1.The relationship of serum miR-124,miR-134 and CT regional assessment grading of pia mater collateral,core infarct volume and prognosis in patients with acute ischemic stroke

		                        			Objective To explore the relationship of serum micro ribonucleic acid(miR)-124,miR-134 and CT regional assessment grading of pia mater collateral,core infarct volume and prognosis in patients with acute ischemic stroke.Methods A total of 192 patients with acute ischemic stroke(observation group)and 100 healthy volunteers(control group)were selected.The serum miR-124 and miR-134 were compared between the two groups.At the same time,the serum miR-124,miR-134 and the score of pia mater collateral in CT region of patients with different clinical characteristics in the observation group were analyzed.Results The relative expression levels of serum miR-124 and miR-134 in the observation group were significantly higher than those in the control group(P<0.05).The relative expression serum miR-124 and miR-134 in patients with core infarct volume≥50 mL in the observation group were significantly higher than those in patients with core infarct volume<50 mL(P<0.05).In the observation group,the score of pia mater collateral in CT region of patients with core infarct volume ≥50 mL was significantly lower than that of patients with core infarct volume<50 mL(P<0.05).The relative expression levels of serum miR-124 and miR-134 were positively correlated with the core infarct volume(r=0.378 and 0.466,P<0.05),and the score of pia mater collateral in CT region was negatively correlated with the core infarct volume(r=-0.463,P<0.05).The relative expression levels of serum miR-124 and miR-134 in patients with poor prognosis were significantly higher than those in patients with good prognosis(P<0.05),while the score of pia mater collateral in CT region was significantly lower than that in patients with good prognosis(P<0.05).Conclusion The serum miR-124 and miR-134 in patients with acute ischemic stroke are up-regulated,and miR-124 and miR-134 are positively correlated with the core infarct volume,while the score of pia mater collateral in CT region is negatively correlated with the core infarct volume.At the same time,miR-124,miR-134,and the score of pia mater collateral in CT region may be related to the prognosis of patients.

		                        			Ulcerative colitis(UC)is a chronic im-mune-mediated disease characterized by diffuse mucosal inflammation and clinical symptoms of ab-dominal pain,diarrhea,mucopurulent bloody stool and tenesmus.Due to its complex etiology,difficult treatment and poor prognosis,UC has been listed as one of the modern refractory diseases by the World Health Organization.MicroRNAs(microR-NAs)regulate various biological processes such as cell growth,differentiation,and development by regulating gene expression.Studies have shown that miRNA is involved in the pathogenesis of UC through various pathways such as intestinal muco-sal barrier homeostasis disorder,intestinal flora im-balance and excessive differentiation of immune cells,and is expected to become a new target for clinical treatment of UC.In order to better under-stand the role of miRNA in the pathogenesis of UC and provide new research directions for the treat-ment of UC,this article reviews the correlation be-tween miRNA and the pathogenesis of UC,the po-tential of miRNA as a biomarker of UC,and the treatment strategies of miRNA in UC in recent years.
		                        		
		                        		
		                        		
		                        	
4.Clinical efficacy of staged individualized treatment of severe clubfoot deformity

		                        			Objective To investigate the clinical efficacy of staged individualized surgical method for treating neuromuscular origin severe clubfoot complicating complex lower limb deformity.Methods Ten ca-ses of neurogenic severe clubfoot deformity treated in the Affiliated Hospital of Guizhou Medical University from May 2021 to June 2023 were collected as the research subjects.In the first stage,the soft-tissue release,tendon transfer combined with Ilizarov technique were adopted to correct the clubfoot deformity,and in the second stage,according to the patients' postoperative gait and the degree of lower limb deformity,the patients were given individualized and appropriate osteotomy assisted by the increase of Ilizarov external frame length-ening,supra-ankle osteotomy,and distal femoral osteotomy.The clinical results of treatment in the patients were observed.The American Orthopaedic Association Division of Foot and Ankle Surgery (AOFAS) scores and visual analog scale (VAS) scores were compared between before operation and 1,3,6,12 and 18 months after operation.The lower extremity biodynamic line related imaging parameters were compared between be-fore and 18 months after operation.Results All patients successfully completed the surgery without nerve or vascular injury during and after operation and there were no serious complications such as pinhole or incision infection.All patients were followed up for 16-24 months.The ankle joint AOFAS score at each time point after operation was increased compared with the physical examination before operation (P<0.05),and the VAS score was decreased compared with the physical examination before operation (P<0.05).The lower ex-tremity biodynamic line related imaging parameters at 18 months after operation were significantly improved compared with before operation (P<0.05).Conclusion Adopting the staged individualized surgical method for the treatment of neuromuscular origin severe clubfoot complicating complex deformity of the lower limbs could achieve more satisfactory and relatively ideal therapeutic effect.
		                        		
		                        		
		                        		
		                        	
5.WDR62-deficiency Causes Autism-like Behaviors Independent of Microcephaly in Mice.

		                        			Brain size abnormality is correlated with an increased frequency of autism spectrum disorder (ASD) in offspring. Genetic analysis indicates that heterozygous mutations of the WD repeat domain 62 (WDR62) are associated with ASD. However, biological evidence is still lacking. Our study showed that Wdr62 knockout (KO) led to reduced brain size with impaired learning and memory, as well as ASD-like behaviors in mice. Interestingly, Wdr62 Nex-cKO mice (depletion of WDR62 in differentiated neurons) had a largely normal brain size but with aberrant social interactions and repetitive behaviors. WDR62 regulated dendritic spinogenesis and excitatory synaptic transmission in cortical pyramidal neurons. Finally, we revealed that retinoic acid gavages significantly alleviated ASD-like behaviors in mice with WDR62 haploinsufficiency, probably by complementing the expression of ASD and synapse-related genes. Our findings provide a new perspective on the relationship between the microcephaly gene WDR62 and ASD etiology that will benefit clinical diagnosis and intervention of ASD.

		                        			Chronic refractory wound (CRW) is one of the most challengeable issues in clinic due to complex pathogenesis, long course of disease and poor prognosis. Experts need to conduct systematic summary for the diagnosis and treatment of CRW due to complex pathogenesis and poor prognosis, and standard guidelines for the diagnosis and treatment of CRW should be created. The Guideline forthe diagnosis and treatment of chronic refractory wounds in orthopedic trauma patients ( version 2023) was created by the expert group organized by the Chinese Association of Orthopedic Surgeons, Chinese Orthopedic Association, Chinese Society of Traumatology, and Trauma Orthopedics and Multiple Traumatology Group of Emergency Resuscitation Committee of Chinese Medical Doctor Association after the clinical problems were chosen based on demand-driven principles and principles of evidence-based medicine. The guideline systematically elaborated CRW from aspects of the epidemiology, diagnosis, treatment, postoperative management, complication prevention and comorbidity management, and rehabilitation and health education, and 9 recommendations were finally proposed to provide a reliable clinical reference for the diagnosis and treatment of CRW.
		                        		
		                        		
		                        		
		                        	
7.Preliminary MRI evaluation of anterior repositioning splint in treatment of disc displacement with reduction of temporomandibular joint.

		                        			Objective: MRI images were used to study the efficacy of anterior repositioning splint (ARS) in the treatment of different types of disc displacement with reduction (DDWR) in temporomandibular joint. Methods: From September 2020 to December 2021, 26 patients with DDWR were enrolled in the Department of Oral and Maxillofacial Surgery, the First Affiliated Hospital of Zhengzhou University. There were 5 males and 21 females with an average age of (20.8±5.8) years. ARS was used for 3-6 months. The changes of joint clicking, opening type and joint pain before and after treatment were compared. The changes of disc position, disc-condyle angle and condylar bone mass before and after treatment were compared by MRI. Paired t-test was performed on the disc-condyle angle before and after treatment, Fisher's exact test was performed on the change of disk position, and other count data were expressed as rate (%). Results: After ARS treatment, the effective rates of joint clicking,abnormal opening, joint pain and disc displacement were 97%(35/36), 14/18, 7/9 and 95%(36/38). MRI analysis found that there was a significant difference between the disc position before and after treatment (P<0.001), MRI analysis showed that the anterior disc displacement (48%, 25/52) and the anterolateral disc displacement (17%, 9/52) were the most common before treatment. In contrast, the normal superior disc (75%, 39/52) and the anterior disc displacement (17%, 9/52) were the most common after treatment, no significant changes were seen after treatment in the anteromedial disc displacement. The disc-condylar angle was (36.09±19.02) ° before ARS treatment and (3.94±10.12) ° after ARS treatment(t=9.23, P<0.001). After treatment, 46% (12/16) of the patients showed new bone formation, and the height of the condyle recovered. Conclusions: The clinical efficacy of ARS in the treatment of anterior disc displacement and anterolateral displacement of temporomandibular joint is remarkable, which can restore the disc-condylar relationship of most patients with indications.

8.Risk factors for premature proximal femur physeal closure after femoral neck fractures in children treated surgically

		                        			Objective:To identify the risk factors for premature proximal femur physeal closure (PPC) in children treated surgically for femoral neck fractures.Methods:Data of 106 children with an open triradiate cartilage who were treated surgically for a femoral neck fracture were retrospectively analyzed. Age, gender, laterality, mechanism of injury, the type of fracture, initial displacement, time to reduction, fixation method, whether the implant crossed the physeal plate, reduction method, reduction quality and development of femoral head avascular necrosis (AVN) were collected. PPC of the proximal femur was assessed through postoperative 6-12 months radiographs.Results:A total of 106 patients with an open triradiate cartilage were followed up, with an average duration of 20.4±13.3 months (range, 6-86 months). The overall rate of PPC following paediatric femoral neck fractures treated surgically was 36.8% (39/106). Among the 39 patients with PPC, 25 were males and 14 were females; the average age at the time of injury was 9.7±3.6 years (range, 3-15 years); 23 patients were involved in left hips and 16 were in right; the mechanisms of injury included motor vehicle accident in 5 fractures, falling injury in 21, sports-related injury in 12 and other causes in 1 fracture; two hips were Delbet type I, and 26 hips and 11 hips were Delbet type II and III, separately; type II and III of initial displacement were involved in 26 and 13 patients, separately; the mean duration from injury to surgery was 3.3±2.8 d (range, 1-14 d); 2 hips were treated with Kirschner wires, 35 hips with screw fixation, and the remaining 2 hips underwent screw and plate fixation; 2 hips had the hardware crossing the proximal femoral growth plate, and the remaining 37 hips didn’t; 15 hips were treated by closed reduction and internal fixation, and the other 24 hips underwent open reduction and internal fixation; anatomical reduction was achieved in 14 patients, acceptable reduction in 24 hips and unacceptable reduction in 1 hip; 24 hips developed AVN at the latest follow-up and the remaining 15 hips didn’t. Statistical analysis indicated that age ( t=3.875, P< 0.001), the severity of initial displacement ( Z=-2.118, P=0.034) and the rate of AVN ( χ2=42.280, P< 0.001) in patients with PPC were significantly higher than those in patients without; Logistic regression analysis confirmed age ( OR=1.288, P=0.011) and AVN ( OR=40.336, P< 0.001) as risk factors for PPC. ROC curve analysis indicated 10 years was the cut off age to significantly increase the rate of PPC. The rate of PPC in patients aged over 10 years (63.6%, 21/33) was significantly higher than that (24.7%, 18/73) in those aged less than 10 years ( χ2=14.848, P< 0.001). 9.Expression levels of high mobility group box protein 1, tumor necrosis factor-α and interleukin-6 and their clinical significance in elderly patients with viral pneumonia

		                        			Objective:To investigate high mobility group protein 1(HMGB1), tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)levels and their clinical significance in elderly patients with viral pneumonia.Methods:One hundred and sixty elderly patients with viral pneumonia admitted to the Sixth Hospital Affiliated to Anhui Medical University were enrolled as research subjects.In addition, 40 elderly people who underwent regular physical examination were considered as the control group.Patients with viral pneumonia were divided into the low-risk group, middle-risk group and high-risk group according to CURB-65 scores and pneumonia severity index(PSI)scores.HMGB1, TNF-α and IL-6 levels were compared between different groups.The correlations of CURB-65 scores and PSI scores with HMGB1, TNF-α, IL-6 levels were analyzed.Multivariate Logistic regression analysis was used to examine influencing factors for the severity of viral pneumonia in elderly patients.Results:HMGB1, TNF-α and IL-6 levels were higher in research subjects than in the control group.As the severity of viral pneumonia increased, so did HMGB1, TNF-α and IL-6 levels(all P<0.05). HMGB1, TNF-α and IL-6 levels in the severe viral pneumonia group were significantly higher than those in the non-severe viral pneumonia group( P<0.05). HMGB1, TNF-α and IL-6 levels were positively correlated with CURB-65 scores and PSI scores(CURB-65 score: r=0.463, 0.392 and 0.497, P=0.015, 0.003 and 0.025; PSI score: r=0.596, 0.515 and 0.381, P=0.007, 0.011 and 0.009). HMGB1, TNF-α and IL-6 levels were influencing factors for the severity of viral pneumonia in elderly patients( OR=1.344, 1.422 and 1.351, P=0.006, 0.015 and 0.009). Conclusions:HMGB1, TNF-α and IL-6 levels are closely correlated with the severity of viral pneumonia and are helpful for early assessment of viral pneumonia.
		                        		
		                        		
		                        		
		                        	
10.Effects of mobile health management system on dietary behavior of patients with coronary heart disease

		                        			Objective:To explore the effect of mobile health (mHealth) management system on dietary behavior of patients with coronary heart disease (CHD) .Methods:Using the convenient sampling method, CHD patients who were hospitalized in Department of Cardiology of Beijing Anzhen Hospital and Beijing Chaoyang Hospital of Capital Medical University were selected as the research objects from June 2019 to January 2020. The research objects were randomly divided into the intervention group and the control group using SAS software. Finally, 68 cases in the intervention group and 66 cases in the control group completed the study. The intervention group received mhealth management, while the control group received routine management, and they were followed up to 1 month after discharge. The proportion of patients with the low-salt and low-oil diet, the intake of salt, oil, staple food, vegetables, fruits, fish, shrimp, poultry and eggs, BMI and blood pressure were compared between the two groups.Results:At 1 month follow-up, the intervention group was better than the control group in terms of low salt and low oil diet, salt intake, oil intake and vegetable intake compliance, and the differences were statistically significant ( P<0.05) . At 1 month follow-up, there were no statistically significant differences in BMI and blood pressure between two groups ( P>0.05) . Conclusions:The mobile health system can effectively increase the proportion of patients with a low-salt and low-oil diet, and improve the intake compliance of salt, edible oil and vegetables in patients with CHD. In future studies, the intervention time should be extended to further explore the long-term intervention effect of mobile health and give full play to the role of mobile health in continuous behavior management of patients with CHD after discharge.
