1.Epidemic characteristics of epidemic Japanese encephalitis and antibody levels in healthy people in Longnan City, Gansu Province from 2015 to 2020

Objective:To understand the epidemic characteristics of epidemic Japanese encephalitis and the antibody level of healthy people in Longnan City, Gansu Province, analyze and evaluate its epidemic risk, and provide scientific basis for future prevention and control of Japanese encephalitis.Methods:By retrospective analysis, the data of Japanese encephalitis cases in Longnan City from 2015 to 2020 were collected from the "Japanese Encephalitis Monitoring Information Report Management System" of the "China Disease Prevention and Control Information System", and the distributions of the cases(time, region and population distribution) were statistically described and analyzed. In 2018, the cluster stratified sampling method was adopted to select Li County and Xihe County with higher incidence rate of Japanese encephalitis in Longnan City, and Wudu District and Hui County with lower incidence rate as monitoring sites, and 3 administrative villages (communities) were selected from each monitoring site to collect blood samples from healthy people to detect the level of Japanese encephalitis IgG antibodies.Results:From 2015 to 2020, a total of 261 cases of Japanese encephalitis were reported in Longnan City, with an average annual incidence of 1.66 per 100 000; among them, the number of reported cases in 2017 and 2018 accounted for 82.38% (215/261) of the total number of cases. The incidence of Japanese encephalitis was mainly concentrated in August, accounting for 72.03% (188/261); 9 counties (districts) in Longnan City had reported cases of Japanese encephalitis, and the top 5 counties (districts) with the number of cases were Li County (68 cases), Xihe County (64 cases), Wudu District (40 cases), Kang County (33 cases) and Cheng County (18 cases), accounting for 85.44% (223/261) of the total number of cases. The age of onset of Japanese encephalitis cases was mainly concentrated in the 40 - < 60 years old, accounting for 40.23% (105/261); male to female ratio was 1.00 ∶ 1.14 (122 ∶ 139); the occupation was mainly farmers, accounting for 75.86% (198/261). A total of 887 sera samples from healthy people were tested, and the antibody positive rate was 91.09% (808/887); the difference in antibody positive rate of different age groups was statistically significant (χ 2 = 5.77, P < 0.05); the differences of positive rate of antibodies between different genders and regions were not statistically significant (χ 2 = 0.45, 2.06, P > 0.05). Conclusions:The incidence of Japanese encephalitis in Longnan City is mainly middle-aged farmers, with a high incidence in August each year. After the outbreak of Japanese encephalitis in Longnan City in 2017 and 2018, it has showed a low epidemic trend, and the positive rate of Japanese encephalitis antibody in healthy people is high. It is recommended to carry out regular monitoring of Japanese encephalitis immunization level of healthy people and strengthen the immunization of key populations in time.
2.Establishment of HPLC Fingerprint and Principal Component Analysis of Xiaojin Capsules

OBJECTIVE: To establish HPLC fingerprint of Xiaojin capsules, and to conduct principal component analysis. METHODS: HPLC method was adopted. The determination was performed on Agilent TC-C18(2) column with mobile phase consisted of acetonitrile-0.2% formic acid solution (gradient elution) at the flow rate of 1.0 mL/min. The detection wavelength was 240 nm, and column temperature was 25 ℃. The sample size was 20 μL. Acetyl-11-keto-β-boswellic acid was used as reference and HPLC fingerprints of 15 batches of Xiaojin capsules were determined. The similarity evaluation of common peaks was conducted by using the TCM Chromatographic Fingerprint Similarity Evaluation System (2004A edition) to confirm common peaks. Principal component analysis was conducted by using Minitab 17.0 software. RESULTS: The similarity of 1 batch of sample was lower than 0.800; there was no common peaks No. 13, 14, 15 in HPLC chromatogram of the batch, compared with other 14 batches. There were 15 common peaks in the HPLC fingerprints of 14 batches of samples and three chromatographic peaks were identified, such as quercetin,amentoflavone, acetyl-11-keto-β-boswellic acid. The similarity of 14 batches ranged 0.889-0.990. Through principal component analysis, accumulative contribution rate of 2 principal component factors was 94.4%. It was indicated that the content change of corresponding components of common peaks No. 8, 9, 11, 12, 14 in samples was an important reason for the quality difference of samples, especially common peaks No.8, 9. CONCLUSIONS: The established HPLC fingerprint and principal component analysis can provide reference for quality evaluation of Xiaojin capsules.
3.Mechanisms of peripheral basophils activation in patients with rheumatoid ar-thritis

Objective:To investigate the activation of peripheral basophils from patients with rheumatoid arthritis ( RA) and its mechanisms. Methods:The activation markers including CD203c and the proportion of IL-4 positive rate of peripheral basophils from RA patients and healthy controls were detected by flow cytometry. Serum levels of IgE in RA patients and healthy controls were detected by electrochemilu minescence immunoassay. Basophils were negatively isolated and co-cultured with or without purified IgE,anti-IgE as positive control,then,the expression of CD203c and propotion of IL-4 positive basophils were detected by flow cytometry. Results:The expression of CD203c and IL-4 positive rate of basophils from RA patients were higher than that of healthy controls (P<0. 05). Serum levels of IgE in RA were higher than that of healthy controls (P<0. 05). After co-cultured with isolated IgE from RA patients,basophils negatively isolated from healthy controls were activated,and higher expression of CD203c and proportion of IL-4 (P<0.05). Conclusion:Basophil activation is related with development of RA,and its activation is mainly mediated by IgE. Targeting basophil and its activation pathway would be expected to provide new strategies for the treatment of RA.
4.Correlation studies of P-glycoprotein and lymphocyte subgroup in patients with immune thrombocytopenia

Objective To discuss the correlation of P-glycoprotein(P-gp) and lymphocyte subgroup(CD4+,CD8+) in patients with immune thrombocytopenia.Methods A total of 20 patients with immune thrombocytopenia were selected from August 2015 to September 2016 in the Eighth Hospital Affiliated to Sun Yat-sen University.According to the platelet count,all patients were divided into mild group and severe group,20 cases of health persons were selected at the same period as the healthy group,flow cytometry technique was used to detect the plasma lymphocyte subsets(CD4+,CD8+) and P-gp level of all the objects.Pearson correlation analysis was used to analyze the relationship between plasma P-gp and plasma CD4+,CD8+,CD4+ / CD8+ level,analyzed all the objects plasma platelet count,P-gp and CD4+,CD8+,CD4+ / CD8+ levels.Results Plasma CD4+,CD8+,CD4+ / CD8+ and P-gp level of severe group were significant higher than those of mild group(t=3.587,2.957,2.315,2.646,P<0.05),plasma CD4+,CD8+,CD4+ / CD8+ and P-gp level of mild group patients were significant higher than those of healthy group(t=5.479,2.921,4.536,7.750,P<0.05).Pearson correlation analysis results showed that the plasma levels of P-gp and plasma CD4+,CD8+,CD4+ / CD8+ levels were positively correlated(r=0.683,0.617,0.548,P<0.05).Conclusion Plasma CD4+,CD8+,CD4+/CD8+and P-gp level is associated with immune thrombocytopenia patients condition,plasma levels of P-gp closely links with abnormal immune function in patients with hyperthyroidism,prompting detection the level of change could help doctors to assess patients medication curative effect.
5.Characterization of serum metabolite profile in patients with rheumatoid arthritis

Objective To investigate and characterize the serum metabolite profile of patients with rheumatoid arthritis (RA),a common autoimmune disease,which will be of help for early diagnosis in clinic.Methods Serum specimens from 26 patients with RA and 19 age-matched healthy volunteers were collected from the First Affiliated Hospital of Hunan University of Chinese Medicine from January through April 2015.Samples were detected on the gas chromatography-mass spectrometry (GC-MS),and serum metabolites were identified by the chemometric methods.Discriminative model of RA and healthy volunteers was established using partial least squares-linear discriminant analysis (PLS-LDA),and furthermore the established model was evaluated with double cross validation (DCV) for predicting ability.Finally,differential metabolites with clinical diagnostic potential were screened out by using subwindow permutation analysis (SPA).Results The established PLS-LDA discriminative model identified 48 metabolites.The total accuracy of the model approached 97.73%,in which the accuracy of the model for predicting RA and healthy volunteers was 96.15% and 100%,respectively.Our studies screened out 3-hydroxy butyric acid,phosphoric acid,isoleucine,mannose and hexadecanoic acid with clinical diagnostic potential based on the SPA.Conclusions Metabolomics with the application of GC-MS combined with the PLS-LDA method can distinguish patients with RA and healthy volunteers,and can aid in potential early diagnosis of RA.
6.Tujia ethnomedicine Xuetong suppresses onset and progression of adjuvant-induced arthritis in rats

Aim Kadsuraheteroclita ( Roxb ) Craib ( Schizandraceae) is a medicinal plant termed Xuetong in Chinese Tujia ethnomedicine. Xuetong possesses therapeutic effects of, in the terms of Chinese medical theories, reinforcing vital energy, promoting blood cir-culation, expelling wind-evil, and removing damp-e-vil, and has been long used for the prevention and treatment of rheumatic and arthritic diseases, especial-ly in the southern China. The HPLC analysis has iden-tified that the ethanol extract of Xuetong contains large-ly biologically active lignans and triterpenoids. Our previous studies have shown that KHS exhibits very fa-vorable safety profile and potent anti-inflammatory and analgesic activities. In the present study, we investiga-ted anti-arthritic effects and the possible mechanisms of Xuetongon adjuvant-induced arthritis ( AIA ) in rats. Methods AIA was established in male Sprague-Daw-ley ( SD ) rats as described previously, and animals were daily treated by gavage with Xuetong ethanol ex-tract ( 1. 0 g · kg-1 ) or vehicle ( 0. 3% CMC-Na ) throughout the 30-day experiment. The incidence and severity of arthritis were evaluated using clinical pa-rameters. On day 30, bone destruction of the arthritic joints was assessed by computed tomography( CT) and histopathological analyses. The serum levels of pro-in-flammatory cytokines TNF-α, IL-1β, and IL-6 were measured by ELISA. Results Treatment with 1. 0 g/kg Xuetong significantly inhibited the onset and pro-gression of AIA. The vehicle-treated rats all developed severe arthritis, while the incidence of AIA in the Xue-tong-treated rats was as low as 55%( P=0. 035 ) . The Xuetong -treated rats exhibited 1. 8 to 2. 3 fold reduc-tion of paw swelling, and gained 10 to 20% more body weight than the vehicle-treated AIA rats throughout the experiment. CT and histopathological examinations re-vealed that Xuetong markedly protected AIA rats from cartilage and bone destruction of joints. Moreover, the serum levels of TNF-α, IL-1β, and IL-6 were signifi-cantly decreased in the Xuetong-treated rats than the vehicle-treated AIA rats. Conclusions These data strongly support the clinical use of Xuetong for rheu-matic and arthritic diseases, and suggest that Xuetong is a valuable candidate for further investigation to be a new anti-arthritic drug with favorable safety profile.
7.Image features and clinical significance of pneumoconiosis with large shadow

Objective To investigate the image features,categories,and clinical significances of Pneumoconiosis Ⅲ with large shadow.Methods The research is based on the analysis of image features of 168 pneumoconiosis Ⅲ patients who are directly diagnosed by qualified medical professionals in our institution.These image features are large shadows bigger than 2 cm×1 cm which can be observed by high kilovolt chest radiographs,DR chest radiography and chest CT scan.Results Large shadows bigger than 2 ×1 cm show noticeable imaging characteristics,like distribution of superior and middle lung regions (95.2%),irregular shapes (97.6%),splayed or sausage like changes on both sides perpendicular with ribs (89.2%),small shadows of pneumoconiosis (98.8%),and convergence of large shadow to hilum or mediastinal (53.6%).CT scan is significantly better than DR radiography on detection rate of focal emphysema,focal cavities and swollen lymph node of mediastinal.Conclusion Combining the image features of high kilovolt chest radiographs,DR chest radiographs and CT scan are the main methods to identify the shadows,and thus direct diagnose pneumoconiosis Ⅲ;CT scan plays an important role in differential diagnosis.

9.Observations on Characteristic Decoction Cup Hot Compress Dose

Objective To administer a lower dose according to different indoor temperatures to give play to the effect of characteristic decoction cup hot compress and be able to save the cost of medicine. Method Twenty subjects were allocated, according to different indoor temperatures, to four groups, 5 cases each. Each subject received 10 sessions of decoction cupping with different doses. The decoction in all the cups was 45℃ before cupping. The temperature of decoction in the cups was measured after cupping. The average value of temperatures of the same doses at the same indoor temperature was calculated after cupping. The graph of average temperature curve of decoction in the cups was produced according to the indoor temperature after cupping with different doses. Finally obtained were the lowest doses of decoction with hot compress effect in the cup required at different indoor temperatures. Result To produce the effect of characteristic decoction cup hot compress, the decoction in the cup must be more than 55 mL in a indoor temperature of 10℃~14℃, more than 45 mL in a indoor temperature of 15℃~19℃ and more than 25 mL in a indoor temperature of 20℃~24℃. Only 20 mL of decoction was needed in indoor temperatures above 25℃. Conclusion A dose of decoction is flexibly selected according to a seasonal change in indoor temperature. That can not only have the effect of hot compress but also save the dosage.
10.Innovation of characteristic medicinal cupping devices.

OBJECTIVETo compare the differences in the characteristic medicinal cupping therapy between the traditional cupping device and the innovated cupping device.
METHODSFifty patients of neck and low back pain were selected. The self-comparison was adopted. The cupping therapy was applied to the acupoints located on the left or right side with the traditional cupping device and the innovated cupping device. The cupping sites were centered at bilateral Quyuan (SI 13) and Dachangshu (BL 25). The cups were retained for 10 min. The traditional cupping device was the glass with smooth border, 100mL. The innovated cupping device was the vacuum-sucking cup. The operative time, medicinal leakage, comfort and cupping marks were observed for the two different cupping devices.
RESULTSThe operative time with the innovated medicinal cupping device was shorter obviously compared with the traditional one at Quyuan (SI 13) and Dachangshu (BL 25, both P<0. 05). The comfort with the innovated medicinal cupping device was remarkably improved as compared with the traditional one at the two acupoints (both P<0. 05). The medicinal leakage was similar between the two different devices during the cupping operation (both P>0. 05). The cupping marks with the innovated medicinal cupping device were much deeper than those with the traditional one after cupping therapy.
CONCLUSIONThe innovated cupping device is more convenent and comfortable in operation during the characteristic medicinal cupping therapy.
