1.Application of fall prevention and management strategies based on patient participation and Internet plus in elderly inpatients

		                        			Objective:To explore the effectiveness of patient participation and Internet plus in fall prevention management strategies of elderly inpatients and analyze the causes of falls, so as to provide a basis for continuous improvement in fall prevention to investigate their continuous improvement.Methods:A pre- and post-control study was conducted. Totally 8 480 elderly inpatients hospitalized in the Department of Internal Medicine from 1 June 2020 to 31 May 2021 in Chenzhou NO. 1 People′s Hospital were selected by convenient sampling as the control group, and 8 662 elderly inpatients hospitalized in the Department of Internal Medicine from 1 June 2021 to 31 May 2022 were in the experimental group. The routine fall prevention measures were used in the control group, and on this basis, the experimental group formulated and implemented fall prevention management strategies involving patients based on the patient participation framework "informing, participating, empowering, cooperating, and electronic information support" and introduced Internet plus. Then the differences between the two groups in terms of the incidence of falls and the satisfaction rate of nursing care were compared.Results:The experimental group included 8 662 cases (5 110 males and 3 552 females) with (73.96 ± 8.78) years old, while the control group included 8 480 cases (4 918 males and 3 562 females) with (74.11 ± 8.59) years old. The incidence of falls in experimental group (0.092%, 8/8 662) was lower than that in control group (0.224%, 19/8 480), and the difference was statistically significant ( χ 2=4.71, P<0.05); the nursing care satisfaction rate of experimental group (98.880%, 8 565/8 662) was higher than that of control group (96.450%, 8 179/8 480), and the difference also was statistically significant ( χ 2=106.50, P<0.01); the analysis of the fall causes of the patients revealed that the toilet squatting commode was an important hidden risk of falls in elderly patients. Conclusions:Fall prevention management strategies based on patient participation can reduce the incidence of falls in elderly patients and improve the satisfaction rate of nursing care. Patient participation introduced "Internet plus" can prevent patient falls. The root causes of patient falls will continue to change, and care managers should continually track real-time changes in the root causes of falls to identify problems, develop and adjust prevention strategies accordingly, and pay attention to the importance of infrastructure in the safety of older patients.
		                        		
		                        		
		                        		
		                        	
2.Signal mining and analysis of adverse drug events of doxycycline based on FAERS database

		                        			Objective To mine adverse drug event(ADE)signals of doxycycline using the U.S.Food and Drug Administration Adverse Event Reporting System(FAERS)database,and provide scientific evidence for clinical medication safety.Methods The data from the FAERS database between the first quarter of 2004 and the first quarter of 2024 were extracted.After data cleaning and standardization,ADE reports with doxycycline as the main suspected drug were screened.The system organ class(SOC)of ADE was performed using MedDRA,and the reporting odds ratio method and Medicines and Healthcare products Regulatory Agency method were used to mine ADE signals.The information component method was also used to evaluate signal strength.Results A total of 43 126 ADE reports with doxycycline as the primary suspected drug were collected,involving 14 642 patients,with a higher proportion of female patients(57.32%).There were 555 related ADE signals involving 26 SOC,with the top 5 SOC being gastrointestinal disorders,skin and subcutaneous tissue disorders,injuries,poisonings,and procedural complications,psychiatric disorders,and infections and infestations.The top 5 ADE signals with the highest signal intensity were Hatch reaction,sclerosing cholangitis,esophageal ulcer,gastrointestinal mucosal necrosis,and gastrointestinal injury.Among the ADE signals with the strongest signal strength not listed in the package insert,the top five were sclerosing cholangitis,nephrogenic diabetes insipidus,minimal change glomerular nephritis,diabetes insipidus and Sixth cranial nerve paralysis.Conclusion In clinical practice,particular attention should be paid to the frequent ADEs caused by doxycycline,as well as those not yet documented in the package insert,which involve multiple SOC such as renal and urinary disorders,hepatobiliary diseases,blood and lymphatic system disorders,and endocrine disorders.Therefore,clinical pharmacists should play a key role in assisting clinicians to develop and implement prevention plans for ADEs,thereby improving the safety of doxycycline in clinical use.
		                        		
		                        		
		                        		
		                        	
3.Application of double guide wire combined with balloon catheter sheathing technique in tunnel-cuffed catheter in situ tube replacement

		                        			Long term retention of hemodialysis catheters in blood vessels can lead to the formation of fibrin sheath, which is one of the main reasons causing catheter dysfunction. This study aims to explore a new in situ tube replacement technique that breaks through the binding of fibrin sheath. The tunnel-cuffed catheters of right internal jugular vein due to dysfunction were reimplanted with a double guide wire combined with balloon catheter sheathing technique in seven patients. The surgery successfully broke through the fibrin sheath constraint, and postoperative catheter dialysis proceeded smoothly, and none had catheter dysfunction during 6 months of follow-up. 4.Effect of FTY720 on transfusion-related acute lung injury: a preliminary investigation

		                        			【Objective】 To investigate the effects and mechanisms of different doses of fingolimod (FTY720) on non-antibody-mediated transfusion-related acute lung injury (TRALI). 【Methods】 A TRALI mouse model was constructed using lipopolysaccharide (LPS) pre-stimulation and platelets (Plt) of different storage days for second strike. The success of the modeling was determined by protein concentration in lung tissue homogenates, myeloperoxidase (MPo) activity, lung wet/dry weight ratio (W/D ratio), lung tissue damage score and pathological sections. Ceramide and sphingosine-1-phosphate (S1P) contents in platelets of different storage days were detected. FTY720 was administered 1 h after LPS injection to investigate the role of FTY720 in TRALI. The expression levels of vascular endothelial cadherin (VE-cadherin) and zonula occludens-1 (ZO-1) were analyzed by WB. 【Results】 Mice infused with stored 5-day Plt (d5Plt group) exhibited typical signs of TRALI, and the differences in lung tissue homogenate protein concentration (6 546.38±409.50) μg/mL, MPO activity (49.38±4.43) U/L, W/D ratio 4.79±0.21, and lung tissue damage score 7.24±0.38 from the rest of the groups were statistically significant (P<0.05). With the increase of platelet storage time, the ceramide content gradually increased and S1P content gradually decreased, and the ratio of the two was imbalanced. d5Plt showed statistically significant differences (P<0.01) in ceramide content (58.37±5.69) μmol/L and S1P content (149.81±4.86) nmol/L from the rest of the groups. After preventive administration of FTY720, 1 mg/kg FTY720 had no significant effect on TRALI mice, whose lung tissue homogenate protein concentration (6 170.26±545.50) μg/mL, MPO activity (45.97±4.79) U/L, W/D ratio 4.88±0.25, and lung tissue damage score 7.92±0.65 were significantly higher than those of the normal and LPS control groups (P<0.01). The low-dose (0.5, 0.2, and 0.1 mg/kg) FTY720 group alleviated lung injury, and its protein concentration, MPO activity, W/D ratio, and lung tissue injury score were significantly lower than those of the d5Plt group (P<0.05). Pathological sections also showed similar results. In terms of endothelial intercellular junction protein expression, the VE-cadherin expression levels in the 1 mg/kg FTY720 group were significantly lower than those in the normal and LPS control groups (P<0.05), and the VE-cadherin and ZO-1 expression levels in the low-dose (0.5, 0.2, and 0.1 mg/kg) FTY720 group were significantly higher than those in the d5Plt group (P<0.05), which tended to be normalized. 【Conclusion】 In this study, a TRALI mouse model was successfully established by one strike of LPS and two strikes of d5Plt. Low doses of FTY720 (0.5, 0.2, 0.1 mg/kg) were protective against TRALI, while high doses of FTY720 (1 mg/kg) may aggravate the symptoms of TRALI. 5.Expert consensus on the biobank development of oral genetic diseases and rare diseases and storage codes of related biological samples from craniofacial and oral region

		                        			The biological samples of oral genetic diseases and rare diseases are extremely precious. Collecting and preserving these biological samples are helpful to elucidate the mechanisms and improve the level of diagnose and treatment of oral genetic diseases and rare diseases. The standardized construction of biobanks for oral genetic diseases and rare diseases is important for achieving these goals. At present, there is very little information on the construction of these biobanks, and the standards or suggestions for the classification and coding of biological samples from oral and maxillofacial sources, and this is not conducive to the standardization and information construction of biobanks for special oral diseases. This consensus summarizes the background, necessity, principles, and key points of constructing the biobank for oral genetic diseases and rare diseases. On the base of the group standard "Classification and Coding for Human Biomaterial" (GB/T 39768-2021) issued by the National Technical Committee for Standardization of Biological Samples, we suggest 76 new coding numbers for different of biological samples from oral and maxillofacial sources. 6.Characteristics of the cognitive domains of the montreal cognitive assessment scale in the elderly from the perspective of structural equation modeling

		                        			Objective:To analyzed the characteristics of the cognitive domains of the montreal cognitive assessment(MOCA)scale in cognitively impaired or normal elderly people from the perspective of structural equation modeling(SEM).Methods:There were 335 old individuals in this study, including 166 cognitively normal individuals, 130 patients with mild cognitive impairment(MCI)and 39 individuals with dementia.The average age of the individuals was 81.5±9.0.Data on the Montreal cognitive assessment(MoCA, Beijing version)were gathered.Following exploratory factor analysis and selection of latent and manifest variables, a structural equation model was established.After assessment of data from the domains of the scale in the elderly, participants were divided into a normal group and a patient group, which formed the basis of the model.Results:The KMO value of the data calculated by exploratory factor analysis was 0.762.The dimensions measured by the scale were divided into four main latent variables: memory, visuospatial execution, language ability and attention.The CMIN value of the overall model was 44.039 and the P value was 0.168.The parameters of the overall model and individual dimensions all indicated a good fit.The model showed that visuospatial execution had the largest impact on cognitive function, with a path coefficient of 0.742, and language ability had the least impact, with a path coefficient of only 0.091.As a latent variable, attention had path coefficients of 0.372 and 0.236 for memory, 1.663 and 1.102 for visuospatial execution, and 1.090 and 0.798 for language ability, respectively, for the two groups, with clear statistically significant differences between the groups(all P<0.05). Conclusions:SEM can improve researchers' overall understanding of the impacts of the individual components of the scale and their use and interpretation of the scale.
		                        		
		                        		
		                        		
		                        	
7.Increasing the rate of timely completion of a rapid frozen-section pathological report using the PDCA cycle

		                        			Objective:To investigate the application value of the PDCA cycle in increasing the rate of timely completion of a rapid frozen-section pathological report.Methods:The basic data of 1 926 rapid frozen section pathological reports not managed by the PDCA cycle in the Department of Pathology, Zhoushan Hospital, during January to August 2019 were collected. The number of pathological reports completed within 30 minutes and the rate of timely completion of pathological reports were calculated and compared with those calculated based on 1 051 pathological reports managed by the PDCA cycle during September to December 2019.Results:After management by the PDCA cycle, the rate of timely completion of frozen-section pathological reports was significantly increased from (84.51 ± 3.61)% to (91.87 ± 1.37)% ( t = 3.86, P < 0.05). Conclusion:Application of the PDCA cycle to pathology management can help monitor the completion of pathological reports on frozen sections. 10.Protective Effect of Protocatechuic Aldehyde on Neurovascular Unit Homeostasis Damage in Rats after Cerebral Ischemia-reperfusion Injury

		                        			OBJECTIVE:To obs erve the protective effect of protoca techuic aldehyde(PAL)on neurovascular unit (NVU) homeostasis damage in rats after cerebral ischemia-reperfusion injury (CIRI). METHODS :SD rats were randomly divided into sham operation group ,model group ,PAL high-dose and low-dose groups (10,20 mg/kg),with 11 rats in each group. Administration groups were given relevant medicine intragastrically. Sham operation group and model group were given the same volume of normal saline intragastrically ,10 mL/kg once a day ,for 5 days. After last administration ,CIRI model was induced by suture method ;the ultrastructural changes of NVU were observed by transmission electron microscope. Western blot assay was used to detect the expression of NUV related proteins (MAP-2,GFAP,AQP-4)in cerebral tissue. Immunofluorescence staining was used to observe the positive expression of above proteins in cerebral cortex. RESULTS :Compared with sham operation group , blood-brain barrier (BBB)structure of model group was destroyed severely ,the vascular lumen became narrower ,lateral edema of endothelial cells was severe ,and the thickness of basement membrane varied ;the nuclei of neurons were pyknosis and there was a large area of edema in the surrounding tissues ;the structure of glial cells was seriously damaged ,the cell body was shrunk and organelles were lost ;protein expression (or positive expression )of MAP- 2 in brain tissue (or cerebral cortex )were significantly decreased (P<0.05 or P<0.01),while protein expression (or positive expression ) of GFAP and AQP- 4 were increased significantly(P<0.01). After PAL intervention ,the rats had less BBB damage ,and the morphology of vascular lumen and basement membrane were not completely destroyed ;the damage of neurons was alleviated ,the pyknosis of neurons was decreased , the chromatin was homogeneous and the heterochromatin was decreased;the damage of glial cell structure was alleviated protein expression of GFAP and AQP- 4(except for low-dose group) in cerebral tissue and positive expression of MAP- 2  and GFAP protein in cerebral cortex were reversed @qq.com significantly (P<0.05 or P<0.01). CONCLUSIONS :PAL can protect the stability of NVU from damage in CIRI model  rats; the mechanism may be related to up-regulating the expression of MAP- 2 protein in cerebral cortex and down-regulating the expression of GFAP and AQP- 4 protein in brain tissue.
