1.Preliminary study on the reduction of streak artifacts in thyroid CT examination with optimized position in prone position

		                        			Objective To explore the factors influencing the image quality of thyroid CT and the application value of optimization schemes in reducing streak artifacts in thyroid CT examinations.Methods A retrospective review of the data of 141 patients who underwent thyroid enhanced CT examinations was conducted to analyze the factors affecting image quality.After determining the influencing factors,a self-controlled study was conducted on 48 patients who underwent thyroid CT examinations to analyze the feasibility of the optimization scheme.Results The patients'body mass index(BMI),neck circumference,pharynx-jaw angle,and pharynx-jaw angle-to-neck circumference ratio showed statistically significant differences in their impact on image quality(P<0.05).Compared to the conventional scheme,the optimization scheme showed significantly higher subjective and objective image evaluations.The optimization scheme also significantly improved the diagnostic performance of the images,with statistically significant differences(P<0.05).There was no statistically significant difference in radiation dose between the two positions(P>0.05).Conclusion The optimization scheme can effectively reduce streak artifacts in thyroid CT examinations.
		                        		
		                        		
		                        		
		                        	
2.Establishment and application of drug use evaluation standards for lauromacrogol

		                        			Objective To establish the drug use evaluation(DUE)standard of lauromacrogol,evaluate its clinical use,and provide a reference for rational clinical application.Methods Based on the drug instructions,related relevant guidelines and literature of lauromacrogol,the DUE standard was formulated through the expert consultation method.A retrospective study was conducted to evaluate the drug utilization of lauromacrogol in hospitalized patients in 3 201 Hospital of Xi'an Jiao tong University in Shaanxi province from January 2021 to August 2022.Results A total of 143 medical records were included,48(33.57%)of which fully met the evaluation criteria.Unreasonable use was mainly manifested as inappropriate indications(65.03%)and unsuitable usage and dosage(1.40%).The unreasonable indications are mainly due to the existence of off-label medication.Conclusion The established DUE standard of lauromacrogol has a certain reference effect on the rational use of lauromacrogol in clinical practice.Irrational usages still exists in the clinical application of lauromacrogol in the hospital,and interventions should be developed to optimize its clinical application and promote the rational drug use.
		                        		
		                        		
		                        		
		                        	
3.Expression of optineurin in multiple myeloma and its clinical significance

		                        			【Objective】 To investigate the expression of optineurin (OPTN) in multiple myeloma (MM) and explore the mechanism and clinical value of OPTN gene in the occurrence and development of MM. 【Methods】 In this study, three gene expression omnibus (GEO) data sets were used to analyze the expression level of OPTN in MM. Clinical bone marrow samples of MM patients were collected. qRT-PCR was used to further verify the expression of OPTN in MM patients. The Kaplan-Meier survival curve and receiver operating characteristic (ROC) curve were used to analyze the value of OPTN in the prognosis and diagnosis of MM. At the same time, MM transcriptome data were downloaded from the Cancer Genome Atlas (TCGA) database. According to the median boundary of OPTN mRNA expression level, the MM patients were divided into OPTN high- and low-expression groups. In order to investigate the possible molecular mechanisms of OPTN in MM, gene set enrichment analysis (GSEA) was made after the differentially expressed genes were filtered using the limma package of the R language. 【Results】 The expression level of OPTN was significantly lower in MM tissues than in normal tissues (P<0.05). OPTN expression level was significantly correlated with International Staging System (ISS) in MM patients (P<0.05). ROC results showed that the expression level of OPTN could distinguish between normal and MM patients. Survival analysis showed that the overall survival (OS) of patients with low OPTN expression was significantly lower than that of patients with high OPTN expression (P<0.05). GO, KEGG and GSEA enrichment analyses indicated that OPTN might affect apoptosis and autophagy, and regulate cellular immune response by regulating Nod-like receptors, NF-κB, TNF and RAS/MAPK pathways. 【Conclusion】 Low expression of OPTN in MM is associated with poor prognosis of patients, and thus may be an important potential biomarker for the diagnosis and treatment of MM.
		                        		
		                        		
		                        		
		                        	
4.The application effect of different contrast agent injection schemes in neck enhanced CT examination

		                        			Objective To investigate the influence of different contrast agent injection schemes on imaging quality in neck enhanced CT examination.Methods A total of 150 patients who underwent neck enhanced CT examination were selected and randomly divided into three groups:groups A,B,and C,with 50 cases in each group.Group A adopted individualized injection scheme,group B used three-phase dual-flow contrast agent injection scheme,and group C used measured constant-speed injection scheme,respectively.The objective and subjective evaluation methods were used to assess the image quality of cervical vessel and non-vessel structures.Results Among the three groups of contrast agent injection schemes,the CT values of group A and group B were higher than those of group C except for the aortic arch and submandibular gland in objective evaluation,and the contrast-to-noise ratio(CNR)of group A and group B except for the left common carotid artery and right common carotid artery bifurcation was higher than that of group C.In subjective evaluation,there was no statistically significant difference in image quality among the three groups(P>0.05),and group B effectively reduced the impact of hardening artifacts.Conclusion The post-injection dual-flow contrast agent injection scheme can achieve the same enhancement effect as other injection schemes while ensuring image quality and reducing the amount of contrast agent used in the examination.
		                        		
		                        		
		                        		
		                        	
5.GNMT inhibits intrauterine adhesion fibrosis through TGF-β1/Smad3 signaling pathway and its mechanism

		                        			Objective To investigate the effect of glycine N-methyl transferase (GNMT)on intrauterine adhesion (IUA)fibrosis and its related mechanism.Methods In vivo experiment:A total of 36 healthy female SD rats (SPF grade,6~8 weeks old and weighing from 180~220 g)were subjected in this study.IUA model of SD rats and IUA model of GNMT overexpressed rats were established.RT-qPCR and immunofluorescence assay were applied to detect GNMT expression level in normal uterus and model group.RT-qPCR and Western blotting were used to detect the mRNA and protein levels of fibrosis-related molecules and the activation of TGF-β1/Smad3 signaling pathway in each group.The number of endometrial glands in each group was observed by HE staining.Masson staining was used to analyze the severity of endometrial fibrosis in each group.In vitro experiment:transformed human endometrial stromal cells (THESCs)fibrotic phenotype model was constructed using TGF-β1,and THESCs stably transfected with GNMT overexpression lentvirus were treated with TGF-β1.RT-qPCR and Western blotting were used to detect the mRNA and protein expression of fibrosis-related molecules.The expression of TGF-β1/Smad3 signaling pathway was detected by Western blotting.TGF-β1/Smad3 signaling pathway was activated by TGF-β1/Smad signaling pathway activator (SRI-011381),and the expression of TGF-β1/Smad3 signaling pathway and key molecular proteins of fibrosis phenotype was measured with Western blotting.Results In vivo experiment,the mRNA and protein expression levels of GNMT were significantly decreased in the IUA rats than the control rats (P<0.05).Overexpression of GNMT decreased the mRNA and protein levels of fibrosis related molecules,Collagen Ⅰ,Collagen Ⅲ and FN in the IUA rats (P<0.05),and decreased the phosphorylation levels of TGF-β1 and its downstream Smad3 protein (P<0.05).HE and Masson staining showed that overexpression of GNMT could increase the number of endometrial glands and reduce the severity of fibrosis in the IUA rats (P<0.05).In vitro experiments:overexpression of GNMT decreased the mRNA and protein levels of Collagen Ⅰ,Collagen Ⅲ and FN associated with fibrotic phenotype of THESCs (P<0.05),and reduced the phosphorylation level of Smad3 protein,downstream of TGF-β1 (P<0.05).After activation of TGF-β1/Smad3 signaling pathway,the protein levels of TGF-β1/Smad3 signaling pathway and downstream fibrosis phenotype molecules,Collagen Ⅲ and FN,were significantly decreased in the LV-GNMT+SRI-011381 group.Conclusion Overexpression of GNMT can inhibit endometrial fibrosis by regulating TGF-β1/Smad3 signaling pathway,thus achieving therapeutic effect on IUA.
		                        		
		                        		
		                        		
		                        	
6.Prenatal diagnosis of microcephaly due to CTNNB1 frameshift variation: a case report

		                        			This article reported a case of neurodevelopmental disorder accompanied by spastic diplegia and visual impairment with the manifestation of small fetal head circumference. Prenatal ultrasonography performed at 33 +5 weeks of pregnancy revealed small fetal head circumference (-2.61SD) and oligohydramnios. Whole-exome sequencing identified a heterozygous frameshift variation of c.1623_1624insA (p.R542Tfs*30) in the CTNNB1 gene (NM_001904.4) of the fetus. No phenotypic abnormalities or corresponding gene variations were detected in the parents, suggesting it was a de novo variation. Based on the clinical manifestations, the fetus was diagnosed with a neurodevelopmental disorder accompanied by spastic diplegia and visual defects. 7.Risk factors for pulmonary infection after laparoscopic surgery in treatment of hepatocellular carcinoma with liver cirrhosis and portal hypertension

		                        			 Objective To investigate the incidence rate of pulmonary infection after laparoscopic surgery and related risk factors in patients with hepatocellular carcinoma (HCC) comorbid with liver cirrhosis and portal hypertension (PHT). Methods A retrospective analysis was performed for the clinical data of 105 HCC patients with liver cirrhosis and PHT who underwent laparoscopic surgery in Beijing Ditan Hospital, Capital Medical University, from January 2017 to February 2022. A total of 30 factors that might cause pulmonary infection were recorded, including general information, disease factors, surgical factors, and postoperative factors. Postoperative recovery was observed and the occurrence of pulmonary infection was recorded. The chi-square test or the Fisher's exact test was used for comparison of categorical data between two groups, and the multivariate logistic regression analysis was used to investigate the independent risk factors for pulmonary infection. Results Among the 105 patients, 66 underwent laparoscopic devascularization combined with hepatectomy and 39 underwent laparoscopic devascularization combined with radiofrequency ablation (RFA). The surgery was successful for all patients, with no case of conversion to laparotomy or unscheduled reoperation. No death was observed within 30 days after surgery and during hospitalization, with a median length of hospital stay of 20 days (range 14-25 days). The incidence rate of pulmonary infection was 25.71% (27/105). Smoking (odds ratio [ OR ]=3.362, 95% confidence interval [ CI ]: 1.282-8.817, P =0.014), MELD score ( OR =3.801, 95% CI : 1.007-14.351, P =0.049), tumor location ( OR =1.937, 95% CI : 1.169-3.211, P =0.010), surgical procedure ( OR =0.006, 95% CI : 0.001-0.064, P =0.000), intraoperative infusion volume ( OR =4.871, 95% CI : 1.211-19.597, P =0.026), and postoperative pleural effusion ( OR =9.790, 95% CI : 1.826-52.480, P =0.008) were independent risk factors for pulmonary infection. Conclusion There is a relatively high risk of pulmonary infection in HCC patients with liver cirrhosis and PHT undergoing laparoscopic surgery. Postoperative pleural effusion is the high risk factor for pulmonary infection, and devascularization combined with RFA can significantly reduce the risk of pulmonary infection. It is recommended to strengthen preoperative rehabilitation, perioperative liver function maintenance, intraoperative damage control, and goal-oriented fluid therapy and reduce postoperative fluid accumulation in the third space, so as to reduce the incidence rate of pulmonary infection. 
		                        		
		                        		
		                        		
		                        	
8.Analysis of constipation status and influencing factors in patients with lung cancer during postoperative hospitalization

		                        			Objective     To investigate the current status of constipation during postoperative hospitalization and the factors associated with moderate to severe constipation at discharge in lung cancer patients. Methods     Lung cancer patients who underwent surgery in 6 tertiary hospitals in Sichuan Province from November 2017 to January 2020 were enrolled. The MD Anderson Symptom Scale-Lung Cancer Module was used to collect postoperative constipation scores. Unconditional logistic stepwise regression was used to analyze the related influencing factors for moderate to severe constipation on the day of discharge. Results     Finally 337 patients were collected. There were 171 males and 166 females, with an average age of 55.0±10.3 years. Constipation scores of lung cancer patients increased from postoperative day 1 to day 3, and showed a decreasing trend from day 3 to day 7. Moderate to severe constipation was present in 68 (20.2%) patients at discharge. The postoperative hospital stay (OR=0.743, P<0.001) and the dose of morphine used during postoperative hospitalization (OR=1.002, P=0.015) were influencing factors for moderate to severe constipation at discharge in lung cancer patients. Conclusion    Lung cancer patients have the most severe constipation on postoperative day 3. Moderate to severe constipation at discharge is associated with the postoperative hospital stay and the dose of morphine used during postoperative hospitalization.

		                        			【Objective】 To analyze the basic characteristics of whole blood donors from blood stations before and after the outbreak of COVID-19. 【Methods】 After excluding invalid data, data related to the basic characteristics of whole blood donors collected from 26 blood stations in China during 2018 to 2021 were statistically analyzed, including the trend of total whole blood donors, the number of repeated blood donors, the frequency of blood donation, the average age of donors and the recruitment of first-time blood donors. 【Results】 Affected by the epidemic, 8 out of 14 indicators were with large variations, accounting for 57%. The overall growth rate of total whole blood donors during the epidemic was higher than before the epidemic (P<0.05).The number of repeated blood donors has shown an increased trend, with a higher number during the epidemic than before (P<0.05). The frequency of blood donation was lower during the epidemic than before(P<0.05).Average ages of blood donors and female blood donors fluctuated widely during the epidemic, both higher than those before the epidemic(P<0.05).The donation rate of first-time blood donors <25 years old and ≥25 years old varied widely and irregularly during the epidemic (both P<0.05). The percentage of first-time blood donors fluctuated irregularly during the epidemic, with overall percentage lower than that before the epidemic(P<0.05). 【Conclusion】 Whole blood donors from 26 blood stations increased after the outbreak of COVID-19, and some indicators in certain areas showed significant fluctuations during the epidemic.
		                        		
		                        		
		                        		
		                        	
10.Application of chain management of perioperative pressure injury in patients undergoing supine craniocerebral injury

		                        			Objective:To investigate the value of chain management of perioperative pressure injury in patients undergoing supine craniocerebral injury, so as to provide reference for the prevention of pressure injury and the application of chain management.Methods:In this retrospective study, a total of 116 patients with craniocerebral injury who underwent supine surgery in Shanxi Cardiovascular Hospital from December 2019 to December 2020 were included in the control group, using traditional perioperative pressure injury management protocol. And 124 patients admitted from January 2021 to December 2021 were included in the observation group, and were managed by the pressure injury chain management scheme. The risk assessment/judgment rate of pressure ulcers, the risk assessment/judgment accuracy rate of pressure ulcers, the implementation rate of intervention measures, the qualified rate of intervention records and the reported rate of high-risk patients with pressure ulcers in the two groups were analyzed. The occurrence of stress injury and medical expenses of the two groups of patients were investigated.Results:The risk assessment/judgment rate of pressure ulcers, the risk assessment/judgment accuracy rate of pressure ulcers, the implementation rate of intervention measures, the qualified rate of intervention records and the reported rate of high-risk patients with pressure ulcers were 100.00%(124/124), 96.77%(120/124), 92.74%(115/124), 93.55%(116/124), 100.00%(29/29) in the observation group, which were 77.59%(90/116), 72.41%(84/116), 68.97%(80/116), 68.97%(80/116), 83.33%(30/36) in the control group, the difference was statistically significant ( χ2 values were 22.24-31.17, all P<0.05). The overall incidence of stress injury in the observation group and the control group was 15.52%(18/116) and 3.23%(4/124), respectively, and the difference between the two groups was statistically significant ( χ2=10.88, P<0.05). There was no significant difference in the total hospitalization cost between the two groups ( P>0.05). The cost of stress-related consumables was (144.94 ± 47.28) yuan in the observation group and (194.21 ± 55.76) yuan in the control group, and the difference was statistically significant ( t=7.40, P<0.05). Conclusions:Perioperative stress injury chain management can reduce the incidence of pressure injury in patients with craniocerebral injury, improve service quality, and reduce medical costs, with good clinical application value.
