triple combination antiviral therapy and lopinavir/ritonavir and interferon dual combination antiviral therapy to hospitalized novel coronavirus pneumonia 2019 in Zhejiang province.
Methods:
A multi-center prospective study was carried out to compare the effect of triple combination antiviral therapy with dual combination antiviral therapy in 15 medical institutions of Zhejiang Province. All patients were treated with recombinant interferon α-2b (5 million U, 2 times/d) aerosol inhalation. 196 patients were treated with abidol (200 mg, 3 times/d) + lopinavir / ritonavir (2 tablets, 1 time/12 h) as the triple combination antiviral treatment group. 41 patients were treated with lopinavir / ritonavir (2 tablets, 1 time/12 h) as the dual combination antiviral treatment group. The patients who received triple combination antiviral therapy were divided into three groups: within 48 hours, 3-5 days and > 5 days after the symptom onset. To explore the therapeutic effects of triple combination antiviral drugs and dual combination antiviral drugs, as well as triple combination antiviral drugs with different antiviral initiate time. SPSS17.0 software was used to analyze the data.
Results:
The time of virus nucleic acid turning negative was (12.2 ± 4.7) days in the triple combination antiviral drug group, which was shorter than that in the dual combination antiviral drug group [(15.0 ± 5.0) days] (

Objective:To compare the efficacy of the combination of abidol, lopinavir/ritonavir plus recombinant interferon α-2b (rIFNα-2b) and the combination of lopinavir/ritonavir plus rIFNα-2b for patients with COVID-19 in Zhejiang province.Methods:A multicenter prospective study was carried out to compare the efficacy of triple combination antiviral therapy and dual combination antiviral therapy in 15 medical institutions of Zhejiang province during January 22 to February 16, 2020. All patients were treated with rIFNα-2b (5 million U, 2 times/d) aerosol inhalation, in addition 196 patients were treated with abidol (200 mg, 3 times/d) + lopinavir/ritonavir (2 tablets, 1 time/12 h) (triple combination group) and 41 patients were treated with lopinavir/ritonavir (2 tablets, 1 time/12 h) (dual combination group). The patients who received triple combination antiviral therapy were further divided into three subgroups: <48 h, 3-5 d and >5 d according the time from the symptom onset to medication starting. The therapeutic efficacy was compared between triple combination group and dual combination group, and compared among 3 subgroups of patients receiving triple combination antiviral therapy. SPSS 17.0 software was used to analyze the data.Results:The virus nucleic acid-negative conversion time in respiratory tract specimens was (12.2±4.7) d in the triple combination group, which was shorter than that in the dual combination group [(15.0±5.0) d] ( t=6.159, P<0.01). The length of hospital stay in the triple combination group [12.0 (9.0, 17.0) d] was also shorter than that in the dual combination group [15.0 (10.0, 18.0) d] ( H=2.073, P<0.05). Compared with the antiviral treatment which was started within after the symptom onset of in the triple combination group, the time from the symptom onset to the viral negative conversion was 13.0 (10.0, 17.0), 17.0 (13.0, 22.0) and 21.0 (18.0, 24.0) d in subgroups of 48 h, 3-5 d and >5 d, respectively ( Z=32.983, P<0.01), while the time from antiviral therapy to viral negative conversion was (11.8±3.9), (13.5±5.1) and (11.2±4.3) d, respectively( Z=6.722, P<0.05). Conclusions:The triple combination antiviral therapy of abidol, lopinavir/litonavir and rIFNα-2b shows shorter viral shedding time and shorter hospitalization time, compared with the dual combination antiviral therapy; and the earlier starting triple combination antiviral therapy will result in better antiviral efficacy.
9.Effects of TNF-related apoptosis-inducing ligand on cell apoptosis of pancreatic cancer

Objective To investigate the mechanism of TNF-related apoptosis-inducing ligand (TRAIL) promoting apoptosis of pancreatic cancer cells SW1990, Patu8988 and BxPC3. Methods Three kinds of pancreatic cancer cells SW1990, Patu8988 and BxPC3 were transfected with the pCA13 plasmid carrying TRAIL gene ( pCA13 TRAIL group) and the blank plasmid control ( pCA13 group) , respectively. The expression of TRAIL mRNA in transfected cells was detected by RT-PCR, and the expression of TRAIL protein was detected by Western blot. The apoptosis rate and expression of TRAIL receptor R1 and R2 were detected by flow cytometry. Apoptosis was detected by TUNEL and Hoechst double staining, and observed by electron microscopy. The expression of caspase-3 in transfected cells was detected by immunohistochemistry. Results SW1990, Patu8988 and BxPC3 cells can expresse TRAIL mRNA and protein within 24 h after transfection. The apoptotic rate at 24 h after transfection was (27. 30 ± 5. 14)%, (13. 52 ± 0. 95)% and (31. 40 ± 8. 70)%,respectively, which was higher than that of pCA13 group [(10. 58 ± 1. 88)%,(8. 42 ± 0. 46)% and (16.11 ±1.66)%], respectively. The expression rates of TRAIL-R1 were (61.37 ± 3.05)%,(42.10 ± 5. 11)% and (36. 64 ± 4. 84)%, respectively, and the expression rates of TRAIL-R2 were (36. 20 ± 4. 83)%,(37. 26 ± 8. 46)% and (24. 32 ± 3. 71)%, respectively,which were higher than those of pCA13 group except PATU8988 cells. Positivity rates of caspase-3 were ( 14. 64 ± 5. 35 )%, ( 9. 92 ± 5. 50 )% and (16. 12 ± 6. 74)%, which were obviously higher than ( 3. 01 ± 1. 50 )%, ( 1. 75 ± 0. 50 )% and ( 3. 79 ± 1. 58)% in pCA13 group,and the differences were statistically significant(P<0. 05). Conclusions TRAIL could up-regulate the expression of TRAIL R1 and R2 in multiple pancreatic cancer cell lines in vitro, and thus promote cell apoptosis.
10.Influence of social cognition and interaction training on social function and life quality of patients with schizophrenia in remission

Objective To investigate the influence of social cognition and interaction training(SCIT) on psychiatric symptoms, social function and life quality of patients with schizophrenia in remission. Methods 90 patients with schizophrenia were randomized into intervention group and control group. Combined with primary medication, patients in intervention group were treated with SCIT for 10 weeks while patients in control group were treated with routine mental supportive treatment. Patients' psychiatric symptoms, social function and life quality after treatment were observed. Results Scores of positive and negative syndrome scale (PANSS), social disability screening schedule(SDSS), family assessment device (FAD) and general quality of life inventory (GQOLI) in two groups before intervention were not significantly different(P>0. 05). Compared with those before intervention, scores of positive syndrome, negative syndrome and total score of PANSS decreased significantly in two groups respectively (P<0. 05) and these indicators in intervention group improved more significantly when compared with control group (P<0. 05). Scores of SDSS and FAD in intervention group decreased when compare with those before intervention and were lower than those in control group(P<0. 05). After intervention, scores of body function, physiological function, social function and quality of life in intervention group increased significantly (P<0. 05) and were higher than those in control group after intervention (P<0. 05). There were no significant differences of scores of material life satisfaction between before and after intervention or be tween groups (P>0. 05). Conclusion SCIT could improve the social function and quality of life in patients with schizophrenia in remission significantly and could relieve the clinical symptoms of patients with schizophrenia in remission to some extent.
