1.Expert Consensus on Clinical Application of Ruyi Zhenbaowan

Osteoarthritis (OA) and stroke are common clinical diseases that reduce patients' quality of life and place a burden on families and society. Ruyi Zhenbaowan, a classic prescription in Tibetan medicine, have the functions of clearing heat, awakening the brain and opening orifices, relaxing tendons and promoting meridian circulation, and eliminating yellow water. Clinically, they are used to treat osteoarthritis, post-stroke sequelae, neuropathic pain, and other related conditions. Modern pharmacological studies have demonstrated their anti-inflammatory, analgesic, and nerve-repairing effects. However, current research remains insufficient regarding the appropriate indications, timing, and efficacy of this medicine in treating relevant diseases. To enhance clinicians' understanding of this medicine and promote its standardized and rational clinical use, a panel of national experts, including clinical specialists, Tibetan medicine practitioners, pharmacologists, and methodologists, formulated this consensus based on clinical experience and evidence-based practice. The Cochrane systematic review framework, the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system, and the nominal group method were employed to generate seven graded recommendations and 19 consensus-based suggestions. These recommendations clearly define the key points in the clinical application of Ruyi Zhenbaowan, including therapeutic indications, dosage and administration, treatment duration, and medication safety. The consensus specifically addresses the clinical efficacy, appropriate timing of administration, dosage strategies, treatment cycles, and combination medication strategies for treating osteoarthritis and stroke and provides an overview of safety considerations. The aim is to provide standardized guidance for hospitals and healthcare institutions nationwide to ensure the rational application of Ruyi Zhenbaowan in the treatment of osteoarthritis and stroke, reduce medication-related risks, and further leverage its clinical advantages. This consensus has been approved and issued by the China Association of Chinese Medicine, with the standard number GS/CACM 369-2024.
2.Prevention and treatment of monkeypox in pregnant women

Using China National Knowledge Infrastructure, Wanfang, and PubMed database, literature search was conducted with the keywords “pregnancy” and “monkeypox”, and 27 related research articles were selected for analysis. Through a comprehensive review of the related literature, we aim to improve our knowledge of this viral disease, better our prevention, treatment and responses to future monkeypox outbreaks in China, so as to better protect the safety of mothers and infants. Maternal monkeypox can be prevented and controlled, if active and effective measures are taken in time. Drawing on the experience and lessons from monkeypox outbreaks at home and abroad, it is suggested that hospitals and public health agencies at all levels should raise awareness, and establish an effective emergency preparedness system for the prevention and control of potential future outbreaks.
3.Circuit-Specific Control of Blood Pressure by PNMT-Expressing Nucleus Tractus Solitarii Neurons.

The nucleus tractus solitarii (NTS) is one of the morphologically and functionally defined centers that engage in the autonomic regulation of cardiovascular activity. Phenotypically-characterized NTS neurons have been implicated in the differential regulation of blood pressure (BP). Here, we investigated whether phenylethanolamine N-methyltransferase (PNMT)-expressing NTS (NTSPNMT) neurons contribute to the control of BP. We demonstrate that photostimulation of NTSPNMT neurons has variable effects on BP. A depressor response was produced during optogenetic stimulation of NTSPNMT neurons projecting to the paraventricular nucleus of the hypothalamus, lateral parabrachial nucleus, and caudal ventrolateral medulla. Conversely, photostimulation of NTSPNMT neurons projecting to the rostral ventrolateral medulla produced a robust pressor response and bradycardia. In addition, genetic ablation of both NTSPNMT neurons and those projecting to the rostral ventrolateral medulla impaired the arterial baroreflex. Overall, we revealed the neuronal phenotype- and circuit-specific mechanisms underlying the contribution of NTSPNMT neurons to the regulation of BP.

4.Training willingness and influencing factors of nursing management qualification certification for hospital nursing managers

Objective:To investigate the training willingness of nursing management qualification certification of hospital nursing managers, so as to provide reference for relevant departments and personnel.Methods:The convenient sampling was used to conduct a self-designed questionnaire survey on nursing managers and training intentions of nursing management qualification certification in secondary and above hospitals in Beijing from March 2019 to January 2020, and its influencing factors were analyzed. In this study, 100 questionnaires were distributed in the pre-experiment, and 98 valid questionnaires were recovered, with an effective recovery rate of 98.0%. During the formal study, 930 questionnaires were distributed and 920 valid questionnaires were recovered, with an effective recovery rate of 98.9%.Results:A total of 920 nurse managers were investigated. Only 212 (23.0%) managers mastered or were proficient in nursing management, and most of them were greater than or equal to 35 years old (χ 2=12.326, P<0.01) , worked for more than 20 years (χ 2=22.376, P<0.01) , and chief head nurse or above position levels (χ 2=7.079, P=0.008) . 722 (78.5%) managers were in great demands of nursing management knowledge, especially those under the age of 35 (χ 2=4.064, P=0.044) , with bachelor 's degree or above levels (χ 2=4.520, P=0.033) , working within 20 years (χ 2=9.332, P=0.002) , and chief head nurse or above position levels (χ 2=5.499, P=0.019) . 879 (95.5%) managers were willing to participate in nursing management qualification certification training, especially those with a bachelor 's degree or above levels (χ 2=6.372, P=0.012) and worked within 20 years (χ 2=18.174, P<0.001) . Conclusions:Hospital nursing managers, especially those with low seniority, have high demand for nursing management knowledge and desire for qualification certification training. Nursing management qualification certification training courses should be set up scientifically, ensure quality and authority and be appropriately adjusted according to the training objects.
5.Development and reliability and validity test of post quality standard scale for wound, ostomy and continence nurses

Objective:To compile the post quality standard scale for wound, ostomy and continence nurses in China and to test the reliability and validity of the scale.Methods:The first draft of the scale was formed through literature review, semi-structured interview and Delphi expert letter consultation. Using the purposive sampling method, a total of 796 wound, ostomy and continence nurses from 31 provinces, municipalities and autonomous regions of China were selected from April to May 2019 to test the reliability and validity of the scale. A total of 796 questionnaires were sent out and 693 were effectively received, with effective recovery of 87.1%.Results:The post quality standard scale for wound, ostomy and continence nurses included 3 first-level items and 77 second-level items. Three common factors were extracted from exploratory factor analysis and the cumulative variance contribution rate was 72.466%. The correlation coefficient between the first-level items was 0.709-0.898 and the correlation coefficient between the first-level item and the total scale was 0.859-0.937. The Cronbach's α coefficient of the total amount table was 0.992 and the half-fold reliability coefficient was 0.917 ( P<0.01) . Conclusions:The post quality standard scale for wound, ostomy and continence nurses has good reliability and validity, which can be used to evaluate the post quality of wound, ostomy and continence nurses.
6.Design and application of an intelligent nursing teaching platform based on training path of medical education

Objective:To explore design of an intelligent teaching platform based on the training path of medical education and its application in clinical nursing teaching and management.Methods:Starting from three stages of medical education in colleges and universities, post-graduation education, continuing education and teacher development and training, combining the whole process of nursing teaching, relying on the nursing management information platform, the intelligent nursing teaching platform was designed and developed by using the front and rear separation design framework and the whole station HTTPS encrypted connection. The platform included teaching plans, teaching training, exam management, teaching feedback, teaching resources and system management modules. Among them, the teaching training module was divided into six sub-modules, such as college education, post-graduation education, continuing education, teacher development, advanced training and base training. The intelligent teaching platform was applied to clinical nursing teaching and management.Results:The use of the platform improved the quality of nursing teaching, provided independent learning environment for nurses and helped the hospital save time and labor costs.Conclusions:The application of intelligent nursing teaching platform effectively saves nursing labor cost and teaching management cost and promotes the development of nursing teaching training assessment and management towards the direction of informatization, which is a training management platform worthy of wide application by nursing teaching managers.
7.Construction of position responsibilities index system of wound, ostomy and continence nurses based on Delphi method

Objective:To construct a position responsibilities index system of wound, ostomy and continence nurses (WOCN) based on the Delphi method.Methods:Through literature review, semi-structured interviews, and investigation and research on the current working situation of WOCN, the expert inquiry questionnaire was self-designed with reference to opinions of the expert coordination group. From January to April 2019, a total of 45 senior experts including professional doctors, WOCN representatives, representatives of international Enterostomal Therapist Training Program (ETNEP) and chairmen of various provincial professional committees in China were invited to conduct 3 rounds of Delphi expert consultation, and construct the position responsibilities index system of WOCN.Results:The positive coefficients of the 3 rounds of expert consultation were respectively 100.0% (45/45) , 93.3% (42/45) and 100.0% (42/42) , and the authority coefficient of the experts was (0.92±0.16) . The final position responsibilities index system of WOCN included 5 first-level indexes such as clinical practice, education, research, coordinated management and professional promotion and 33 second-level indexes.Conclusions:The index system constructed in the study clarifies the position responsibilities of WOCN, which helps to provide a certain theoretical basis for the work, performance and classification of specialist nurses.
8.The construction of training model of new head nurse based on demands: a grounded theory research

Objective:To explore new head nurses' demands of training and construct the theoretical framework of training demands of new head nurses. In order to provide reference for management department about making training plans and improving the competence of new head nurses.Methods:From November 2018 to March 2019, semi-structured in-depth interviews were conducted with 15 new head nurses in a ClassⅢ Grade A hospital in Beijing by using the objective sampling method. The data were sorted out and analyzed by the grounded theory, the themes were separated out and then the training model is built.Results:34 concepts, 13 categories and 4 main categories were extracted. The core categories including external-driven, self-development, training needs and key elements of training. Eventually, the theoretical framework of training demands in new head nurses was developed.Conclusions:Under the role of external drive and self-development, the new head nurse has a strong desire for training. The management department should formulate training strategies according to the specific needs of the new head nurse to meet their training intentions, help them adapt to the working environment as soon as possible and promote the development of their own education.
9.Best evidence summary of nursing measures in the second and third stages of normal delivery

Objective:To retrieve and analyze the relevant guidelines for the second and third stages of normal delivery, and summarize the evidence of high-quality guidelines so as to provide references for the formulation of clinical practice programs.Methods:We took Johns Hopkins evidence-based nursing practice model as theoretical guidance and set up an evidence-based team.We systematically searched major guideline publication websites, electronic resource databases, related academic groups and guideline networks at home and abroad, evaluated guidelines and summarized the best evidence, and the search time limit was up to June 1, 2019. The literature quality evaluation and evidence extraction were carried out independently by four researchers with evidence-based training.Results:This study included two guidelines, summarizing the 20 best evidences in 9 aspects of humanistic support, delivery companion, delivery position, fetal monitoring, delayed exertion, perineal protection, fundus compression, prevention of postpartum hemorrhage, and delayed umbilical cord clamping.Conclusions:Based on the existing evidence, it integrates the best evidence of nursing measures for the second and third stages of normal delivery. It is recommended that maternal and child institutions promote the transformation of the best evidence based on their actual conditions.
10.Construction of post quality standard index system for wound, ostomy and continence nurses

Objective:To establish a post quality standard index system for wound, ostomy and continence nurses (WOCN) .Methods:An expert coordination group was established in January 2019. Through literature search, semi-structured interviews, WOCN work status surveys, and concerning the opinions of the expert coordination group, the WOCN post quality standard index system was initially constructed. The convenient sampling method was adopted to conduct three rounds of Delphi expert letter consultation among 45 senior experts from all over China from January to April 2019.Results:The expert positive coefficients of three rounds of expert letter inquiries were respectively 100.0%, 93.3% and 100.0%, the expert authority coefficient was (0.92±0.16) , Kendall coordination coefficient was 0.241-0.383 ( P<0.01) and the coefficient of variation was all less than 0.25. The final WOCN post quality standard index system includes 5 first-level indicators (clinical practice ability, education ability, research ability, coordination management ability and professional promotion ability) and 79 second-level indicators. Conclusions:Clarifying WOCN post quality standards can promote the formulation of human resources decisions in various aspects, such as retraining and promotion of specialist nurses, and improve the scientific and effective management of specialist nurses.
