1.Effect of miR-1298-5p on biological behavior of non-small cell lung cancer cells and tumor immune microenvironment by targeting MSH2 gene

		                        			Objective:To investigate the potential mechanism of miR-1298-5p in non-small cell lung cancer(NSCLC)to regu-late the MSH2 gene and its effect on the biological behavior of tumor cells and the tumor immune microenvironment.Methods:Bioin-formatics was used to identify the key genes and miRNA involved in NSCLC.Cell proliferation was detected by CCK-8 assay,and cell invasion and migration were detected by Transwell assay.Levels of inflammatory factors were detected by ELISA assay.Western blot was used to measure the expression of MSH2 in cells,and fluorescence quantitative polymerase chain reaction(RT-qPCR)was used to detect the expressions of miR-1298-5p and MSH2 gene in NSCLC cells.Dual luciferase reporter gene assay was performed to verify the targeting relationship between miR-1298-5p and MSH2.Spearman correlation analysis was performed to correlate miR-1298-5p with immune cells and immune factors in the tumor immune microenvironment.Results:The level of miR-1298-5p was down-regulated in NSCLC cells compared with normal lung tissue cells.miR-1298-5p overexpression inhibited the proliferation,migration and inva-sion of NSCLC cells.MSH2 was confirmed to be a target gene of miR-1298-5p using luciferase reporter gene assay.Furthermore,down-regulation of miR-1298-5p in NSCLC cells could be reversed by silencing MSH2.miR-1298-5p expression levels were negatively corre-lated with the levels of Treg,IL-10,and TGF-β,and positively correlated with the levels of CD3+T,CD4+T,CD8+T,NK cells,IL-2,and IFN-γ.Conclusion:miR-1298-5p negatively regulates MSH2 to inhibit the proliferation,invasion and migration of NSCLC cells and improve the tumor immune microenvironment.
		                        		
		                        		
		                        		
		                        	
2.Analysis of the efficacy and safety of bone disease treatment in patients with newly diagnosed multiple myeloma treated with denosumab or zoledronic acid

		                        			Objective:This study investigated the efficacy and safety of denosumab (DENOS) versus zoledronic acid (ZOL) in the bone disease treatment of newly diagnosed multiple myeloma.Methods:The clinical data of 80 patients with myeloma bone disease (MBD) at the Fifth Medical Center of PLA General Hospital between March 1, 2021 and June 30, 2023 were retrospectively reviewed. Eighteen patients with severe renal impairment (SRI, endogenous creatinine clearance rate<30 ml/min) were treated with DENOS, and 62 non-SRI patients were divided into DENOS (30 patients) and ZOL group (32 patients) .Results:Hypocalcemia was observed in 26 (33%) patients, and 22 patients developed hypocalcemia during the first treatment course. The incidence of hypocalcemia in the non-SRI patients of DENOS group was higher than that in the ZOL group [20% (6/30) vs 13% (4/32), P=0.028]. The incidence of hypocalcemia in SRI was 89% (16/18). Multivariate logistic regression analysis revealed that endogenous creatinine clearance rate<30 ml/min was significantly associated with hypocalcemia after DENOS administration ( P<0.001). After 1 month of antiresorptive (AR) drug application, the decrease in the serum β-C-terminal cross-linked carboxy-telopeptide of collagen type I concentrations of SRI and non-SRI patients in the DENOS group were significantly higher than that in the ZOL group (68% vs 59% vs 27%, P<0.001). The increase in serum procollagen type Ⅰ N-terminal propeptide concentrations of patients with or without SRI in the DENOS group were significantly higher than that in the ZOL group (34% vs 20% vs 11%, P<0.05). The level of intact parathyroid hormone in each group increased after AR drug treatment. None of the patients developed osteonecrosis of the jaw and renal adverse events, and no statistically significant differences in the overall response rate, complete remission and stringent complete remission rates were found among the groups ( P>0.05), and the median PFS and OS time were not reached ( P>0.05) . Conclusions:In the treatment of MBD, DENOS minimizes nephrotoxicity and has strong AR effect. 3.Surgical Options for Appropriate Length of J-Pouch Construction for Better Outcomes and Long-term Quality of Life in Patients with Ulcerative Colitis after Ileal Pouch-Anal Anastomosis

		                        			 Background/Aims:
		                        			Total proctocolectomy with ileal pouch-anal anastomosis (IPAA) is widely accepted as a radical surgery for refractory ulcerative colitis (UC). Definite results on the appropriate pouch length for an evaluation of the risk-to-benefit ratio regarding technical complications and long-term quality of life (QOL) are still scarce. 
		                        		
		                        			Methods:
		                        			Data on UC patients who underwent IPAA from 2008 to 2022 in four well-established pouch centers affiliated to China UC Pouch Center Union were collected. 
		                        		
		                        			Results:
		                        			A total of 208 patients with a median follow-up time of 6.0 years (interquartile range, 2.3 to 9.0 years) were enrolled. The median lengths of the patients’ short and long pouches were 14.0 cm (interquartile range, 14.0 to 15.0 cm) and 22.0 cm (interquartile range, 20.0 to 24.0 cm), respectively. Patients with a short J pouch configuration were less likely to achieve significantly improved long-term QOL (p=0.015) and were prone to develop late postoperative complications (p=0.042), such as increased defecation frequency (p=0.003) and pouchitis (p=0.035). A short ileal pouch was an independent risk factor for the development of late postoperative complications (odds ratio, 3.100; 95% confidence interval, 1.519 to 6.329; p=0.002) and impaired longterm QOL improvement (odds ratio, 2.221; 95% confidence interval, 1.218 to 4.050, p=0.009). 
		                        		
		                        			Conclusions
		                        			The length of the J pouch was associated with the improvement in long-term QOL and the development of late post-IPAA complications. 4.Medication rules of famous Beijing TCM doctor Wang Pei in the treatment of colon cancer based on data mining

		                        			Objective:To explore the medication rules of Professor Wang Pei in the treatment of colon cancer.Methods:The medical records of Professor Wang Pei's outpatient treatment for colon cancer were collected, entered into the Traditional Chinese Medicine Inheritance Computing Platform V3.0, and the laws of the prescription's nature, flavor, meridian, drug frequency, drug combination, association rules, and cluster analysis were explored.Results:A total of 65 prescriptions were collected, including 150 Chinese materia medica. The medicinal properties of these Chinese materia medica are mainly warm, cold and flat, the tastes were mainly sweet, bitter and pungent, and the meridians were mainly liver, spleen and kidney meridians. Dampness category and blood circulation promoting and blood stasis category were mainly used, and the most frequently used drugs include Poria, Polyporus, Scutellariae Barbatae Herba, Astragali Radix, Curcumae Rhizoma, and the pairs of Polyporus- Poria, Astragali Radix- Astragali Radix Praeparata cum Melle, Scutellariae Barbatae Herba- Curcumae Rhizoma. Twenty nine high-related drug pairs were obtained from association rules, including Polyporus- Astragali Radix Praeparata cum Melle- Astragali Radix, Poria- Scutellariae Barbatae Herba- Polyporus, Scutellariae Barbatae Herba- Curcumae Rhizoma- Polyporus. The cluster analysis showed the combination of 6 types of medicines mainly focuses on strengthening the spleen and removing dampness, nourishing qi and promoting blood, clearing heat and detoxifying, and astringing the intestines to stop bleeding. Conclusion:Professor Wang Pei mostly discusses the treatment of colon cancer from "spleen dampness, rectification deficiency, heat toxin, blood stasis, bleeding", and the treatment is "invigorating the spleen and removing dampness, invigorating Qi and strengthening the body, clearing away heat and detoxifying, promoting blood circulation and removing blood stasis, and astringent intestines to stop bleeding", which is in line with the thought of Traditional Chinese Medicine treatment of tumors, strengthening the righteousness and eliminating pathogenic factors, and and can be used for clinical reference.
		                        		
		                        		
		                        		
		                        	
5.Survey of clinicians' needs of support for conducting clinical research

		                        			Objective To explore and understand limitations and needs of support for conducting clinical research among clinicians in China.Methods The in-service clinician in our hospital were enrolled in as survey participants,survey questionnaires were designed and filled up by target audience in the questionnaire star platform.Results Main problems identified as restrictions for conducting clinical studies including lack of data platform,have ideas but not protocols,lack of research funding and project management.On the other hand,identified needs for support including research-related technical support,project management facilitation,training,external communication and exchange,as well as quality risk management.Conclusions Hospital should establish Clinical Research Unit to help clinicians to improve research quality by training,providing technical support and administrative service of clinical research.
		                        		
		                        		
		                        		
		                        	
6.Preliminary exploration and practice of clinical research assessment system based on a tertiary hospital

		                        			Objective:To construct assessment and evaluation system of clinical research and its application in practice. To explore the reliability, validity and discrimination of this index system, and evaluate its effect.Methods:The cluster was randomly sampled from 54 clinical departments. Taking the department as units, all individuals in each department was conducted a questionnaire survey for empirical analysis. Cronbach's coefficient and factor analysis were used to verify the reliability and validity of the index system. The correlation coefficient method was used to analyze the discrimination. The t-test method was used to calculate the discrimination. Extreme grouping method was used to compare the difference between high grouping and low grouping to validate the difference of each index.Results:Cronbach′s α of the whole index system is 0.840. Through exploratory factor analysis, four common factors were extracted, and the cumulative contribution rate is 56.278%. Through correlation coefficient method and t-test method, results show that except for the " concluding situation" , " number of visitors to clinical scientists workstation" and " personnel selected into talent category clinical research program" , the differences of other indicators were statistically significant ( P<0.05), and the distinguish index was between 0.31 and 0.64. Conclusions:The clinical research assessment system has good reliability and validity. And the discrimination is good. The index system can accurately assess the clinical research situation. 7.Practical exploration of clinical research unit platform construction and management mode based on research-oriented hospitals:The case study of a tertiary hospital in Shanghai

		                        			Objective To summarize the practical experiences of Clinical Research Unit (CRU) platform established by Xinhua Hospital affiliated to Shanghai Jiao Tong University School of Medicine,provide reference for domestic institutions that have prepared or to prepare to establish CRU platform.Methods Oriented by demand,this platform has set up six modules including project management module,methodology support module,data management module,quality risk management module,project transformation module and training and international exchange module.Besides,it also establishes and improves the corresponding module system,quality assurance,technical support and information data platform system.Results With this platform,the enthusiasm of departments to carry out clinical research has been continuously enhanced,the talent pool of clinical research has been expanded;the awareness of clinical research cooperation has been improved and the international cooperation fields have been further extended;the number of clinical research projects and funding have increased rapidly.Conclusions CRU Platform in Xinhua Hospital affiliated to Shanghai Jiao Tong University School of Medicine provides all-dimensional technical support and whole-process informatization management to improve clinical research quality and efficiency.While learning from the advanced international concepts,we still need to explore and improve the platform construction and management mode in line with general research-oriented hospitals in China,further to enhance the academic research internationally.
		                        		
		                        		
		                        		
		                        	
8.Analysis of inherited dysfibrinogenemia pedigree associated with a heterozygous and deletion mutantion in the FGG gene

		                        			Objective To analyze the phenotype and genotype of inherited dysfibrinogenemia pedigree associated with a novel heterozygous and deletion mutation in the FGG gene,and to investigate its molecular mechanism.Methods The clinical data were collected from the proband found at our hospital and her family members in April 2016.The activity plasma fibrinogen(Fg:C), activated partial thromboplastin time(APTT),prothrombin time(PT), thrombin time(TT)were detected by coagulation method and the antigen plasma fibrinogen(Fg:Ag), D-Dimer(D-D), fibrinogen degradation products (FDPs)were analyzed by immunoturbidimetry method.All of the exons and exon-intron boundaries of the genes of FGA, FGB and FGG with the fibrinogen(Fg)were amplified by PCR and followed by direct sequencing.And further verification were performed by cloning sequence and non-denatured polyacrylamide gel electrophoresis and silver staining.The conservatism of mutated gene locus were analyzed by ClustalX-2.1-win.The change of the protein spatial structure and the intermolecular forces with mutation were analyzed by Pymol.Results The Fg:C of the proband was significantly reduced(0.30 g/L)and the Fg:Ag of the proband was normal(2.00 g/L).Their Fg:C were both significantly reduced and the Fg:Ag were both normal(0.42 g/L,2.09 g/L & 0.47 g/L,2.42 g/L, respectively), these were found in her mother and grandma.Genetic analysis revealed a novel heterozygous and deletion mutation with c.944 _c.952 delCCTTTGATG in exon 8 of FGG gene in the proband,predicting a heterozygous 289_291delAla,Phe,Asp mutation.The same mutations were carried by her mother and grandma, but her father and grandpa were normal.Homology analysis indicated that the Ala 289,Phe290 and Asp291 were maintained highly conservative in homogenous species.Protein model analysis found that the original hydrogen bonds were disappeared when the deletion mutation happened with the Ala 289,Phe290 and Asp291.Conclusion The heterozygous and deletion mutation with 289_291delAla,Phe,Asp in the γchain of fibrinogen were identified that could cause the rearrangement of the Fg molecular space structure and the reduction of the structure stability,so the mutation probably underly the dysfibrinogenemia in this pedigree.(Chin J Lab Med,2018, 41:305-311)
		                        		
		                        		
		                        		
		                        	
9.Retrospective sampling survey to the implementation situation of research projects in one large general hospital from Shanghai area

		                        			Objective To investigate the implementation situation of scientific research projects in large general hospital,analyze the quantitative evaluation parameters to explore the improvement measures.Methods Sampling survey of all levels of research projects from one general hospital in Shanghai.Based on parameter index quantization,the progress of projects in different levels was com pared and analyzed.Results The progress index and achievement index are reliable quantitative indexes,and there is a linear correlation between these two indexes.Based on progress index,forecast for SCI publication of project can be carried out.There was no significant difference in the progress situa tion of all levels of projects.Higher level of project showed better SCI publication.Conclusions For the process management of scientific research projects at different levels,the quantitative indexes can be used to facilitate the early warning and supervision.Classified assessment should be implied to SCI publication for research projects at different levels.
		                        		
		                        		
		                        		
		                        	
10.Biomechanical properties of thoracic spine with various locations of metastatic defects:three-dimensional finite element analysis

		                        			BACKGROUND:The spinal column is the most common site of cancer metastases. Most of the previous biomechanical experiments utilized models with defects only in the vertebral body or posterior elements, but the biomechanical changes of the thoracic vertebrae and posterior part with various locations of metastasis deserve further research. OBJECTIVE:To set up the three-dimensional (3D) finite element model to investigate biomechanical effects by simulating combined destruction of vertebral body and other posterior components. METHODS:Based on CT data, we constructed the 3D geometric models of the thoracic vertebrae (T9-11), including intervertebral discs, ligaments and ribs using the Mimics software. The 3D models of T9 vertebra and different parts of the posterior thoracic vertebrae related with the metastasis could be simulated, including the control group with the intact vertebrae, the group of the T10 vertebrae with the right defective hemi-vertebrae, the group of the defective hemi-vertebrae with the defective ipsilateral pedicle, the group of the defective hemi-vertebrae with the defective ipsilateral costovertebral joint, the group of the defective hemi-vertebrae with the defective ipsilateral pedicle and costovertebral joint, the group of the defective hemi-vertebrae with the defective ipsilateral pedicle, costovertebral joint and transverse process. The corresponding 3D finite element models were established using the Abaqus software. The displacement and Von Mises stress distribution of the models were analyzed when the anterior compressive flexure load was applied. RESULTS AND CONCLUSION:When the anterior compressive flexure load was applied, the entire stiffness was proportionaly decreased when the more posterior parts destroyed, especialy destruction of vertebral body and pedicle significantly decreased. The destruction of posterior structures such as the thoracic rib joints and transverse processes was not great. However, the maximal Von Mises stress increased significantly when the vertebral body and pedicle were destructed, but additional costovertebral joint destruction slightly decreased the maximal Von Mises stress because of the stress was re-distributed.
