1.Therapeutic Strategy and Mechanism of Xijiao Dihuangtang in Recurrence of Psoriasis Vulgaris After Remission Based on Theory of "Latent Fire Causing Blood Stasis"

		                        			Psoriasis vulgaris is notoriously difficult to treat and prone to recurrence. Traditional Chinese medicine (TCM), however, has shown considerable efficacy in mitigating or suppressing such recurrence. The underlying reason lies in the TCM concept of "latent pathogens", which are prone to be reactivated by external pathogenic factors, thereby triggering relapse. At the early stage of recurrence, manifestations of "latent fire" often appear externally. If treatment is not thorough, the condition may shift into a state of "stalemate between healthy Qi and pathogenic factors", in which the disease appears on the skin but is rooted in deeper pathological layers, remaining unresolved and accumulating internally. Over time, blood stasis arises from fire, and the fire further congeals due to stasis, leading ultimately to recurrent flare-ups. This aligns with the modern immunological concept of "immunological memory" mediated by tissue-resident memory T cells (TRM) in the skin, which corroborates the TCM view of "latent fire inducing blood stasis". The interaction between TRM and keratinocytes (KC) parallels the entanglement of latent fire and latent stasis, both of which are deeply entrenched and difficult to resolve. The core pathogenesis of recurrent psoriasis vulgaris lies in "latent fire causing blood stasis". The hallmark is the deep concealment and persistence of latent fire and stasis, which linger and await an opportunity to reemerge. Based on this understanding, Xijiao Dihuangtang is employed to cool the blood, resolve stasis, and eliminate latent pathogens, and treatment is tailored according to the disease stage through three-phase syndrome differentiation. In the progressive stage, both exterior and interior are treated, with emphasis on clearing latent fire. In the stationary stage, the focus shifts to dispelling latent stasis and simultaneously regulating the Zang-fu organs. In the regressive stage, efforts are made to prevent the retention of latent pathogens and to strengthen healthy Qi. Accordingly, drugs effective in dispersing wind and clearing heat, pungent-moistening and dredging the collaterals, and tonifying deficiency and moistening dryness are often employed to achieve optimal outcomes. The precise mechanisms by which Xijiao Dihuangtang treats recurrent psoriasis vulgaris remain to be fully elucidated. Current research suggests it may intervene in the recurrence process through inhibiting KC proliferation via the PI3K/Akt/mTOR signaling pathway and glycolysis, regulating the Th1/Th2 and Th17/Treg cell balances to restore immune homeostasis, suppressing inflammatory cytokine production to alleviate the inflammatory response, modulating angiogenesis-related factors, such as vascular endothelial growth factor A (VEGF-A) and matrix metalloproteinase-9 (MMP-9), to control disease progression, and restructuring the gut microbiota to modulate systemic immunity and thereby influence the course of disease recurrence. 
		                        		
		                        		
		                        		
		                        	
2.Evaluation of the effect of contrast medium injection in CT enhanced examination of malignant tumor patients with standardized nursing procedure of totally implantable venous power port

		                        			Objective:To explore the standardization of totally implantable venous power port of nursing process in CT enhancement and application effect of contrast medium injection, so as to provide a safer and more efficient way for contrast medium injection in CT enhanced examination for patients with malignant tumors.Methods:A non-randomized prospective study was conducted, 358 patients with malignant tumors were selected in Sun Yat-sen Memorial Hospital, Sun Yat-sen University who underwent CT enhanced examination from August 1, 2022 to July 31, 2023, 179 patients who had been implanted totally implantable venous power port were selected as the experimental group, and the standardized nursing procedure was given. The other 179 patients were the control group, using radiology routine high-pressure intravenous indwelling needle as the contrast medium access, with routine peripheral venous nursing process. The incidence of contrast medium extravasation during CT enhanced examination was observed and compared between the two groups.Results:All the patients were included. There were 85 males and 94 females, aged (55.50±11.72) years old in the control group. There were 83 males and 96 females, aged (54.50±12.24) years old in the experimental group. The incidence of contrast medium extravasation was 0 in the experimental group and 3.35%(6/179) in the control group. The difference between the two groups was statistically significant (Fisher exact probability, P<0.05). Conclusions:The application of standardized nursing procedure of totally implantable venous power port to the injection of contrast medium in CT enhanced examination of malignant tumor patients, can significantly reduce the incidence of contrast medium extravasation.
		                        		
		                        		
		                        		
		                        	
3.Practice of navigation bronchoscopy and artificial intelligence in the basic navigational bronchoscopy skills and technique training

		                        			Objective:To investigate the effects of navigation bronchoscopy and artificial intelligence in the standardized training of residents on basic bronchoscopy skills, with the goal of enhancing their practical abilities.Methods:We selected 26 trainees (14 on standardized training of residents and 12 on standardized training of specialists) who participated in the basic navigational bronchoscopy skills and technique training (NBSTAT) at Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from September 2023 to January 2024. The training was structured around a modular curriculum with navigation as the core design concept, consisting of five modules: lesion identification, precise localization, route planning, navigation operation, and real-time error correction. Theoretical teaching and skill practice were combined. The skill proficiency of the trainees was assessed using the Ruijin Hospital bronchoscopy simulation training system and the electromagnetic navigation bronchoscopy system. SPSS 26.0 was used to perform the t-test and Wilcoxon rank-sum test for comparisons. Results:After the training, the trainees as a whole showed significant improvements in practicing the skills, with the simulated operation time was reduced from (125.08±48.16) seconds to (77.69±24.75) seconds, the number of errors decreased from 1 to 0, the total score increased from (60.77±15.01) to (75.19±17.63), and the navigation operation time was reduced from 3 (3, 4) minutes to 2 (2, 3) minutes (all P<0.05). There were no significant differences in these assessment items between resident trainees and specialist trainees. The satisfaction rate of the trainees with the training reached 100.00%. Conclusions:The curriculum is effective in basic bronchoscopy skill training, remarkably enhancing the bronchoscopy skills of resident and specialist physicians, which is a useful approach for physicians to cope with the continuous development of diagnostic and therapeutic techniques for pulmonary diseases.
		                        		
		                        		
		                        		
		                        	
4.Analysis of risk factors for death after acute Stanford type A aortic dissection compli-cated with organ malperfusion

		                        			Aim To investigate the risk factors of death after acute Stanford type A aortic dissection(ATAAD)complicated with malperfusion syndrome(MPS).Methods 244 patients with ATAAD complicated with MPS who ad-mitted to Nanchong Central Hospital from June 2020 to June 2023 were selected as the study objects.The postoperative survival of the patients was followed up and they were classified into survival group(156 cases)and death group(88 ca-ses).After propensity score matching(PSM)was applied in 1 ∶1 matching,there were 54 cases in both groups.Uni-variate and Logistic regression analysis was performed to analyze the risk factors of postoperative death in patients with ATA-AD complicated with MPS.Area under curve(AUC)of receiver operating characteristics(ROC)was used to analyze the prognosis of ATAAD complicated with MPS.The prediction model was established by using the regression equation y=1-1/(1+e-z)and the stability of the model was verified by cross-checking method.Results After matching,compared with the survival group(n=54),in the death group(n=54),the proportion of sex(male),the proportion of alcohol con-sumption,acute physiology and chronic health status Ⅱ(APACHE Ⅱ)score,sequential organ failure(SOFA)score,al-anine aminotransferase(ALT),aspartate aminotransferase(AST),total serum bilirubin(TSB),cholinesterase,serum creatinine(SCr),blood urea nitrogen(BUN),N-terminal pro-brain natriuretic peptide(NT-proBNP),D-dimer(D-D),white blood cell(WBC),neutrophile granulocyte(NEU),fibrinogen degradation product(FDP),platelet(PLT),fi-brinogen(FIB),C-reactive protein(CRP),hypersensitive troponin,operation time,ICU stay time,ventilator stay time,hospital stay,distal extremity hypoperfusion,renal hypoperfusion were significantly increased(P<0.05).Logistic analy-sis displayed that gender(male),history of drinking,NT-proBNP ≥271.86 ng/L,D-D≥0.74 mg/L and NEU≥13.06× 109 L-1 were independent risk factors in ATAAD patients complicated with MPS for postoperative death(P<0.05).The combination of NT-proBNP,D-D,gender(male),alcohol drinking history and NEU(referred to as"five factors")had the highest value in predicting ATAAD patients with MPS.The AUC of its ROC curve was 0.979(95%CI:0.937~0.984),the sensitivity was 94.3%,and the specificity was 91.8%,which was higher than the independent predictor.The best critical value predicted by the five factors was 5.02.The survival rate of the group>5.02 was significantly high-er than that of the group ≤5.02.Log Rank test P<0.01.A prediction model was established based on the important factors of postoperative death in ATAAD patients with MPS.The results showed that the model had good prediction accu-racy.Conclusion NT-proBNP≥271.86 ng/L,D-D≥0.74 mg/L,gender(male),history of alcohol consumption,and NEU≥×109 L-1 were independent risk factors for long-term prognosis in patients with ATAAD combined with MPS,and their combined application could effectively increase the accuracy of prognosis assessment.
		                        		
		                        		
		                        		
		                        	
5.Clinical and genetic analysis of patients of PCDH19 gene related epilepsy

		                        			Objective:To investigate the clinical and gene variant characteristics of PCDH19 gene related epilepsy, and improve the ability of clinicians in early disease identification. Methods:The clinical data of 3 PCDH19 gene related epilepsy patients admitted to Children′s Hospital Affiliated to Zhengzhou University from October 2018 to August 2023 diagnosed by gene detection were reviewed and analyzed. Results:All the patients are female, and the onset age of seizure ranged in their infancy. Seizures in clusters and fever sensitivity were observed in all patients, and were very hard to control by single-drug treatment. Proband 1 was seizure-free after 2 kinds of anti-epileptic drug treatment, but with mild degree of intellectual disability. Proband 2 had refractory epilepsy with severe degree of intellectual disability. Proband 3 was seizure-free after 2 kinds of anti-epileptic drug treatment and without intellectual disability. In the first family, the proband carried heterozygous c.369C>G variant in the PCDH19 gene which was identified as de novo after parental validation. In the second family, the proband carried c.1652T>A variant inherited from her mother. In the third family, the proband carried c.278G>A variant inherited from her father. The 3 mutations had not been reported in the Human Gene Mutation Database. Conclusions:PCDH19 gene related epilepsy is one special kind of X-linked inherited epilepsy syndrome characterized by seizures in clusters and sensitivity to fever. And gene detection can help with early diagnosis and make rational clinical strategies in time. 6.Clinical value of 1, 3-β-D glucan and mannan IgM/IgG antibodies in the early diagnosis of invasive candidiasis

		                        			Objective:To evaluate the diagnostic value of 1, 3-β-D glucan (BDG), mannan IgM antibody (Mn-IgM) and mannan IgG antibody (Mn-IgG) in invasive candidiasis and to compare the differences in the diagnostic capability of serological markers used alone or in combination.Methods:Serum samples of 126 patients with invasive candidiasis and 104 healthy people who took physical examination during the same period were collected. BDG was detected by dynamic chromogenic method, and Mn-IgM and Mn-IgG were detected by ELISA. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index, coincidence rate and Kappa value of the three serological markers used alone or in combination in the diagnosis of invasive candidiasis were analyzed and compared. The receiver operating characteristic (ROC) curves were drawn and the areas under the curves (AUCs) were calculated.Results:The levels of BDG, Mn-IgM and Mn-IgG in patients with invasive candidiasis were significantly higher than those in healthy people ( P<0.01). The sensitivity, specificity, Kappa value and AUC of BDG were 48.41%, 92.31%, 0.389 and 0.842. The sensitivity, specificity, Kappa value and AUC of Mn-IgM were 64.29%, 91.35%, 0.540 and 0.829. The sensitivity, specificity, Kappa value and AUC of Mn-IgG were 27.78%, 95.19%, 0.214 and 0.737. The sensitivity, specificity, Kappa value and AUC of BDG+ Mn-IgM were 76.19%, 88.46%, 0.637 and 0.921. The sensitivity, specificity, Kappa value and AUC of BDG+ Mn-IgG were 59.52%, 91.35%, 0.491 and 0.856. The sensitivity, specificity, Kappa value and AUC of Mn-IgM+ Mn-IgG were 69.84%, 90.38%, 0.588 and 0.891. The sensitivity, specificity, Kappa value and AUC of BDG+ Mn-IgM+ Mn-IgG were 80.16%, 88.46%, 0.679 and 0.922. Conclusions:The sensitivity of Mn-IgM was higher than that of BDG and Mn-IgG in the diagnosis of invasive candidiasis. When the serological biomarkers were used in combination, BDG+ Mn-IgM and BDG+ Mn-IgM+ Mn-IgG had relatively high Kappa value and AUC, showing high accuracy. The clinical diagnostic value of multiple serological biomarkers used in combination was significantly higher than that of any serological biomarkers used alone. Early combined detection and continuous monitoring of multiple serological biomarkers in patients with high risk of invasive candidiasis could be used clinically to adjust antifungal treatment strategies timely.

8.How " blood station-hospital" information management system based on Internet affect blood collection and supply

		                        			【Objective】 To evaluate the influence of establishing " blood station-hospital" information management system, based on the concept of Internet, on blood supply and use. 【Methods】 Blood information management system was established in our blood station, and connected to 21 secondary and above hospitals with blood storage function in Maoming to achieve interconnection and timely observation and recording of blood collection, supply and use. The working intensity, blood appointment, incidence of adverse reactions of clinical blood transfusion and satisfaction rate of clinical blood consumption before (April 2017 to March 2018) and after (April 2018 to March 2019) the application of the blood station-hospital information system were compared. 【Results】 In the same period before and after the implementation of blood station-hospital information system, the blood volume (U) collected was 78 249 vs 87 044.5, and the total blood supplied (U) was 225 276.5 vs 249 303, with growth rates at 11.24% and 10.67%, respectively; The average daily working intensity (s) of blood supply staff was 68.68±4.13 vs 41.71±3.76 (P<0.01), and average daily area (m2) was 9.82±3.51 vs 3.31±3.49 (P<0.05). The appointment time of clinical blood by telephone (s) was 110.34±6.79 vs 56.38±4.18 (P< 0.01), by network was 28.55±2.27 vs 13.48±2.76 (P<0.01); The incidence of transfusion adverse reactions was 0.035% (11/31 250) vs 0.012% (5/42 314) P<0.05); The satisfaction rates of clinical blood consumption were 85.71% (18/21) vs 100% (21/21) (P<0.01). 【Conclusion】 The implementation of blood station-hospital information system improved the efficiency of blood collection and supply in blood stations, and reduced the work intensity of blood supply staff. 9.Analysis of the health status, health-related behaviors and health needs of the elderly in China

		                        			Objective:To investigate the health status, health-related behaviors and health needs of elderlies in eastern, central and western China and to understand their health experience, for exploring an efficient medical service model, and optimizing allocation of medical resources.Methods:This study used the 2008-2018 database from Chinese longitudinal healthy longevity survey organized by Peking University. It described the concentration trend of medical distance by median, compared the health status, health-related behaviors and health needs of the elderly in eastern, central and western China by χ2 tests, and analyzed time-distribution trends of the poor health status, negative health behaviors and health needs by linear relation test. Results:Among 49 611 subjects included in the study, 7 881 of them self-evaluated their health status as poor, 19 445 evaluated their recent health status as deteriorated, while 14 954 lack routine physical examination, 35 141 lack physical exercise, 8 178 smoked, 7 859 drunk, 35 814 had no access to health education services in their communities, 35 676 expressed their health education needs, and 2 637 lack adequate medical services. The difference of distribution of health status, health-related behaviors and health needs of the elderly in eastern, central and western China was statistically significant ( P<0.05). The health status, negative health behaviors and health needs of the elderly in 2008-2018 presented a linear time distribution trend( P<0.05), while the overall medical distance in eastern, central and western China was 1.02 km, 1.00 km and 1.56 km respectively, and the medical distance in the rural area further than that in the urban areas. Conclusions:The health status of the elderly in China needed to be further improved, the proportion of negative health behaviors was high, and the health needs of the elderly in central and western China were large. Promoting an active medical service mode in central and western China, strengthening the interactions between medical institutions and residential communities will contribute to health needs fulfillment and medical resource utilization.
		                        		
		                        		
		                        		
		                        	
10.Best evidence summary of nursing measures in the second and third stages of normal delivery

		                        			Objective:To retrieve and analyze the relevant guidelines for the second and third stages of normal delivery, and summarize the evidence of high-quality guidelines so as to provide references for the formulation of clinical practice programs.Methods:We took Johns Hopkins evidence-based nursing practice model as theoretical guidance and set up an evidence-based team.We systematically searched major guideline publication websites, electronic resource databases, related academic groups and guideline networks at home and abroad, evaluated guidelines and summarized the best evidence, and the search time limit was up to June 1, 2019. The literature quality evaluation and evidence extraction were carried out independently by four researchers with evidence-based training.Results:This study included two guidelines, summarizing the 20 best evidences in 9 aspects of humanistic support, delivery companion, delivery position, fetal monitoring, delayed exertion, perineal protection, fundus compression, prevention of postpartum hemorrhage, and delayed umbilical cord clamping.Conclusions:Based on the existing evidence, it integrates the best evidence of nursing measures for the second and third stages of normal delivery.
