1.A multicenter survey on the current status of human caring in hospital wards in China

Objective:To explore the current situation of nursing human caring in hospital wards and analyze its influencing factors, so as to facilitate the development of nursing human caring practice.Methods:From July to November 2022, a total of 107 hospitals were surveyed through stratified convenience sampling method, and 4 072 ward nursing managers were recruited to finish the general information questionnaire and the ward nursing human caring status questionnaire. The general information included the region, class and type of the hospital, etc. The ward nursing human caring status questionnaire included 38 items in 5 dimensions of nursing human caring system and process, humanistic quality and training of nursing staff, humanistic environment and facilities, human caring procedures and measures, and human caring quality evaluation and improvement, with a full score of 190 points. Descriptive statistics were used to analyze the general data, independent samples t-test, ANOVA and correlation analysis were used to analyze the factors influencing the current status of nursing human caring in the ward, while multiple linear regression analysis was used to conduct a multivariate analysis. Results:The score of nursing human caring in hospital wards was 156.91±27.78. Whether the hospital had carried out nursing human caring pilot(demonstration) wards, whether the ward had previously been a hospital nursing human caring pilot(demonstration) nursing unit, the type of ward, and whether nursing managers had participated in human caring training were the influencing factors of the implication of nursing humanistic caring in wards( P<0.05). Conclusions:The practice of nursing human caring in hospital wards is at a good level, but needs to be further strengthened. Nursing managers should take systematically strategies to promote the development of nursing human caring practice.
2.Effect of basic fibroblast growth factor on autophagy of nerve cells in rats after traumatic brain injury

Objective To investigate the effect of basic fibroblast growth factor (bFGF) on autophagy of nerve cells in rats after traumatic brain injury (TBI).Methods A total of 120 healthy adult male SD rats were randomly divided into sham group,TBI + vehicle group,and TBI + bFGF group by random number table method,with 40 rats in each group.PinPointTM Precision Cortical Impactor was used to simulate the pathological damage after TBI.In the sham group,the dura was exposed without impact.In the TBI + bFGF group,250 μg/kg of human recombinant bFGF was given in the nasal cavity 1 hour before the modeling,while in the sham group and TBI + Vehicle group,the same amount of saline was given in the nasal cavity 1 hour before the modeling.The necrotic cells were observed by propidium iodide(PI) staining 6 hours after injury.The effect of bFGF on the nerve function after TBI in rats was evaluated with modified neurological severity score (mNSS) 24 hours after injury.The water content of brain tissue was measured by dry and wet method 48 hours after injury.The ratio of Beclin-1,P62 protein and microtubule-associated protein 1 light 3 (LC3)-Ⅱ/Ⅰ protein was detected by western blot.The volume of brain injury was calculated by integral method of brain tissue section.The positive neuron specific nuclear protein (NeuN) cells were observed by immunofluorescence staining.The apoptotic cells were observed by TUNEL.Results Compared with the sham group [(4.0 ± 1.2) %],the percentage of necrotic cells in TBI + vehicle group [(54.3 ± 10.1) %] and TBI + bFGF group [(34.5 ± 10.5) %] increased significantly (P < 0.05),but the percentage of necrotic cells in TBI + bFGF group increased less than that in TBI + vehicle group (P < 0.05).Compared with the sham group [(0.3 ± 0.5)points],the mNSS in the TBI + vehicle group [(5.8 ±0.8)points] and TBI + bFGF group [(4.7 ± 1.1) points] were significantly increased (P < 0.05),but the mNSS of TBI + bFGF group was lower than that of TBI + vehicle group (P < 0.05).Compared with the sham group [(76.7 ± 0.7) %],the water content of brain tissue of TBI + vehicle group [(79.2 ± 0.5) %] and TBI + bFGF group [(78.4 ± 1.0) %] were significantly increased (P <0.05),but the water content of TBI + bFGF group was significantly lower than that of TBI + vehicle group (P <0.05).Compared with the sham group,protein expression of Beclin-1 and LC3-I/Ⅰ in TBI + vehicle group and TBI +bFGF group were significantly improved (P < 0.05),P62 protein expression was significantly decreased (P < 0.05),the volume of brain tissue injury was significantly increased (P < 0.05),the number of positive NeuN cells increased significantly (P < 0.05),and the number of apoptotic cells and apoptotic cells were significantly increased (P <0.05).Compared with the TBI + Vehicle group,the up-regulation of Beclin-1 protein and LC3-Ⅱ/Ⅰ protein ratio was obviously inhibited in the TBI + bFGF group (P < 0.05),the down-regulation of P62 protien was significantly suppressed,the volume of brain tissue injury was significantly decreased,and the number of positive NeuN cells and apoptotic cells was significantly reduced (P < 0.05).Conclusion The bFGF can significantly inhibit excessive autophagy in rats after TBI,reduce brain edema,reduce cell apoptosis and necrosis,and improve neural function.
3.Characteristics analysis and management countermeasures of 33 general hospital inpatients who attempted suicide

Objective? To explore the characteristics of inpatients who attempted suicide and to propose targeted management strategies to prevent inpatients from committing suicide once again. Methods? The data of 33 inpatients who attempted suicide between January 2008 and January 2018 from a Class Ⅲ Grade A hospital. Chief nurses, nurses-in-charge and bedside doctors associated with attempted suicide were interviewed to fully understand the conditions of patients who attempted suicide and to propose corresponding management and improvement protocols. Results? There were 19 (57.58%) females and 16 (48.48%) middle-aged patients in the patients who attempted suicide. Totally 20 of them (60.61%) committed suicide between 10:00 pm and 6:00 am. 14 of them (42.42%) committed suicide cutting wrists. 8 of them (24.24%) were from the department of oncology. Conclusions? Inpatients' attempted suicide has become a severe issue that cannot be ignored and should draw the attention of medical and nursing workers. We should formulate more standardized management strategies to preventing the inpatients who attempted suicide from committing suicide once again and reduce the incidence of suicide among inpatients.
4.Outcomes study on improving humanistic care efficacy for nursing personnel through 3 in 1 construction of mental health training platform

Objective To build a 3 in 1 mental health training platform and explore its application in mental health training for nurses working in geriatric wards. MethodsTotally 120 clinical nurses were selected by cluster sampling from the Department of Geriatrics, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology in May 2017. A mental health and humanistic care training platform which integrated "mental-humanistic-social" aspects was built for the nurses in the 3 in 1 form of "microcourse-learning platform-online interaction and discussion", in order to enhance their mental knowledge, improve their mental health management ability, expand scope of knowledge about humanistic care and intensify their practical ability training. A self-made questionnaire for nurses' evaluation of 3 in 1 mental health training platform and Caring Ability Inventory (CAI) were used to assess the effects of the platform. ResultsNursing workers' recognition of the platform scored over 3.5 in all items. This platform enabled the organic unity of informatized training and satisfied most of the nurses. Nursing workers' humanistic care ability was higher after the platform was applied than that before it was applied, and there was statistically significant difference (P<0.05). ConclusionsThe 3 in 1 mental health platform can effectively enhance nurses' humanistic care ability, which is worth promoting in clinical practice.
5. The enlightenment of world suicide prevention day theme for nurses to prevent the suicide of hospitalized patients

On the analysis of world suicide prevention day theme throughout the years and its basic connotation, the article elucidates the core idea of "suicide, can be prevented" from different angles. And it enlightens the nurses should combine clinical practice, according to the specific situation of the hospitalized patients to carry out suicide prevention nursing work, mainly includes: think globally, make life full of hope, diversified social support, promote understanding, focus on high-risk age bracket, attach great importance to the cultural background, strengthen the cause of survival, eliminate discrimination, psychological intervention and continuous human care.
6.Investigation and analysis on the knowledge and attitude of nurses on common complications among bedridden patients

Objective To investigate the knowledge and attitude of nurses on the prevention and nursing of common complications among bedridden patients in Wuhan City, and provide scientific basis for the implementation of targeted training in improving clinical nursing quality. Methods A total of 748 clinical nurses from 4 hospitals in Wuhan City were selected in November 2016 using convenient sampling method. A self-designed questionnaire was applied to investigate the common complications nursing knowledge and attitude of nurses on bedridden patients. Results The accuracy of nurses' knowledge on the prevention and nursing of common complications among bedridden patients were listed as follows: deep vein thrombosis (DVT) (77.44%), pneumonia (73.88%), urinary tract inflammation (70.63%), and pressure ulcers (61.44%). The scores of nurses' attitude were listed successively: pressure ulcers (88.21%), urinary tract inflammation (86.81%), pneumonia (86.66%), and DVT (85.92%). There were positive correlation between the scores of knowledge and attitude of nurses on four kinds of complications (P< 0.05). Conclusions Nursing administrators should improve the training and update of common complications knowledge on nurses, and promote their positive attitude to the prevention of common complication to increase the quality of nursing.
7.Association of MYH9 SNPs with end-stage renal disease in Chinese Han popula-tion

Objective:To explore the associations of MYH9 gene polymorphisms with ESRD in Han population in the fragment between exon 23 and 24.Methods:A hospital-based case control study was carried out including 180 patients and 118 controls in this study.Single nucleotide polymorphisms of MYH9 gene were determined using PCR sequencing,and the haplotypes were calculated using phase software(version 2.0),and transcription factor binding sites were predicted using AliBaba2.Univariate analysis was conducted for exploring the associations between polymorphisms and ESRD.Results: Five newly discovered and three previously reported SNP loci [Rs4821480(MYH9-92),Rs2032487(MYH9-273) and Rs4821481(MYH9-787)]were homozygote genotyped by bidirectional se-quencing.Among newly discovered polymorphisms,two were found at the 489 locus(G→A)and the 616 locus(A→C) in the 901 bp fragment which located in the intron 23 of MYH9 gene.A G489A transversion was very likely a risk mutation contribute to the occurrence of ESRD(P=0.013).No association was observed between ESRD and three previous reported sites [Rs4821480(MYH9-92),Rs2032487(MYH9-273)and Rs4821481(MYH9-787)].The most common haplotype was TCTCGGAT,which was less frequent in the cases than that in the controls.Moreover, TCTCGGCT and TCTCAGAT haplotypes were more in the cases than that in the controls.The number of transcription binding sites increased from 82 ( wild ) to 85 ( mutation ) in the 23th intron of MYH9 gene.Conclusion:Polymorphisms of MYH9 at intron 23 may influence the prevalence of ESRD in Chinese Han population and TCTCGGAT haplotype may be one protective haplotype.TCTCGGCT and TCTCAGAT may be risk haplotypes attributed to ESRD.The polymorphism of MYH9 at the 23th intron may company with the amount alteration of transcription factor binding sites.
8.Outpatient experiences of caring in the outpatient clinic:a qualitative research

Objective To explore the outpatients′experience of caring and provide references for improving their experience of medical care.Methods The phenomenology research method was adopted in this study,and twenty-one outpatients were subject to an in-depth interview to learn their outpatient experiences.Results Ten themes of experience with caring were extracted as follows:passionate and patient,careful and considerate,respect for others,offers to help,encouraging and comforting,apologies and courteously,humor,truthful considerations,conscientious and responsible,and a caring environment setup.Five themes of experience without caring were extracted as follows:impatient communication, careless seeing of outpatients,delayed service,passive reminders,and facilities without enough details. Conclusions The outpatients may have abundant experiences of caring at the outpatient clinic,also lack of caring sometimes as well.Outpatient managers should keep an eye on outpatients′needs and satisfaction of caring from time to time,to improve the quality of outpatient service.
9.Standardized management and effects for hospital′s humanistic nursing care

Objective To explore the methods of the standardized management used in humanistic nursing care and to assess its effects.Methods Measures of the standardized management began to be used in such care at the hospital since January 2013.Questionnaires of patient satisfaction with nursing care for inpatients and those discharged were used to assess the effects of the standardized management measures periodically and synthetically.Results The patient satisfaction with humanistic nursing care keeps increasing year by year from 2013 to 201 5.The differences among three sets of data are statistically significant(P < 0.05 ).Conclusions Humanistic nursing care calls for promotion of standardized management.It is high time to build such a standardized management system at the hospitals.
