1.Evaluation of Effect of Tongnaoyin on Blood-brain Barrier Injury in Acute Ischemic Stroke Patients Based on Dynamic Contrast-enhanced Magnetic Resonance Imaging

		                        			ObjectiveTo evaluate the effects of Tongnaoyin on the blood-brain barrier status and neurological impairment in acute ischemic stroke (AIS) patients with the syndrome of phlegm-stasis blocking collaterals by dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). MethodsA total of 63 patients diagnosed with AIS in the Jiangsu Province Hospital of Chinese Medicine from October 2022 to December 2023 were enrolled in this study. According to random number table method,the patients were assigned into a control group (32 cases) and an observation group (31 cases). The control group received conventional Western medical treatment,and the observation group took 200 mL Tongnaoyin after meals,twice a day from day 2 of admission on the basis of the treatment in the control group. After 7 days of treatment,the patients were examined by DCE-MRI. The baseline data for two groups of patients before treatment were compared. The National Institute of Health Stroke Scale (NIHSS) score and modified Rankin Scale (mRS) score were recorded before treatment and after 90 days of treatment for both groups. The rKtrans,rKep,and rVe values were obtained from the region of interest (ROI) of the infarct zone/mirror area and compared between the two groups. ResultsThere was no significant difference in the NIHSS or mRS score between the two groups before treatment. After 90 days of treatment,the NIHSS and mRS scores declined in both groups,and the observation group had lower scores than the control group (P<0.05). After treatment,the rKtrans and rVe in the observation group were lower than those in the control group (P<0.01). ConclusionCompared with conventional Western medical treatment alone,conventional Western medical treatment combined with Tongnaoyin accelerates the repair of the blood-brain barrier in AIS patients,thereby ameliorating neurological impairment after AIS to improve the prognosis.

		                        			BACKGROUND:After the internal fixation of cannulated screws in femoral neck fractures,because the affected limb is often unable to bear weight in the short term and the implants with high stiffness have a stress shielding effect on the fracture end,it is easy to cause osteoporosis of the affected limb and changes in the biomechanical distribution of the proximal femur,the incidence of osteonecrosis of the femoral head is high after surgery.At present,few studies have been conducted on the biomechanical effects of osteoporosis at the proximal end of the femur occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws. OBJECTIVE:Using finite element analysis,to investigate the biomechanical effects of osteoporosis occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws and explore the role of biomechanical factors in osteonecrosis of the femoral head. METHODS:Based on the obtained CT scan data of the femur in a patient with a femoral neck fracture,a proximal femoral model for internal fixation for femoral neck fracture was established by Mimics 19.0,3-Matic,UG 11.0,Hypermesh 14.0,and Abaqus software.One finite element model of the proximal femur without osteoporosis and three finite element models of the proximal femur with osteoporosis were analyzed using Abaqus software.The stress,contact pressure,displacement peak and cloud map under different components of the four models were measured and analyzed,and the internal stress changes and distribution of the femoral head were compared and analyzed. RESULTS AND CONCLUSION:The stresses and contact pressures of the femoral head and lower anterior cannulated screws varied more with the degree of osteoporosis.The peak displacement of the four models increased slowly with the degree of osteoporosis.By one-way analysis of variance,there was no significant effect of the degree of osteoporosis on the peak stress,contact pressure,and displacement of the different components.The internal stress distribution of the femoral head changed with the degree of osteoporosis.Changes in the biomechanical environment of the proximal femur have an important impact on osteonecrosis of the femoral head.
		                        		
		                        		
		                        		
		                        	
3.Investigation on acute chromium poisoning accident occurred in process of electroplating tank cleaning

		                        			Background Electroplating technology is widely used in the mechanical manufacturing industry. Electroplating workers may be exposed to occupational hazards such as chromium, zinc, cyanides, and sulfuric acid. With the continuous improvement of protective conditions, occupational hazards have been effectively controlled, but there is less focus on the potential acute occupational poisoning that may occur during the electroplating process. Objective To identify causes of an acute chromium poisoning accident occurred during the cleaning process of electroplating tank in a hardware processing plant, and to formulate relevant prevention and control measures to avoid similar poisoning accidents. Methods Occupational history, medical history, and treatment process of the poisoned worker were inquired in detail, his inpatient medical records were consulted , and his blood samples were tested. Occupational health investigation and testing were carried out at the scene of the poisoning accident. Results The air concentration of peak exposure (CPE) and permissible concentration-time weighted average (CTWA) of chromium and its compounds in the workplace were 0.49 mg·m−3 and 0.31 mg·m−3 respectively. 4.Prognostic value of serum levels of HMGB1,IL-34 and ANGPTL4 in patients with acute respiratory distress syndrome

		                        			Objective To investigate the prognostic value of serum levels of high mobility group protein B1(HMGB1),interleukin-34(IL-34)and angiopoietin like protein 4(ANGPTL4)in patients with acute respira-tory distress syndrome(ARDS).Methods A total of 97 patients with acute respiratory distress syndrome from June 2021 to June 2023 were selected as the observation group,and 97 healthy patients from the hospital during the same period were selected as the control group.The levels of HMGB1,IL-34 and ANGPTL4 were compared between the two groups.The clinical data and serum levels of HMGB1,IL-34 and ANGPTL4 of pa-tients with different prognosis were compared and analyzed,and the influencing factors of prognosis were ana-lyzed.The predictive value of prognostic factors on prognosis was analyzed,and the predictive value of serum HMGB1,IL-34 and ANGPTL4 combined detection on prognosis was evaluated.Results The serum levels of HMGB1,IL-34 and ANGPTL4 in observation group were higher than those in control group,with statistical significance(P<0.05).Oxygenation index(PaO2/FiO2)in death group was lower than that in survival group,acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score,Murray lung injury score(MLIS),HMGB1,IL-34 and ANGPTL4 levels were higher than those in survival group,and the differences were statistically significant(P<0.05).PaO2/FiO2,APACHE Ⅱ score,MLIS,HMGB1,IL-34 and AN-GPTL4 levels were all prognostic factors of ARDS patients(P<0.05).The area under the curve(AUC)of PaO2/FiO2,APACHE Ⅱ score,MLIS and HMGB1,IL-34 and ANGPTL4 for predicting prognosis were 0.779,0.799,0.808,0.844,0.772 and 0.822,respectively.There was no significant difference in AUC among prognostic factors(P>0.05).The analysis of receiver operating characteristic curve showed that the AUC of se-rum HMGB1,IL-34 and ANGPTL4 combined to predict the prognosis of ARDS patients was 0.915(95%CI:0.841-0.962),which was significantly higher than that predicted by each indicator alone.Conclusion The combined detection of serum HMGB1,IL-34 and ANGPTL4 levels in ARDS patients has certain prognostic value.
		                        		
		                        		
		                        		
		                        	
5.Pyridine degradation characteristics of Rhodococcus sp. LV4 under high salinity conditions.

		                        			Biodegradation of pyridine pollutant by microorganisms is one of the economical and effective methods to solve the environmental pollution of pyridine under high salinity conditions. To this end, screening of microorganisms with pyridine degradation capability and high salinity tolerance is an important prerequisite. In this paper, a salt-resistant pyridine degradation bacterium was isolated from the activated sludge of Shanxi coking wastewater treatment plant, and identified as a bacterium belonging to Rhodococcus on the basis of colony morphology and 16S rDNA gene phylogenetic analysis. Salt tolerance experiment showed that strain LV4 could grow and degrade pyridine with the initial concentration of 500 mg/L completely in 0%-6% saline environment. However, when the salinity was higher than 4%, strain LV4 grew slowly and the degradation time of pyridine by strain LV4 was significantly prolonged. Scanning electron microscopy showed that the cell division of strain LV4 became slower, and more granular extracellular polymeric substance (EPS) was induced to secrete in high salinity environment. When the salinity was not higher than 4%, strain LV4 responded to the high salinity environment mainly through increasing the protein content in EPS. The optimum conditions for pyridine degradation by strain LV4 at 4% salinity were 30 ℃, pH 7.0 and 120 r/min (DO 10.30 mg/L). Under these optimal conditions, strain LV4 could completely degrade pyridine with an initial concentration of 500 mg/L at a maximum rate of (29.10±0.18) mg/(L·h) after 12 h adaptation period, and the total organic carbon (TOC) removal efficiency reached 88.36%, indicating that stain LV4 has a good mineralization effect on pyridine. By analyzing the intermediate products in pyridine degradation process, it was speculated that strain LV4 achieved pyridine ring opening and degradation mainly through two metabolic pathways: pyridine-ring hydroxylation and pyridine-ring hydrogenation. The rapid degradation of pyridine by strain LV4 in high salinity environment indicates its application potential in the pollution control of high salinity pyridine environment.

		                        			Despite growing prevalence and incidence, the management of gout remains suboptimal. The intermittent nature of the gout makes the long-term urate-lowering therapy (ULT) particularly important for gout management. However, patients are reluctant to take medication day after day to manage incurable occasional gout flares, and suffer from possible long-term toxicity. Therefore, a safe and easy-to-operate drug delivery system with simple preparation for the long-term management of gout is very necessary. Here, a chitosan-containing sustained-release microneedle system co-loaded with colchicine and uricase liposomes were fabricated to achieve this goal. This microneedle system was confirmed to successfully deliver the drug to the skin and maintain a one-week drug retention. Furthermore, its powerful therapeutic potency to manage gout was investigated in both acute gouty and chronic gouty models. Besides, the drug co-delivery system could help avoid long-term daily oral colchicine, a drug with a narrow therapeutic index. This system also avoids mass injection of uricase by improving its stability, enhancing the clinical application value of uricase. 7.The application value of tibial transverse transport microvascular reconstruction combined with antibiotic bone cement in the limb salvage of diabetic foot

		                        			Objective:To analyze the application value of tibial transverse transport (TTT) microvascular reconstruction combined with antibiotic bone cement in the limb salvage of diabetic foot.Methods:Seventy-seven patients with diabetic foot admitted to the Traditional Chinese Medicine Hospital of Zhongshan from January 2018 to January 2021 were enrolled retrospectively, and they were divided into the control group (performed TTT microvascular reconstruction, 47 cases) and the combined group (performed TTT microvascular reconstruction combined with antibiotic bone cement, 30 cases) according to the surgical method. Wound healing rate, limb salvage rate, the ulcer healing time, bone window healing time, wound infection control status, affected side lower limb vascular blood supply situation, and peripheral nerve function recovery were compared between the two groups.Results:The amputation rate between the two groups had no significant difference ( P>0.05), but the healing rate in the combined group was higher than that in the control group: 90.00%(27/30) vs. 68.08%(32/47); and the ulcer healing time, bone window healing time in the combined group were shorter than those in the control group: (12.55 ± 2.14) weeks vs. (15.01 ± 3.17) weeks, (17.47 ± 2.09) weeks vs. (19.00 ± 2.58) weeks, there were statistical differences ( P<0.05). The positive percentage of bacteria culture in wound secretions in the combined group after the surgery for 6 d was lower than that in the control group: 20.00%(6/30) vs. 44.68%(21/47), there was statistical difference ( P<0.05). After the surgery for 3 months, the dorsal skin temperature and ankle brachial index in the combined group were higher than those in the control group: (31.05 ± 1.74) ℃ vs. (29.27 ± 1.66) ℃, 0.93 ± 0.29 vs. 0.80 ± 0.19, there were statistical differences ( P<0.05), but the scores of visual analog scale (VAS) and 10 g nylon line test values between the two groups had no significant differences ( P>0.05). Conclusions:TTT combined with antibiotic bone cement can achieve better wound healing and infection control effect in the limb salvage of diabetic foot, and the blood supply improvement of the affected limb is more significant.
		                        		
		                        		
		                        		
		                        	
8.Application of MRI-related indicators of posterior cruciate ligament in the diagnosis of anterior cruciate ligament injury: an update

		                        			The main function of anterior cruciate ligament (ACL) is to maintain stability of the knee joint and prevent anterior displacement of the tibial plateau. ACL injury accounts for more than 50% of the knee joint injuries. If not timely handled, it will increase the risk of secondary injuries to structures such as the meniscus and cartilage, causing chronic pain and degeneration of the knee joint. Although most ACL injuries can be determined by their direct signs on MRI, the identification of complex situations and partial tears of ACL are still not satisfactory, which subsequently affects treatment strategies. After ACL injury, changes in anatomical relationship of the knee joint can also lead to morphological changes in other structures such as the posterior cruciate ligament (PCL) on MRI, and these indirect signs can assist in the diagnosis of ACL injury. 9.Epidemiological characteristics and pathogen spectrum changes of hand-foot-mouth disease in Anhui province from 2015 to 2022

		                        			Objective     :
		                        			To  analyze  the  epidemiological  characteristics  and  pathogen  spectrum  of hand,foot  mouth disease  (HFMD) in Anhui province from 2015 to 2022,and to provide scientific evidence for prevention and control measures of HFMD.
		                        		
		                        			Methods    :
		                        			 The  surveillance data of hand,foot and mouth disease in Anhui province from 2015  to 2022 were analyzed by descriptive epidemiology. Real-time PCR was used to detect and classify HFMD samples.  
		                        		
		                        			Results    :
		                        			A total of 650 590  HFMD cases were reported in Anhui province from 2015  to 2022,including 1 406 se- vere cases and 17 deaths.The annual reported incidence was 131. 45 /100 000.The epidemic features of“low incidence in odd years and high incidence in even years”were presented from 2015 to 2019.The incidence showed a  continuous decline from 2020 to 2022.The monthly distribution showed the characteristics of bimodal epidemic,and  the main peak was not obvious in 2020.Hefei,Fuyang,Bozhou,Chuzhou and Suzhou ranked the top five cities in  terms of cumulative incidence.The age of onset was mainly distributed in children aged 5 years and below,accounting for 89. 26%  of the total cases.The male to female ratio was 1. 48  ∶  1.A total of 28 657 laboratory-confirmed cases had  been  reported from  2015  to 2022.EV71   cases  accounted for  10. 57% ,Cox A16   cases  accounted for 24. 90% ,and other enterovirus cases accounted for 64. 53%.The  dominant pathogens showed dynamic changes in  different years.Since 2018,the proportion of EV71 decreased significantly,and the proportion of other enteroviruses gradually increased  to  become  the  dominant  pathogens.Among other enteroviruses,Cox A6 strain was dominant (80. 48% ) .
		                        		
		                        			Conclusion    
		                        			 This study  suggests that the prevention and control of HFMD in Anhui province should  be paid more attention from April to July and from October to December.The focus areas are the cities in northern  Anhui and Hefei where the floating population is large.The focus of prevention and control is on children aged 5  years and  below.Other  enteroviruses  have  become  the  dominant  pathogens of  hand-foot-mouth disease in Anhui province,Cox A6 strain is dominant.
		                        		
		                        		
		                        		
		                        	
10.DeeReCT-APA:Prediction of Alternative Polyadenylation Site Usage Through Deep Learning

		                        			Alternative polyadenylation(APA)is a crucial step in post-transcriptional regulation.Previous bioinformatic studies have mainly focused on the recognition of polyadenylation sites(PASs)in a given genomic sequence,which is a binary classification problem.Recently,computa-tional methods for predicting the usage level of alternative PASs in the same gene have been pro-posed.However,all of them cast the problem as a non-quantitative pairwise comparison task and do not take the competition among multiple PASs into account.To address this,here we propose a deep learning architecture,Deep Regulatory Code and Tools for Alternative Polyadenylation(DeeReCT-APA),to quantitatively predict the usage of all alternative PASs of a given gene.To accommodate different genes with potentially different numbers of PASs,DeeReCT-APA treats the problem as a regression task with a variable-length target.Based on a convolutional neural network-long short-term memory(CNN-LSTM)architecture,DeeReCT-APA extracts sequence features with CNN layers,uses bidirectional LSTM to explicitly model the interactions among com-peting PASs,and outputs percentage scores representing the usage levels of all PASs of a gene.In addition to the fact that only our method can quantitatively predict the usage of all the PASs within a gene,we show that our method consistently outperforms other existing methods on three different tasks for which they are trained:pairwise comparison task,highest usage prediction task,and rank-ing task.Finally,we demonstrate that our method can be used to predict the effect of genetic variations on APA patterns and sheds light on future mechanistic understanding in APA regulation.
