Objective:To determine the erythromycin resistance of Bordetella pertussis isolates and their ptxP1 and ptxP3 phenotypic composition and compare clinical manifestations of children with pertussis caused by the two types of strains. Methods:This was a cross-sectional study, the pertussis cases diagnosed using bacterial culture from January 2019 to December 2022 in Beijing Children′s Hospital and the First People′s Hospital of Wuhu were collected.Any suspected Bordetella pertussis colonies were identified by the slide agglutination test. The susceptibility of isolates to erythromycin was detected by the E-test and K-B test. The ptxP gene was amplified by polymerase chain reaction and sequenced to determine its genotype. t-test, Mann-Whitney U-test, Chi-square test and Fisher′s exact test were use to statistical analysis. Results:A total of 192 strains of Bordetella pertussis were identified, including 188 (97.9%) erythromycin-resistant strains.Among the 188 strains, 30.3%(57/188) belonged to the ptxP1 genotype and 69.7%(131/188) belonged to the ptxP3 genotype. In children aged below 1 year old, the incidence of paroxysmal cough caused by infection with the ptxP3 strain was higher than that with the ptxP1 strain (57.1% vs.29.4%, P<0.05), and children infected with the ptxP3 strain were more likely to develop apnea or asphyxia (23.8% vs.17.6%), post-tussive vomiting (44.4% vs.32.4%), whooping cough (72.0% vs.50.0%) and pneumonia or bronchitis (85.7% vs.73.5%) compared to those infected with the ptxP1 strain, but the differences were not statistically significant(all P>0.05). In children aged 1 year old and above, the white blood cell count of children infected with the ptxP1 strain was higher than that of infections with the ptxP3 strain [13.5(9.9, 24.5)×10 9/L, 10.3 (7.0, 16.4)×10 9/L, P<0.05], and children infected with the ptxP1 strain were more likely to contract other pathogen infections than those infected with the ptxP3 strain (17.4% vs.4.4%, P>0.05). Conclusions:ptxP3 erythromycin-resistant Bordetella pertussis has become the main pathogen of pertussis.Infants with pertussis caused by the ptxP3 erythromycin-resistant strain show more significant manifestations and a higher possibility of severe symptoms than those infected with the ptxP1 erythromycin-resistant strain.

Chinese Journal of Perinatal Medicine 2024;27(10):822-828
Objective:To summarize the clinical characteristics of neonatal Ureaplasma parvum (Up) infection. Methods:From June 2021 to July 2023, a total of 2 890 neonates were admitted to the Neonatal Intensive Care Unit of the Children's Hospital Capital Institute of Pediatrics. Metagenomic next generation sequencing (mNGS) was performed on 373 specimens from 157 infants, detecting Up sequences in 12 specimens from ten infants, with no detection of Ureaplasma urealyticum sequences. All ten infants with detected Up were included in a retrospective analysis. Descriptive statistical analysis was used to summarize the clinical characteristics of Up-infected neonates. Results:All ten Up-infected neonates were preterm infants with a gestational age of (28.3±2.6) weeks (25 +3-33 +1 weeks). Seven were delivered vaginally; eight had mothers with premature rupture of membranes; seven had mothers with elevated white blood cell counts and/or C-reactive protein levels prenatally; one had a mother with Ureaplasma Urealyticum vaginitis. All ten infants experienced clinical deterioration after initial stabilization of their underlying conditions, primarily presenting with respiratory symptoms, including decreased blood oxygen saturation, diffuse reticular changes on chest X-rays by the second day of life, pneumonia, and atelectasis. Some also had fever, decreased heart rate, poor skin perfusion, and scattered bruises, with two cases of heart failure. Despite empirical antibiotic treatment, nine infants continued to have significantly elevated white blood cell counts, with only mildly elevated or normal C-reactive protein levels. Seven developed bronchopulmonary dysplasia, including four moderate to severe cases. After mNGS confirmed Up infection, all infants received macrolide antibiotics and symptomatic treatment, with individualized treatment courses. All were discharged after recovery with a median hospital stay of 58.5 d (range 26-100 d), though three had respiratory sequelae on follow-up. Conclusions:In preterm infants, clinical deterioration after initial stabilization, primarily with respiratory symptoms and persistent leukocytosis despite routine antibiotic treatment, should raise suspicion for Up infection. And mNGS aids in definitive diagnosis, and early initiation of macrolide antibiotics can improve clinical outcomes and long-term prognosis.

Asian Pacific Journal of Tropical Medicine 2022;15(4):153-160
Objective: To describe the current reporting of pneumonia of unknown etiology (PUE) and factors that affect reporting by clinicians in China using the PUE surveillance system in order to provide a reference for improving PUE reporting rates in the future. Methods: Clinicians were recruited via the Sojump platform and requested to complete an anonymous self-administered questionnaire. Multivariate logistic regression analysis was used to assess factors influencing clinicians' reporting activities. Results: This study showed a low PUE case reporting rate and a poor understanding of PUE reporting among the investigated clinicians. Of the 136 clinicians who had diagnosed PUE cases, multivariate logistic regression analysis results showed that clinicians who had attended in-hospital training were more likely to report PUE than those who had not (OR 4.48, 95% CI 1.49-13.46). Clinicians with an expert panel on PUE in their hospital were more likely to report PUE cases than those without (OR 5.46, 95% CI 1.85-16.11). Conclusions: There is a need to promote and reinforce PUE surveillance system training among medical staff. In addition, PUE testing technologies in hospital laboratories should be upgraded, especially in primary and unclassified hospitals, to increase surveillance efficiency and improve PUE reporting rates.
