Objective:To explore the situation and characteristics of the practice of severe cardiac analgesia, sedation and delirium in children in China, so as to lay a research foundation for standardizing the prevention process of severe cardiac analgesia, sedation and delirium in children. Methods:This study was a cross-sectional study, using questionnaire survey. The Analgesia, Sedation and Delirium Practice Questionnairewas designed according to consensus, clinical suggestions and clinical practice. In August 2021, the electronic questionnaire was issued to the Children's Cardiac Intensive Care Unit (ICU) in children's hospitals through the Pediatric Nursing Alliance Pediatric Congenital Nursing Group of the National Children's Medical Center. A total of 15 ICUs were included, all of which completed the survey, and the effective recovery rate of the questionnaire was 100%.Results:The 15 ICUs all had pain assessment system, including 11 with pain assessment standard operating procedures, 8 with sedation assessment standard operating procedures, and 4 with delirium assessment. However, there was no specific process or system for delirium assessment in ICU. All 15 ICUs used analgesics, 8 of them used opioids for analgesia immediately after cardiac surgery, 5 of them used analgesics according to the patient's condition, 2 of them used analgesics according to the pain score. Non-steroidal paracetamol was the first choice for non-postoperative children, and 6 of 15 ICUs used analgesics according to the pain score. All 15 ICUs used sedative drugs, 6 of which used sedative drugs within 1 hour after admission, 6 of which were decided by the doctor according to the situation of the child, 3 of which used sedative drugs according to the sedation score, and 7 of which used midazolam as the first choice. A total of 6 the 15 ICUs carried out some projects in the "ABCDEF" delirium prevention program. Conclusions:In this investigation, the use of analgesic and sedative drugs in ICU is heterogeneous, the evaluation process of analgesia and sedation has not been established, and delirium has not attracted enough attention. Rational sedation and analgesia need to develop standardized sedation and analgesia evaluation procedures. At present, high-quality clinical studies are still needed to provide data support for the construction of standardized processes.

Objective:To investigate the application value of ultrasound-guided selective pudendal nerve block during vestibular gland resection. Methods:Ninety patients who underwent vestibular gland resection in Lishui Maternal and Child Health Hospital from January 2017 to January 2020 were included in this study. These patients were divided into a control group and an observation group ( n = 45/group) according to the order of admission. Patients in the control group underwent blind nerve block and those in the observation group underwent ultrasound-guided selective pudendal nerve block. Hemodynamics before and after anesthesia, anesthetic effects, pain severity, and the incidence of complications were compared between the control and observation groups. Results:There were no significant differences in hemodynamics and pain severity between the control and observation groups (both P > 0.05). Systolic blood pressure [(124.37 ± 8.12) mmHg], diastolic blood pressure [(91.68 ± 5.44) mmHg] and heart rate [(74.62 ± 3.56) beats/min] were significantly lower than those in the control group [(128.62 ± 9.27) mmHg, (95.24 ± 6.17) mmHg, (76.12 ± 3.32) beats/min, t = 2.248, 2.903, 2.067, all P < 0.05]. In the observation group, pain scores at 2 and 24 hours after surgery were (3.06 ± 0.25) points and (4.21 ± 0.52) points, respectively, which were significantly lower than those in the control group [(3.28 ± 0.24) points and (4.54 ± 0.48) points, t = 2.618, 3.128, both P < 0.05). Anesthetic effect in the observation group was superior to that in the control group, and incidence of complications in the observation group was significantly lower than that in the control group ( χ2 = 12.869, 4.285, 4.864, 3.920, all P < 0.05). Conclusion:Ultrasound-guided selective pudendal nerve block in vestibular gland resection exhibits better anesthetic effect, has no obvious effect on hemodynamics, and has good analgesic effect with low incidence of complications. Therefore, it deserves clinical popularization.

Objective:To determine the dose-effect relationship of norepinephrine in the treatment of hypotension after spinal anesthesia in cesarean section. Methods:Ninety American Society of Anesthesiologists physical status Ⅰ or Ⅱ parturients who were at full term with a singleton fetus, body mass index of 20-35 kg/m 2, blood pressure 100-140 mmHg, heart rate 60-100 beats/min, scheduled for elective cesarean section, were divided into 6 different doses of norepinephrine groups (NE2, NE4, NE6, NE8, NE10 and NE12 groups) using a random number table method, with 15 cases in each group. The maternal basal systolic blood pressure was measured after entering the operating room.Anesthesia was performed by injecting hyperbaric bupivacaine 9 mg into the subarachnoid space over 45 s. When hypotension occurred for the first time after anesthesia (systolic blood pressure was lower than 80% of the baseline value), norepinephrine 2, 4, 6, 8, 10 and 12 μg (diluted to 5 ml in normal saline) were intravenously injected in NE2, NE4, NE6, NE8, NE10 and NE12 groups, respectively.Systolic blood pressure was measured at 60 s after completion of injection. The effective treatment of hypotension was defined as the recovery of systolic blood pressure to more than 80% of the baseline value. The logistic regression analysis method was used to draw the dose-effect curve of norepinephrine in treating hypotension after spinal anesthesia in cesarean section. The median effective dose (ED 50), 95% effective dose (ED 95) and 95% confidence interval (CI) were calculated. The time to first hypotension, effective treatment of hypotension, and occurrence of bradycardia and nausea and vomiting after intravenous injection of norepinephrine were recorded. The Apgar scores of the neonates at 1 and 5 min after birth were recorded. The umbilical artery blood samples of neonates were collected immediately after cutting the cord for blood gas analysis. Results:There was no significant difference in the incidence of maternal basal systolic blood pressure, time to first hypotension, bradycardia, and nausea and vomiting among the six groups ( P>0.05). The rate of effective treatment of hypotension increased with the increase of the dose in the six groups ( P<0.05). There was no significant difference in Apgar score and indexes of umbilical artery blood gas analysis at 1 and 5 min after birth among the six groups ( P>0.05). The ED 50 (95% CI) of norepinephrine in the treatment of hypotension after spinal anesthesia in cesarean section was 4.0 (3.0 to 5.0) μg, and the ED 95 (95% CI) was 11.8 (8.9-20.4) μg. Conclusion:The ED 50 and ED 95 of norepinephrine are 4.0 and 11.8 μg, respectively, when used for treating hypotension after spinal anesthesia in cesarean section.

		                        			OBJECTIVE:
To analyze the changes in tumor lymphatic vessel density (LVD) in patients with lung adenocarcinoma in situ (AIS), minimally invasive adenocarcinoma (MIA), and invasive adenocarcinoma (IA) and explore the regulatory factors of LVD.

		                        			METHODS:
Complete clinicopathological data were collected form a total of 301 patients with lung adenocarcinoma, including 28 (9.3%) with AIS, 86 (28.6%) with MIA, and 187 (62.1%) with IA. The LVD of all the adenocarcinomas were calculated after D2-40 immunohistochemical staining, and MT1-MMP and VEGF-C expression levels were also evaluated. The differences in LVD among the groups and the correlations of tumor LVD with the expressions of MT1-MMP and VEGF-C and the clinicopathological factors were analyzed.

		                        			RESULTS:
The LVD differed significantly among AIS, MIA, and IA groups (= 0.000). The LVDs was significantly correlated with the level of VEGF-C protein expression (=0.917, =0.009), tumor size (= 0.686, =0.017), lymph node metastasis (=0.739, =0.000), and clinical stage (=0.874, =0.012) of the patients.

		                        			CONCLUSIONS
Tumor lymphangiogenesis plays an important role in lung adenocarcinoma progression, and VEGF-C may promote this process.

Objective To detect the plasma concentration of fluorouracil in patients with advanced colorectal cancer who uesd fluorouracil , and to explore the relationship between chemotherapy efficacy , tumor response, toxicity and survival of patients with advanced colorectal cancer after chemotherapy with fluorouracil. Methods 174 patients with advanced colorectal cancer were collected from the General Hospital of Ningxia Medical University from January 2014 to December 2016.Patients with advanced colorectal cancer were treated with FOLFOX scheme , venous blood was taken from patients 18-24 hours after intravenous infusion of fluorouracil .The concentration of 5-FU in blood was measured by nami-enhanced immune assay. The targeted AUC range of 5-FU was 20-30 mg· h/L.The patients were divied into three groups-targeted range group , lower than the targeted range group and higher than the targeted range group . The therapeutic efficacy, toxicity and survival of patients were recorded , in order to observe the difference of different target range groups. To analyze the correlation between the different target groups and the clinical characteristics,a logistic method was used.Chi-square test was used to compare the efficacy and toxicity of the three groups. The survival curves were plotted by kaplan-meier method. Results To detected the concentration of fluorouracil at the same time after infusion of fluorouracil .The AUC was 2.267-31.06 mg· h/L,it was very different, the median AUC was 15.368 mg· h/L, and the highest AUC was 31.06 mg· h/L,it was 13.7 times that of the lowest blood concentration (2.267 mg· h/L) .The relationship between the concentration of fluorouracil and the clinical indicators was found as follows :(1)Clinical characteristics : the blood concentration of 5-Fu after infusion was different , which was related to the preoperative CEA status (r=0.318,P<0.05), EGFR(r=0.558,P<0.05), Lymph node metastasis status(r=0.325,P<0.05), and the number of metastatic organs ( r=0.404, P<0.05 ) .( 2 ) Effect of chemotherapy : There were significant differences in the effect of chemotherapy between different target groups (χ2=6.78,P<0.05). The effect of chemotherapy in the targeted range group was lower than that in the target range group , and the difference has statistically significant(χ2=2.030,P<0.05).(3)toxicity: there have statistical difference among the three groups, the toxicity of higher targeted range group was higher than the other two groups (χ2=2.01,P<0.05).(4)Survival: The survival time of patients with targeted range group was lower than that of targeted range group, and the difference has statistically significant (χ2=7.263,P<0.05).Conclusion The method of testing the concentration of fluorouracil in patients with advanced colorectal cancer is valuable to predict the efficacy , toxicity and prognosis of patients with colorectal cancer.

Purpose To evaluate the diagnostic value of contrast-enhanced ultrasound (CEUS) in papillary thyroid carcinomas (PTC) by exploring the relationship between quantitative parameters of time-intensity curve (TIC) of CEUS for PTC and tumor size and metastasis of cervical lymph nodes.Materials and Methods 124 patients with PTC confirmed by surgery and pathology in the Affiliated Hospital of Qingdao University were retrospectively analyzed.According to maximum diameter of lesions (D),the lesions were divided into three groups with D<1.0 cm,1.0 cm ≤ D ≤ 2.0 cm and D>2.0 cm.The lesions were also divided into LN(+) group with lymph node metastasis and LN(-) group without lymph node metastasis based on pathology of cervical lymph nodes. The features of CEUS and quantitative parameters of TIC of the above groups were analyzed. Results ① The CEUS showed that the PTC nodules were mainly concentric and heterogeneous enhancement.Thyroid carcinoma with D<1.0 cm and 1.0 cm≤D≤2.0 cm showed low enhancement (45/57,31/42),while thyroid carcinomas with D>2.0 cm exhibited high enhancement (14/25),and the difference was significant (P<0.05).① With the increase of the diameter of PTC,the peak intensity [(12.75 ± 3.77)%,(15.53 ± 3.62)%,(18.11 ± 4.28)%],the area under the curve [(820.52±289.19)%.s,(873.84± 156.19)%· s,(1118.8± 152.48)% ·s] and the ratio of the perfusion defects (24.56%,52.38%,72.00%) were increased, and the differences were statistically significant (P<0.05).③ The cervical lymph node metastasis rate of PTCs with isoenhancement or hyperenhancement patterns showed by CEUS was significantly higher than that with hypoenhancement (P<0.05).The peak intensity and the area under the curve of LN (+) group were higher than that of LN (-) group, and the differences were statistically significant (P<0.05).Conclusion There were significant differences in imaging features of CEUS between the PTC nodules with different size and lymph node metastasis, which can provide valuable information for clinical diagnosis.
