1.Protective effects and mechanism of Dachengqi decoction on intestinal septic mice

OBJECTIVE To study the protective effects of Dachengqi decoction (DCQD) on intestinal septic mice, and to explore the possible mechanisms from the Toll-like receptor 4(TLR4)/myeloid differentiation factor 88(MyD88) signaling pathway. METHODS The SPF male C57BL/6J mice were randomly divided into Sham group, Sham+DCQD-H group, model (CLP) group, DCQD-L group, DCQD-H group and Positive group. The model of intestinal sepsis was established by cecal ligation and puncture in CLP group, DCQD-L group, DCQD-H group and Positive group. Three days before the operation and seven days after the operation, DCQD-L group and DCQD-H group were given DCQD intragastrically at 4, 8 g/kg (calculated by crude drug), respectively. Positive group was given ulinastatin intraperitoneally 2 h before operation and 7 d after the operation (at 50 000 U/kg). In Sham group and Sham+DCQD-H group, only cecum of mice was exposed without ligation and puncture. Sham+DCQD- H group was given DCQD intragastrically (8 g/kg,calculated by crude drug) 3 days before the operation and 7 days after the operation. Both the Sham group and CLP group were given normal saline 0.2 mL intragstrically and intraperitoneally each day, for 10 consecutive days. After the operation, the severity of sepsis was assessed, and the 7 d survival rate of mice was assessed. One hour after the last medication, the levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in serum and ileum of mice were determined; the pathological and morphological changes of mice’s liver, lung, kidney and ileum were observed; mRNA expressions of the TLR4 and MyD88 in ileum were tested. RESULTS Compared with CLP group, sepsis score, the levels of TNF-α and IL-6 in serum and ileum (except for IL-6 in ileum of DCQD-L group), damage score of the liver, lung, kidney and ileum, mRNA expressions of TLR4 and MyD88 in ileum were all decreased significantly in DCQD-L group and DCQD-H group (P<0.05 or P<0.01), while 7 d survival rate (except for DCQD-L group) was increased significantly (P<0.05). The damage to liver tissue in mice was significantly improved, and inflammation infiltration and apoptosis were reduced; lung tissue damage had been alleviated, with varying degrees of improvement in alveolar atrophy, bleeding and edema; the renal tissue damage was improved and weakened dilation of renal tubular lumen was weakened; the damage and edema of ileal tissue were significantly improved. CONCLUSIONS DCQD may exert a protective role on intestinal septic model mice. The mechanism may be related to the inhibition of systemic inflammation, the reduction of multiple organ damage, and down-regulation of TLR4/MyD88 signaling pathway.
2.Analysis of the characteristics of HIV-1 molecular transmission network among people aged 50 years and above in Pengzhou city of Sichuan province

Objective:To describe the characteristics of molecular transmission network of newly diagnosed HIV-1 infected patients, analyze their risk factors related to network access and provide a scientific basis for precise prevention of HIV infection.Methods:For 340 blood samples collected from confirmed HIV-1 infection cases aged ≥50 years in Pengzhou city of Sichuan province from April 2019 to August 2021, nested PCR amplification was used to amplify, clean up and splice clips the pol gene region. The phylogenetic tree was constructed by multi-sequence comparison to distinguish subtypes, and the pairwise genetic distance was calculated. When the genetic distance threshold was 0.90%, the number of clusters was the largest (41), and the molecular transmission network was constructed.The χ2 test and logistic regression analysis were performed.The software SPSS 19.0 was used for statistical analysis. Results:A total of 340 samples were successfully amplified (97.06%, 330/340) in 330 samples. 6 HIV-1 subtypes identified, including:CRF01_AE(56.67%,187/330), CRF07_BC(27.88%,92/330), B(11.21%,37/330), CRF08_BC(3.33%,11/330), CRF55_01B(0.61%,2/330) and C(0.30%,1/330).The network entry rate was 58.79% (194/330).The results of logistic regression analysis of the risk factors of HIV-1 molecular transmission network in the research subjects showed that compared with illiteracy, junior high school ( OR=0.35, 95% CI:0.13-0.97) and high school/technical secondary school ( OR=0.14, 95% CI: 0.02-0.97) had lower possibility of network entry. Compared with farmers, unknown occupations ( OR=0.40,95% CI: 0.17-0.95) are less likely to enter the network .Compared with CRF01_AE, CRF07_BC ( OR=0.20, 95% CI: 0.11-0.35) and CRF08_BC subtype ( OR=0.09, 95% CI: 0.02-0.45) were less likely to enter the network. Conclusions:The sources of AIDS transmission among middle-aged and elderly people of rural areas are diversified in Pengzhou city of Sichuan province. AIDS intervention should focus on middle-aged and elderly farmers with low educational level, and strengthen detection and traceability investigation.

The expert consensus reviews current literatures and provides clinical practices for thermal ablation of GGN. The main contents include: (1) clinical evaluation of GGN, (2) procedures, indications, contraindications, outcomes evaluation and related complications of thermal ablation for GGN and (3) future development directions.

4.Effect of dexmedetomidine on pyroptosis in mice with acute renal injury induced by endotoxin and the relationship with miRNA-223-3p

Objective:To evaluate the effect of dexmedetomidine on pyroptosis in mice with acute renal injury induced by endotoxin and the relationship with miRNA-223-3p.Methods:Thirty-two clean-grade healthy male ICR mice, aged 8-12 weeks, weighing 20-25 g, were divided into 4 groups ( n=8 each) using the random number table method: control group (group C), lipopolysaccharide (LPS) group (group L), LPS plus dexmedetomidine group (group LD), and LPS plus dexmedetomidine plus atipamezole group (group LDT). The model of acute renal injury induced by endotoxin was established by intraperitoneal injection of LPS 400 μg/kg, followed by intraperitoneal injection of LPS 10 mg/kg 8 h later.Dexmedetomidine 40 μg/kg was intraperitoneally injected once every 2 h for 3 times in total starting from 30 min after establishing the model in group LD.Atipamezole 750 μg/kg was intraperitoneally injected immediately after establishing the model, and 30 min later dexmedetomidine 40 μg/kg was intraperitoneally injected once every 2 h for 3 times in total in group LDT.The equal volume of normal saline was intraperitoneally injected in group C. Blood samples were collected from the heart at 24 h after establishing the model, and serum creatinine (Cr) and blood urea nitrogen (BUN) concentrations were measured with an automatic biochemical analyzer.The animals were sacrificed and the left kidney tissues were obtained for microscopic examination of pathological changes after HE staining (with a light microscope) and for determination of the expression of caspase-1 p20, NOD-like receptor thermoprotein structural domain-related protein 3 (NLRP3) and ASC protein and mRNA (by quantitative real-time polymerase chain reaction and Western blot), contents of interleukin-1beta (IL-1β) and IL-18 (by enzyme-linked immunosorbent assay), and rate of pyroptosis in renal cortical cells (by TUNEL). Results:Compared with group C, the concentrations of serum Cr and BUN were significantly increased, the expression of NLRP3, caspase-1 p20 and ASC protein and mRNA in the renal tissues was up-regulated, the contents of IL-1β and IL-18 were increased, the rate of pyroptosis in renal cortical cells was increased ( P<0.05), no significant change was found in the expression of miRNA-223-3p ( P>0.05), and pathological changes of kidney were accentuated in group L. Compared with group L, the concentrations of serum Cr and BUN were significantly decreased, the expression of NLRP3, caspase-1 p20 and ASC protein and mRNA in the renal tissues was down-regulated, the contents of IL-1β and IL-18 were decreased, the rate of pyroptosis in renal cortical cells was decreased, the expression of miRNA-223-3p was up-regulated ( P<0.05), and pathological changes of kidney were attenuated in group LD.Compared with group LD, the concentrations of serum Cr and BUN were significantly increased, the contents of IL-1β and IL-18 were increased, the expression of NLRP3, caspase-1 p20 and ASC protein and mRNA in the renal tissues was up-regulated, the rate of pyroptosis in renal cortical cells was increased, the expression of miRNA-223-3p was down-regulated ( P<0.05), and the pathological changes of kidney were accentuated in group LDT. Conclusion:The mechanism by which dexmedetomidine reduces acute renal injury may be related to up-regulating the expression of miRNA-223-3p and inhibiting pyroptosis in mice.
5.Epidemiological characteristics of children with hand, foot, and mouth disease and herpetic angina in Wenzhou City and their correlation with meteorological factors

Objective:To analyze the epidemiological characteristics of children with hand, foot, and mouth disease (HFMD) and herpetic angina (HA) in Wenzhou City and the influence of meteorological factors on the pathogenesis, and to provide basis for early warning and disease prevention.Methods:A total of 62 809 children diagnosed with HFMD and 56 005 with HA in the 2nd Affiliated Hospital and Yuying Children′s Hospital of Wenzhou Medical University during 2012 to 2017 were enrolled. The meteorological factors during 2012 to 2017 were collected monthly. The data were analyzed using Spearman rank correlation and multivariant linear stepwise regression model.Results:The number of cases of HFMD and HA began to rise in the spring, and decreased after reaching the peak from May to July. Then there was a secondary peak from September to December, and the incidence decreased significantly in winter. Univariate correlation analysis showed that the effects of different meteorological factors on HFMD and HA were basically the same. Multivariant linear stepwise regression analysis showed that there was a positive correlation between monthly mean temperature (X) and the number of HFMD cases. The regression equation was Y=-161.450+ 53.828X ( F=22.250, P<0.01). Monthly mean relative humidity (X 1) and temperature (X 2) were positively associated with the number of HA cases. The regression equation was Y=-3 521.196+ 46.814X 1+ 41.762X 2 ( F=18.351, P<0.01). Conclusions:The trends of onset time of HFMD and HA are similar. The meteorological factors are closely related to HFMD and HA, and their incidence trends are significantly affected by meteorological changes.

BACKGROUND:Previous research indicated that iron-fluorouracil-phenanthroline complex has good antitumor activity in vitro, which can inhibit the proliferation of human cancer cels. OBJECTIVE:To detect the antitumor activity and toxicity of iron-fluouracil-phenanthroline complex, [Fe(5-Fu)2(Phen)SO4],in vivo. METHODS:A total of 40 Kunming mice were randomly divided into four groups, which were intraperitoneally injected with 72, 102.9, 147, 210 mg/kg [Fe(5-Fu)2(Phen)SO4] and the half lethal dose of the complex was detected. One day after the establishment of mouse S180 sarcoma models, the model mice were randomly divided into eight groups, and administered with the intraperitoneal injection of 15 mg/kg (low dose group), 30 mg/kg (middle dose group), 60 mg/kg (high dose group) [Fe(5-Fu)2(Phen)SO4], normal saline (negative control group), cisplatin (positive control group), 5-fluorouracil, iron-salt and phenanthroline, respectively. The injection was done once a day, lasting for 7 days. The weight of sarcomas, body weight, the main organ coefficient and histopathological changes of the main organs were detected. RESULTS AND CONCLUSION: The half lethal dose of [Fe(5-Fu)2(Phen)SO4] was 103.9 mg/kg. Compared with the negative control group, high dose group, positive control group and 5-fluorouracil could significantly inhibit the growth of the tumor (P< 0.05 orP< 0.01), and the effect of high dose group was the most obvious (P < 0.01). Compared with cisplatin, 60mg/kg [Fe(5-Fu)2(Phen)SO4] had a weaker inhibitory effect on the kidney, but higher inhibitory effect on the liver, spleen and thymus, indicating the complex has a lower nephrotoxicity, but stronger immunotoxicity and hepatotoxicity than cisplatin.

Objective To evaluate the effects of multi-functional self-help lower extremity training band on elderly patients after hip fracture surgery. Methods A total of 120 elderly patients after hip fracture surgery were randomly divided into two groups experimental group (n=60) and control group (n=60). The patients of two groups were treated with routine care, but patients of experimental group also received functional exercise using multi-functional self-help lower extremity training. The compliance of functional exercise were compared between two groups, and the effects of postoperative rehabilitation exercise were evaluated. Results The exercise effects of knee joint and hip joint in experimental group were significantly better than those in control group (χ2 =14. 90,13. 89;P<0. 05). The patients′compliance of functional exercise in the experimental group was significantly higher than that in the control group (P<0. 01). Conclusions The using of multi-functional self-help lower extremity training band can improve the compliance of functional exercise and the effects of rehabilitation on elderly patients after hip fracture surgery, and can promote the progress of rehabilitation.
10.The effect of TGP assisted treatment of SLE on the expression of CD4+CD25+T in patient peripheral blood

Objective to investigate the effect of TGP assisted treatment of SLE on the expression of CD4+CD25+ T in patient peripheral blood .Methods flow cytometry was used to detect the peripheral blood CD4+ CD25+ T cells in healthy group ,routine group and TGP group .Results The expression rate of CD4+CD25+ T cells in SLE patients was (6 .15 ± 1 .21)% ,and that of the healthy controls was (12 .30 ± 1 .78)% .The expression rate of CD4+CD25+ T cells in active SLE patients and healthy controls were significantly different (t=22 .03 ,P<0 .05) .In the routine group ,the expression rate of peripheral blood CD4+CD25+ T cells before and after treatment were significantly different (t= 12 .30 ,P<0 .05);in the TGP group ,the expression rate of peripheral blood CD4+ CD25+ T cells before and after treatment were significantly different ,too (t=16 .68 ,P<0 .05) .The expression rate of periph‐eral blood CD4+CD25+ T cells in routine group and TGP group were (9 .34 ± 1 .37)% and (11 .49 ± 1 .14)% respectively ,and the difference was statistically significant (t=6 .46 ,P<0 .05) .Conclusion The expression rate of CD4+ CD25+ T cells in active SLE patients was significantly lower than that of healthy controls ;the expression rate of CD4+CD25+ T cells in SLE patients increased significantly after treatment .The TGP treatment may work on CD4+CD25+ T cells .
