1.Polymorphism and Tissue Expression Analysis of TYR and MC1R Genes in Guinea Pigs with Different Coat-Color Phenotypes

Objective To explore the polymorphism of tyrosinase (TYR) and melanocortin 1 receptor (MC1R) genes and their mRNA expression levels in relation to coat-color phenotypes in guinea pigs, providing genetic markers for locating dominant traits in guinea pigs. Methods A total of 57 self-bred ordinary-level guinea pigs were selected and divided into three groups based on coat color: white (n=22), variegated (n=22) and black (n=13). The guinea pigs were euthanized with an overdose of pentobarbital sodium via intraperitoneal injection. DNA was then extracted from the dorsal skin tissue. Polymorphism in the coding sequence (CDS) of the exons of the TYR and MC1R genes in each group was detected by cloning and sequencing. The mRNA expression of the two genes in skin tissues was detected by real-time fluorescent quantitative PCR to investigate the relationship between these genes and guinea pig coat color. Results A single nucleotide polymorphism (SNP) site was found in the CDS region of TYR exon Ⅰ, where the base A was replaced by G. All white guinea pigs had the G/G genotype for TYR, while no deep-colored (variegated and black) guinea pigs exhibited the G/G genotype for TYR. Most deep-colored guinea pigs had the A/A genotype, and a few had A/G genotype. The A/A genotype frequency in black guinea pigs was higher than in variegated guinea pigs. A 2 760 bp sequence deletion was identified in the exon of the MC1R gene, marked as the - gene, with non-deleted samples marked as N gene. Most white guinea pigs had the -/- genotype for MC1R, variegated guinea pigs mainly had the -/N genotype, and black guinea pigs mainly had the N/N genotype, with a few showing the -/N. The TYR gene expression level was higher in white guinea pigs, lower in variegated guinea pigs, and intermediate in black guinea pigs, but there was no significant difference among the three groups (P>0.05). The MC1R gene expression level in white guinea pigs was extremely low, while both variegated and black guinea pigs showed significantly higher levels than white guinea pigs (P<0.01). Black guinea pigs showed significantly higher levels than variegated guinea pigs (P<0.05). ConclusionThe TYR and MC1R genes synergistically regulate coat color of guinea pigs. The G-site mutation in the TYR gene may lead to albinism, and the change of N-site in the MC1R gene affects the depth of the coat color.
2.Advance of research on Hereditary spastic paraplegia type 4

Spastic paraplegia type 4 (SPG4) is the most common type of autosomally inherited spastic paraplegia. Its main clinical features include typical simple hereditary spastic paraplegia, with neurological impairments limited to lower limb spasticity, hypertonic bladder dysfunction, and mild weakening of lower limb vibration sensation, without accompanying features such as nerve atrophy, ataxia, cognitive impairment, seizures, and muscle tone disorders. SPAST is the main pathogenic gene underlying SPG4, and various pathogenic SPAST variants have been discovered. This disease has featured a high degree of clinical heterogeneity, and the same pathogenic variant can have different age of onset and severity among patients and even within the same family. There is a lack of systematic research on the correlation between the genotype and phenotype of SPG4, and the pathogenic mechanism has remained controversial. This article has provided a review for the clinical characteristics, pathogenic gene characteristics, correlation between the genotype and phenotype, and pathogenic mechanism of this disease, with an aim to provide reference for its clinical diagnosis and treatment.
3.Application of nanobodies in autoimmune diseases

Autoimmune disease is a kind of heterogeneous disease caused by the loss of immune tolerance to autoantigens.In recent years,antibody drug has become an important choice for the treatment of autoimmune diseases.Monoclonal antibodies are limited by their large relative molecular mass and difficult to penetrate solid tissues,while nanobodies,the smallest antibody fragment known to date with intact antigen recognition,have gained much attention in the treatment of autoimmune diseases due to their ease of modifi-cation,high penetration,and ability to target more cryptic epitopes.The review focuses on the application of nanobodies in the treat-ment of diseases such as rheumatoid arthritis,acquired thrombotic thrombocytopenic purpura,systemic lupus erythematosus,psoria-sis,and looks forward to the prospect of nanobodies in the treatment of autoimmune diseases.
4.Study on improvement effects and mechanism of imperatorin on cachexia model mice

OBJECTIVE To investigate the improvement effects and mechanism of imperatorin on cachexia model mice. METHODS Fifteen male C57BL/6J mice were randomly divided into blank control group, model group and imperatorin group, with 5 mice in each group. Except for blank control group, the remaining mice were inoculated with LLC cell suspension subcutaneously on the dorsal surface, and the drug was administered by gavage daily from the 7th day of inoculation. The imperatorin group was gavaged with imperatorin suspension (0.5% sodium carboxymethylcellulose solution as solvent) at 60 mg/kg; blank control group and model group were given an equal volume of 0.5% sodium carboxymethylcellulose solution, for 13 d of continuous administration. During the administration period, food intake and body mass of mice were recorded daily and regularly, tumor long and short diameters were measured every two days, and tumor volume was calculated. The skeletal muscle mass and tumor mass of each group were weighed and the tumor-free body weight was calculated; the pathological changes of skeletal muscle were observed and the cross-sectional area of skeletal muscle fibers was calculated; the phosphorylation levels of signal transduction and activator of transcription 3 (STAT3) (measured as p-STAT3/STAT3 ratio), muscle atrophy box F gene (MAFbx), myostatin (Myog), B-cell lymphoma-2 (Bcl-2), Bcl-2 associated X protein (Bax), Caspase3 protein and mRNA expression were all detected. RESULTS Compared with blank control group, body mass and skeletal muscle mass of model group were decreased significantly (P<0.05), and reduced food intake, loose arrangement of skeletal muscle, large cell space were observed; the cross-sectional area of skeletal muscle fiber was significantly reduced, while p-STAT3/STAT3 ratio, protein and mRNA expressions of MAFbx, Bax and Caspase3 were somo_amour@163.com increased significantly (P<0.05). The protein and mRNA expressions of Myog and Bcl-2 were significantly reduced (P< 0.05). Compared with model group, body weight, tumor-free weight and skeletal muscle weight were increased significantly in imperatorin group (P<0.05); food intake increased, while the expressions of tumor weight and volume were decreased significantly (P<0.05); the expressions of above proteins and genes were improved significantly (P<0.05). CONCLUSIONS Imperatorin can improve the tumor cachexia state, the mechanism of which may be related to the regulation of ubiquitin-proteasome pathway and anti-apoptosis.
5.Effects of different intensity of wearable lower limb rehabilitation robot-assisted training on lower limb function after stroke

ObjectiveTo explore the effects of different intensity of wearable lower limb rehabilitation robot-assisted training on walking function, lower limb motor function, balance function and functional independence of stroke patients. MethodsFrom November, 2021 to December, 2022, 60 stroke patients hospitalized in Beijing Bo'ai Hospital were randomly divided into control group (n = 20), observation group 1 (n = 20) and observation group 2 (n = 20). All the groups received routine rehabilitation, while the control group received routine walking training 30 minutes a day, the observation group 1 received wearable lower limb rehabilitation robot-assisted training 30 minutes a day, and the observation group 2 received wearable lower limb rehabilitation robot-assisted training 60 minutes a day, for four weeks. They were assessed with Functional Ambulation Category scale (FAC), Fugl-Meyer Assessment-Lower Extremities (FMA-LE), Berg Balance Scale (BBS) and Rivermead Mobility Index (RMI) before and after treatment. ResultsOne case in the observation group 1 and three cases in the observation group 2 dropped down. The FAC, FMA-LE, BBS and RMI scores improved in all the three groups after treatment (|Z| > 3.448, |t| > 8.102, P < 0.001), and there was no significant difference in all the indexes among the three groups (|H| < 4.643, F = 1.454, P > 0.05); however, the improvement of BBS score was more in the observation group 1 than in the control group (P < 0.05), and the improvement of all the indexes was more in the observation group 2 than in the control group (P < 0.05). ConclusionThe wearable lower limb rehabilitation robot-assisted training may promote the recovery of walking function, lower limb motor function, balance function and functional independence of stroke patients, and high-intensity training seems to be more effective.
6.Clinical characteristics and related factors of recurrence after colorectal polypectomy

Objective :
The study provided evidence for clinical prevention of colorectal cancer by analyzing the characteristics of recurrence after colorectal polypectom.
Methods :
According to the number of polyps during the second colonoscopy,patients were divided into multiple recurrence group ( ≥3) and non-multiple recurrence group (<3) .The characteristics and risk factors of polyp recurrence were analyzed.
Results :
The clinical data of 370 patients who underwent two colorectal polypectomy were retrospectively analyzed.In univariate statistical analysis, polyp size (P = 0. 033) ,polyp site (P = 0. 002) ,and polyp color (P = 0. 032) were the risk factors for multiple polyp recurrence.In multivariate regression analysis,the number of polyps detected at the first colonoscopy (P = 0. 001) and the polyp site (P = 0. 001) were independent risk factors for multiple recurrence.
Conclusion
Multi- ple polyps on the first colonoscopy and polyps in the distal colon indicate an increased risk of multiple recurrence.
7.Analysis of pathogenic gene variant in two children with Treacher-Collins syndrome

Objective:To explore the clinical and genetic characteristics of two children with a clinical diagnosis of Treacher Collins syndrome (TCS).Methods:Whole-exome sequencing was used to screen potential variants in the two children. Confirmation of suspected variants was performed through Sanger sequencing , multiplex ligation dependent probe amplification and real-time PCR in probands and their parents.Results:A heterozygous deletion variant, c. 4357_4360delGAAA, was detected in case one, while was de novo and verified by Sanger sequencing. Thevariant was classified as pathogenic(PVS1 + PM2+ PM6)according to ACMG guideline. The heterozygous deletion of exon 1-7 was seen in the same gene in case 2, which MLPA verified as heterozygous deletion of exon 1-6. This deletion was inherited from the father with a normal phenotype, and the father’s TCOF1 gene was suspected to be chimeric heterozygous deletion of exon 1-6 verified by MLPA. Conclusion:The identified variants in the TCOF1 gene probably underlie the two cases of TCS. There was no apparent correlation between genotype and phenotype. In addition, it shows a high interfamilial variability ranging from normal to full presentation of TCS. Genetic detection provided clinical diagnosis and genetic counselling for TCS patients .
8.Effect of different working time on the prognosis of ischemic stroke patients undergoing intravenous thrombolysis.

OBJECTIVE:
To investigate the effect of working time on the prognosis of patients with ischemic stroke undergoing intravenous thrombolysis.
METHODS:
Clinical data of 3050 patients with ischemic stroke received intravenous thrombolysis from 71 hospitals in Zhejiang Province during June 2017 and September 2018 were retrospective analyzed. Whole day of Saturday and Sunday were defined as weekend; whole day of Monday to Friday were defined as weekdays; Monday to Friday 8:00-17:00 were defined as daytime of weekdays; Monday to Friday 17:01-07:59 on next day were defined as nights of weekdays; unconventional working time were defined as weekend and nights of weekdays. Good outcome was defined as mRS 0-2 at 3 months. Univariate analyses of baseline and prognostic variables in group of weekend and weekdays, nights of weekdays and daytime of weekdays, unconventional working time and daytime of weekdays were performed. Binary logistic regression was used to investigate whether weekend, nights of weekdays and unconventional working time were independent predicting factors of outcome after intravenous thrombolysis, respectively.
RESULTS:
There was no difference in 7-day mortality, 3-month mortality and good outcome at 3-month between weekend group and weekdays group, nights of weekdays group and daytime of weekdays group, unconventional working time group and daytime of weekdays group (all >0.05). Binary logistic regression results showed that weekends, nights of weekdays and unconventional working time were not independent predicting factors for outcome after intravenous thrombolysis (all >0.05).
CONCLUSIONS
The working time has not affected the outcomes of patients with ischemic stroke undergoing intravenous thrombolysis in studied hospitals of Zhejiang province.

9.Analysis of intravenous thrombolysis time and prognosis in patients with in-hospital stroke.

OBJECTIVE:
To compare the time delay between in-hospital stroke and out-of-hospital stroke patients, and to explore the influence factors for the prognosis of in-hospital stroke patients treated by intravenous thrombolysis.
METHODS:
Clinical data of 3050 patients with ischemic stroke who received intravenous thrombolysis in 71 hospitals in Zhejiang province from June 2017 to September 2018 were analyzed. Differences of time delay including door to imaging time (DIT), imaging to needle time (INT) and door to needle time (DNT) between in-hospital stroke (=101) and out-of-hospital stroke (=2949) were observed. The influencing factors for the outcome at 3 month after intravenous thrombolysis in patients with in-hospital stroke were analyzed using binary logistic regression analysis.
RESULTS:
Patients with in-hospital stroke had longer DIT[53.5 (32.0-79.8) min vs. 20.0 (14.0-28.0) min, <0.01], longer IDT[47.5(27.3-64.0)min vs. 36.0(24.0-53.0)min, <0.01], and longer DNT[99.0 (70.5-140.5) min vs. 55.0 (41.0-74.0) min, <0.01], compared with patients with out-of-hospital stroke; patients in comprehensive stroke center had longer DIT[59.5(44.5-83.3) min vs. 37.5(16.5-63.5) min, <0.01], longer DNT[110.0(77.0-145.0) min vs. 88.0 (53.8-124.3) min, <0.05], but shorter INT[36.5(23.8-60.3)min vs. 53.5 (34.3-64.8) min, <0.05], compared with patients in primary stroke center. Age (=0.934, 95% 0.882-0.989, <0.05) and baseline National Institute of Health Stroke Scale score (=0.912, 95% 0.855-0.973, <0.01) were independent risk factors for prognosis of in-hospital stroke patients.
CONCLUSIONS
In-hospital stroke had longer DIT and DNT than out-of-hospital stroke, which suggests that a more smooth thrombolysis process of in-hospital stroke should be established.

10.Association of atrial fibrillation with hemorrhagic transformation after intravenous thrombolysis in patients with ischemic stroke.

OBJECTIVE:
To investigate the association of atrial fibrillation (AF) with hemorrhagic transformation (HT) in patients with ischemic stroke treated by intravenous thrombolysis.
METHODS:
Clinical data of 3272 patients treated by intravenous thrombolysis from 71 hospitals in Zhejiang Province during June 2017 and December 2018 were retrospectively reviewed. Intracranial HT was defined as intracranial hemorrhage suggested by imaging examination 24 hours after intravenous thrombolysis. Patients were dichotomized into HT group (=533) and non-HT group (=2739). The association of AF and HT was analyzed by univariate analysis and binary logistic regression.
RESULTS:
Compared with the non-HT group, the HT group were older, had longer onset to needle time (ONT), higher baseline National Institute of Health Stroke Scale (NIHSS) score, higher baseline glucose level, and higher AF rate (<0.05 or <0.01). Binary logistic regression analysis revealed that AF was independently associated with HT (=2.527, 95%:2.030-3.146, <0.01).
CONCLUSIONS
AF is independently associated with the occurrence of HT in ischemic stroke patients treated with intravenous thrombolysis.
