1.Analysis of the efficacy and safety of bone disease treatment in patients with newly diagnosed multiple myeloma treated with denosumab or zoledronic acid

Objective:This study investigated the efficacy and safety of denosumab (DENOS) versus zoledronic acid (ZOL) in the bone disease treatment of newly diagnosed multiple myeloma.Methods:The clinical data of 80 patients with myeloma bone disease (MBD) at the Fifth Medical Center of PLA General Hospital between March 1, 2021 and June 30, 2023 were retrospectively reviewed. Eighteen patients with severe renal impairment (SRI, endogenous creatinine clearance rate<30 ml/min) were treated with DENOS, and 62 non-SRI patients were divided into DENOS (30 patients) and ZOL group (32 patients) .Results:Hypocalcemia was observed in 26 (33%) patients, and 22 patients developed hypocalcemia during the first treatment course. The incidence of hypocalcemia in the non-SRI patients of DENOS group was higher than that in the ZOL group [20% (6/30) vs 13% (4/32), P=0.028]. The incidence of hypocalcemia in SRI was 89% (16/18). Multivariate logistic regression analysis revealed that endogenous creatinine clearance rate<30 ml/min was significantly associated with hypocalcemia after DENOS administration ( P<0.001). After 1 month of antiresorptive (AR) drug application, the decrease in the serum β-C-terminal cross-linked carboxy-telopeptide of collagen type I concentrations of SRI and non-SRI patients in the DENOS group were significantly higher than that in the ZOL group (68% vs 59% vs 27%, P<0.001). The increase in serum procollagen type Ⅰ N-terminal propeptide concentrations of patients with or without SRI in the DENOS group were significantly higher than that in the ZOL group (34% vs 20% vs 11%, P<0.05). The level of intact parathyroid hormone in each group increased after AR drug treatment. None of the patients developed osteonecrosis of the jaw and renal adverse events, and no statistically significant differences in the overall response rate, complete remission and stringent complete remission rates were found among the groups ( P>0.05), and the median PFS and OS time were not reached ( P>0.05) . Conclusions:In the treatment of MBD, DENOS minimizes nephrotoxicity and has strong AR effect. Hypocalcemia is a common adverse event but is usually mild or moderate and manageable.
2.Medication rules of famous Beijing TCM doctor Wang Pei in the treatment of colon cancer based on data mining

Objective:To explore the medication rules of Professor Wang Pei in the treatment of colon cancer.Methods:The medical records of Professor Wang Pei's outpatient treatment for colon cancer were collected, entered into the Traditional Chinese Medicine Inheritance Computing Platform V3.0, and the laws of the prescription's nature, flavor, meridian, drug frequency, drug combination, association rules, and cluster analysis were explored.Results:A total of 65 prescriptions were collected, including 150 Chinese materia medica. The medicinal properties of these Chinese materia medica are mainly warm, cold and flat, the tastes were mainly sweet, bitter and pungent, and the meridians were mainly liver, spleen and kidney meridians. Dampness category and blood circulation promoting and blood stasis category were mainly used, and the most frequently used drugs include Poria, Polyporus, Scutellariae Barbatae Herba, Astragali Radix, Curcumae Rhizoma, and the pairs of Polyporus- Poria, Astragali Radix- Astragali Radix Praeparata cum Melle, Scutellariae Barbatae Herba- Curcumae Rhizoma. Twenty nine high-related drug pairs were obtained from association rules, including Polyporus- Astragali Radix Praeparata cum Melle- Astragali Radix, Poria- Scutellariae Barbatae Herba- Polyporus, Scutellariae Barbatae Herba- Curcumae Rhizoma- Polyporus. The cluster analysis showed the combination of 6 types of medicines mainly focuses on strengthening the spleen and removing dampness, nourishing qi and promoting blood, clearing heat and detoxifying, and astringing the intestines to stop bleeding. Conclusion:Professor Wang Pei mostly discusses the treatment of colon cancer from "spleen dampness, rectification deficiency, heat toxin, blood stasis, bleeding", and the treatment is "invigorating the spleen and removing dampness, invigorating Qi and strengthening the body, clearing away heat and detoxifying, promoting blood circulation and removing blood stasis, and astringent intestines to stop bleeding", which is in line with the thought of Traditional Chinese Medicine treatment of tumors, strengthening the righteousness and eliminating pathogenic factors, and and can be used for clinical reference.
3.Flavonoids from the roots and rhizomes of Sophoratonkinensis and their in vitro anti-SARS-CoV-2 activity.

Acute respiratory infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) had caused a global pandemic since 2019, and posed a serious threat to global health security. Traditional Chinese medicine (TCM) has played an indispensable role in the battle against the epidemic. Many components originated from TCMs were found to inhibit the production of SARS-CoV-2 3C-like protease (3CLpro) and papain-like protease (PLpro), which are two promising therapeutic targets to inhibit SARS-CoV-2. This study describes a systematic investigation of the roots and rhizomes of Sophora tonkinensis, which results in the characterization of 12 new flavonoids, including seven prenylated flavanones (1-7), one prenylated flavonol (8), two prenylated chalcones (9-10), one isoflavanone (11), and one isoflavan dimer (12), together with 43 known compounds (13-55). Their structures including the absolute configurations were elucidated by comprehensive analysis of MS, 1D and 2D NMR data, and time-dependent density functional theory electronic circular dichroism (TDDFT ECD) calculations. Compounds 12 and 51 exhibited inhibitory effects against SARS-CoV-2 3CLpro with IC50 values of 34.89 and 19.88 μmol·L-1, repectively while compounds 9, 43 and 47 exhibited inhibitory effects against PLpro with IC50 values of 32.67, 79.38, and 16.74 μmol·L-1, respectively.

4.Retrospective sampling survey to the implementation situation of research projects in one large general hospital from Shanghai area

Objective To investigate the implementation situation of scientific research projects in large general hospital,analyze the quantitative evaluation parameters to explore the improvement measures.Methods Sampling survey of all levels of research projects from one general hospital in Shanghai.Based on parameter index quantization,the progress of projects in different levels was com pared and analyzed.Results The progress index and achievement index are reliable quantitative indexes,and there is a linear correlation between these two indexes.Based on progress index,forecast for SCI publication of project can be carried out.There was no significant difference in the progress situa tion of all levels of projects.Higher level of project showed better SCI publication.Conclusions For the process management of scientific research projects at different levels,the quantitative indexes can be used to facilitate the early warning and supervision.Classified assessment should be implied to SCI publication for research projects at different levels.
5.An Algorithm for Correcting Fetal Heart Rate Baseline.

Fetal heart rate (FHR) baseline estimation is of significance for the computerized analysis of fetal heart rate and the assessment of fetal state. In our work, a fetal heart rate baseline correction algorithm was presented to make the existing baseline more accurate and fit to the tracings. Firstly, the deviation of the existing FHR baseline was found and corrected. And then a new baseline was obtained finally after treatment with some smoothing methods. To assess the performance of FHR baseline correction algorithm, a new FHR baseline estimation algorithm that combined baseline estimation algorithm and the baseline correction algorithm was compared with two existing FHR baseline estimation algorithms. The results showed that the new FHR baseline estimation algorithm did well in both accuracy and efficiency. And the results also proved the effectiveness of the FHR baseline correction algorithm.

6.Development of an Analgesia Therapy System for Delivery Based on Bio-feedback Transcuataneous Electrical Nerve Stimulation.

Transcuataneous electrical nerve stimulation (TENS) analgesia as a non-drug method has received people's more and more attention recently. Considering problems of existing products, such as unstable performance and unsatisfied effectiveness, we developed a new analgesia therapy system for delivery based on bio-feedback TENS in our laboratory. We proposed a new idea for stimulation signal design, that is, we modulated a middle frequency signal by a traditional low frequency TENS wave in the new system. We designed different prescription waves for pain relief during a uterine contraction or massage between contractions. In the end, a bio-feedback TENS method was proposed, in which the waveforms of stimulation signals were selected and their parameters were modified automatically based on feedback from uterine pressure, etc. It was proved through quality tests and clinical trials that the system had good performance and satisfied analgesia effectiveness.

7.Assessment of risk factors of schistosomiasis transmission control in Jingzhou City

Objective To understand the key risk factors of schistosomiasis transmission in Jingzhou City,so as to provide the evidence for improving the treatment of these risk factors. Methods Each village of six counties was investigated and 3 envi-ronments were surveyed each village for the distribution of Oncomelania snails and animal stools in the field. The results were ana-lyzed and the risk factors of schistosomiasis transmission were assessed. Results The density of living snails was 0.43 snails per 0.1 m2,the frequency of the frames with snails was 9.12%,and no schistosome infected snails were found. All of the animal stools collected from the field were from bovines. The schistosome positive rate of animal stools was 37.50%(3/8)among the environ-ments,and the schistosome infection rate of stools was 8.11%(3/37). The schistosome infection rate of animal stools was 0 near the residence living sites,and the positive rates were 12.50%and 8.33%in the ditches and slopes,respectively(χ2=0.07,P>0.05). Conclusions Bovine is still the main infectious source of schistosomiasis,i.e. the main risk factor of the disease transmis-sion. Therefore,the strategy of controlling bovine should be strengthened.
8.Application value of ventilator positive airway pressure therapy in acute myocardial infarction patients with left ventricular failure

Objective To investigate the application value of breathing machine noninvasive positive pressure ventilation in acute myocardial infarction patients with left ventricular failure.Methods 70 acute myocardial infarction patients with left ventricular failure were randomly divided into observation group and control group,the control group received conventional treatment,on the basis of the control group,patients in the observation group were treated with noninvasive positive pressure ventilation.The treatment indicators and the proportion of converted to endotracheal intubation,mortality and length of hospital stay were compared between the two groups.Results Compared with the control group,the PaCO2,RR,HR and SBP of observation group reduced more significantly,PaO2 and SaO2 increased more significantly(t =4.2588,5.144,7.127,4.2814,6.4719,6.2629,all P < 0.05).Compared with the control group,the SV and EF of observation group increased more significantly,CVP decreased more significantly (t =12.0527,14.0937,14.6425,all P <0.05).The patients converted to endotracheal intubation and mortality ratio were significantly lower than the control group,the average length of stay was significantly shorter than the control group(x2 =4.6289,4.2000,t =6.3892,all P < 0.05).Conclusion In the treatment of acute myocardial infarction patients with left ventricular failure,noninvasive positive pressure ventilation can significantly improve cardiac function,lung ventilation and ventilatory function,increase the therapeutic effect.
9.The effects of inhaled budesonide on interleukin-8, C-reactive protein and procalcitonin in bronchoalveolar lavage fluid

ObjectiveTo investigate the effects of budesonide inhaled on the levels of interleukin-8 (IL-8), C-reactive protein (CRP) and procalcitonin (PCT) in bronchoalveolar lavage fluid (BALF) of patients with AECOPD treated by using inhalation of budesonide in different doses or by using injection of dexamethasone. MethodsNinety AECOPD patients with mechanical ventilation in ICU ward were enrolled from Jan. 2008 through Sep. 2010. All patients were selected in this study as per the criteria of AECOPD set by the Chinese Medical Association. Ninety AECOPD patients treated with routine therapy were randomly (random number) divided into 3 groups. The patients of group A were given budesonide 2 mg inhaled 1 time/d. The patients of group B was given budesonide 4 mg inhaled 1 time/d. The patients of group C had dexamethasone 2. 5 mg injected 1 time/12h. The changes of IL-8, PCT and CRP in both BALF and serum were respectively detected in three groups at the beginning of treatment, and 3 days and 7days after treatment. The detected data of variables were analyzed by SPSS 13.0 package. ResultsThe level of IL-8 in BALF declined in 3days and 7days after treatment, and the magnitude of decrease in the following order:Group B ( budesonide 4mg/d) > Group A ( budesonide 2 mg/d) > Group C ( intravenous dexamethasone)with significant differences among them ( P < 0. 05 ). The decrease in IL-8 in serum showed the similar trend, but there were no statistical differences among them. The CRP and PCT in both BALF and serum had no significant changes. There was a correlation between the concentration of IL-8 in BALF and the duration of mechanical ventilation.Conclusions The treatment with the inhalation of budesonide could more significantly reduced the IL-8 level in BALF compared with intravenous dexamethasone, whereas the CRP and PCT were not changed. The dynamic changes of IL-8 in BALF might be used as an indicator of prognosis in AECOPD patients with mechanical ventilation.
10.Effects of Xuebijing injection on serum protein level in early phase of septic rats.

OBJECTIVETo observe the effect of Xuebijing injection on serum protein level in the early phase of septic rats and explain the mechanism from the perspective of molecular biology.
METHODFifty-four healthy wistar rats were randomly divided into control group, sepsis group and Xuebijing treatment group. The rat model of sepsis was established with injecting lipopolysaccharide (LPS) through caudal vein. Serum total protein (TP) and albumin (ALB) were measured at the point of 6, 12 and 24 h with the established model. The expression of AMPK, eEF2 protein in liver in the three groups were detected by Western blot analysis.
RESULTCompared with control gruop, the concentrations of TP and ALB of sepsis group and Xuebijing group were no significant difference with 6, 12 h treatment TP and ALB in sepsis group was lower (P<0.01) than control group after 24 h, and the expression of phos-AMPK, pho-eEF2 protein in livers was increased (P<0.01) simultaneously. All measured indexes in Xuebijing group has no difference with control group. Compared with sepsis group, TP and ALB of Xuebijing group was significantly higher (P<0.05), but litle lower than control group, the expression of phos-AMPK, pho-eEF2 protein in livers was decreased (P<0.05) simultaneously.
CONCLUSIONThese data suggest that Xuebijing injection prohibits catabolising serum albumin and inhibit liver protein catabolism by method of AMPK way.
