1.Comparison of long-term prognosis in elderly stageⅠnon-small cell lung cancer patients undergoing lobectomy or segmentectomy: A propensity score matching study

Objective To compare the long-term prognosis of elderly patients with stageⅠnon-small cell lung cancer (NSCLC) after lobectomy or segmentectomy. Methods Data of elderly patients with stageⅠNSCLC between 2010 and 2020 were collected from the SEER database. According to the resection method, patients were divided into a lobectomy group and a segmentectomy group. The overall survival (OS) and lung cancer-specific survival (LCSS) of the two groups were compared by propensity score matching (lobectomy : segmentectomy=2 : 1). Results A total of 9990 patients were included, including 5840 (58.46%) females and 4150 (41.54%) males, with an average age of (70.48±6.47) years. Among them, 9029 patients were in the lobectomy group and 961 patients were in the segmentectomy group. After propensity score matching, a total of 2883 patients were matched, including 1 922 patients in the lobectomy group and 961 patients in the segmentectomy group. There was no statistical difference in baseline data between the two groups (P>0.05). The 10-year OS rate and LCSS rate of the lobectomy group were higher than those of the segmentectomy group (OS: 51.15% vs. 38.35%, P<0.01; LCSS: 79.68% vs. 71.52%, P<0.01). Subgroup analysis showed that the survival advantage of lobectomy was found in patients aged 60-<70 years and ≥80 years; for patients 70-<80 years, there was no statistical difference in OS or LCSS between the two surgical methods (P>0.05). In addition, for patients with tumor diameter ≤2 cm (stages ⅠA1-ⅠA2), lymph node dissection number≥10, and receiving adjuvant radiotherapy/chemotherapy, segmentectomy could also achieve a similar prognosis as lobectomy. Conclusion Overall, for elderly patients with stage ⅠNSCLC, lobectomy can achieve better OS and LCSS. However, individual differences, tumor characteristics, and perioperative treatment plans should be considered comprehensively to determine the surgical method for elderly patients with stageⅠNSCLC.
2.Protective effect of cordycepin on cardiac function in aged rats and its mechanism

Objective:
To investigate the protective effect of cordycepin( Cor) on cardiac function in aged rats and its mechanism.
Methods :
The aged rats were divided into Model group,Cor administration group ( Cor 5,10,20g / kg) ,and young rats were used as Control group ( Control) .The levels of heart rate (HR) ,left ventricular e- jection fraction (LVEF) and fractional shortening (FS) were detected by echocardiography.The levels of creatine kinase MB ( CK-MB) ,myohemoglobin (Mb) ,cardiac troponin Ⅰ (cTnI) ,superoxide dismutase (SOD) ,malon- dialdehyde (MDA) and lactate dehydrogenase ( LDH) were detected by ELISA.The pathological morphology of heart tissue was observed by HE staining. The apoptosis of myocardial cells was observed by TUNEL staining. Western blot detected the protein expression of cysteine aspartic protease 9 ( Caspase 9) ,cleaved cysteine aspartic protease 9 (cleaved Caspase 9) ,cysteine aspartic protease 3 ( Caspase 3 ) ,cleaved cysteine aspartic protease 3 ( cleaved Caspase 3) ,nuclear factor E2 related factor 2 (Nrf2) ,phosphorylation nuclear factor E2 related factor 2 (p-Nrf2) ,quinone oxidoreductase 1 (NQO1) ,heme oxygenase 1 ( HO-1) .
Results :
Compared with the elderly model group,HR , LVEF ,FS increased ,CK-MB ,Mb ,cTnI levels decreased ,MDA ,LDH content decreased, SOD activity increased,cleaved Caspase 9 / Caspase 9,cleaved Caspase 3 / Caspase 3 expression decreased,The ex- pressions of Nrf2,NQO1 and HO-1 were increased,and the pathological morphology of cardiac tissue and myocar- dial apoptosis were improved in Cor groups.
Conclusion
Cor can improve cardiac function of elderly rats,relieve oxidative stress response,inhibit myocardial cell apoptosis and have a certain protective effect on cardiac tissue in- jury in rats,and the mechanism may be related to the promotion of Nrf2 / HO-1 pathway related protein expression.
3.Application of magnetic surgery technique in thoracic surgery

The earliest research of magnetic surgery was the application of magnetic anastomotic device to anastomose the blood vessels. Now, it has been widely used for anastomosis of blood vessels, gastrointestinal tract and biliary tract. The concept of "magnetic surgery" was named firstly by LU Yi in 2010 and magnetic surgery was classified into magnetic anchoring technique, magnetic navigation technique, magnetic compression technique, magnetic tracing technique, and magnetic suspension technique. The applications of magnetic surgery in the field of thoracic surgery mainly include magnetic compression technique, magnetic anchoring technique and magnetic navigation technique. This paper summarizes the application of magnetic surgery in thoracic surgery and prospects its future development in the field of thoracic surgery.
4.Role of penehyclidine in acute organophosphorus pesticide poisoning

BACKGROUND:
Penehyclidine is a newly developed anticholinergic agent. We aimed to investigate the role of penehyclidine in acute organophosphorus pesticide poisoning (OP) patients.
METHODS:
We searched the Pubmed, Cochrane library, EMBASE, Chinese National Knowledge Infrastructure (CNKI), Chinese Biomedical literature (CBM) and Wanfang databases. Randomized controlled trials (RCTs) recruiting acute OP patients were identified for meta-analysis. Main outcomesincluded cure rate, mortality rate, time to atropinization, time to 60% normal acetylcholinesterase (AchE) level, rate of intermediate syndrome (IMS) and rate of adverse drug reactions (ADR).
RESULTS:
Sixteen RCTs involving 1,334 patients were identified. Compared with the atropine-or penehyclidine-alone groups, atropine combined with penehyclidine significantly increased the cure rate (penehyclidine+atropine vs. atropine, 0.97 vs. 0.86, RR 1.13, 95% CI [1.07–1.19]; penehyclidine+atropine vs. penehyclidine, 0.93 vs. 0.80, RR 1.08, 95% CI [1.01–1.15]) and reduced the mortality rate (penehyclidine+atropine vs. atropine, 0.015 vs. 0.11, RR 0.17, 95% CI [0.06–0.49]; penehyclidine+atropine vs. penehyclidine, 0.13 vs. 0.08, RR 0.23, 95% CI [0.04–1.28]). Atropine combined with penehyclidine in OP patients also helped reduce the time to atropinization and AchE recovery, the rate of IMS and the rate of ADR. Compared with a single dose of atropine, a single dose of penehyclidine also significantly elevated the cure rate, reduced times to atropinization, AchE recovery, and rate of IMS.
CONCLUSION
Atropine combined with penehyclidine benefits OP patients by enhancing the cure rate, mortality rate, time to atropinization, AchE recovery, IMS rate, total ADR and duration of hospitalization. Penehyclidine combined with atropine is likely a better initial therapy for OP patients than atropine alone.
5.The immediate implant and immediate restoration in the anterior maxilla with the digital technique: case report and literature review

Objective :
To describe a novel workflow for the anterior dental esthetics in immediate implant and immediate aesthetic restoration with digital technique.
Methods :
Before the implant placement, a intra-oral optical scan(IOS) combined with CBCT was performed to virtually design the 3D implant position and restoration. A surgical guide and an individualized Ti abutment restoration was CAD/CAM-fabricated out of the PMMA-based in a fully digital workflow and seated at the stage of minimal invasive surgery.
Results :
The technique achieved immediate implant and immediate restoration after tooth extraction. Immediate restoration of temporary crown could be completed immediately in second patient visit after the operation, finally realized the aesthetic implantation with preserved soft tissue contour.
Conclusin
The fully digital technique changed the conventional workflow which achieves more efficiency and better aesthetic effect.
