1.Ethical issues and reflections on clinical research of radiopharmaceuticals

Radiopharmaceuticals play an important role in the diagnosis and treatment of cardiovascular and cerebrovascular diseases, malignant tumors, central nervous system diseases, and other diseases. Under the urgent need for clinical diagnosis and treatment as well as medical development, the clinical research of radiopharmaceuticals has become a hotspot in international research. By analyzing the current situation of clinical research on radiopharmaceuticals in Europe, America, and China, the ethical issues of clinical research on radiopharmaceuticals were elaborated from four aspects, including lack of relevant laws and regulations, a higher risk of radiopharmaceuticals, dilemmas in ethical review, and insufficient radiation protection. Response principles and measures were proposed from four aspects, including improving regulations and policies, enhancing radiological protection for all parties involved in the research, strengthening ethical review, and reinforcing the training of relevant personnel, to enhance the quality and level of clinical research on radiopharmaceuticals.
2.Factors affecting dyslipidemia among residents in Chengdu City

Objective:
To investigate the prevalence and influencing factors of dyslipidemia among residents in Chengdu City, so as to provide insights into improving the prevention and control of dyslipidemia.
Methods:
Based on the baseline survey of the Natural Population Cohort Study in Southwest China, residents aged 30 to 79 years was selected from 34 towns (communities) in 5 counties (districts) of Chengdu City using the multi-stage stratified cluster random sampling method in 2018. Demographic information and lifestyle behaviors were collected through questionnaires. Blood pressure, fasting blood glucose, serum uric acid, total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were collected through physical examination and laboratory tests. A multivariable logistic regression model was used to identify the factors affecting dyslipidamia.
Results:
A total of 21 113 participants were surveyed, including 9 331 males (44.20%) and 11 782 females (55.80%), and had a mean age of (50.80±12.32) years. The prevalence rate of dyslipidemia was 35.64%, and the prevalence rates of high TG, low-HDL-C, high TC and high LDL-C were 17.25%, 11.88%, 10.11% and 7.35%, respectively. Multivariable logistic regression analysis identified gender (male, OR=1.584, 95%CI: 1.463-1.716), age (50 to 79 years old, OR:1.221-1.444, 95%CI: 1.079-1.632), residence (urban, OR=1.123, 95%CI: 1.052-1.198), marital status (not married, OR=1.246, 95%CI: 1.128-1.376), educational level (high school and above, OR=0.914, 95%CI: 0.849-0.983), current smoking (OR=1.220, 95%CI: 1.121-1.327), drinking (1 to 2 d/week, OR=1.525, 95%CI: 1.368-1.700; 3 to 5 d/week, OR=1.857, 95%CI: 1.575-2.191; almost every day, OR=1.512, 95%CI: 1.269-1.801), sedentary time in leisure time (>2 h/d, OR=1.123, 95%CI: 1.046-1.206), central obesity (OR=2.212, 95%CI: 1.986-2.265), hypertension (OR=1.489, 95%CI: 1.388-1.598), diabetes (OR=1.998, 95%CI: 1.833-2.157) and hyperuricemia (OR=2.012, 95%CI: 1.848-2.192) as factors affecting dyslipidemia.
Conclusion
The prevalence of dyslipidemia among residents in Chengdu City was mainly associated with smoking, drinking, sedentary time, central obesity, hypertension, diabetes and hyperuricemia.
3.Effects of emodin on autophagy in fibroblast-like synoviocytes in rheumatoid arthritis

Objective To study the cGAS/STING signaling pathway and investigate the potential effect of emodin(EMD)on autophagy of human rheumatoid arthritis fibroblast synovial cells(MH7A).Methods CCK-8 method was used to detect MH7A cell proliferation,and the experimental concentration of EMD was screened according to cell survival rate.Then,autophagy inhibitor 3-MA was added to further verify the effect of EMD on autophagy.Autophagy of MH7A cells was detected via the monodansylcadaverine staining method.Protein expression levels of cGAS,STING,p-STING,LC3-Ⅰ,LC3-Ⅱ,P62 and Beclin-1 were detected by Western blot.Results Monodansylcadaverine staining indicated that EMD enhanced the autophagy of MH7A cells.Western blot indicated that EMD decreased the expression of autophagy related proteins cGAS,STING,p-STING and P62,and increased that of LC3-Ⅱ and Beclin-1 in MH7A cells.After addition of the autophagy inhibitor 3-MA,the expression of P62 protein in MH7A cells increased,while that of LC3-Ⅱ and Beclin-1 decreased.Conclusions EMD may accelerate autophagy and inhibit MH7A cell proliferation by down-regulating cGAS/STING signaling pathway proteins.
4.Amide proton transfer-weighted MRI of cervical squamous carcinoma: correlation with Ki-67 proliferation status

Objective:To investigate the correlation between amide proton transfer-weighted (APTw) values and Ki-67 labeling index of cervical squamous cell carcinoma.Methods:From October 2017 to December 2018, 24 patients with cervical squamous cell carcinoma [International Federation of Gynecology and Obstetrics (FIGO) stage Ⅰ-Ⅲ] were prospectively enrolled in Peking Union Medical College Hospital and underwent pelvic morphological MRI on a 3.0 T MR scanner, including three-dimensional turbo-spin-echo APTw imaging and DWI. The maximum diameters of the lesions, APTw values and ADC values on the slice with the maximum diameter of the lesion were independently measured by two radiologists. The ICC was computed to evaluate the inter-observer consistency. Ki-67 immunohistochemical expression status was assessed by one pathologist. The Pearson correlation analysis was performed between the APTw values, maximum diameters, ADC values and Ki-67 labeling index.Results:The APTw values of cervical squamous cell carcinoma were (2.9±0.5)%. Inter-observer ICC was 0.972 (95%CI 0.937-0.988). The APTw values were positively moderately correlated with Ki-67 labeling index [(61.9±18.7)%, r=0.532, P=0.008]. The maximum diameters of the lesions were (28.7±10.6) mm. The mean ADC values were (0.998±0.217)×10 -3 mm 2/s. No correlations were found between maximum diameters, ADC values and Ki-67 labeling index ( r=0.038, P=0.859; r=0.238, P=0.263). Conclusion:APTw values can partially reveal the proliferation status of cervical squamous cell carcinoma.
5.Risk factors and nutritional status analysis in patients with liver cirrhosis and concomitant chronic periodontitis

Objective:To study and explore the prevalence, characteristics, preliminary risk factors, as well as their relationship with nutritional scores in liver cirrhotic patient with chronic periodontitis.Methods:163 patients with liver cirrhosis who were hospitalized in the Hepatology Division, Department of Internal Medicine at Tianjin Third Central Hospital from June to September 2018 were enrolled as the case group, while the control group consisted 140 healthy individuals enrolled during the same period. Periodontal examination, biochemical examination and oral hygiene habits were investigated. The prevalence of periodontitis in the two groups was compared, and the risk factors of severe periodontitis were conducted by multivariate regression analysis.Results:The prevalence of chronic periodontitis was significantly higher in patients with liver cirrhosis than healthy control population, and the differences were statistically significant ( P < 0.05). The prevalence of severe periodontitis and full edentulous jaws was significantly higher in patients with liver cirrhosis than healthy control group, and the differences were statistically significant ( P < 0.05 and P < 0.001). Compared with the healthy control group, the depth of periodontal pocket and the degree of attachment loss were significantly increased in the liver cirrhosis group ( P < 0.001). Multivariate regression analysis showed that liver cirrhosis was the independent risk factors for both groups of patients with severe periodontitis (χ 2 = 11.046, P < 0.001). Univariate and multivariate regression analysis showed that toothbrushing frequency, nutritional risk score, prealbumin level and Child-Pugh grade were independent risk factors for occurrence of severe periodontitis in liver cirrhotic patient (χ 2 = 5.252, P = 0.022; χ 2 = 24.162, P < 0.001; χ 2 = 4.159, P = 0.041; χ 2 = 9.249, P = 0.002). Conclusion:The prevalence of periodontitis is significantly higher in patients with liver cirrhosis than healthy individuals, and liver cirrhosis is an independent risk factor for the occurrence of severe periodontitis. Toothbrushing frequency, nutritional risk score, prealbumin level and Child-Pugh grade are risk factors for severe periodontitis in patients with liver cirrhosis.

Objective:To establish a disease risk prediction model for the newborn screening system of inherited metabolic diseases by artificial intelligence technology.Methods:This was a retrospectively study. Newborn screening data ( n=5 907 547) from February 2010 to May 2019 from 31 hospitals in China and verified data ( n=3 028) from 34 hospitals of the same period were collected to establish the artificial intelligence model for the prediction of inherited metabolic diseases in neonates. The validity of the artificial intelligence disease risk prediction model was verified by 360 814 newborns ' screening data from January 2018 to September 2018 through a single-blind experiment. The effectiveness of the artificial intelligence disease risk prediction model was verified by comparing the detection rate of clinically confirmed cases, the positive rate of initial screening and the positive predictive value between the clinicians and the artificial intelligence prediction model of inherited metabolic diseases. Results:A total of 3 665 697 newborns ' screening data were collected including 3 019 cases ' positive data to establish the 16 artificial intelligence models for 32 inherited metabolic diseases. The single-blind experiment ( n=360 814) showed that 45 clinically diagnosed infants were detected by both artificial intelligence model and clinicians. A total of 2 684 cases were positive in tandem mass spectrometry screening and 1 694 cases were with high risk in artificial intelligence prediction model of inherited metabolic diseases, with the positive rates of tandem 0.74% (2 684/360 814)and 0.46% (1 694/360 814), respectively. Compared to clinicians, the positive rate of newborns was reduced by 36.89% (990/2 684) after the application of the artificial intelligence model, and the positive predictive values of clinicians and artificial intelligence prediction model of inherited metabolic diseases were 1.68% (45/2 684) and 2.66% (45/1 694) respectively. Conclusion:An accurate, fast, and the lower false positive rate auxiliary diagnosis system for neonatal inherited metabolic diseases by artificial intelligence technology has been established, which may have an important clinical value.
7. T2WI female pelvic MRI using the MultiVane XD technique at 3.0 T in uterine adenomyosis and fibroids: a preliminary study

Objective:
To explore the application value of 3.0 T MultiVane XD (MVXD) technique in female patients with uterine adenomyosis and fibroids.
Methods:
Patients diagnosed with uterine fibroids with ultrasound and suspected of adenomyosis were involved prospectively from March to May 2018, 3.0 T pelvic MRI examinations were performed during peri-ovulatory period. Axialconventional turbo spin echo (TSE) T2WI, axial MVXD T2WI, sagittal conventional TSE T2WI and MVXD sagittal T2WI were acquired. Two observers rated those 4 series in the aspects of sharpness of uterine border, motion artifacts, identification capability of lesions, confidence of diagnosis and overall image quality. Cohen Kappa analysis was used to evaluate the consistency of scores between 2 observers. Scores of TSE T2WI and MVXD T2WI qualities were compared using Wilcoxon matched-pairs signed-ranks test.
Results:
Twenty patients were enrolled. Axial conventional TSE T2WI, axial MVXD T2WI were aquired on all of them. Sagittal conventional TSE T2WI, sagittal MVXD T2WI were aquired on 19 among them. Nine patients had only obvious adenomyosis, 6 had only uterinefibroids, and 5 had adenomyosis and uterine fibroids. To investigate the profiles of blood amino acid and acylcarnitine in early neonates with neonatal intrahepatic cholestasis caused by citrin deficiency (NICCD) and the sensitivity of newborn screening, and to explore potential biochemical metabolic markers for newborn screening program.

Amino acid and acylcarnitine profiles in dried blood spots of newborn screening program were analyzed by tandem mass spectrometry (MS/MS). Objective To investigate the modulation effects of mesenchymal stem cells(MSCs)overex-pressing IL-10 transplanted in a rat model of myocardial infarction and its possible mechanism. Methods The MI rats were established by left anterior descending coronary artery ligation and the rats were then randomly divided into three groups:group C(MSC+PBS),group P(pcDNA3-IL-10+MSC),group K(pcDNA3+MSC).Echocardiography and hemodynamic examinations were used to evaluate the cardiac function.Myocardial infarction size were evaluate were evaluate by Immumohistochemical stainingmyocardial.At the same time,Immunofluorescence and western blot was applied to show the expression of Caspase-3,TNF-α and IL-1β,respectively.Results The left ventricular ejec-tion fraction and fractional shortening in three groups showed no significant difference(P>0.05)at different time;There were no statistically significant differences between the groups K and group C and the left ventricular ejection fraction,fractional shortening in group P were highest(P<0.05);The left ventricular ejection fraction(Finteractive=2.564,Pinteractive=0.015)and fractional shortening(Finteractive=2.233,Pinteractive=0.022)have interactive effect in three groups.After 4 weeks,LVSP,+dp/dtmax and-dp/dtmax in group P were significantly higher than that of C group and K group,while the LVEDP was lower(P<0.01);immunofluorescence and Western blot showed that Caspase-3, TNF-α and IL-1β in group P were significantly lower than that of C group and K group,and the difference was statis-tically significant(P<0.05).Conclusion MSC overexpressing IL-10 can promote the recovery of cardiac function after MI,which may be related to inhibition of Caspase-3 apoptosis gene and TNF-α and IL-1β inflammatory factors.
10.Efficacy of botulinum toxin type A injection on axillary hyperhidrosis

Objective To evaluate clinical efficacy on treatment of axillary hyperhidrosis by botulinum toxin type A injection and to introduce a corresponding method evolved from Minor's starch iodine test with MATLAB analysis.Methods Ten patients were involved in the study and evaluated preoperatively by both Hyperhidrosis Disease Severity Scale (HDSS) and Minor's starch iodine test with MATLAB analysis.All cases were treated with botulinum toxin type A injection dosed by 50 U on one side focally and evaluated postoperatively by both above mentioned methods with interval of 1 month and by HDSS only of 6 months.Results The clinical efficiency by the therapy evaluated by HDSS accounted for 100% and 60% postoperatively with intervals of 1 month and 6 months respectively.By the Minor's starch iodine test with MATLAB analysis,all patients showed no statistical difference between two matched sides preoperatively (P > 0.05),while postoperatively statistical difference was observed between the treated side and the control (P<0.05) and also between the treated group and zero control (P<0.05).Conclusions The clinical efficacy of botulinum toxin type A injection in treatment of axillary hyperhidrosis is highlighted in short term.The evolved Minor's starch iodine test combined with MATLAB analysis serves as an alternative which facilitates operability and objectivity evaluating the therapy outcomes on hyperhidrosis.
