Central serous chorioretinopathy(CSC), the first described pachychoroid disease, is characterized by visual distortions and loss of vision, which are commonly seen in middle-aged male. Research has demonstrated that ocular blood flow in CSC is in a state of overload, characterized by the dilation of vortex vein ampullae and choroidal vasculature. The obstruction of venous outflow is linked to scleral thickness, while the choriocapillaris exhibits perfusion deficits due to compression from the engorged vascular layer. Over time, vascular remodeling occurs, with venous anastomoses forming to create alternative drainage pathways and mitigate blood stasis. These abnormalities in vortex vein dynamics and choroidal circulation play a critical role in elucidating the underlying pathogenesis and clinical manifestations of CSC. This review highlights the alterations in vortex vein and choroidal vasculature in CSC, hoping to understand how the changes of blood flow affect the course of CSC and their correlation with treatment response. By evaluating blood flow dynamics, we aim to determine the disease stage more accurately, optimize therapeutic strategies, and ultimately enhance patient outcomes.

Objective:
To understand the pubertal development of primary and secondary school students in urban and rural areas of the Inner Mongolia Autonomous Region in 2021, so as to provide clues for further clarification of the health hazards of early pubertal initiation.
Methods:
From October to November 2021, primary and secondary school students in 12 provinces and cities of Inner Mongolia were selected by stratified cluster random sampling, including 21 662 girls who had reached menarche and 10 600 boys who had their first seminal emission, as well as 46 183 girls who had not reached menarche and 78 995 boys who had not experienced their first seminal emission. The height and weight of the primary and secondary school students were measured, and the time of first seminal emission or menarche was recorded. The study used the Mann-Whitney U test to analyze the differences between urban and rural students, Spearmans rank correlation to analyze the correlation between overweight and obesity and the age of pubertal initiation, and the Logistic regression model to analyze the correlation between pubertal initiation and age of pubertal initiation.
Results:
The age of menarche for urban girls [(12.31±1.29) years old] was earlier than that of suburban girls [(12.43±1.24) years old], and the age of first spermatozoa for urban boys [(13.85±1.20) years old] was statistically significant compared with that of suburban boys [(13.79±1.36) years old] (t=12.76, 10.58, P<0.05). There was a negative correlation between the age of menarche and overweight/obesity among girls aged 10-14 years (r=-0.169, P<0.05), and there was no statistically significant correlation between the age of spermarche and overweight/obesity in boys aged 5-9, 10-14, and 15-20 years (r=0.122, -0.022, -0.004, P>0.05). The occurrence of menarche in overweight girls was 1.06(95%CI=1.01-1.11) times higher than that in the normal group, and 1.13(95%CI=1.06-1.20) times higher among obese girls than normalweight girls. Compared to urban areas, primary and secondary school students in suburban counties observed the onset of puberty later [OR=0.88(95%CI=0.85-0.91) for menarche, OR=0.94(95%CI=0.89-0.99) for first spermatozoa] (P<0.05).
Conclusions
Obesity during childhood and adolescence is associated with earlier onset of puberty among female students in Inner Mongolia, and later onset of puberty among primary and secondary school students in suburban counties compared with those in urban areas.

Objective:To analyze the factors associated with residual low back pain in older adults with osteoporotic thoracolumbar compression fractures after surgery, and to construct a predictive model based on the collected data and assess its effectiveness. Methods:The clinical data of 101 older adult patients with osteoporotic thoracolumbar compression fractures who underwent surgical treatment at Xi 'an International Medical Center Hospital from January 2020 to December 2022 were retrospectively analyzed. The patients were divided into two groups based on the presence or absence of low back pain after surgery: those with low back pain and those without.Postoperative pain intensity was recorded in detail. Univariate and multivariate analyses were performed to identify independent influential factors. In addition, a predictive model was constructed and its accuracy and prediction ability were tested. Results:Among 101 patients with osteoporotic thoracolumbar compression fractures, 43 (42.57%) experienced low back pain after surgery, while 58 (57.43%) did not. Univariate analysis revealed that patients with low back pain were older compared with those without low back pain [(67.81 ± 3.65) years vs. (64.21 ± 3.11) years, t = 5.34]. Patients with low back pain had a higher level of education, with 62.79% holding a college degree or higher compared to 41.38% of patients without low back pain (χ2 = 4.52). The prevalence of psychological disorders was higher in patients with low back pain compared with those without low back pain (39.53% vs. 20.69%, χ2 = 4.28), as was the proportion of patients who smoke (34.88% vs. 13.79%, χ2 = 6.25). Additionally, the proportion of patients with a history of fractures was greater in patients with low back pain compared with those without low back pain (25.58% vs. 10.34%, χ2 = 4.09). The bone density in patients with low back pain was lower than that in patients without low back pain [(3.18 ± 0.48) g/cm3 vs. (3.67 ± 0.43) g/cm3, t = -5.38]. The American Society of Anesthesiologists (ASA) classification was also higher in patients with low back pain, with proportions of ASA Ⅰ, Ⅱ, Ⅲ, and Ⅳ being 18.60%, 25.58%, 32.56%, and 23.26% compared with 34.48%, 37.93%, 20.69%, and 6.90% in patients without low back pain (χ2 = 9.51). The volume of bone cement injected was lower in patients with low back pain than in those without low back pain [(4.62 ± 0.21) mL vs. (4.85 ± 0.18) mL, t = -5.91]. The incidence of cement leakage was higher in patients with low back pain than in those without low back pain (18.60% vs. 1.72%, χ2 = 6.71). All differences were statistically significant (all P < 0.05). Multivariate logistic regression analysis indicated that age and cement leakage were independent risk factors for residual low back pain ( OR = 1.634, 3.379, both P < 0.05), while bone density and the amount of injected cement were protective factors against residual low back pain ( OR = 0.017, 0.003, both P < 0.05). Model validation showed that the regression model had good fit (Hosmer-Lemeshow χ2 = 6.73, P > 0.05). A risk prediction model was constructed based on the selected variables, yielding a C-index of 0.792. Using the independent variables and P values, a receiver operating characteristic curve was generated to predict the likelihood of low back pain in patients, with areas under the curve of 0.749, 0.754, 0.754, 0.642, and 0.945, respectively. Conclusion:Residual low back pain after surgery in older adult patients with osteoporotic thoracolumbar compression fractures is associated with age, bone density, volume of bone cement injected, and cement leakage. The constructed nomogram model, based on these factors, demonstrates good predictive capability for residual low back pain in this population.

ObjectiveTo validate the alleviating effect of Gegen Qinliantang (GGQLT) on insulin resistance in db/db diabetic mice by regulating the silent information regulator 1 (SIRT1)/forkhead transcription factor O1 (FoxO1) autophagy pathway. MethodSeventy-five SPF-grade spontaneous type 2 diabetic db/db mice and 15 control db/m mice were selected and maintained on regular feed for one week before measuring blood glucose. They were randomly divided into six groups, with 15 mice in each group. The groups included a normal group (physiological saline, 0.2 g·kg-1), a metformin group (0.2 g·kg-1), high-, medium-, and low-dose GGQLT groups (31.9, 19.1, 6.9 g·kg-1), and a model group (physiological saline, 0.2 g·kg-1). They were orally treated with corresponding drugs for eight weeks, once daily. Fasting blood glucose (FBG) was measured using a Roche glucometer. Serum levels of high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), triglyceride (TG), and total cholesterol (TC) were measured using an automated biochemical analyzer. Fasting serum insulin (INS) levels were determined using enzyme-linked immunosorbent assay (ELISA), and the homeostasis model assessment of insulin resistance (HOMA-IR) was calculated. Western blot was used to detect the expression of Beclin-1, microtubule-associated protein 1 light chain 3 (LC3), and SIRT1/FoxO1 autophagy pathway-related proteins in liver tissues. Immunohistochemistry was performed to assess the expression of SIRT1, FoxO1, Beclin-1, and LC3B proteins in liver tissues. Transmission electron microscopy was used to observe the formation of autophagosomes in the liver. ResultCompared with the normal group, the model group showed significant increases in FBG, FINS, HOMA-IR, TC, TG, LDL-C, and HDL-C levels (P<0.01), and significant increases in the expression of SIRT1, Beclin-1, LC3, and FoxO1 proteins in liver tissues (P<0.01). Transmission electron microscopy revealed the highest number of autophagosomes in the model group. Compared with the model group, the metformin group and the low-, medium-, and high-dose GGQLT groups showed significant decreases in serum FBG, FINS, HOMA-IR, TC, TG, LDL-C, and HDL-C levels (P<0.05, P<0.01), significant decreases in the expression of SIRT1, Beclin-1, LC3 (P<0.05, P<0.01), and up-regulated FoxO1 protein (P<0.01). Transmission electron microscopy showed a reduction in the degree of autophagy in the treatment groups. Compared with the metformin group, the medium- and high-dose GGQLT groups showed significant decreases in FBG, FINS, and TG levels (P<0.01), significant decreases in the expression of SIRT1, Beclin-1, and LC3 in liver tissues (P<0.05, P<0.01), and reduced FoxO1 protein (P<0.01). The high-dose GGQLT group showed reduced HOMA-IR, TC, LDL-C, and HDL-C levels (P<0.05, P<0.01). Transmission electron microscopy revealed a significant reduction in autophagosomes in the medium- and high-dose GGQLT groups. ConclusionGGQLT can significantly improve glucose and lipid metabolism disorders, alleviate insulin resistance in db/db mice, and prevent and treat type 2 diabetes by activating the SIRT1/FoxO1 autophagy pathway.
