1.Effects of Sushen Jiubao decoction on Th1/Th2 factors in patients with acute exacerbation of bronchial asthma

Objective To observe the clinical efficacy of Sushen Jiubao decoction in the treatment of acute exacerbation of bronchial asthma and its effect on Th1/Th2 factors.Methods Ninety-eight patients with acute exacerbation of bronchial asthma treated in Sishengbianji Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from February 2022 to May 2023 were selected and divided into treatment group and control group according to random number table method,with 49 cases in each group.The control group was given conventional treatment such as oxygen inhalation and atomization,and the treatment group was additionally treated with Sushen Jiubao decoctionon based on the treatment of control group.The clinical efficacy,TCM syndrome score,pulmonary function indicators[forced expiratory volume in first second(FEV1),peak expiratory flow(PEF)],and laboratory indicators[interleukin(IL)-4,IL-13,interferon-γ(IFN-γ),immunoglobulin E(IgE)].Results The total effective rate of treatment group was significantly higher than that of control group(χ2=9.287,P=0.010).After treatment,the TCM syndrome scores,serum IL-4,IL-13 and IgE levels of two groups were significantly lower than before treatment,and FEV1,PEF and IFN-γ were significantly higher than before treatment(P<0.05).The TCM syndrome score,serum IL-4,IL-13 and IgE levels in treatment group were significantly lower than those in control group,and FEV1,PEF and IFN-γ were significantly higher than those in control group(P<0.05).Conclusion Sushen Jiubao decoction can significantly improve clinical efficacy,reduce the level of inflammatory factors,and improve pulmonary function in the treatment of acute exacerbation of bronchial asthma.
2.The effect of Ba Duan Jin on the balance of community-dwelling older adults: a cluster randomized control trial

Objective:To assess the effectiveness of a 6-month Ba Duan Jin exercise program in improving the balance of community-dwelling older adults.Methods:A two arms, parallel-group, cluster randomized controlled trial was conducted in 1 028 community residents aged 60-80 years in 40 communities in 5 provinces of China. Participants in the intervention group (20 communities, 523 people) received Ba Duan Jin exercise 5 days/week, 1 hour/day for 6 months, and three times of falls prevention health education, and the control group (20 communities, 505 people) received falls prevention health education same as the intervention group. The Berg balance scale (BBS) score was the leading outcome indicator, and the secondary outcome indicators included the length of time of standing on one foot (with eyes open and closed), standing in a tandem stance (with eyes open and closed), the closed circle test, and the timed up to test.Results:A total of 1 028 participants were included in the final analysis, including 731 women (71.11%) and 297 men (28.89%), and the age was (69.87±5.67) years. After the 3-month intervention, compared with the baseline data, the BBS score of the intervention group was significantly higher than the control group by 3.05 (95% CI: 2.23-3.88) points ( P<0.001). After the 6-month intervention, compared with the baseline data, the BBS score of the intervention group was significantly higher than the control group by 4.70 (95% CI: 4.03-5.37) points ( P<0.001). Ba Duan Jin showed significant improvement ( P<0.05) in all secondary outcomes after 6 months of exercise in the intervention group compared with the control group. Conclusions:This study showed that Ba Duan Jin exercise can improve balance in community-dwelling older adults aged 60-80. The longer the exercise time, the better the improvement.
3.Research progress of traditional external preparations and novel nanoformulations of Arnebiae Radix

Arnebiae Radix is used widely in TCM external treatment. It has obvious curative effect on skin diseases, wound infection and local inflammation, and is used to treat water and fire burns, skin ulcers, eczema, psoriasis, vitiligo and atopic dermatitis, etc. The clinical and commercial preparations mainly include ointment, liniment and suppository. Modern research has proved that microcapsules, nano-micelles, nanofiber membranes, nanogels and other novel nanoformulations can significantly improve the stability of drug-effective substances, improve local drug concentration and targeting, and perform sound drug release properties in vitro. This paper reviews the variety and application of Arnebiae Radix traditional preparations for external use and the research progress of novel nanoformulations of Arnebiae Radix, from which we prospect to provide some valuable references for the future application and development of Arnebiae Radix external preparations.
4.Inhibitory Effect of Gancao Fuzitang on Bone Destruction in Collagen-induced Arthritis Mice by Regulating NF-κB Signaling Pathway

ObjectiveTo explore the mechanism of Gancao Fuzitang (GCFZ)in inhibiting the bone destruction of collagen-induced arthritis (CIA) model in mice. MethodThirty male DBa/1J mice were randomly divided into normal group, CIA group, low-dose GCFZ group (GCFZ-L, 2.4 g·kg-1), high-dose GCFZ group (GCFZ-H, 4.8 g·kg-1), and methotrexate group (MTX, 1 mg·kg-1), with six mice in each group. The CIA model was induced by secondary immunization method. The arthritis index of mice in each group was observed and recorded, and the histopathological changes in ankle joint were observed by hematoxylin-eosin (HE) staining. The damage to ankle cartilage was detected by safranin O-fast green staining. Micro-CT scanning was used to detect the bone destruction of ankle joint, and the expression of nuclear factor-κB p65 (NF-κB p65), p-NF-κB p65, inhibitory-κB kinase α/β (IKKα/β), and p-IKKα/β was observed by immunohistochemical staining. ResultCompared with the normal group, the CIA group showed manifest joint swelling and increased arthritis index score (P<0.01). Compared with the CIA group, the groups with drug intervention could inhibit joint swelling and reduce arthritis index score (P<0.05, P<0.01). As revealed by HE staining and safranine O-green staining, compared with the CIA group, the groups with drug intervention could inhibit synovial invasion and reduce the destruction of articular cartilage. Micro-CT scanning analysis showed that compared with the CIA group, the GCFZ-H group and the MTX group showed reduced bone destruction scores (P<0.01). The immunohistochemical results showed that compared with the normal group, the CIA group showed increased optical density values of NF-κB p65, p-NF-κB p65, IKKα/β, and p-IKKα/β(P<0.01). Compared with the CIA group, the GCFZ-H group and the MTX group showed reduced optical density values of NF-κB p65, p-NF-κB p65, IKKα/β, and p-IKKα/β(P<0.05,P<0.01). In the GCFZ-L group, only the NF-κB p65 optical density value decreased(P<0.01). ConclusionGCFZ may inhibit bone destruction in CIA mice by regulating the NF-κB signaling pathway.
5.Analysis of the characteristics of HIV-1 molecular transmission network among people aged 50 years and above in Pengzhou city of Sichuan province

Objective:To describe the characteristics of molecular transmission network of newly diagnosed HIV-1 infected patients, analyze their risk factors related to network access and provide a scientific basis for precise prevention of HIV infection.Methods:For 340 blood samples collected from confirmed HIV-1 infection cases aged ≥50 years in Pengzhou city of Sichuan province from April 2019 to August 2021, nested PCR amplification was used to amplify, clean up and splice clips the pol gene region. The phylogenetic tree was constructed by multi-sequence comparison to distinguish subtypes, and the pairwise genetic distance was calculated. When the genetic distance threshold was 0.90%, the number of clusters was the largest (41), and the molecular transmission network was constructed.The χ2 test and logistic regression analysis were performed.The software SPSS 19.0 was used for statistical analysis. Results:A total of 340 samples were successfully amplified (97.06%, 330/340) in 330 samples. 6 HIV-1 subtypes identified, including:CRF01_AE(56.67%,187/330), CRF07_BC(27.88%,92/330), B(11.21%,37/330), CRF08_BC(3.33%,11/330), CRF55_01B(0.61%,2/330) and C(0.30%,1/330).The network entry rate was 58.79% (194/330).The results of logistic regression analysis of the risk factors of HIV-1 molecular transmission network in the research subjects showed that compared with illiteracy, junior high school ( OR=0.35, 95% CI:0.13-0.97) and high school/technical secondary school ( OR=0.14, 95% CI: 0.02-0.97) had lower possibility of network entry. Compared with farmers, unknown occupations ( OR=0.40,95% CI: 0.17-0.95) are less likely to enter the network .Compared with CRF01_AE, CRF07_BC ( OR=0.20, 95% CI: 0.11-0.35) and CRF08_BC subtype ( OR=0.09, 95% CI: 0.02-0.45) were less likely to enter the network. Conclusions:The sources of AIDS transmission among middle-aged and elderly people of rural areas are diversified in Pengzhou city of Sichuan province. AIDS intervention should focus on middle-aged and elderly farmers with low educational level, and strengthen detection and traceability investigation.
6.Influencing factors on the death of HIV/AIDS patients treated with antiviral treatment in Butuo county, Liangshan Yi Autonomous Prefecture, 2010-2019

Objective:To understand influencing factors on the deaths of HIV/AIDS patients receiving antireviral treatment in Butuo county of Liangshan Yi Autonomous Prefecture (Liangshan) from 2010 to 2019, to provide data for drug replacement and sustainable antiviral treatment strategy.Methods:A matched case-control study was used to collect basic and follow-up information on AIDS death patients receiving antiviral treatment in Butuo county of Liangshan from 2010 to 2019. The control group was formed by sampling twice the number of cases. The logistic regression model was used to analyze the risk factors affecting mortality.Results:In 3 355 patients of HIV/AIDS treated with antiviral therapy, 1 179 cases in the death group and 2 176 cases in the control group. Including 81.34% were 30-49 years old, 69.09%males, 99.55% Yi nationality, 91.12% were married or cohabitated, 95.77% had junior high school education or below, and 88.41% peasants. Amultivariate logistic stepwise regression model showed that among the death risk factors, age ≥50 years old was 5.08 times (95% CI:3.05-8.48) that of the 18-29, female was 0.70 times (95% CI: 0.52-0.94) than male, the transmission rate of intravenous drug use was 1.43 times (95% CI: 1.06-1.91) that of heterosexual transmission, CD4 +T lymphocyte (CD4) count ≥350 cells/μl before treatment was 0.38 times (95% CI: 0.30-0.48) that of CD4 <200 cells/μl before treatment, the most recent antiviral treatment regimen containing LPV/r was 0.04 times (95% CI: 0.01-0.18) than that of stavudine (d4T) + lamivudine (3TC) + nevirapine (NVP)/efavirenz (EFV) regimen, drug resistance was 3.40 times (95% CI: 2.13-5.42) of non-drug resistance, non-viral load and non-drug resistance test results were 12.98 times (95% CI: 10.28-16.40) of non-drug resistance. Conclusions:Age, gender, transmission route, CD4 before treatment, the latest antiviral treatment program, and drug resistance test after antiviral therapy were the influencing factors of HIV/AIDS death in Butuo county. It is necessary to expand the coverage of viral load and drug resistance test to change the antiviral therapeutic schedule scientifically and carry out publicity and education on the compliance of patients with antiviral treatment and medical staff training in order to reduce the mortality of patients with antiviral treatment.
7.Anti-apoptosis and anti-inflammation effects and mechanism of berberine on high-concentration glucose induced rat retinal Müller cells

Objective:To investigate the effects of berberine on Sprague Dawley (SD) rat retinal Müller cells cultured by high concentration glucose.Methods:The cultured SD rat retinal Müller cells were divided into normal-glucose group, high-glucose group, high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group, and the cells were cultured in 5 mmol/L glucose, 25 mmol/L glucose, 25 mmol/L glucose+ 10 μmol/L berberine, and 25 mmol/L glucose+ 25 μmol/L berberine, respectively.After 72 hours cultured, cell apoptosis rate was detected by flow cytometry; the expressions levels of tumor necrosis factor-α (TNF-α), interleukin-8 (IL-8) and cyclooxygenase-2 (COX-2) in the culture supernatant were detected by enzyme linked immunosorbent assay (ELISA); the L-glutamate-L-aspartate transporter (GLAST) and the related protein expression levels were detected by real-time fluorescence quantitative PCR; the expressions levels of GLAST, protein phosphatase magnesium-dependent 1A (PPM1A), nuclear factor-κB (NF-κB) and cleaved caspase-3 in the cytoplasm, and the expression level of NF-κB protein in the nucleus were detected by Western blot.Results:The cell apoptosis rate was (1.37±0.21)%, (17.67±1.17)%, (10.60±0.17)% and (5.57±0.35)% in the normal-glucose group, high-glucose group, high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group, respectively, and the overall comparative difference was statistically significant ( F=375.97, P<0.01). The cell apoptosis rates in the high-glucose group was increased in comparison with those in the normal-glucose group ( P<0.01). The cell apoptosis rates in the high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group were significantly reduced in comparison with that in the high-glucose group (both at P<0.01). And the cell apoptosis rate in the high-glucose+ 25 μmol/L berberine group was lower than that in the high-glucose+ 10 μmol/L berberine group ( P<0.01). ELISA results revealed that the overall comparative differences of the concentrations of TNF-α, IL-8 and COX-2 among the four groups were statistically significant ( F=28.36, 35.88, 41.59; all at P<0.01). The concentrations of TNF-α, IL-8 and COX-2 in the high-glucose group were significantly higher than those in the normal-glucose group ( P<0.01). Compared with the high-glucose group, the concentrations of TNF-α and COX-2 were significantly decreased in the high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group (both at P<0.05). The concentrations of TNF-α and IL-8 in the high-glucose+ 25 μmol/L berberine group were lower than those in the high-glucose+ 10 μmol/L berberine group (both at P<0.05). Real-time fluorescence quantitative PCR showed that the relative expression levels of GLAST mRNA in Müller cells among the four groups were statistically significant ( F=268.60, P<0.01). Compared with the normal-glucose group, the relative expression level of GLAST mRNA was significantly decreased in the high-glucose group ( P<0.01). In the high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group, the relative expression level of GLAST mRNA was significantly elevated compared with the high-glucose group (both at P<0.05). Western blot analysis results showed that the overall comparative differences of the GLAST, PPM1A, cleaved caspase-3, NF-κB in cytoplasm and NF-κB in nucleus among the four groups were statistically significant ( F=135.20, 156.98, 80.96, 128.07, 47.36; all at P<0.01). The relative expression levels of GLAST, PPM1A and NF-κB protein in cytoplasm in the high-glucose group were significantly lower than those in the normal-glucose group, high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group (all at P<0.05). The relative expression levels of cleaved caspase-3 and NF-κB protein in nucleus in the high-glucose group were significantly higher than those in the normal-glucose group, high-glucose+ 10 μmol/L berberine group and high-glucose+ 25 μmol/L berberine group (all at P<0.05). Conclusions:High-concentration glucose can induce cell apoptosis and inflammatory response of SD rats retinal Müller cells in vitro.However, berberine can inhibit cell apoptosis and inflammatory response induced by high-concentration glucose via suppressing NF-κB translocation and transcription activity, and thereby inhibiting the expression of inflammatory cytokines.
8.Effects of T-2 toxin on expression of hepatocyte growth factor and its receptor in articular cartilage and epiphyseal cartilage of rats under selenium deficiency

Objective:To observe the effects of T-2 toxin on expression of hepatocyte growth factor (HGF) and HGF receptor (C-Met) in articular cartilage and epiphyseal cartilage of rats under low selenium condition.Methods:Twenty-four healthy male SD rats weighted 60-80 g were randomly divided into conventional diet group (selenium content of 101.5 μg/kg) and low-selenium diet group (selenium content of 1.1 μg/kg), with 12 rats in each group. After 30 days of feeding, the conventional diet group was further divided into conventional group and T-2 toxin group (100 μg·kg -1·d -1), and the low-selenium diet group was further divided into low-selenium group and low-selenium+T-2 toxin group (100 μg·kg -1·d -1), with 6 rats in each group. After 30 days of feeding, the rats were sacrificed and the cartilage of knee joint was taken, the morphological changes of knee articular cartilage and epiphyseal cartilage were observed by HE staining under light microscope. Immunohistochemical method was used to detect the expression of HGF and C-Met in knee articular cartilage and epiphyseal cartilage, and positive expression rates of HGF and C-Met were calculated. Results:Under light microscope, chondrocytes of articular cartilage and epiphyseal cartilage in low-selenium+T-2 toxin group were sparse, and the necrosis and structural area were found in the deep layer, and the extracellular matrix of chondrocytes in the region was degraded and light stained, and proliferating granulation tissue was visible nearby. The positive expression rates of HGF in articular cartilage [(21.97 ± 6.90)%, (49.41 ± 8.24)%, (76.39 ± 5.88)%] and epiphyseal cartilage [(23.36 ± 12.49)%, (58.43 ± 14.48)%, (66.59 ± 10.83)%] of rats in low-selenium, T-2 toxin and low-selenium+T-2 toxin groups were higher than those in conventional group [(9.13 ± 6.01)%, (11.14 ± 4.67)%, P < 0.05]. The positive expression rates of C-Met in articular cartilage [(25.34 ± 7.53)%, (58.21 ± 12.54)%, (81.46 ± 7.89)%] and epiphyseal cartilage [(35.21 ± 4.71)%, (40.84 ± 2.03)%, (49.41 ± 6.29)%] of rats in low-selenium, T-2 toxin and low-selenium+T-2 toxin groups were higher than those in conventional group [(11.21 ± 5.11)%, (12.12 ± 4.71)%, P < 0.05]. Conclusion:T-2 toxin may affect the expression of HGF and C-Met in articular cartilage and epiphyseal cartilage of rats under low selenium condition.
9.Effect evaluation of 2% anisodamine cream in preventing chemotherapy phlebitis caused by 5-fluorouracil

Objective To explore the interventional effect of two different methods in the prevention of chemotherapy phlebitis caused by 5-fluorouracil.Methods Eighty cases of patients treated with cisplatin+5-fluorouracil (PF) chemotherapy in the First Affiliated Hospital of Guangxi Medical University from May 2015 to July 2016 were selected and divided into the observation group and control group according to admission time,40 cases in each group.The observation group was smeared with 2% anisodamine along the direction of vein puncture after peripheral venous puncture success,while the control group was smeared with mucopolysaccharide polysulphate emulsion paste.The occurrence of phlebitis and degree of local pain after intervention were compared between the two groups.The blood biochemical indexes and inflammatory factors levels before and after intervention were compared too.Results After the intervention,the distribution of phlebitis level and local pain degree had statistical difference between the two groups (Z=2.57,2.53;P=0.01);the observation group was superior to the control group in the aspects of phlebitis lesion degree and local pain alleviation;WBC,MPV and PDW in the observation group had statistical difference between before and after intervention;WBC,PLT and PCT in the control group had statistical difference between before and after intervention (P<0.05);PLT count was positively correlated with PCT in the two groups (r=0.952,P<0.05);the CRP level and WBC count after intervention had no statistical difference between the two groups(P>0.05).Conclusion 2% anisodamine can effectively prevent and alleviate the occurrence of 5-FU caused chemotherapy phlebitis and local pain.At the same time,PLT can provide a basis for preventing chemotherapy phlebitis.
10.Qualitative research on the humanistic care of cardiac surgery patients in the operating room

Objective To investigate the status of humanistic care on patients with cardiac surgery of operating room nurse and explore the implementation of perioperative humanistic care on patients with cardiac surgery.Methods Purposive sampling method was used to deeply semi-structured interview 10 operating room nurses in a class-three grade A hospital of Guangxi from July 2016 to August 2016. The interview data was analyzed by using Colaizzi analysis method.Results There were three themes. The knowledge level of humanistic care was related to seniority, professional title and practical experience. The poor performances in the implementation of humanistic care were as following: the preoperative interview was not in place, nurses were lack of effective communication skills, and nurses did not pay attention to the protection of the privacy of patients and postoperative visits. The implementation of humanistic care improved the satisfaction of patients. Conclusions Operation room managers should arrange humanistic care topics and fully implement the humanistic nursing care of patients undergoing cardiac surgery in operation room so as to improve the patients' satisfaction.
