1.Clinical Questions Construction in Clinical Practice Guidelines: Based on Case-guided Approach

Currently, there are practical and technical difficulties in the construction of clinical questions in the development of clinical practice guidelines. Clinicians or guideline developers seldom construct clinical questions based the actual case scenario, leading to some information loss between structured and actual clinical connotation. To overcome this challenge, we proposed a case-guided questions construction approach, and carried out case research and verification in the formulation of the guideline. We found that this method could more efficiently and scientifically assist the formulation of clinical questions, and provide reference for clinicians or guideline developers.
2.Factors related to blurred vision after general anesthesia

Objective:To analyze the influential factors of blurred vision after general anesthesia.Methods:The clinical data of 997 patients who underwent elective general anesthesia at The No. 1 People's Hospital of Pinghu from September 2022 to May 2023 were retrospectively analyzed. The data collected included age, sex, body mass index, American Society of Anesthesiologists classification, history of hypertension, history of diabetes, operation duration (specifically whether it exceeded 3 hours), surgical position (whether the patient was in the supine position), operating room temperature, use of penehyclidine hydrochloride, use of muscle relaxant antagonists, use of atropine, blood pressure (specifically whether it was ≥ 30% of the baseline value), fluid input, blood loss, and use of pneumoperitoneum. Collinearity diagnosis and univariate logistic regression analysis were conducted to select factors with statistical significance. Subsequently, multivariate logistic regression analysis was performed.Results:Univariate and multivariate logistic regression analyses showed that age > 65 years ( OR = 1.47, 95% CI: 1.01-2.15, P = 0.043), surgical position (non-supine position) ( OR = 1.54, 95% CI: 1.06-2.25, P = 0.025), operation time exceeding 3 hours ( OR = 1.76, 95% CI: 1.05-2.94, P = 0.031), and the use of penehyclidine hydrochloride ( OR = 4.91, 95% CI: 3.35-7.21, P < 0.001) were identified as factors contributing to postoperative blurred vision in patients undergoing general anesthesia. Conclusion:Factors contributing to postoperative blurred vision in patients undergoing general anesthesia include age > 65 years, the use of penehyclidine hydrochloride during surgery, operation time exceeding 3 hours, and non-supine surgical position. Clinically, it is essential to implement early and effective preoperative education, enhance intraoperative nursing quality, and optimize preoperative medication for general anesthesia to reduce the incidence of blurred vision after surgery.
3.The study of Sirt1 promotes the osteogenic differentiation of inflammatory PDLSCs

Objective:
To study the positive effect and mechanism of histone deacetylase Sirtuin 1 (Sirt1) on the osteogenic capacity of inflammatory PDLSCs.
Methods:
PDLSCs were isolated and cultured from the healthy individuals and periodontitis patients , the expression of Sirt1 in two types of PDLSCs was observed;osteogenic induction of inflammatory PDLSCs was performed , concurrently Sirt1 agonist resveratrol was used to up⁃regulate the expression of Sirt1 , then the osteogenic differentiation was observed , the expressions of Runx2 , acetylated NF⁃κB , acetylated FoxO1 and FoxO1 were assessed by Western blot.
Results :
The protein expression of Sirt1 was suppressed in in flamed PDLSCs( P < 0. 01) ;resveratrol could up⁃regulate the expression of Sirt1 in inflamed PDLSCs;compared with the osteogenic induction group , the osteogenesis + resveratrol group increased the expression of BSP ( t =14. 045 ,P < 0. 01) , Runx2( t = 3. 349 ,P < 0. 01) , OCN( t = 7. 218 ,P < 0. 01) and Osx ( t = 4. 544 ,P < 0. 01) mRNA in inflamed PDLSCs , and enhanced ALP staining(P < 0. 01) ;expression of FoxO1( t = 8. 737 , P < 0. 01) and Runx2( t = 6. 152 , P < 0. 01) increased after osteogenesis of inflamed PDLSCs ; in the osteogenesis + resveratrol
group , the expression of Sirt1 was up⁃regulated , and the expression of FoxO1( t = 5. 912 ,P < 0. 01) and Runx2( t =6. 277 ,P < 0. 01) further increased compared with the osteogenic induction group ,while the expression of acetylated NF⁃κB(F = 184. 033 ,P < 0. 01) and acetylated FoxO1(F = 301. 454 , P < 0. 01) was significantly lower than that of control group and osteogenic induction group.
Conclusion
The deacetylase Sirt1 could promote the osteogenic differentiation of inflammatory PDLSCs through deacetylating NF⁃κB and FoxO1 .
4.Construction and Application of National Rare Diseases Registry System of China

Rare diseases have been a major challenge for clinical medicine and public health challenge in China. One of the effective measures is to conduct proactive research on rare diseases to deal with the disease burden of the diseases. However, low prevalence, disperse distribution of patients, lack of knowledge about the disease course, and phenotype heterogeneity hamper the development of research for rare diseases. Recently, it has been found that patients registry is effective in understanding the course of the disease and accu- mulating the cases and data of clinical research or clinical trial design. At present, most of developed countries or regions in the world have promoted clinical research and clinical trials of new medications on rare diseases by using the registration of rare disease. In 2016, Peking Union Medical College Hospital established China's first registry system at the national level-National Rare Disease Registry System of China(NRDRS). NRDRS has accumulated 68 137 cases data registered by the researchers from China's 101 collaborating hospitals in 29 provinces/municipalities/autonomous regions, covering 171 different, and forming 188 cohorts. To date, NRDRS complete the initial stage of resources buildup.Nex stage will be focused on clinical research and clinical trials related to rare diseases based on NRDRS. This article is on the process of building NRDRS, the potential support for conducting clinical research and clinical trials related to rare diseases, and the challenges will be faced.
5.Value of dual-layer spectral detector CT in differentiating the diagnosis of lung cancer and inflammatory nodules

Objective:To explore the value of dual-layer spectral detector CT in differentiating the diagnosis of lung cancer and inflammatory nodules.Methods:A total of 92 patients undergoing enhanced chest scan from March 2019 to September 2020 at Renji Hospital, School of Medicine, Shanghai Jiaotong University, were retrospectively enrolled in the study. The conventional CT parameters, spectral CT parameters were measured and the nodules′ morphological characteristics were analyzed. Later the factors with statistical significance were identified as independent variables in a logistic regression model to establish models for predicting malignant nodules. ROC curve was used to assess the diagnostic performance for the conventional CT model, spectral CT parameters and combined model, respectively. Differences in the area under the ROC curve (AUC) were analyzed by the DeLong test.Results:Lobulated sign (42 and 8, respectively, χ2=10.779, P=0.001), short burr sign (41 and 7, respectively, χ2=11.911, P=0.001), pleural indentation sign (45 and 9 respectively, χ2=11.705, P=0.001), vascular convergence sign (35 and 8, respectively, χ2=5.337, P=0.021) and the venous phase iodine concentrations (IC) value [(2.1±0.5) mg/ml, (2.3±0.5) mg/ml, t=-2.464, P=0.016], normalized iodine concentrations (NIC) value (0.40±0.06, 0.45±0.08, t=-6.943, P<0.001), and Z-effective (Z eff) values (8.38±0.21, 8.49±0.19, t=-2.122, P=0.037) were significantly different between the lung cancer group and the inflammatory group, while other CT signs and CT indicators were not significantly different between the lung cancer group and the inflammatory group ( P>0.05). The conventional CT model was established with lobulated sign, short burr sign, pleural indentation sign, vascular convergence sign, and the AUC for differential diagnosis of lung cancer and inflammatory nodules was 0.827. The spectral CT parameter model was established with venous phase IC, venous phase NIC, and venous phase Z eff value, and the AUC for differential diagnosis of lung cancer and inflammatory nodules was 0.899. The conventional CT model combined spectral CT parameter model was established with the significant factors in the univariate analysis, and the AUC for differential diagnosis of lung cancer and inflammatory nodules was 0.925. The AUC of the combined model showed no significant difference from that of the spectral CT parameter model ( Z=1.794, P=0.073). However, AUC of the combined model was significantly higher than that of evaluation based on conventional CT alone ( Z=2.156, P=0.031). Conclusion:Spectral CT parameters combined with conventional CT signs can improve the differential diagnosis efficiency between lung cancer and inflammatory nodules.
6.Discussion on pancreatic morphology and pancreaticojejunostomy technique selection

Objective The risk of current pancreaticojejunostomy is carefully considered from the perspective of the morphology of remnant pancreas,and we aimed to discuss the clinical outcomes of selecting different pancreaticojejunostomy techniques based on pancreatic morphology.Methods This was a prospective cohort study.The histopathology of remnant pancreatic tissues was categorized into four types based on preoperative radiological images and intraoperative palpation:Type Ⅰ:pancreas with hard texture in palpation,pancreatic atrophy,dilated pancreatic duct larger than 5 mm and remnant pancreatic surface <3 cm;Type Ⅱ:pancreas with hard texture in palpation,pancreatic atrophy and mild dilatation of pancreatic duct with the diameter of 3-5 mm and remnant pancreatic surface <3 cm;Type Ⅲ:pancreas with slightly hard texture,no atrophy,and normal or slightly dilated pancreatic duct with the diameter of 3-5 mm and remnant pancreatic surface ≥3 cm;Type Ⅳ:pancreas with soft texture,normal morphology and pancreatic duct.Results From January 2008 to August 2017,116 consecutive patients underwent pancreaticoduodenectomy in our center.Among them,10 patients with type Ⅰ underwent classic pancreatic ductal mucosa to mucosa anastomosis.19 patients with type Ⅱ underwent classic end to end invaginated pancreaticojejunostomy.45 patients with type Ⅲ underwent classic end to end invaginated pancreaticojejunostomy with overlapping U sutures;42 patients with type Ⅵ underwent total invaginated pancreaticojejunostomy.The post-operative pancreatic fistula occurred in 6 patients (5.2%) with one patient died.Postoperative bleeding occurred in 10 patients (8.6%),and gastroparesis occurred in 22 patients (19.0%).Overall complication rate was 33.6%.Conclusions Classification of pancreatic morphology based on preoperative radiological images and intraoperative palpation and the selection of corresponding pancreaticojejunostomy technique is theoretically rational and has the advantage of potentially reducing the risk of remnant pancreatic tissue.
7.Exposure risk assessment of aflatoxin B1 in edible vegetable oil by using the margin of exposure in Guangxi

Objective This report aims to assess the exposure risk of aflatoxin B1 in edible vegetable oil in Guangxi.Methods By using margin of exposure (MOE),the report analyzes the dietary exposure of aflatoxin B1 in edible vegetable oil with the data from contamination survey and dietary intake survey.Results For the vegetable oil sample,the content of aflatoxin B1 was between 0.50-320.00 μg/kg.The detection rate of peanut oil was 78.08% (114/146) which was higher than other vegetable oil,and the exceeding rate was 31.51% (46/146).For peanut oil,the average content was 30.80 μg/kg,the dietary exposure of the population was 17.30 ng/kg BW,and the MOE was 18.For the prepackaged peanut oil samples,the average content of aflatoxin B1 was 6.33 μg/kg,which was below the limit.While for the bulk peanut oil,the average content of AFB1 was 41.50 μg/kg,which was more than 1.08 times of the limit,and the dietary exposure was 25.59 ng/kg BW.The MOE of bulk peanut oil was 12,1/8 of the prepackaged peanut oil.Conclusion Food safety regulators should pay more attention to bulk peanut oil products,the priority in the risk management measures.At the same time,related department should also promote healthy education for the residents.

Objective To explore the evolution and treatment of traumatic subdural effusion(TSE).Methods The clinicsl materials of 66 patients with TSE were analyzed retrospectively.Results 53 patients were cured with comervative therapy,and other patients were evolved into chronic subdural hematoma(CSDH).8 patients with CSDH were cured with surgery and others with conservative therapy.Conclusion Patients with TSE don't need surgery,and then patients with clinical characteristics will be operated when TSE evolves into CSDH.

10.Changes of bcl-x(L) and bax mRNA expression following traumatic brain injury in rats.

OBJECTIVETo investigate the changes of bcl-2 gene family and the molecular mechanism of neuronal apoptosis following traumatic brain injury (TBI) in rats.
METHODSMale Sprague-Dawley (SD) rats were subjected to lateral fluid percussion brain injury (FPBI) of moderate severity. The bcl-x(L) and bax mRNA expression was detected by reverse transcription polymerase chain reaction (RT-PCR). In addition to morphological evidence of apoptosis, terminal deoxynucleotide transferase-mediated dUTP-biotin nick-end labeling (TUNEL) histochemistry was used to identify the DNA fragmentation in situ at both light and electron microscope levels, whereas characteristic internucleosomal DNA fragmentation of apoptosis was demonstrated by DNA gel electrophoresis.
RESULTSThe apoptotic response to trauma was regionally distinct and may be involved in both acute and delayed cell death. The bcl-x(L) mRNA expression of the impact site was significantly lower (67.42%+/-7.54%) than that of the ipsilateral hemisphere at 6 hours after injury (P<0.01). The decrease of bcl-x(L) mRNA expression preceded apoptosis at 24 hours after injury. The bax mRNA expression rose slowly, doubled at 3 days after injury and returned to the sham level slowly.
CONCLUSIONSDecreased expression of bcl-x(L) mRNA and increased expression of bax mRNA coincides with apoptosis following brain injury. The bcl-2 gene family is involved in neuronal apoptosis after TBI, and the changes of mRNA expression of the family members lead the neuronal cells to apoptosis.
