1.Disease burden of chronic kidney disease attributable to high BMI in China and trend prediction in 1992-2021

Objective To analyze the disease burden of chronic kidney diseases (CKD) attributed to high body mass index (BMI) in China from 1992 to 2021 and predict the disease burden for the next decade, and to provide evidence for the prevention and treatment of CKD. Methods Using the Global Burden of Disease (GBD) database and the Joinpoint model, the average annual percentage rate change (AAPC) of the mortality rate and disability-adjusted life year (DALY) rate was calculated to describe and analyze the CKD disease burden attributed to high BMI in China from 1992 to 2021. The ARIMA model was employed to predict and analyze the change trend of the CKD disease burden. Results From 1992 to 2021, the mortality rate and DALY rate attributed to high BMI-induced chronic kidney disease showed an upward trend. Compared to 1992, the attributed number of deaths increased by 324.38%, and DALYs increased by 268.56%; the mortality rate increased by 64.00%, and the DALY rate grew by 51.62%. From 1992 to 2021, the mortality rate and DALY rate for males were lower than those for females, but the growth rate for males exceeded that of females. From 1992 to 2021, the mortality rate and DALY rate of chronic kidney disease attributed to high BMI in China increased with age. The average annual change rate of chronic kidney disease attributed to high BMI in China from 1992 to 2021 (mortality rate: 1.40 per 100,000 (95% CI: 1.04–1.76), DALY rate: 1.43 per 100 000 (95% CI: 1.17–1.70)) was higher than thHuaiyin Normal University, Huai'anher social demographic index (SDI) regions. The ARIMA model predicted that the age-standardized mortality rate increased from 2.91 per 100 000 in 2022 to 3.05 per 100 000 in 2026, and the age-standardized DALY rate increased from 69.65 per 100 000 in 2022 to 73.58 per 100 000 in 2026. Conclusion Chronic kidney disease attributed to high BMI in China is on the rise, and it will continue to grow in the future. The focus of CKD prevention and control should be on males and the elderly, while active measures should be taken to reduce the occurrence and progression of chronic kidney disease.
2.Factors affecting microvascular complications among patients with type 2 diabetes mellitus

Objective :
To identify the factors affecting microvascular complications among patients with type 2 diabetes (T2DM), so as to provide insights into the management of microvascular complications of T2DM.
Methods:
T2DM patients hospitalized in the Department of Endocrinology of a tertiary hospital in Weifang City, Shandong Province from January 2021 to January 2022 were enrolled, and subjects' basic information, lifestyle and medical history were collected using questionnaire surveys. Fasting insulin, fasting blood glucose and glycated hemoglobin were measured, and factors affecting microvascular complications were identified among T2DM patients using a multivariable logistic regression model and a decision tree model.
Results:
Totally 1 003 T2DM inpatients were enrolled, including 515 men (51.35%) and 488 women (48.65%), and the prevalence of microvascular complications was 40.18%. Multivariable logistic regression analysis showed that age of 60 years and older (OR=2.510, 95%CI: 1.441-4.374), T2DM duration of 10 years and longer (OR=3.205, 95%CI: 2.242-4.581), fasting insulin of lower than 3.21 μIU/mL (OR=1.749, 95%CI: 1.239-2.469), using of agents or insulin to control blood glucose (OR=1.880, 95%CI: 1.143-3.092), glycated hemoglobin level of 7% and higher (OR=1.751, 95%CI: 1.172-2.615) as factors affecting microvascular complications among T2DM patients. Decision tree analysis identified course of T2DM as a major factor affecting the risk of microvascular complications among T2DM patients, and the prevalence of microvascular complications was 70.22% among T2DM patients with disease course of 10 years and longer and fasting insulin of lower than 3.21 μIU/mL or 16.32 μIU/mL and higher, 44.23% among T2DM patients with disease course of 5 to 10 years and at ages of 60 years and older, and 43.10% among T2DM patients with disease course of less than 5 years and fasting insulin of lower than 3.21 μIU/mL.
Conclusion
Advanced age, long course of T2DM, low fasting insulin and high glycated hemoglobin may increase the risk of microvascular complications among T2DM patients.

4.Effect of single locked-plate internal fixation combined with autogenous iliac bone graft in the treatment of Rorabeck type II periprosthetic distal femoral fracture after total knee arthroplasty

Objective:To investigate the efficacy of single locked-plate internal fixation combined with autologous iliac bone graft in the treatment of Rorabeck type II periprosthetic distal femural fracture (PDFF) after total knee arthroplasty (TKA).Methods:A retrospective case series study was made on 13 patients suffering from Rorabeck type II PDFF after primary TKA together with severe osteoporosis (T value≤ -2.5 SD) admitted to 940th Hospital of Joint Logistics Support Force of PLA from January 2016 to December 2020, including 4 males and 9 females, aged 65-85 years [(75.2±6.5)years]. All patients were treated with single locked-plate internal fixation combined with autologous iliac bone graft. Anti-osteoporosis and early standardized joint function rehabilitation were undertaken postoperatively. The operation time and intraoperative blood loss were recorded. The range of motion of knee joint was compared before operation, at postoperative 3, 6 and 12 months and at the last follow-up. The Hospital for Special Surgery (HSS) knee score was assessed at postoperative 3, 6 and 12 months and at the last follow-up to evaluate the recovery of knee joint function. The bone mineral density was reexamined at postoperative 6 months and 12 months to evaluate the therapeutic effect of anti-osteoporosis. Complications were detected as well.Results:All patients were followed up for 12-72 months [(43.2±19.9)months]. The operation time was 90-135 minutes [(103.8±12.6)minutes], with the intraoperative blood loss of 100-250 ml [(150.0±45.6)ml]. The range of motion of knee joint was (114.6±7.8)°, (90.4±8.0)°, (97.3±4.8)° and (98.1±6.3)° before operation and at postoperative 3, 6 and 12 months (all P<0.05). The HSS knee score was (80.2±2.2)points, (84.6±2.9)points and (87.3±3.3)points at postoperative 3, 6 and 12 months (all P<0.05). The knee joint function was excellent in 10 patients and good in 3 at postoperative12 months, and the excellent and good rate was 100%. The T value of bone mineral density was (-3.8±0.6)SD, (-3.4±0.6)SD and (-2.9±0.6)SD preoperatively and at postoperative 6 months and 12 months (all P<0.05). One patient experienced nonunion and was cured after secondary autologous iliac bone grafting combined with recombinant human bone morphogenetic protein-2 (rhBMP-2) bone grafting. Three patients developed venous thrombosis of lower limbs and were cured with oral administration of rivaroxaban. One patient had mild knee flexion and extension limitation and was improved after manual release under femoral nerve block anesthesia and subsequent functional rehabilitation. Conclusion:For patients with Rorabeck type II PDFF after TKA, single locked-plate internal fixation combined with autologous iliac bone graft has advantages of short operation time, few intraoperative bleeding, satisfactory knee range of motion and functional recovery as well as significant improvement of bone mineral density.
5.Arthroscopic Bristow procedure using suture-button fixation for treatment of recurrent anterior shoulder instability

Objective:To investigate the clinical outcome of arthroscopic suture-button Bristow procedure using suture-button fixation for treatment of recurrent anterior shoulder instability.Methods:A retrospective case series study was conducted to analyze 28 patients with recurrent anterior shoulder instability admitted to Shanghai Sixth People’s Hospital from March 2015 to September 2018,including 26 males and 2 females,aged 18-36 years [(24.1 ± 3.2)years]. Preoperative MRI showed Bankart injury and three-dimensional CT showed glenoid bone defect > 15%. The Hill-Sachs injury was found in 27 patients. Arthroscopic Bristow procedure was used to fix coracoid process graft with the suture-button plate and glenoid labial complex was repaired with the suture anchor to treat anterior instability of the shoulder joint. The visual analogue scale (VAS),American shoulder and elbow surgeon (ASES) scale,Constant score and Rowe score were used to evaluate the shoulder joint function preoperatively,at postoperative 3,6,12 months and at the last follow-up. The postoperative infection,neurovascular injury,redislocation and other complications were recorded,and the incidence rate was calculated. The bone resorption and bone healing of coracoid process graft were analyzed by CT.Results:All patients were followed up for 22 - 36 months [(24.5 ± 6.3) months]. Three months after operation,the VAS,ASES scale and Constant score were not significantly different from those before operation ( P > 0.05). In comparison,the Rowe score was significantly higher than that before operation ( P < 0.05). Six and twelve months after operation,the VAS,ASES scale,Constant score and Rowe score were significantly improved from those before operation ( P < 0.05). At the last follow-up,the VAS decreased from (2.7 ± 1.5)points preoperatively to (0.8 ± 0.3)points,the ASES scale increased from (78.6 ± 12.7)points preoperatively to (92.4 ± 8.4)points,the Constant score increased from (43.4 ± 5.2) points preoperatively to (81.6 ± 6.7) points,the Rowe score increased from (52.3 ± 7.1)preoperatively to (92.7 ± 5.4) points ( P < 0.05). During the follow-up,there was no infection,neurovascular injury,re-dislocation and other complications. In addition,24 (86%) out of the 28 patient showed healing of coracoid process grafts,and the overall bone resorption rate was (18.4 ± 6.1)%. No patients had glenohumeral joint osteoarthritis. Conclusion:Arthroscopic Bristow procedure using suture-button fixation for treatment of recurrent anterior shoulder instability has advantages of a low rate of postoperative complications and a high union rate,indicating an effective and safe surgical procedure.
6.Assessment of nutritional status in a group of children aged 3-6 years in Lanzhou City by adjusting the dietary balance index

Objective To evaluate the dietary quality of children aged 3 to 6 years during the kindergarten period in Lanzhou City using the Dietary Balance Index (DBI). Methods Based on kindergarten grades and geographical locations, 304 recipes for different semesters from 2014 to 2017 in 40 kindergartens in Lanzhou City were randomly selected with a sampling ratio of 5%(291 recipes with no recipes 13 copies) as the research object. Adjusted DBI indicators for children aged 3 to 6 years were used to evaluate the dietary quality of the recipes during the kindergarten period in the sample. Using the Kruskal-Wallis H rank-sum test, the food consumption rates and DBI evaluation results of the kindergartens in Lanzhou City were analyzed. Results Results of the sub-group diet scores that were close to the recommended intake (DBI scores between-2 to 2) showed that cereals, fresh vegetables, nuts and beans, fish, shrimp, eggs, and snacks accounted for the ideal intake of more than 80%of all the recipes. The diet scores of fruits, meat, and processed products ranged from 50%to 80%, and that for milk accounted for only 26.8%. Analyzing the type of food intake showed that the proportion of recipes that included eating more than six types of food per day was 98.3%. Analyzing by kindergarten level and year showed that the overall dietary quality problems with the studied recipes were mainly caused by insufficient intake and dietary imbalance, and the difference was statistically significant. Among them, the lack of intake in 2017 and the degree of dietary imbalance were greater compared to the other years. The level of insufficiency and dietary imbalance in the third-grade kindergartens were higher than those in the first- and second-grade kindergartens. Dietary pattern A recipes accounted for 63.6% of the kindergarten recipes, and there were no recipes for dietary patterns C, F, G, H, and I. Conclusion The dietary quality problems of kindergartens in Lanzhou City were mainly reflected in the inadequate dietary intake and dietary imbalance. The supervision of dietary quality by kindergartens should be strengthened and the concept of a balanced diet should be promoted. The dietary management of third-grade kindergartens should be simultaneously strengthened.
7.Effects of family function, resilience and focal lesion characteristics on acute stress disorder in patients with cerebral injury

Objective To investigate the status quo of the cerebral injury in patients with acute stress disorder (ASD) and the impact of family function,mental toughness,and the characteristics of brain lesions on it.Methods A total of 349 patients were enrolled from North China University of Science and Technology affiliated hospital neurology department and neurosurgery department from May 2016 to November 2016 and they were tested with Stanford Acute Stress Reaction Questionnaire (SASRQ),the family function assessment scale (APGAR),Chinese version of mental toughness scale (CD-RISC).Results 349 cases of patients with SASRQ score (57.21±44.97),8 to 39 160 people (45.8%),40 to 56 85 people (24.4%),57 to 150 104 people (29.8%).The results showed that whether the hemiplegia (β=-0.030),family function (β=-0.032),mental toughness (β=0.886),disturbing degree (β=0.052),bad days (β=0.060)were picked in the regression equation (P<0.05).Conclusion There were obvious acute stress symptoms in patients with brain injury.The individuals who have family dysfunction,the worse the psychological resilience and the higher the disturbance degree of the patients with hemiplegia,may be likely to get acute stress disorder.
8.Comparison on efficacy of hollow screws and" L" type plate for treatment of middle and lower 1/3 spiral fracture of tibia combined with posterior malleolar fracture

Objective To compare the clinical efficacy of hollow screws and "L" type plate for the treatment of middle and lower 1/3 spiral fracture of tibia combined with posterior malleolar fracture.Methods A retrospective case control study was performed on the clinical data of 73 patients with middle and lower 1/3 spiral fracture of tibia combined with posterior malleolar fracture treated from January 2010to January 2017.There were 47 males and 26 females,with the mean age of 38.5 years (range,23-70 years).According to the posterior malleolus fixation method,38 cases adopted the internal fixation with three hollow screws (screw group),and 35 cases with" L" type plate fixation (plate group).The middle and lower 1/3 spiral fracture of tibia were treated with medial anatomical plate internal fixation.The intraoperative blood loss,operation time,infection,fracture healing time,and complications were recorded.The Baird-Jackson score was used to evaluate the ankle joint function.Results There were no significant differences in the blood loss and operation time between the two groups (P > 0.05).A total of 71 patients were followed up for 6-18 months (mean,15.8 months).Two cases in the screw group were lost to follow up.The wounds healed well after operation in two groups,and no signs of wound infection were found.All fractures healed well without delayed union or nonunion.There was no significant difference in fracture healing time between the two groups (P > 0.05).No obvious reduction loss in posterior ankle fracture was found during follow-up.In the plate group,one patient was seen vertical shape toe deformity after operation,which might be related to muscle spasm of flexor hallucis longus,and the condition was not improved after removing the plate.At the last follow up,ankle joint function was evaluated according to the Baird-Jackson scoring system:in the screw group,18 cases were excellent,13 cases were good,four cases were allowed to pass,and one case was poor,with the excellent and good rate of 82%;in the plate group,17 cases were excellent,13 cases were good,four cases were allowed to pass,and one case was poor,with the excellent and good rate of 86% (P > 0.05).Conclusion The clinical efficacy of hollow screws and "L" type plate is not significantly different,both of which can provide rigid fixation,reduce the incidence of hip dysfunction and improve quality of life of patients with middle and lower 1/3 spiral fracture of tibia combined with posterior malleolar fracture.
9.Effect of Living Mode on Quality of Life and Life Satisfaction of Patients with Spinal Cord Injury Survived Tang-shan Earthquake

Objective To explore the effect of living mode on quality of life and life satisfaction of patients with spinal cord injury who survived Tangshan earthquake. Methods From February to May, 2016, 76 patients with spinal cord injury survived the earthquake were in-vestigated, among whom 38 patients lived in the nursing home or rehabilitation village and the other 38 patients discretely lived at home. Their quality of life and life satisfaction were assessed with WHO Quality of Life-BREF (WHOQOL-BREF) and Life Satisfaction Assess-ment respectively. Results The scores of physical, psychological, social adaptation and environment field in WHOQOL-BREF were signifi-cantly higher in the concentrated living group than in the scattered living group (t>8.97, P<0.001), as well as the life satisfaction (t=6.55, P<0.001). Conclusion The concentrated living mode could improve the quality of life and life satisfaction of the patients with spinal cord inju-ry survived Tangshan earthquake.
10.Correlation between reliability and validity of Brunel balance assessment with activities of daily living

Objective To explore the correlation between the reliability and validity of Brunel balance assessment(BBA)with the activities of daily living(ADL).Methods The BBA,ADL and walking ability test were performed on 215 patients conforming to the inclusion standard by two professionals.The Pearson correlation analysis,factor analysis and descriptive statistical analysis were adopted.Results The BBA total score and Cronbach's α coefficient in 3 dimensions was 0.852~0.941(P<0.01),moreover the relative coefficient of each item with its dimension was higher that that of other dimensions;3 common factors were extracted by the factor analysis,the contribution rate was 83.916%;but BBA had the ceiling effect,the relative coefficient of BBA with ADL and walking ability was 0.284-0.709(P<0.05).Conclusion BBA has a good reliability and validity and can better reflect the functions of ADL and walk.
