1.Network analysis and core dimensions of the biased constitution in Chinese medicine based on 3, 691 cases

Objective:
To conduct network analysis of biased constitution to provide theoretical basis for daily health management.
Methods:
Purposive sampling and snowball sampling methods were used to collect the required data via a web-based questionnaire from February 3rd, 2022 to January 21st, 2023. The tendency scores of 8 biased constitutions (qi-deficiency type, yang-deficiency type, yin-deficiency type, phlegm-dampness type, damp-heat type, blood-stasis type, qi-depression type, and special diathesis type) were evaluated by 30-Item Short Version of Constitution in Chinese Medicine Questionnaire. Data analysis was conducted in R. The qgraph package was used to construct network analysis models and calculate the edge weight, namely regularised partial correlation coefficients (r) and centrality coefficients (including strength, closeness, betweenness and expected influence), while mgm was used to calculate the predictability of each node within the network. The ggplot2 package was used to visualize network analysis result, and bootnet was applied to assess the structural reliability and robustness of the network, including the r accuracy assessment, the stability assessment of the centrality coefficients, and the difference test result of node centrality coefficients.
Results:
A total of 3, 691 valid samples were obtained from the survey. The result of the network analysis showed that: (1) the strength of the association between phlegm dampness type and damp-heat type (r=0.370), qi-deficiency type and qi-depression type (r =0.315), qi-deficiency type and special diathesis type (r=0.260), qi-deficiency type and phlegm-dampness type (r=0.247), and the phlegm-dampness type and qi-depression type (r=0.247) was high. (2) The centrality of phlegm-dampness type and qi-deficiency type was the strongest, and the differences between the centrality coefficients of phlegm-dampness type and qi-deficiency type were not significant. There were no other nodes where all 4 centrality coefficients were not significantly different from qi-deficiency type and phlegm-dampness type, and the predictability of phlegm-dampness type and qi-deficiency type was the highest. (3)The result of the r accuracy assessment showed that the r of this network did not differ significantly from the average of multiple samples, so the r accuracy of this network was high. (4) Correlation stability(CS) coefficient of the four centrality indicators were CS Strength=0.59, CS Closeness=0.75, CS Betweenness=0.75, and CS Expected influence=0.75. This indicated that the stability of the centrality indicators of this network was very good.
Conclusion
Complex connections exist between various biased constitutions, and phlegm-dampness type and qi-deficiency type are the two key biased constitutions in the entire biased constitution.
2.Exploration on the Paths of Traditional Chinese Medicine Constitution Intervention to Improve the Quality of Life:Based on the Perspective of Configuration

ObjectiveTo adapt to the dynamic multidimensional changes in traditional Chinese medicine (TCM) constitution, the paths of improving the quality of life through TCM constitution intervention are proposed from a configuration perspective. MethodsFive hundred and nine subjects were included in the study, all of whom voluntarily participated in and completed TCM constitution interventions, and completed 60-item Constitution in Chinese Medicine Questionnaire (CCMQ) (including 9 subscales: neutral type, qi deficient type, yang deficient type, yin deficient type, phlegm-dampness type, damp-heat type, blood stasis type, qi stagnation type, and special constitution type) to evaluate the TCM constitution, and the Short Form Health Questionnaire (SF-36) in Chinese version (including overall, physical component scores (PCS), and mental component scores (MCS)) before and 6 months after the intervention to evaluate the quality of life. The necessary condition analysis (NCA) and fuzzy set qualitative comparative analysis (fsQCA) were used to explore the necessary conditions and advantage configurations for improving the overall, physical and mental quality of life of TCM constitution dimension changes. ResultsThe results of NCA showed that the changes in the scores of neutral type and qi stagnation type can serve as necessary conditions for improving the SF-36 overall quality of life scores (P<0.05). The changes in the scores of neutral type and blood stasis type can serve as necessary conditions for improving PCS scores. The changes in scores of the other 7 types can serve as necessary conditions for improving MCS scores. The fsQCA results showed that the necessary consistency of a change in a particular physical subscale score alone did not reach to a necessary condition for a change in SF-36 total score, PCS score, and MCS score (consistency < 0.9 for all), so the results of NCA were considered. The results of configuration analysis found that 6 configurations were included in increasing the overall quality of life scores and PCS scores, and 7 in increasing the MCS scores, with overall consistency > 0.7. Improving the overall quality of life scores required a focus on lowering the qi deficient type scores, or reducing the yang deficient type scores while increasing the neutral type scores; reducing the scores of phlegm-dampness type and qi stagnation type is beneficial for improving the PCS scores; increasing the neutral type scores and reducing the scores of deficient type, yang deficient type, phlegm-dampness type, and stagnation type is beneficial for increasing MCS scores. ConclusionTCM constitution interventions can improve physical, mental and overall quality of life, and its improvement in the mental domain of quality of life is closely related to the paths of overall quality of life. In order to improve the quality of life, in addition to increasing the tendency of the neutral type, more focused and targeted measures should also be taken to decrease the degrees of qi deficient type, yang deficient type, qi stagnation type, and phlegm-dampness type.
3.Longitudinal Measurement Invariance of Constitution in Chinese Medicine Questionnaire

ObjectiveTo explore the longitudinal measurement invariance of standardized Constitution in Chinese Medicine Questionnaire (CCMQ) among Chinese adult populations, and to provide evidence for the longitudinal effect analysis of this questionnaire. MethodsA total of 509 adults who voluntarily received 26 weeks of comprehensive traditional Chinese medicine (TCM) constitution fitness intervention were included (3 time points: before intervention, 13 weeks intervention, and 26 weeks intervention), evaluated by CCMQ with 9 subscales. A single-group confirmatory factor analysis was performed to establish a single-group baseline model with a well-fitting model, and then nested models, that is multiple groups of confirmatory factors analysis, are used to analyze the longitudinal measurement invariance, followed by configural invariance, metric invariance, scalar invariance, and strict invariance. Comparative Fit Index (CFI), Root Mean Square Error of Approximation (RMSEA), and Standardized Root Mean Square Residual (SRMR) were used as model fit indicators for the analysis. ResultsThe single-group confirmatory factors analysis proved that the 9 subscales of CCMQ fit well at the three time points (CFI: 0.933~0.992, RMSEA: 0.027~0.080, SRMR: 0.022~0.045). The multiple-group confirmatory factors analysis showed that the configural, metric, scalar, and strict invariance across time of qi-stagnation and special diathesis constitution were established (ΔCFI<0.01, ΔRMSEA<0.01); the configural, metric, scalar, and partial strict invariance across time of phlegm-dampness constitution subscale were established (ΔCFI<0.01, ΔRMSEA<0.01); the configural, metric, partial scalar and partial strict invariance of 6 subscales, including gentleness, qi-deficiency, yang-deficiency, yin-deficiency, dampness-heat, and blood-stasis constitutions, were all established (ΔCFI<0.01, ΔRMSEA<0.01). ConclusionThe CCMQ met at least metric invariance at 3 time points, which can be applied to the study of whether different time points have the same unit or meaning. The measurement invariance of the 3 subscales of phlegm-dampness, qi-stagnation and special diathesis, special diathesis have longitudinal measurement invariance, so the mean comparison over time could be made; while the 6 subscales of gentleness, qi-deficiency, yang-deficiency, yin-deficiency, dampness-heat, and blood-stasis constitutions meet partial scalar and partial strict invariance, which could be explained partially for the difference in the comparison of the mean over time.
4.Research on the application of debate-style ethics teaching among undergraduate nursing interns under the background of "doctor-patient conflicts"

Objective:To explore the application effect of debate-style ethics teaching among undergraduate nursing interns under the background of "doctor-patient conflicts".Method:A total of 128 nursing interns,who interned in Yantai Affiliated Hospital of Binzhou Medical University from January 2022 to July 2023,were selected as the research subjects.Using the historical controlled study and according to the random envelope method,undergraduate nursing interns from June 2022 to December 2022 were divided into the intervention group with a total of 64 cases,and undergraduate nursing interns from January 2023 to July 2023 were divided into the control group with a total of 64 cases.Finally,60 undergraduate nursing interns both the intervention group and the control group completed this study.The control group adopted the traditional nursing ethics teaching mode,while the intervention group adopted debate-style ethics teaching based on the background of "doctor-patient conflicts".The teaching effect was evaluated by using the Ethical Sensitivity Questionnaire for Nursing Students,the Professional Identity Questionnaire for Nursing Students,and the Clinical Nursing Instructors before and after the intervention.Result:The research results showed that the scores of ethical sensitivity,professional identity,and clinical nursing instructors of nursing interns in the intervention group were higher than those in the control group after the intervention,and the differences were statistically significant (P<0.05).Conclusion:Debate-style ethics teaching based on the background of "doctor-patient conflicts" can improve the ethical sensitivity,professional identity,and clinical nursing instructors of nursing interns,as well as significantly improve the effectiveness of clinical nursing ethics mentoring.
5.Impact of biased constitutions on health-related quality of life and mediating effects of sleep quality

Objective To explore the effects of biased constitutions on health-related quality of life (HRQOL) and mechanisms of the role of sleep quality in the relationship between biased constitutions and HRQOL.Methods Data collected from the propulation of Beijing and Shijiazhuang,Hebei province for health check-up from March 2015 to December 2016. The biased constitutions were evaluated using the Constitution in Chinese Medicine Questionnaire (CCMQ) score;CCMQ consists of gentleness type (A type),qi-deficiency type (B type),yang-deficiency type (C type),yin-deficiency type (D type),phlegm-wetness type (E type),wet-heat type (F type),blood-stasis type (G type),qi-depression type (H type),and special diathesis type (I type),the eight constitutions except A type are classified as biased constitution (BC),Sleep quality (SQ) was evaluated using the Pittsburgh Sleep Quality Index (PSQI). HRQOL was evaluated using the 36-item Short Form Health Survey (SF-36). Pearson's correlation analysis was used to explore the correlation among the eight BCs,SQ,and HRQOL,and the structural equation model was further constructed to explore the effects of the eight BCs on HRQOL and the mediating effect of SQ.Results A total of 2,170 samples were included in the database. Finally,1,982 samples were included as study subjects after excluding 188 unqualified samples. The SF-36 and PSQI scores of the samples were 79.73±12.89 and 4.29±2.66,respectively,and the scores for the eight BCs were 27.46±14.84,25.12±19.17,23.55±14.99,21.57±15.20,23.67±15.49,21.68±14.49,22.00±16.14,and 16.13±14.19 for B,C,D,E,F,G,H,and I types,respectively. The result of Pearson's correlation analysis showed that all biased constitution scores were negatively correlated with the SF-36 score (r=-0.562 to-0.327,P<0.01),and were positively correlated with the PSQI score (r=0.272 to 0.431,P<0.01),and PSQI score was negatively correlated with the SF-36 score (r=-0.471,P<0.01). The result of the structural equation model showed that all biased constitutions had a negative effect on HRQOL and that the mediating role of SQ in the relationship between eight BCs and HRQOL was all statistically significant. The total effects of the eight BCs on HRQOL were-0.776,-0.528,-0.603,-0.684,-0.596,-0.678,-0.751,and-0.452 for the B,C,D,E,F,G,H,and I types,respectively. Moreover,the absolute effects of the B and H types were significantly higher than in other BCs;the mediation effect of SQ was-0.242,-0.272,-0.291,-0.261,-0.265,-0.395,-0.203,and-0.287 for the B,C,D,E,F,G,H,and I types,respectively,the absolute value of the mediation effect between G type and HRQOL was significantly higher than that of other BCs. Conclusion The eight BCs had negative effects on HRQOL,and SQ played a mediator role in the relationship between the eight BCs and HRQOL.
6.Application of thoracic endovascular aortic repair in the treatment of ruptured descending thoracic aortic aneurysms

Objective To evaluate the efficacy of endovascular treatment for ruptured descending thoracic aortic aneurysm(rDTAA),and to summarize the treatment experience.Methods The clinical data of 8 patients with rDTAA,who were admitted to the Department of Vascular Interventional Surgery,Zhangjiajie Municipal People's Hospital of China to receive treatment between January 2016 and June 2023,were retrospectively analyzed.The patients included 4 males and 4 females,with a median age of 70.5 years(61-78 years).Preoperative computed tomography angiography(CTA)was performed in all the 8 patients,and the diagnosis of rDTAA complicated by hematoma in the thoracic cavity and/or mediastinum was confirmed.Emergency thoracic endovascular aortic repair(TEVAR)was carried out in all the 8 patients.Results Complete isolation of the thoracic descending aortic aneurysm was achieved in 6 patients,and 2 of them received reconstruction of left subclavian artery by using chimney stent technique.The remaining two patients had a small amount of type Ⅰa endoleak,which disappeared after implantation of a cuff stent at the proximal site.During hospitalization,one patient died of septic shock caused by thoracic aortic-esophageal fistula,and 3 patients developed respiratory failure after operation,which was recovered after thoracic drainage,anti-infection medication and ventilator-assisted breathing therapy.Two patients developed postoperative ischemic stroke,one of them presented as grade Ⅲ motor muscle weakness on the affected side accompanied by speech impairment,and the another patient presented as grade Ⅳ motor muscle weakness on the affected side accompanied by speech impairment.During the follow-up period,one patient died of sudden chest pain 7 months after surgery,and another patient who had not received chimney stent implantation developed type Ⅰa endoleak 18 months after operation,which disappeared after implantation of a cuff stent at the proximal site.The remaining 5 patients showed good clinical condition during the follow-up period.Conclusion For the treatment of rDTAA,TEVAR is clinically safe and effective.
7.Randomized controlled study on the application effect of a new type of intravenous radiofrequency closed therapy system made in China and an imported system

Objective:To compare the application effect of domestic and imported intravenous radiofrequency closure system in the treatment of primary varicose veins of lower extremities.Methods:This single-center prospective, non-inferiority randomized controlled trial was performed in the Department of Vascular Surgery, the Fourth Affiliated Hospital, Zhejiang University School of Medicine from January 2021 to January 2022. Patients with primary varicose veins of lower extremities who met the ataxation criteria were randomly assigned to the experimental group(domestic novel venous radiofrequency closure system) or the control group(imported venous radiofrequency closure system) in a ratio of 1∶1. The two groups of subjects were compared in terms of target vein closure rate, technical success rate, system operation performance, incidence of adverse events and incidence of serious adverse events(SAE) within 6 months after surgery. Quantitative data were compared by Mann-Whitney U test, and categorical data were compared by χ2 test and non-inferiority test. Results:A total of 80 subjects were included in the trial (41 in the experimental group and 39 in the control group), including 27 males and 53 females, aged ( M(IQR)) 55(23) years (range:40 to 78 years). There were 48 cases of left lower limb and 32 cases of right lower limb. The technical success rate and system control performance between the groups were 100%.The incidence of adverse events (58.5% (24/41) vs. 61.5% (24/39), χ2=0.075, P=0.784), and the incidence of SAE (7.3% (3/41) vs. 5.1% (2/39), χ2=0.163, P=0.686) within 6 months after surgery in experimental group and control group had no statistical significance. There was one device-related adverse event in each of the two groups. In the experimental group, one patient developed endovenous heat-induced thrombosis after surgery and recovered after taking rivaroxaban tablets. One patient in the control group had pain in the upper right thigh for more than 1 day after operation, which was cured after using analgesic cream. No device-related SAE occurred. The venous closure rate of the experimental group was 100% (38/38) at 6 months after surgery, and that of the control group was 97.4% (37/38). The difference between the two groups was 2.63% (95% CI:-3.19 to 8.45, Z=4.865, P<0.01), and the 95% CI lower limit of the difference in target venous closure rate between two groups was greater than the non-inferiority threshold of -10.00%. Conclusion:The early application effect of the new domestic intravenous radiofrequency closure system in patients with primary varicose veins of lower extremities is in line with expectations, it is not inferior to the imported system.
8.Research Progress on Medical Imaging and New Ultrasound Techniques for Assessing the Degree of Carotid Artery Stenosis

The paper summarizes the imaging evaluation methods for assessing the degree of carotid artery stenosis and analyzes the unique advantages and limitations of various imaging techniques in vascular imaging based on existing guidelines and consensus.The paper focuses on reviewing the clinical applications of several novel ultrasound technologies,including the use of advanced hemodynamic parameters such as blood flow dispersion(Tur index)and wall shear stress(WSS).Carotid artery stenosis is closely associated with cardiovascular disease.Although non-invasive and radiation-free ultrasound technology has certain limitations in diagnostic accuracy to a certain extent,with the continuous emergence of advanced functions such as ultrasound hemodynamics and vascular elasticity,the combination of multi-modality and multi-parameter ultrasound is expected to become an important method for efficient diagnosis of arterial stenosis in the future.
9.Randomized controlled study on the application effect of a new type of intravenous radiofrequency closed therapy system made in China and an imported system

10.Newborn screening, clinical features and genetic analysis for Citrin deficiency in Henan province

Objective:To explore the prevalence, clinical features, genetic characteristics and prognosis of Citrin deficiency in Henan province of China.Methods:A total of 986 565 neonates screened by tandem mass spectrometry at the Third Affiliated Hospital of Zhengzhou University from January 2013 to December 2021 were retrospectively analyzed. Analysis of SLC25A13 gene variants and parental verification were carried out for neonates suspected for Citrin deficiency by next-generation sequencing. The clinical, biochemical and genetic characteristics of Citrin deficiency patients were integrated to guide the diet treatment and follow up the growth and development. Paired- t test was used to compare the amino acid levels in the peripheral blood samples before and after the treatment. Results:Nine cases of Citrin deficiency were diagnosed among the 986 565 neonates. Specific elevation of citrulline was observed in all of the 9 cases. Six variants were detected by genetic sequencing, among which c. 852_855delTATG, c. 615+ 5G>A, c. 550C>T and IVS16ins3kb were known pathogenic variants, whilst c. 1111_1112delAT and c. 837T>A were unreported previously. The detection rate for c. 852_855delTATG was the highest (61.6%, 11/18), followed by IVS16ins3kb (16.7%, 3/18). The clinical symptoms of all patients were relieved after the treatment, and the blood amino acid profile and biochemical parameters were significantly improved by gradually falling within the normal range. By June 2022, all patients had shown a good prognosis.Conclusion:The prevalence of Citrin deficiency among neonates from Henan Province by tandem mass spectrometry is 1/109 618, and the carrier rate for the pathogenic variants of the SLC25A13 gene was 1/166. The c. 852_855delTATG may be a hot spot variant among the patients. Discovery of the novel variants has enriched the mutational spectrum of the SLC25A13 gene. Above results have provided a basis for the early diagnosis, treatment, prognosis and genetic counseling for the affected families.
