1.Exploring the effects of "liver-smoothing and spirit-regulating" acupuncture on intestinal flora, lipopolysaccharide, and hippocampal TLR4/NF-κB signaling pathway in depressive disorder mice based on the gut-brain axis

		                        			To investigate the effects of "liver-smoothing and spirit-regulating" acupuncture on the intestinal flora, lipopolysaccharide (LPS) and the hippocampal toll-like receptor 4 (TLR4)/ transcription factor (NF)-κB signaling pathway in depressive disorder mouse model, and to explore its underlying mechanisms.
		                        		
		                        			Methods:
		                        			Eighteen male SPF-grade C57BL/6J mice were randomly assigned to the control, model, and acupuncture groups using a random number method, with six mice in each group. The depression disorder model was induced in mice from both the acupuncture and model groups using CUMS. The mice in the acupuncture group were treated with acupuncture at the acupoints of "Baihui" (DU20), "Yintang" (DU29), "Hegu" (LI4), and "Taichong" (LR3) on the 15th day of modeling, with a duration of 20 min per session, once per day, for 2 consecutive weeks. Behavioral differences were assessed using the sucrose preference test, open field test, and forced swim test. Hematoxylin-eosin staining was used to observe pathological changes in the hippocampus and colon. The levels of the inflammatory factors interleukin (IL)-1β, IL-6, tumor necrosis factor (TNF)-α, and LPS in the hippocampus and colon were measured using Enzyme-linked Immunosorbent Assay. Western blotting was used to detect the expression of TLR4 and NF-κB protein in the hippocampus. Changes in gut microbiota structure and abundance were analyzed by 16 S rDNA sequencing.
		                        		
		                        			Results:
		                        			Compared to the control group, the model group showed reduced sucrose preference rate, time in the center area, and total distance, with an increase in immobility time (P<0.01). Inflammatory pathological changes were observed in the hippocampal CA1 region and colon. The contents of IL-1β, IL-6, TNF-α, and LPS in the hippocampus and colon increased (P<0.01). The protein expression levels of hippocampal TLR4 and NF-κB were increased (P<0.01). The Chao1 index was increased (P<0.01). The relative abundances of Pseudomonadales, Acinetobacter, Moraxellaceae, Solibacillus, Escherichia_shigella, Enterobacteriaceae, Enterobacterales, Dubosiella, and Erysipelottichales were decreased, while the relative abundances of Alloprevotella and gram_negative_bacteriurh_cTPY_13 were increased (P<0.05). The pathways of lipopolysaccharide biosynthesis and pathogenic Escherichia coli infection were upregulated, and the pathway of terpenoid backbone biosynthesis was downregulated (P<0.01). Compared to the model group, the acupuncture group showed increased sucrose preference, time in the center area, and total distance, with a decrease in immobility time (P<0.01). The inflammatory pathological changes in the hippocampal CA1 region and colon were alleviated. The contents of IL-1β, IL-6, TNF-α, and LPS in the hippocampus and colon were reduced(P<0.01). The protein expression levels of hippocampal TLR4 and NF-κB were reduced (P<0.01). The Chao1 index was decreased (P<0.05), and the relative abundances of Dubosiella and Erysipelotrichaceae were increased, while the relative abundance of Rikenellaceae, Alloprevotella, and gram_negative_bacteriuch_cTPY_13 were decreased(P<0.05). The pathways of lipopolysaccharide biosynthesis and pathogenic Escherichia coli infection were significantly downregulated, and the pathway of terpenoid backbone biosynthesis was upregulated (P<0.01).
		                        		
		                        			Conclusion
		                        			" Liver-smoothing and spirit-regulating" acupuncture can improve depressive symptoms in depressive disorder mice, potentially through regulating the LPS and TLR4/NF-κB signaling pathway mediated by intestinal flora, reducing the inflammatory response of the hippocampus, and improving the pathological injury of the hippocampus.

		                        			There are different views on the theory of “spleen governs time”, which is still a hot spot in the study of Zangxiang (藏象) theory. Based on Zangxiang time-space view, it is found that the thinking mode of the spleen governing time theory follows space-time logic. It is believed that the different time views of the spleen governing time are all formed based on the space view that the spleen belongs to earth and resides in the center, and the zang time theory is developed with the unified time and space logic. Guided by Zangxiang time-space view, the origin of the spleen belonging to earth and residing in the center is traced, and the theoretical connotation and its clinical application of spleen governing time under different time-space logic are explored with reference to the four season and five zang theory, five season and five zang theory, six season and six zang theory, and eight season and eight zang theory. 
		                        		
		                        		
		                        		
		                        	
3.Value of Two-dimensional Speckle Tracking Imaging on Assessment of Right Ventricular Function in Patients With Fabry Disease

		                        			Objectives:To explore the clinical value of two-dimensional speckle tracking echocardiography(STE)on evaluating right ventricular function in patients with Fabry disease(FD). Methods:From August 2014 to June 2023,57 patients with FD and 57 age-(±2 year)and sex-matched healthy controls were included in this study.Clinical characteristics,echocardiographic parameters and right ventricular strain parameters were compared between the two groups.According to the left ventricular mass index(LVMI),FD patients were further divided into the left ventricular hypertrophy(LVH)group(n=22,men:LVMI≥115 g/m2,women:LVMI≥95 g/m2)and the non-LVH(n=35,men:LVMI<115 g/m2,women:LVMI<95 g/m2)group,the association between right ventricular strain and LVH in FD patients was analyzed. Results:(1)The brain natriuretic peptide(BNP)level was higher,heart rate and estimate glomerular filtration rate(eGFR)were lower in the FD group than in healthy control group(all P<0.05).Interventricular septum(IVS)and left ventricular posterior wall(LVPW)thickness as well as left ventricular mass index(LVMI)and right ventricular free wall(RVFW)thickness were significantly higher in FD group than in healthy control group(all P<0.05).(2)Subgroup analysis showed that the age,systolic blood pressure,BNP,LVMI and RVFW thickness were significantly higher,while eGFR was lower in the LVH group than in the non-LVH group(all P<0.05).(3)The left ventricular global longitudinal strain(LVGLS),right ventricular global longitudinal strain(RVGLS),right ventricular free wall strain(RVFWS),RVFWS at basal,middle and apical segment were lower in FD patients than in healthy controls(all P<0.05).There was no significant difference in right ventricular strain difference value(ΔRV)between the FD group and controls(P>0.05).(4)In the LVH group,LVGLS and RVGLS were significantly declined and ΔRV increased(all P<0.05),while RVFWS and all three segments preserved compared with the non-LVH group(all P>0.05). Conclusions:Myocardial strain analysis is valuable on the assessment of global and regional myocardial work in FD patients.RVGLS and RVFWS are reduced in FD patients.RVGLS is decreased in the LVH patients,while RVFWS is preserved in FD patients.
		                        		
		                        		
		                        		
		                        	
4.Evidence summary of exercise intervention in patients with multiple myeloma

		                        			Objective To search,evaluate and summarize the relevant evidence of exercise intervention for patients with multiple myeloma,and provide references for medical staff to guide patients to carry out exercise intervention.Methods The researchers conducted a systematical search in national and intemational databases,relevant guideline websites and professional society websites for related clinical decisions,recommended practices,guidelines,evidence summaries,expert consensuses,and systematic reviews for exercise intervention in patients with multiple myeloma since establishment of databases to May 2023.The quality evaluation,extraction and integration for evidence were conducted independently by 2 researchers,respectively.Results A total of 13 studies,including 2 guidelines,8 systematic reviews,2 expert consensuses and 1 randomized controlled trial,were included.There were 23 pieces of best evidence with 5 dimensions,including the exercise intensity effectiveness,contraindications and indications,exercise pre-intervention evaluation,exercise intervention style and intensity,and exercise intervention management.Conclusion The study summarized the best evidence of exercise intervention for patients with multiple myeloma.It is recommended for health professionals to fully consider the clinical situations to develop more detailed personalized exercise intervention programs based on patients'own situation.
		                        		
		                        		
		                        		
		                        	
5.Exploration on the Ecological Medical Model Involved in Seventy-Two Grid of Palm Technique

		                        			The seventy-two grid palm technique is an important theoretical source of traditional Chinese medicine hand diagnosis. Starting from the ecological medical model, we analyse the seventy-two grid palm technique, and believe that its diagnosis of human body integrates biological, ecological, psychological, social and other factors, and each factor is based on physiological and pathological theories, and its external social interpretation of the nature of the human body is inseparable from health state. It is proposed that the seventy-two grid palm technique should be integrated with the ecological and natural viewpoints based on the biomedical models or bio-psycho-social medical models, and the research should be conducted from the perspective of the ecological medical model, in order to promote the development of hand diagnosis.

		                        			Objective:To evaluate the diagnostic efficacy and practicality of the Jaundice color card (JCard) as a screening tool for neonatal jaundice.Methods:Following the standards for reporting of diagnostic accuracy studies (STARD) statement, a multicenter prospective study was conducted in 9 hospitals in China from October 2019 to September 2021. A total of 845 newborns who were admitted to the hospital or outpatient department for liver function testing due to their own diseases. The inclusion criteria were a gestational age of ≥35 weeks, a birth weight of ≥2 000 g, and an age of ≤28 days. The neonate′s parents used the JCard to measure jaundice at the neonate′s cheek. Within 2 hours of the JCard measurement, transcutaneous bilirubin (TcB) was measured with a JH20-1B device and total serum bilirubin (TSB) was detected. The Pearson′s correlation analysis, Bland-Altman plots and the receiver operating characteristic (ROC) curve were used for statistic analysis.Results:Out of the 854 newborns, 445 were male and 409 were female; 46 were born at 35-36 weeks of gestational age and 808 were born at ≥37 weeks of gestational age. Additionally, 432 cases were aged 0-3 days, 236 cases were aged 4-7 days, and 186 cases were aged 8-28 days. The TSB level was (227.4±89.6) μmol/L, with a range of 23.7-717.0 μmol/L. The JCard level was (221.4±77.0) μmol/L and the TcB level was (252.5±76.0) μmol/L. Both the JCard and TcB values showed good correlation ( r=0.77 and 0.80, respectively) and agreements (96.0% (820/854) and 95.2% (813/854) of samples fell within the 95% limits of agreement, respectively) with TSB. The JCard value of 12 had a sensitivity of 0.93 and specificity of 0.75 for identifying a TSB ≥205.2?μmol/L, and a sensitivity of 1.00 and specificity of 0.35 for identifying a TSB ≥342.0?μmol/L. The TcB value of 205.2?μmol/L had a sensitivity of 0.97 and specificity of 0.60 for identifying TSB levels of 205.2 μmol/L, and a sensitivity of 1.00 and specificity of 0.26 for identifying TSB levels of 342.0 μmol/L. The areas under the ROC curve (AUC) of JCard for identifying TSB levels of 153.9, 205.2, 256.5, and 342.0 μmol/L were 0.96, 0.92, 0.83, and 0.83, respectively. The AUC of TcB were 0.94, 0.91, 0.86, and 0.87, respectively. There were both no significant differences between the AUC of JCard and TcB in identifying TSB levels of 153.9 and 205.2 μmol/L (both P>0.05). However, the AUC of JCard were both lower than those of TcB in identifying TSB levels of 256.5 and 342.0 μmol/L (both P<0.05). Conclusions:JCard can be used to classify different levels of bilirubin, but its diagnostic efficacy decreases with increasing bilirubin levels. When TSB level are ≤205.2 μmol/L, its diagnostic efficacy is equivalent to that of the JH20-1B. To prevent the misdiagnosis of severe jaundice, it is recommended that parents use a low JCard score, such as 12, to identify severe hyperbilirubinemia (TSB ≥342.0 μmol/L).
		                        		
		                        		
		                        		
		                        	
7.Feasibility of a Radiomics Nomogram of Multiparametric Magnetic Resonance Imaging to Differentiate Fat-Poor Renal Angiomyolipoma from Nodular Renal Clear Cell Carcinoma

		                        			Purpose To investigate the value of MRI multi-sequence-based radiomic nomogram in identifying clear cell renal cell carcinoma from fat-poor renal angiomyolipoma in small renal masses(≤4 cm).Materials and Methods A retrospective analysis was performed for 78 renal masses in 75 patients with pathologically confirmed cases in the First Affiliated Hospital of Nanjing Medical University from July 2017 to December 2022,including 56 cases of renal clear cell carcinoma and 22 cases of fat-deficient angiomyolipoma,and all participants were divided into a training set(n=55)and a validation set(n=23)in a ratio of 7∶3.Radiomics features were extracted from T2WI and diffusion-weighted imaging sequences,and the t-test and minimum absolute shrinkage and selection algorithm were used for feature selection,the radiomics model was constructed,and the radiomics score was calculated.The clinical characteristics and subjective characteristics of MRI were evaluated to establish a clinical model,and the radiomics nomogram was constructed based on the radiomics score and clinical features,and the calibration,discrimination and clinical practicability of the nomogram were evaluated.Results A total of2 632 radiomics features were extracted from each patient,and 4 features were used to construct a radiomics model.The radiomics model had good discrimination ability in the training set[area under the curve(AUC)=0.979,95%CI 0.937-1.000)]and the validation set(AUC=0.833,95%CI 0.626-1.000).The radiomics nomogram had good calibration and discrimination ability in the training set(AUC=0.988,95%CI 0.963-1.000)and validation set(AUC=0.867,95%CI 0.698-1.000),which was better than the clinical model(AUC=0.725,95%CI 0.478-0.972)and radiomics model(AUC=0.833,95%CI 0.626-1.000)in the test set.Decision curve analysis showed that the clinical utility of nomogram was better than that of clinical factor model and radiomics features.Conclusion MRI-based radiomics nomogram combined with radiomics scores and clinical factors can be used to non-invasively distinguish clear cell renal cell carcinoma from alipid-deficient angiomyolipoma before surgery.

		                        			Objective:To investigate the expression level and clinical significance of HMGB1-TLR4-IL-23-IL-17A axis in se-rum of patients with psoriasis vulgaris.Methods:The expression levels of HMGB1,TLR4,IL-23 and IL-17A in serum of 60 patients with psoriasis vulgaris and 30 healthy volunteers were detected by ELISA.In addition,the differences of cytokines expression levels between moderate and severe psoriasis patients were compared,and the correlation between the expression levels of cytokines and the disease severity expressed by psoriasis area and severity index(PASI)were analyzed.The differences of expression levels of HMGB1-TLR4-IL-23-IL-17A axis before and after IL-17A inhibitor induction treatment were detected and compared in 22 moderate to severe psoriasis patients reached PASI75 and higher level.Results:The expression levels of serum HMGB1,TLR4,IL-23 and IL-17A in patients with psoriasis were obviously higher than those of healthy controls.Moreover,the expression levels of serum HMGB1,TLR4,IL-23 and IL-17A were even elevated in severe patients compared with moderate patients,and were positively correlated with PASI score.After induction treatment of IL-17A inhibitor,the expression levels of HMGB1-TLR4-IL-23-IL-17A aixs decreased significantly in serum of patients with psoriasis.Conclusion:HMGB1-TLR4-IL-23-IL-17A axis is highly expressed in patients with psoriasis vulgaris,and positively related to the disease severity,which may be involved in the disease process of psoriasis vulgaris and provide a new idea for the immunotherapy of psoriasis.
		                        		
		                        		
		                        		
		                        	
9.The relationship between non-alcoholic fatty liver disease and hepatic fibrosis with skeletal muscle mass in patients with type 2 diabetes mellitus

		                        			Objective:To investigate the relationship between non-alcoholic fatty liver disease(NAFLD) and hepatic fibrosis and skeletal muscle mass in patients with type 2 diabetes mellitus(T2DM).Methods:A total of 685 T2DM patients diagnosed at the Endocrinology department of Lanzhou University First Hospital, from April 2022 to May 2023, were divided into NAFLD and Non-NAFLD groups, and the NAFLD group was further categorized into fibrosis and non-fibrosis based on aspartate aminotransferase(AST) /alanine aminotransferase(ALT) level. The differences in appendicular skeletal muscle mass(ASM), appendicular skeletal muscle mass index(ASMI), and the prevalence of muscle mass loss were compared across groups. The correlations between ASMI and NAFLD, as well as liver fibrosis were analyzed by binary logistic regression.Results:Among male T2DM patients, those with NAFLD had lower ASMI levels and a higher prevalence of muscle mass reduction compared to non-NAFLD group. Among female T2DM patients, those with NAFLD had lower levels of ASM and ASMI, and a higher prevalence of muscle mass reduction compared to non-NAFLD group. ASMI levels in both male and female T2DM patients were independently negatively correlated with NAFLD risk( OR=-0.696, 95% CI 0.579-0.837; OR=-0.757, 95% CI 0.629-0.911). In NAFLD patients, ASM and ASMI levels were lower in those with liver fibrosis compared to those without fibrosis; however, the prevalence of muscle mass reduction did not differ significantly. Among male NAFLD patients, ASMI levels were independently negatively correlated with the risk of liver fibrosis( OR=-0.726, 95% CI 0.537-0.983), while no correlation was found in female patients. Conclusion:Reduced muscle mass is independently associated with the risk of NAFLD in both male and female T2DM patients. 10.A clinical study of different doses and frequencies of Sildenafil in the treatment of erectile dysfunction

		                        			【Objective】 To evaluate the efficacy and safety of different doses and frequencies of oral Sildenafil in the treatment of erectile dysfunction (ED). 【Methods】 The randomized,open clinical trial included 120 ED patients who met the inclusion and exclusion criteria. The patients were randomly divided into the following groups:on-schedule (25 mg/day),on-demand (50 mg,taken irregularly half an hour before each sexual life),new regular group (25 mg/day,50 mg more before each sexual life),regular group (100 mg/time,twice/week). All treatments lasted for 8 weeks. The follow-up indexes included the five-item International Index of Erectile Function (IIEF-5),Erection Hardness Scale (EHS) and Sexual Encounter Profile (SEP2/3). The adverse reactions were recorded. 【Results】 The IIEF-5 scores of the four groups were significantly higher than those after baseline treatment (P<0.001),but there was no significant difference among the four groups (P>0.05). In terms of effective rate,at the 16th week,there were significant differences between the on-demand group (10.7%) and new regular group (62.1%),and between the on-demand group (10.7%) and regular group (50.0%) (P<0.001). In terms of EHS, the percentage of grade 4 patients in regular group was significant higher than that in the on-demand group at the 8th week and 16th week (all P<0.05). In terms of positive rate of SEP-3,there was a significant difference between the on-demand group and regular group (P=0.042) at the 16th week. In the course of treatment,there were transient adverse reactions such as headache,blurred vision,stuffy nose and back pain,which did not affect the treatment. 【Conclusion】 All of the four treatment methods of oral sildenafil showed good efficacy. Both regular group and new regular group maintained good clinical efficacy during the follow-up,which is better than that of the on-demand group.
