1.Cellular and cytokine immune responses during varicella zoster virus reactivation of herpes zoster infection

Objective To study changes in immune cells and cytokines during the reactivation stage of varicella-zoster virus(VZV)in patients with herpes zoster.Methods A total of 50 patients with herpes zoster and 30 healthy individuals were selected from Xi'an Ninth Hospital between May 2022 and October 2022.Flow cytometry was used to detect the proportion of peripheral blood CD3+cells,CD4+cells,CD8+T cells,B cells and NK cells,as well as levels of cytokines IL-2,IFN-γ,IL-10 and IL-6.We analyzed the immune mechanism of VZV reactivation stage in herpes zoster patients.Results Compared with the healthy control group,the proportion of CD3+cells and CD4+T cells in herpes zoster patients decreased significantly;the proportion of NK cells significantly increased;the levels of IFN-γ,IL-10 and IL-6 significantly increased;the proportion of CD8+T cells,B cells and IL-2 content showed an increasing trend,but there was no significant difference.In addition,the severity of neurological involvement in herpes zoster patients might affect changes in cytokine levels.Conclusion During the reactivation period of VZV,changes in the proportion of immune cells and cytokine expression levels are closely related to the occurrence and development of herpes zoster.
2.Research progress on early identification and prevention of immunotherapy-related cardiac toxicity in lung cancer patients

With the widespread application of immunotherapy, the overall survival rate of lung cancer patients continues to improve, and the accompanying immune-related adverse events are increasingly valued, with cardiac toxicity being the most severe. Early identification and prevention of immunotherapy-related cardiac toxicity in lung cancer patients significantly improves their quality of life. This paper summarizes the early identification (risk factors, evaluation tools, and prediction models) and preventive measures of immunotherapy-related cardiac toxicity in lung cancer patients so as to provide a reference for the early identification, prevention, and management of immunotherapy-related cardiac toxicity in lung cancer patients.
3.Development status and prospects of Oncology Cardiology Clinic

4.Management and outcomes of gastric leak after sleeve gastrectomy: results from the 2010-2020 national registry.

BACKGROUND:
Management of gastric leak after sleeve gastrectomy (SG) is challenging due to its unpredictable outcomes. We aimed to summarize the characteristics of SG leaks and analyze interventions and corresponding outcomes in a real-world setting.
METHODS:
To retrospectively review of 15,721 SG procedures from 2010 to 2020 based on a national registry. A cumulative sum analysis was used to identify a fitting curve of gastric leak rate. The Kaplan-Meier method and log-rank tests were performed to calculate and compare the probabilities of relevant outcomes. The logistic regression analysis was conducted to determine the predictors of acute leaks.
RESULTS:
A total of 78 cases of SG leaks were collected with an incidence of 0.5% (78/15,721) from this registry (6 patients who had the primary SG in non-participating centers). After accumulating 260 cases in a bariatric surgery center, the leak rate decreased to a stably low value of under 1.17%. The significant differences presented in sex, waist circumference, and the proportion of hypoproteinemia and type 2 diabetes at baseline between patients with SG leak and the whole registry population ( P = 0.005, = 0.026, <0.001, and = 0.001, respectively). Moreover, 83.1% (59/71) of the leakage was near the esophagogastric junction region. Leakage healed in 64 (88.9%, 64/72) patients. The median healing time of acute and non-acute leaks was 5.93 months and 8.12 months, respectively. Acute leak (38/72, 52.8%) was the predominant type with a cumulative reoperation rate >50%, whereas the cumulative healing probability in the patients who required surgical treatment was significantly lower than those requring non-surgical treatment ( P = 0.013). Precise dissection in the His angle area was independently associated with a lower acute leak rate, whereas preservation ≥2 cm distance from the His angle area was an independent risk factor.
CONCLUSIONS
Male sex, elevated waist circumference, hypoproteinaemia, and type 2 diabetes are risk factors of gastric leaks after SG. Optimizing surgical techniques, including precise dissection of His angle area and preservation of smaller gastric fundus, should be suggested to prevent acute leaks.

5.Application research of multiple intelligences theory combined with the ADDIE model in surgical clinical practice teaching

Objective:To explore the effect of multiple intelligences theory combined with the analysis, design, development, implementation, and evaluation (ADDIE) model in surgical clinical practice teaching.Methods:A total of 100 residents trained in Department of Gastrointestinal Surgery of Heping Hospital Affiliated to Changzhi Medical College from July 2019 to April 2020 were randomly divided into the control group ( n=50) and the observation group ( n=50). The control group used the ADDIE model, and the observation group adopted the multiple intelligences theory combined with the ADDIE model. The teaching assessment of the two groups was compared, and the core competence, critical thinking ability, self-evaluation, and teaching satisfaction of the two groups were evaluated. SPSS 22.0 was used for Chi-square test and t-test. Results:The scores of basic knowledges of gastrointestinal surgery, surgical clinical thinking and case analysis, routine skills and operations, and the total scores in the observation group were higher than those in the control group ( P<0.05). The scores of professional knowledge and skills, patient safety and rights, scientific research and academic ability, professional ethics, teamwork, personal and professional development ability in the observation group were higher than those in the control group ( P<0.05). While, there was no significant difference in the mastering of knowledge between the two groups ( P>0.05). The four dimensions of learning interest, self-learning ability, innovation ability, and clinical thinking establishment in the observation group were higher than those in the control group ( P<0.05). Conclusion:Multiple intelligences theory combined with ADDIE model in surgical clinical practice teaching can improve the teaching assessment results, significantly enhance the core competence, stimulate the learning interest, cultivate the self-learning ability and innovation ability of residents, and help them to establish clinical thinking ability.
6.Advances of long non-coding RNA encoded micro-peptides.

Long non-coding RNA (lncRNA) refers to non-coding RNA longer than 200 nt, with one or more short open reading frames (sORF), which encode functional micro-peptides. These functional micro-peptides often play key roles in various biological processes, such as Ca2+ transport, mitochondrial metabolism, myocyte fusion, cellular senescence and others. At the same time, these biological processes play a key role in the regulation of body homeostasis, diseases and cancers development and progression, embryonic development and other important physiological processes. Therefore, studying the potential regulatory mechanisms of micro-peptides encoded by lncRNA in organisms will help to further elucidate the potential regulatory processes in organisms. Furthermore, it will provide a new theoretical basis for the subsequent targeted treatment of diseases and improvement of animal growth performance. This review summarizes the latest research progress in the field of lncRNA-encoded micro-peptides, as well as the progress in the fields of muscle physiological regulation, inflammation and immunity, common human cancers, and embryonic development. Finally, the challenges of lncRNA-encoded micro-peptides are briefly described, with the aim to facilitate subsequent in-depth research on micro-peptides.

7. Taxonomy of Epimedium (Berberidaceae) with special reference to Chinese species

Epimedii Herba is a commonly used traditional Chinese herbal medicine. Five Epimedium species are included in Chinese Pharmacopoeia and most species of Epimedium are used as Epimedii Herba in practical application. However, as the largest herbaceous genus of the Berberidaceae, Epimedium has many taxonomic controversies which hinder the effective use of Epimedii Herba. This paper reviewed the taxonomic research related to Epimedium, including taxonomic history, taxonomic values of morphological characters, species and distribution, infra-genera taxonomic system and the taxonomic research of Chinese Epimedium. For instance, we recognized Epimedium wushanense and clarified that the species, as described in Flora Reipublicae Popularis Sinicae and Flora of China, actually includes four Epimedium species similar in leaflet shape. In general, it was recognized here that Epimedium comprises 62 species, of which 52 species are distributed in China. For Chinese Epimedium species with the most taxonomic problems, the taxonomic research on the taxa was reviewed and the newest species key was proposed along with proposals for those taxonomic problems needing further resolution. This review is of great implication for the identification, exploration and utilization of Epimedii Herba.
8.Meta-analysis of application of palliative care based on multidisciplinary cooperation in patients with heart failure

Objective:To evaluate the application effect of palliative care based on multidisciplinary cooperation in patients with heart failure.Methods:Randomized controlled trials of palliative care in patients with heart failure were searched by computer in PubMed, Embase, CINAHL, The Cochrane Library, China Biology Medicine disc, CNKI, Wanfang Database, VIP and other Chinese and English databases. The search time limit was from the establishment of the databases to December 31, 2020. Two researchers independently screened the literature, extracted the data and evaluated the quality. RevMan 5.4 software was used to conduct a meta-analysis of the included studies.Results:A total of 12 studies were included, including 1 402 patients. Meta-analysis results showed that compared with conventional management, palliative care based on multidisciplinary collaboration could improve quality of life of patients with heart failure ( SMD=0.72, 95% CI: 0.32-1.11, P<0.01), reduce the burden of symptoms on patients ( MD= -12.90, 95% CI: -20.25--5.54, P<0.01), and reduce depression level ( MD=-0.38, 95% CI: -3.69--0.08, P<0.05), anxiety level ( MD=-0.35, 95% CI: -0.66--0.05, P<0.05) and rehospitalization rate of patients ( RR=0.70, 95% CI: 0.57-0.86, P<0.01). There was no statistically significant difference in mortality between patients ( RR=1.02, 95% CI: 0.63-1.63, P=0.94) . Conclusions:Palliative care based on multidisciplinary collaboration can improve the quality of life of patients with heart failure, reduce the burden of symptoms, depression level and anxiety level and reduce the rehospitalization rate of patients, but it has no significant effect on reducing the mortality rate of patients. Given that palliative care is still in its infancy in China and is limited by the quality and quantity of included studies, the above conclusions need to be verified by more large-sample and high-quality studies.
9.Attitude and experience of health care personnel on hospice care for patients with heart failure: a meta-synthesis

Objective:To systematically synthesize the qualitative research on the attitude and experience of health care personnel towards the hospice care of patients with heart failure, so as to provide evidence support for hospice care.Methods:Qualitative research on the attitude and experience of health care personnel towards the hospice care of patients with heart failure was retrieved through computers in PubMed, Embase, Web of Science, CINAHL, China National Knowledge Infrastructure (CNKI), China Biomedical Literature Database, VIP and Wanfang. The retrieval time limit was from the establishment of the database to July 1, 2022. The quality of the included article was evaluated using the quality evaluation criteria for qualitative research of the Joanna Briggs Institute Evidence-Based Health Care Center in Australia. The results were synthesized using the aggregation synthesis method.Results:Totally 15 articles were included. A total of 61 results were extracted, and 9 categories were included and 3 synthetic results were synthesized and summarized, namely, the timing of intervention, obstacle factors and promotion strategies of hospice care for patients with heart failure.Conclusions:Health care personnel should involve in hospice care in the early stage of heart failure and take into account the wishes, needs of patients and their families and prognosis of patients. Health care personnel should strengthen multidisciplinary cooperation and communication with patients and their families. Relevant departments need to provide guidance and support from the policy aspect, and strengthen the publicity and education of health care personnel, patients and their families. We will work together to integrate hospice care into the disease management of patients with heart failure, so as to improve the quality of life of patients.
