1.Treatment of Recurrent Pediatric Cough and Asthma Based on the Theory of "Mutual Dependence of Ascending and Descending" from the Perspectives of Deficiency,Phlegm,and Blood Stasis

Guided by the theory of "mutual dependence of ascending and descending", recurrent pediatric cough and asthma are considered to result from the imbalance of the ascending and descending of zang-fu organ qi, which is closely associated with deficiency, phlegm, and blood stasis. In clinical practice, the disease is treated based on differentiation during the initial stage, progression stage, and stable stage. In the initial stage, the pathogenesis is attributed to lung deficiency complicated by external pathogens and liver qi rising to attack the lung; the treatment should focus on restraining the lung and calming the liver, using the self-formulated Longchai Yuchuan Fomulation (龙柴愈喘方). During the progression stage, the disorder is due to the spleen failing to ascend clear qi and the kidney failing to grasp qi, leading to phlegm formation from deficiency; treatment should focus on tonifying spleen yang, supplementing kidney qi, and resolving phlegm and fluid retention, using a modified combination of Linggui Zhugan Decoction (苓桂术甘汤) and Jinkui Shenqi Pill (金匮肾气丸). In the stable stage, qi deficiency leads to poor consolidation, with phlegm turbidity and blood stasis; the treatment should aim at tonifying qi, transforming phlegm, and dispelling blood stasis, using a modified version of Guizhi Fuling Pill (桂枝茯苓丸).
2.Syndrome Differentiation and Treatment of Children Acute Respiratory Infection from the Perspective of "Triple-Yang Combination of Diseases"

Guided by "triple-yang combination of diseases", it is believed that the core pathogenesis of acute respiratory infection in children is external cold and internal heat, and triple-yang combination, and its transmission can be divided into two forms. First, the taiyang syndrome usually happens earlier, presenting as the exterior cold in taiyang is not yet resolved, and inward penetration of exterior cold transfers into heat, which combines the disease of shaoyang and yangming; second, yangming meridian already has internal heat, then meets with taiyang meridian externally infected with pathogenic qi, both internal and external pathogens accumulated in the two meridians and affected shaoyang meridian. It is proposed that the therapeutic method should release the exterior and clean the interior, and treat the triple-yang at the same time, with self-prescribed Chaige Gaore Formula (柴葛高热方) as the empirical formula for the treatment of acute respiratory infections in children.
3.Analysis of Ancient and Modern Literature of Xiaoyaosan and Examination of Its Key Information

First recorded in an official medical book from the Northern Song Dynasty called Taiping Huimin Heji Ju Fang (Prescriptions of the Bureau of Taiping People's Welfare Pharmacy), Xiaoyaosan has been developed and refined over generations and is preserved to this day. It specializes in soothing the liver,resolving stagnation,fortifying the spleen,and nourishing blood. In this study,ancient traditional Chinese medicine (TCM) books and contemporary studies were reviewed to obtain information on Xiaoyaosan using bibliometrics,including its historical development,dosage,origin,processing methods,decoction dosage,and ancient and modern indications. Furthermore,a question regarding the presence of Zingiberis Rhizoma Recens and Menthae Haplocalycis Herba in Xiaoyaosan was investigated,and a table of key information on Xiaoyaosan was compiled,providing references for developing Xiaoyaosan preparations. According to the weight and measurement system of the Song dynasty,the contemporary equivalent formulation of the decocted Xiaoyaosan consists of 20.65 g of Glycyrrhizae Radix et Rhizoma and 41.3 g of Angelica Sinensis Radix,Poria,Paeoniae Radix Alba,Atractylodis Macrocephalae Rhizoma,and Bupleuri Radix. The formulation is processed to obtain a mixed powder with a particle size of 10 mesh. For each dose,8.25 g of the mixed powder is combined with 1 g of unprocessed Zingiberis Rhizoma Recens and 0.62 g of Menthae Haplocalycis Herba in 300 mL of water. The mixture is decocted until the volume reaches 210 mL,and the residue is then removed,with no specific timing required for administration. After the processing,each dose consists of approximately 0.75 g of Glycyrrhizae Radix et Rhizoma and 1.50 g of Radix Angelica Sinensis,Poria,Paeoniae Radix Alba,Atractylodis Macrocephalae Rhizoma,and Bupleuri Radix. Ancient medical literature shows that Xiaoyaosan primarily treats blood deficiency and overstrain,specifically for symptoms including heat caused by blood deficiency and fatigue,irregular menstruation,headache,eye soreness,pain in the ribs and limbs,and emaciation and bone steaming. In the Qing Dynasty,ZHANG Lu clearly proposed the pathogenesis of liver depression,and since then,the use of Xiaoyaosan in treating various syndromes associated with liver depression has been highly praised by physicians in the Qing dynasty and modern times. Xiaoyaosan has a wide application in modern clinical practices,involving digestive diseases,gynecological diseases,psychological diseases,nervous system diseases,and otorhinolaryngologic diseases. Moreover,it is most commonly used to treat depression and other diseases complicated with depression,hyperplasia of the mammary gland,etc. The key information on Xiaoyaosan and its clinical applications in ancient and modern times investigated in the study could serve as a scientific reference for in-depth research and extended clinical applications of the prescription.
4.Differentiation and Treatment of Pediatric Allergic Rhinitis Based on the Theory of "Spleen Fuctions as Wei Qi"

Based on the theory of "spleen functions as wei qi", this paper believes that the disease mechanism of allergic rhinitis (AR) in children is the nasal dysfunction caused by the loss of spleen's wei qi. The root cause of AR is the failure of splenic transportation as well as its inability to properly distribute nutrients. The inducement of AR is the invasion of pathogenic qi coupled with insecurity of the wei exterior. The key to AR recurrence lies in the deficiency of healthy qi and lingering of pathogenic qi, with pathogenic qi lodging inside the body. The treatment should adhere to the principle of helping the spleen restore wei qi. During the acute phase, the treatment should dispel wind, conso-lidate the wei qi, and relieve stuffy orifices, and the modified Qufeng Tongqiao Decoction (祛风通窍汤) is used. During the remission phase, the treatment should fortify the spleen, raise the clear, and harmonize the wei qi, and the modified Yuhan Decoction (御寒汤) is applied. During the recovery phase, the treatment should reinforce the healthy qi, consolidate the constitution, and strengthen the wei qi, and the modified Huangqi Jianzhong Decoction (黄芪建中汤) is employed.
5.Exploration on the Approach to Syndrome Differentiation and Treatment of Pediatric Infectious Mononucleosis Based on the "Sweat Pore-Qi and Liquid-Collaterals" Theory

Based on the "sweat pore-qi and liquid-collaterals" theory, it is considered that the core pathogenesis of pediatric infectious mononucleosis lies in the obstruction of sweat pores, the failure of qi and liquid to disperse, and damage to the collaterals due to pathogenic toxins. Accordingly, the treatment principles proposed include unblocking the sweat pores, regulating qi and liquid, and smoothing the collaterals. In clinical practice, treatment is differentiated according to stages: initial, acute, and late stages. In the initial stage, invasion of warm pathogenic toxins into the lung defense leads to obstruction of the sweat pores, which should be treated by unblocking the sweat pores and expelling pathogens outward. In the acute stage, the obstruction of the sweat pores worsens, leading to the failure of qi and liquid dispersal, resulting in intense heat toxins with accumulation of dampness, phlegm, and blood stasis, which should be treated by promoting qi movement, resolving dampness and phlegm, clearing heat, detoxifying, and dispersing stasis to regulate qi and liquid. In the late stage, residual pathogens remain, with qi and yin deficiency and unsmooth collaterals, which should be treated by unblocking the collaterals, dissipating nodules, tonifying qi, and nourishing yin to smooth the collaterals. This approach may provide new insights for the clinical treatment of pediatric infectious mononucleosis.
6.Research progress on impacts of air pollutants, gut microbiota, and seminal microbiota on semen quality

In recent years, China has been facing the dual challenges of declining fertility rates and births, with male reproductive health issues, especially the decline in semen quality, identified as a pivotal contributor to this phenomenon. Meanwhile, accumulating evidence indicates that air pollutants, an increasingly severe environmental problem, can damage semen quality not only directly through their biological toxicity but also indirectly by disrupting the composition of microbial communities in the gut and semen, thereby dysregulating immune function, endocrine homeostasis, and oxidative stress responses. The gut microbiota and semen microbiota, as important components of the human microecosystem, play crucial roles in maintaining reproductive health. This article comprehensively reviewed the research progress on the potential effects of air pollutants (particulate matter and gaseous pollutants), gut microbiota, and semen microbiota on semen quality. Specifically, it elucidated the mechanisms of interaction between these factors and explored how they affect male fertility.
7.Investigation of Anisakis infections in coastal marine fishes and awareness of anisakiasis control knowledge among residents in Yantai City in 2021

Objective To investigate the prevalence of Anisakis infections in coastal marine fishes and awareness of anisakiasis control knowledge among local residents in Yantai City, Shandong Province in 2021, so as to provide insights into formulation of anisakiasis control interventions. Methods Marine fishes were purchased from Shunxin Port, Yantai City, Shandong Province in November 2021, and the presence of Anisakis was detected in different species of fishes and different fish sites. The correlations between body length and weight of marine fish and intensity of Anisakis infections were examined using Spearman’s rank correlation analysis, and the dietary habits and anisakiasis control knowledge were investigated using questionnaire surveys among local residents. Results A total of 201 marine fishes belonging to 20 species were dissected, and Anisakis was detected in 77 marine fishes (38.31%) belonging to 11 species (55.00%), with a mean infection intensity of 45.04 parasites per fish (3 468/77). Spearman’s rank correlation analysis revealed that the body length (rs = 0.74, P < 0.05) and weight (rs = 0.79, P < 0.01) of the monkfish correlated positively with the intensity of Anisakis infections, and the body length (rs = 0.68, P < 0.05) of the flatfish correlated positively with the intensity of Anisakis infections, while no correlations were examined between the body length or weight of other marine fishes and the intensity of Anisakis infections. Of all respondents, 53.38% men and 56.67% women did not know anisakiasis control knowledge at all, and there was a significant difference in the proportion of respondents using separate chopping boards for raw and cooked food from different villages (χ2 = 17.89, P < 0.01), while there was an age-specific proportion of respondents with habitats of eating raw or semi-raw seafood (χ2 = 28.27, P < 0.01). Conclusions The prevalence and intensity of Anisakis infections were high in coastal marine fishes in Yantai City in 2021, and the awareness of anisakiasis control knowledge was low among local residents. Intensified health education pertaining to anisakiasis control knowledge is recommended to reduce the risk of Anisakis infections.

Objective To discuss the application of the"rotating guidewire and correcting the filter recovery hook direction technique"("rotation-correction loop technique"for short),a technique invented by the authors in clinical practice,in the retrieval of complex inferior vena cava filter(IVCF),and to discuss its technical skills and advantages.Methods The clinical data of 417 patients carrying an IVCF,who were admitted to the Department of Vascular Surgery of Second Hospital of Shanxi Medical University of China to retrieve IVCF between January 2022 and December 2022,were retrospectively analyzed.Taking the time spent on the retrieval of IVCF and the intraoperative radiation dose as the evaluation indicators,the advantages and disadvantages of the standard filter retrieval technique,the"rotation-correction loop technique"and the other loop-assisted techniques were compared.Results Both the intraoperative radiation dose and the time spent on the retrieval of IVCF using"rotation-correction loop technique"were remarkably lower than those of other loop-assisted techniques(P<0.000 1).Conclusion For the retrieval of complex IVCF,especially for the IVCF which is heavily tilted and/or its recovered hook is attached to the vascular wall,the use of"rotation-correction loop technique"can shorten the time spent on the the retrieval of IVCF and reduce the intraoperative radiation dose.This technique carries high safety and practicability,the device is simple and it can be manipulated by single physician,which is conducive to clinical application and promotion.(J Intervent Radiol,2024,33:289-294)
9.Mechanism Study of Soufeng Yuchuan Formula on Regulating Airway Remodeling in Asthmatic Rats Based on Epithelial-Mesenchymal Transformation

Objective To explore the mechanism of Suofeng Yuchuan Formula on inhibiting airway remodeling in asthma by regulating epithelial-mesenchymal transformation(EMT).Methods Sixty rats were randomly divided into normal control group,model control group,Dexamethasone group,high-,medium-and low-dose groups of Soufeng Yuchuan Formula,with 10 rats in each group.In addition to the normal control group,the other groups were injected and inhaled with ovalbumin(OVA)to replicate the asthma rat model.The rats in each group were killed after 21 days of administration of the corresponding drug.Hematoxylin-eosin(HE)staining was used to observe the pathological changes of lung tissue.The ultrastructure of epithelial cells in rat lung was observed by transmission electron microscope.The contents of transforming growth factor β1(TGF-β1)and interleukin-17(IL-17)in serum were detected by enzyme-linked immunosorbent assay(ELISA).Real-time fluorescence quantitative polymerase chain reaction(RT-PCR)was used to detect mRNA expressions of TGF-β1,α-smooth muscle actin(α-SMA),E-cadherin and N-cadherin in lung homogenate.Protein expression levels of TGF-β1,α-SMA,E-cadherin and N-cadherin in lung tissue were detected by Western Blot.Results Compared with the normal control group,the serum contents of TGF-β1 and IL-17 in the model control group were significantly increased(P<0.01).The mRNA and protein expressions of TGF-β1,α-SMA and N-cadherin in lung tissues were significantly up-regulated(P<0.01),while the mRNA and protein expressions of E-cadherin were significantly down-regulated(P<0.01).HE staining showed thickening of airway wall and alveolar wall,and a large number of inflammatory cells infiltrated in the visual field,and the inflammation score was significantly increased(P<0.01).Transmission electron microscopy showed that the epithelial cells of lung tissue had severe edema.Local dissolution and thinning of intracellular matrix,and obvious swelling of organelles were observed.Compared with model control group,the contents of TGF-β1 and IL-17 in serum of dexamethasone group and all doses of Soufeng Yuchuan Formula groups were significantly decreased(P<0.05,P<0.01).The mRNA and protein expressions of TGF-β1,α-SMA and N-cadherin were significantly down-regulated(P<0.05,P<0.01),while the mRNA and protein expressions of E-cadherin were significantly up-regulated(P<0.05,P<0.01).The inflammatory cell infiltration of lung tissue and thickening of bronchial wall and alveolar wall were found to be reduced.The inflammation score of dexamethasone group and high-dose group of Suofeng Yuchuan Formula was reduced(P<0.05).Ultrastructure showed that the degree of edema of epithelial cells was significantly reduced,the cell membrane was intact,the intracellular matrix was uniform,and most of the organelles were slightly swollen.Conclusion Soufeng Yuchuan Formula can reduce the level of TGF-β1,thereby improve EMT in lung tissue,and achieve the purpose of preventing and treating airway remodeling in asthma.
10.Qualitative study on the influencing factors of medical insurance-related doctor-patient disputes based on grounded theory

The medical insurance policy has entered into a deep reform and can effectively alleviate the tensions between doctors and patients.However,medical insurance-related doctor-patient disputes still occur during the actual implementation process.Based on the grounded theory research method,semi-structured interviews were conducted to intuitively and realistically describe the experiences of both doctors and patients in medical insurance-related doctor-patient disputes.Nvivo 11 software was used to conduct three-level coding on interview data and extract the reasons that trigger medical insurance-related doctor-patient disputes.in response to the influencing factors of medical insurance-related doctor-patient disputes,four core concepts of "medical staff,patient groups,medical environment,and social environment" were extracted.Additionally,countermeasures to alleviate medical insurance-related doctor-patient disputes were proposed,including the government strengthening overall planning and diversifying policy publicity;hospitals refining services and optimizing the evaluation system for medical staff;improving the professional quality of medical staff and guiding the standardized practice;and transforming patients' medical thinking and improving their policy awareness.
