1.Effect of acupuncture on point postoperative nausea and vomiting and intestinal flora in gynecological endoscopic surgery

		                        			Objective To evaluate the effect of acupuncture at Yin Wei point on postoperative nausea and vomiting(PONV)in female patients undergoing abdominal surgery.The clinical efficacy of postoperative nausea and vomiting(PONV)and its impact on gut microbiota.Methods This study included 184 patients who underwent gynecological laparoscopic surgery in Jinhua Central Hospital from January 2021 to April 2024.They were randomly divided into control group(n=93)and acupuncture group(n=91)using a random number method.At the completion of surgery,the control group received intravenous injection of 5mg of tropisetron hydrochloride.In acupuncture group,on the basis of control group,intervention was performed by needling the palmar Yin Wei points of both forearms for 30 minutes before surgery.The incidence and severity of PONV were compared between two groups of patients.In addition,fecal samples were collected from two groups of patients before and after surgery,and differential analysis of gut microbiota community structure was performed using 16S amplicon absolute quantitative sequencing technology.Results In the 0-24 hours after surgery,40 cases of the acupuncture group and 56 cases of control group experienced PONV.The acupuncture group's PONV incidence was lower than control group's(P<0.05).The nausea severity of acupuncture group after surgery was significantly lower than that of control groups.The proportion of patients taking antiemetic drugs after surgery in acupuncture group was also significantly lower than that in control group(P<0.05).Before surgery,the two groups have no significant difference regarding Chao1,ACE,Shannon,and Simpson indices(P>0.05).After surgery,the acupuncture group's Chao1,ACE,and Shannon indices were significantly higher than control group's(P<0.05).The Simpson scores of two groups of patients were compared after surgery,and no significant difference was found(P>0.05).The Observed,Chao1,ACE,and Shannon indices were significantly higher in the acupuncture group after surgery than before(P<0.05).The incidence of adverse reactions in acupuncture group and control group were 8.8%and 8.6%,respectively,and there was no statistical significance(P>0.05).Conclusion Acupuncture at Yin Wei point combined with intravenous injection of tropisetron can reduce the incidence of PONV in patients undergoing gynecological laparoscopic surgery.
		                        		
		                        		
		                        		
		                        	
2.Exploration of Basement Membrane-related Markersand Prediction of Therapeutic Drugs in Idiopathic Pulmonary Fibrosis

		                        			Objective To explore basement membrane markers and potential drugs for treatment in idiopathic pulmona-ry fibrosis(IPF).Methods IPF-related datasets were downloaded from the Gene Expression Omnibus(GEO)database,processed to construct basement membrane gene expression matrices associated with IPF,and screened for differential basement membrane genes(DEBMs);DEBMs were enriched for function and pathways,and machine learning algorithms were used to ob-tain candidate signature genes,receiver operating characteristic(ROC)curves were used to identify signature genes and con-struct a nomogram.We performed ssGSEA analysis to explore the correlation between signature genes and immune cells and their functions and predicted the corresponding miRNAs and therapeutic drugs by signature genes.Results A total of 56 DEBMs were extracted;enrichment analysis showed that DEBMs were mainly enriched in"extracellular matrix tissue","extracellular structural tissue",etc.,and were closely related to"ECM-receptor interaction"and"local adhesion spot"pathways.The ma-chine learning has identified six candidate signature genes(TIMP3,P3H2,ITGA7,ITGA4,ADAMTS2,COL8A2),all of which meet the requirements of the signature genes by the ROC curve test,and the nomogram diagnostic value was outstanding(AUC=0.991 523);B cells and Macrophages in IPF were significantly different from the normal group.Finally,miRNAs were predicted to be dominated by miR-4305,miR-3684,progesterone,and tert-butyl hydroperoxide as therapeutic agents with strong relevance to IPF.Conclusion Signature genes and predictive miRNAs may serve as novel markers for IPF diagnosis,and pre-dictive drugs may be a potential source of drugs for treating IPF.
		                        		
		                        		
		                        		
		                        	
3.Independent risk factors related to unintended initial dissection of the poste-rior plane in small incision lenticule extraction performed by surgeons with different qualifications

		                        			Objective To investigate independent risk factors for unintended initial dissection of the posterior plane(UIDPP)during small incision lenticule extraction(SMILE)by surgeons with different qualifications.Methods A total of 1 600 patients(3 003 eyes)who underwent SMILE performed by three surgeons with different qualifications from April to September 2021 were selected as the research subjects,including 911 males and 689 females,with an average age of(21.82±3.55)years.The incidence of UIDPP in patients was recorded,along with a comprehensive summary of the age,gender,eyes,surgeons,surgical proficiency,subjective refraction,central corneal thickness,lenticule thickness,side cut angle,and peripheral lenticule thickness of the patients.Independent risk factors of UIDPP were analyzed using the binary logistic regression.Test level:α=0.05.Results In this study,the incidence of UIDPP was 6.56%(197/3 003),and the lenticule was successfully removed in all eyes.Multivariate logistic regression analysis demonstrated that surgeon(P=0.035),surgical proficiency(P=0.026),eyes(P=0.007),lenticule thickness(P<0.001),and peripheral lenticule thick-ness(P<0.001)were independent risk factors associated with the UIDPP during SMILE surgery.No significant differences were observed in other parameters(all P>0.05).Conclusion Surgeons,surgical proficiency,eyes,lenticule thick-ness,and peripheral lenticule thickness are independent risk factors of UIDPP.
		                        		
		                        		
		                        		
		                        	
4.Guidelines for Ethical Review Project Entrustment of Life Science and Medical Research Involving Humans

		                        			The seventh article of Measures for the Ethical Review of Biomedical Research Involving Humans (2016) stipulated that medical and health institutions without an ethics committee shall not carry out biomedical research involving Humans. The Opinions on Strengthening the Governance of Ethics in Science and Technology, issued in March 2022, clearly stated that the institutions that do not meet the conditions for establishing a scientific and technological ethics (review) committee should entrust other institutions to conduct the review. The fourteenth article of Measures for Ethical Review of Life Science and Medical Research Involving Humans (2023) proposes that if an institution, which carries out life science and medical research involving humans, has not established an ethics committee or its ethics committee is not competent for ethics review, it can entrust a competent ethics committee or regional ethics committee in writing to carry out ethical review. Most medical institutions at or above the second level in China have set up ethics committees. While most universities and colleges, scientific research institutions, enterprises and grass-roots medical and health institutions have not set up ethics committees, which lack a working system to protect the safety and interests of the participants, and is difficult to conduct life sciences and medical research involving humans. At present, there is a need for some research institutions that do not have the conditions to establish ethics committees to entrust their projects of life science and medical research involving humans to other institutions for ethical review. The entrusted review is still in the exploratory stage, and there is no relevant specification. The hasty implementation of entrusted review may not achieve the goal of effectively protecting the safety and interests of the participants, and even cause legal disputes. Based on the thematic discussion, with reference to the relevant laws and regulations, departmental rules, ethical standards, and the experience of the ethics committees of some domestic institutions in implementing the entrusted review, the guideline was formulated for the reference of the current entrusted review to ensure the safety and interests of the participants.
		                        		
		                        		
		                        		
		                        	
5.The clinical and pathological diagnosis of three cases of uterine corpus primary NHL

		                        			Objective To discuss pathological features of three cases of uterine primary NHL(non Hodgkin′s lymphoma).Methods The clinical and pathological data of 3 cases of NHL were collected.Immunohistochemical method was used to label the tumor cells.Their histological types were classified by the latest WHO Classification of Tumors of Haematopoietic and Lymphoid Tissues,and then differential diagnosis with other uterus diseases was offered.Results Three cases were presented with abnormal vaginal bleeding or ejecting as the first symptom,which was easily misdiagnosed as common diseases of the uterus in preliminary clinical and pathological diagnosis.Histopathology of general uterus specimens demonstrated that case one expressed CD3,CD56,TiA-1 and so on,and detected EBV virus cells expression,which supported the diagnosis of NK/T cell lymphoma.Another two cases expressed B cell-specific markers(CD20) and so on,and these tumor cells volume was 3 times larger than that of lymphocyte.These tumor cells had high proliferation index and supported diffuse large B-cell lymphoma diagnosis.Conclusion The tumor cells are like lymphocytes.We infiltrate endometria and myometrium,which is difficult to identify from endometria stromal sarcoma and other tumors,and need to use immunohistochemistry(IHC) for definitive diagnosis and differential diagnosis.
		                        		
		                        		
		                        		
		                        	
6. Expression and mechanism of Twist2 in glioma

		                        			To investigate the significance of Twist2 in glioma and whether it is involved in the malignant transformation of glioma by epithelial-mesenchymal transition (EMT).
		                        		
		                        			Methods:
		                        			Using immunohistochemical method detected the expression level of Twist2 in 60 cases of gliomas (including WHO grades Ⅱ, Ⅲ and Ⅳ, each for 20 cases) and 20 cases of non-tumor brain tissues. Real-time fluorescence quantitative PCR and Western blot were used to detect the expression level of Twist2 mRNA and protein in 61 cases of fresh glioma tissue (WHO grade Ⅱ 16 cases, Ⅲ 21 cases, Ⅳ 24 cases) and 12 cases of adjacent tissues, and the expression levels of E-cadherin, N-cadherin and vimentin were also investigated in fresh glioma tissue.

8.The influence and mechanism of cisplatin on autophagy and proliferation of endometrial cancer

		                        			Objective To explore the effects of cisplatin on the proliferation and autophagy of endometrial carcinoma Ishikawa cells.Methods Ishikawa cell proliferation was detected by MTS assay after the cells were treated with CDDP.To assess the level of autophagy,transmission electron microscope and Western blot were used to detect LC3 and Beclin1 expression;fluorescence microscopy was used to observe the fluorescence aggregation of green fluorescent protein and microtubule associated protein 1 light chain 3 fusionprotein (GFP-LC3).Results Cisplatin of 10 g/mL inhibited the proliferation of Ishikawa cells,with an increase of time and concentration,the inhibition of cell proliferation was significantly elevated (P < 0.01).Transmission electron microscopy showed that under a condition of cisplatin on Ishikawa endometrial cancer cell,autophagy occurred.With an increase of concentration and dosage,Western blot showed that autophagy related protein LC3 expression was up-regulated,but becline-1 had no obvious change.LC3 expression level was higher in 12h-treatment with 20 μg/mL cisplatin group than in the control group,and was higher in 24h-treatment group than in 12h-treatment group.Conclusions Cisplatin inhibits proliferation of Ishikawa cells and induces autophagy of the cells in a time-and dose-dependent manner.Autophagy related MAP-LC3 is involved in the molecular mechanisms of autophagy induced by cisplatin in endometrial carciHom.
		                        		
		                        		
		                        		
		                        	
9.A comparative study of crystalloid solution mixed with colloidal solutions and pure crystal solution as extracorporeal circulation priming solution in adult simple heart valve replacement with cardiopulmonary bypass

		                        			Objective To investigate physiological changes in peri extracorporeal circulation period of patients who underwent cardiac valve replacement surgeries with crystalloid solution mixed with colloidal solutions and pure crystal solution as extracorporeal circulation priming solution, and explore the clinical value and practicability of crystalloid solution as the sole extracorporeal circulation priming solution.Methods A retrospective analysis was performed in 130 patients who underwent cardiac valve replacement surgeries.Pure lactated Ringer's solution liquid and Lactated Ringer's solution mixed with Voluven as the extracorporeal circulation priming solution were used.We respectively compared hematocrit at different time points, postoperative blood routine, liver and kidney function, blood coagulation index, duration of intensive care and trachea cannula in two groups.Results There were no significant differences in ages, preoperative blood routine, kidney function, blood coagulation function, duration of operation, clamping time, bypass time, intensive care, postoperative blood routine, kidney function, blood coagulation function and hematocrit at different time points in two groups (P >0.05).However, the hospital day of group which used crystalloid solution as extracorporeal circulation priming solution was significant shorter compared to group which used lactated Ringer's solution mixed with Voluven (P < 0.05).Alanine aminotransferase of group which used crystalloid solution as extracorporeal circulation priming solution was significant higher compared to group which used lactated Ringer's solution mixed with Voluven (P <0.01).Conclusions Crystalloid solution as extracorporeal circulation priming solution is safe and economy in cardiopulmonary bypass.Pure crystalloid solution as the sole extracorporeal circulation priming solution can be safely used on patients (New York Heart Association class Ⅱ-Ⅲ) who have normal liver and kidney function before the operation of adult heart valve replacement with cardiopulmonary bypass.
		                        		
		                        		
		                        		
		                        	
10.Echocardiography in mini-invasive surgical device closure of secundum atrial septal defects.

		                        			OBJECTIVE:
		                        			To discuss the preoperative, intraoperative, and postoperative application of echocardiography in mini-invasive surgical device closure of secundum atrial septal defects, including those special and difficulty-occluded defects.
		                        		
		                        			METHODS:
		                        			We performed mini-invasive surgical device closure of secundum atrial septal defects on 287 patients. Before the surgery, transthoracic echocardiography was applied for screening; during the surgery we reassessed the sizes of the defects and their remaining margins, designated the suitable occluders, and guided the placement of the occluders by multiplane transesophageal echocardiography. The patients were postoperatively followed up at regular intervals by multiplane transesophageal echocardiography (MTEE) which was employed to assess the therapeutic efficacy.
		                        		
		                        			RESULTS:
		                        			Out of the 287 atrial septal defects, 276 (96.17%) were successfully closed. There were 37 porous defects and 23 cases with short posterior-inferior margin of defects. Follow-ups at intervals showed the occluders stayed firmly and echoed clearly. No notable residual shunt or valve regurgitation was observed.
		                        		
		                        			CONCLUSION
		                        			Echocardiography plays a vital and reliable role in mini-invasive surgical device closure of secundum atrial septal defects, especially those special and difficulty-occluded defects.
