1.A modified surgical technique of robot-assisted inferior vena cava thrombectomy for patients with left renal cell carcinoma and tumor emboli: a report of 7 cases eliminating preoperative interventional embolization

Objective: To explore the safety and feasibility of the disconnection of the left renal artery preferentially during robot-assisted inferior vena cava (IVC) thrombectomy for patients with left renal cell carcinoma and tumor emboli. Methods: Clinical data of 7 patients who underwent robot-assisted IVC thrombectomy and radical nephrectomy in the First Affiliated Hospital of Zhengzhou University during Dec.2021 and Oct.2024 were retrospectively analyzed.Thrombectomy was performed first,followed by nephrectomy. The “IVC-first, kidney-last”robotic technique was developed to minimize chances of IVC thrombus. When patients in left lateral decubitus position, the left renal artery was severed from the right side through the inferior vena cava and abdominal aorta. After removal of thrombus from IVC was completed, patients changed to the right lateral position to complete radical left nephrectomy. Results: Imaging examinations revealed that the median diameter of the renal cell carcinomas was 83(46-99) mm; the median length of the inferior vena cava cancerous emboli was 49(2-91) mm.According to the Mayo classification,the cancerous emboli were gradeⅠ in 2 cases,gradeⅡ in 4 cases,and grade Ⅲ in 1 case.All surgeries were successful.The median operation time was 248(201-331) minutes,blood loss 500(200-1000) mL,and 6 cases required intraoperative blood transfusion.The median time for transition into the intensive care unit was 1(1-4) days,and drainage tube removal 6(5-12) days.Serum creatinine increased significantly in 5 cases,4 of which returned to normal after 1 week,but 1 had renal insufficiency (creatinine 166 μmol/L).Chylous fistula occurred in 1 patient,and lower extremity venous thrombosis developed in 3 patients.Pathological examinations indicated 6 cases of renal cell carcinoma and 1 case of MiT family translocation renal cell carcinoma.During the median follow-up of 17(1-35) months,5 cases were tumor-free,while 2 had lung and retroperitoneal metastases.They received targeted therapy of axitinib combined immunotheraphy and lived with tumors. Conclusion: In the left lateral position for left renal cell carcinoma with cancerous emboli,robot-assisted laparoscopic thrombectomy by crossing the inferior vena cava and abdominal aorta and disconnecting the left renal artery first is safe and feasible.
2.Analysis of injury mortality characteristics among residents in Yuyao City, Zhejiang Province, 2014‒2023

ObjectiveTo analyze the characteristics and trends of injury-related deaths among the registered residents in Yuyao City, Zhejiang Province from 2014 to 2023, and to provide a scientific basis for the formulation of targeted injury prevention and control strategies. MethodsData on injury-related deaths in Yuyao City from January 1, 2014 to December 31, 2023 were collected from the cause-of-death surveillance module of the chronic disease monitoring and management system of Zhejiang Province. Indicators including constituent ratio, crude mortality rate (CMR), standardized mortality rate (SMR), premature death probability, potential years of life lost (PYLL), average years of life lost (AYLL), and potential years of life lost rate (PYLLR) were calculated. The average annual percentage change (AAPC) was used to analyze the trends in injury mortality over the past 10-year period. ResultsFrom 2014 to 2023, a total of 6 625 injury-related deaths were reported in Yuyao City, accounting for 10.65% of all deaths and ranking fourth among all causes. The CMR was 79.34/100 000 and showed an increasing trend (AAPC=4.396%, t=2.875, P=0.021), while the SMR was 38.99/100 000 with no significant trend (AAPC=-0.585%, t=-0.451, P=0.664). The SMR of females (37.74/100 000) was lower than that of males (40.87/100 000) (χ2=12.468, P<0.001). The top 5 causes of injury-related death were falls, traffic accidents, drowning, suicide, and accidental suffocation. Over the 10-year period, the SMRs for traffic accidents, drowning, and suicide showed downward trends (AAPC=-5.381%, t=-3.428, P=0.009; AAPC=-8.061%, t=-6.924, P<0.001; AAPC=-6.919%, t=-4.039, P=0.004, respectively), while the SMRs for falls and accidental suffocation showed no significant trends (AAPC=3.417%, t=1.767, P=0.115; AAPC=2.228%, t=0.803, P=0.445). Injury mortality increased with age for males, females, and the total population (all P<0.05). The leading causes of injury-related death were drowning among residents aged 0‒<15 years, traffic accidents among those aged 15‒<45 and 45‒<65 years, and falls among those aged 65‒<85 and 85‒110 years. The total PYLL due to injury was 58 708.00 person-years, the AYLL was 21.23 years, and the PYLLR was 7.36‰. ConclusionFrom 2014 to 2023, the crude mortality rate due to injuries in Yuyao City remained at a relatively high level, posing a serious threat to residents’ health and well-being. Falls, traffic accidents, drowning, suicide, and accidental suffocation are key targets for injury prevention and control work.

Hearing loss has become increasingly prevalent and causes considerable disability, thus gravely burdening the global economy. Irreversible loss of hair cells is a main cause of sensorineural hearing loss, and currently, the only relatively effective clinical treatments are limited to digital hearing equipment like cochlear implants and hearing aids, but these are of limited benefit in patients. It is therefore urgent to understand the mechanisms of damage repair in order to develop new neuroprotective strategies. At present, how to promote the regeneration of functional hair cells is a key scientific question in the field of hearing research. Multiple signaling pathways and transcriptional factors trigger the activation of hair cell progenitors and ensure the maturation of newborn hair cells, and in this article, we first review the principal mechanisms underlying hair cell reproduction. We then further discuss therapeutic strategies involving the co-regulation of multiple signaling pathways in order to induce effective functional hair cell regeneration after degeneration, and we summarize current achievements in hair cell regeneration. Lastly, we discuss potential future approaches, such as small molecule drugs and gene therapy, which might be applied for regenerating functional hair cells in the clinic.

4.Acute Myocardial Infarction Caused by Multiple Coronary Thrombosis:a Case Report

Multiple thrombosis in the coronary arteries need to be characterized by a thorough determination of the source of the thrombus to distinguish them as thrombosis or coronary embolism.This case was a 38-year-old male patient with chest pain and an electrocardiogram showing acute inferior wall and right ventricular myocardial infarction.Emergency coronary angiography showed thrombosis in the proximal middle of the left anterior descending artery,the opening of the first diagonal artery,and the middle of the right coronary artery,but no obvious stenosis was seen.Postoperative electrocardiogram showed acute inferior wall,right ventricular and anterior wall myocardial infarction,and intensive antithrombotic treatment was applied,elective re-examination of coronary angiography and intraluminal imaging showed mixed plaques and suspicious intimal dissection,indicating the possibility of thrombosis secondary to unstable plaque and coronary dissection,and intensive drug treatment was given.After discharge,the patient was stable during the regular follow-up visits.
5.Application of left echography in the diagnosis of false ventricular aneurysm and mural thrombus

Objective:To evaluate the value of left echography (LVO) in the diagnosis of false ventricular aneurysm complicated with mural thrombus.Methods:The clinical data of 10 patients with suspected pseudoventricular aneurysm examined by thoracic echocardiography (TTE) in Wuhan Asian Heart Hospital from January 2018 to March 2024 were retrospectively analyzed. All patients underwent LVO examination to further diagnose pseudoventricular tumor and whether it was complicated with mural thrombosis. Computed tomography angiography (CTA) or cardiac magnetic resonance (CMR) examination was used as the gold standard to analyze the diagnostic value of LVO in the diagnosis of pseudoventricular tumor.Results:Among the 10 suspected pseudoventricular tumors examined by TTE, LVO detected 6 cases of left ventricular pseudoaneurysm and 1 case of right ventricular pseudoaneurysm; CTA confirmed that 6 cases of left ventricular pseudoaneurysm detected by LVO were correctly diagnosed, 1 case of right ventricular pseudoaneurysm was misdiagnosed, CMR diagnosed right ventricular diverticula, LVO diagnosis accuracy was 6/7, and 4 cases of thrombi were detected. The detection rate was 4/4. The maximum transverse diameter of the tumor body of the communicating mouth/false ventricular aneurysm was 0.46±0.04. 1 patient underwent coronary artery bypass grafting and resection of false ventricular aneurysm. 1 patient underwent coronary artery interventional stent surgery; 4 routine conservative drug treatment, follow-up observation; One case of right ventricular diverticulum did not require special treatment.Conclusions:LVO contrast agent can clearly show the tumor body and location, measure the tumor entrance and size, and show mural thrombus. It is the first choice for the identification of false ventricular tumor. The diverticula was similar to the image of false ventricular aneurysm, and the sensitivity and specificity of right ventricular wall motion were higher in CMR than in LVO.
6.Propensity score-matched comparison of the clinical efficacy between two approaches of robot-assisted radical prostatectomy

Objective To compare the clinical efficacy and postoperative urinary control between robot-assisted radical prostatectomy(RARP)with posterior-anterior-lateral(PAL)approach and with anterior(conventional)approach using propensity score matching method.Methods Clinical data of 145 patients undergoing RARP in our hospital during Jan.2020 and Jan.2023 were retrospectively analyzed,including 122 patients in the conventional group and 23 in the PAL group.The patients were matched by 2∶1 propensity score matching,including 46 cases in the conventional group and 23 in the PAL group.The perioperative outcomes were compared of prostate cancer(PCa)patients undergoing RARP surgery with different approaches before and after matching,including operation time,intraoperative blood loss,pelvic drainage time,hospitalization days,preservation of neurovascular bundles(NVB)during surgery,deep dorsal venous complex(DVC)suture,reconstruction of bladder neck,and postoperative urinary control recovery rate after extubation immediately,and 1,3,and 6 months after surgery.Results There were no significant differences in baseline data,operation time,bleeding volume,pelvic drainage time,hospitalization days,preservation of NVB,and reconstruction of bladder neck between the two groups(P>0.05).The PAL group used less DVC suture during surgery(30.4%vs.100%,P<0.001),but had better urinary control recovery rate immediately after extubation,1,3 and 6 months after surgery(P<0.05).Conclusion RARP with PAL approach is as safe and effective as the conventional approach,and has significant advantages in early postoperative urinary control.
7.Construction of evaluation index system of core competence of neonatal specialist nurses

Objective:To construct the evaluation index system of the core competence of neonatal specialist nurses, so as to provide reference for clinical training of neonatal specialist nurses.Methods:From January 2020, through literature review, theoretical analysis, interview, Delphi method and superiority chart, the evaluation index and weight of core competence of neonatal specialist nurses were determined.Results:Totally 28 experts in China were invited for 3 rounds of consultation. The effective recovery rate of the questionnaires was 93.33%(28/30) in the first round. The effective recovery rate of the questionnaires was 100.00%(28/28) in the second and third rounds. The authority coefficient of the experts was 0.85 in the third round. The Kendall′s coefficients of concordance of the first-level indexes, second-level indexes and third-level indexes were 0.150, 0.221 and 0.161, respectively. The final evaluation index system of the core competence of neonatal specialist nurses included 5 first-level indicators, 17 second-level indicators and 58 third-level indicators.Conclusions:The evaluation index system of the core competence of neonatal specialist nurses constructed in this study has certain scientific, reliable and clinical application value, which is conducive to the training of newborn specialized nurses in China.
8.Morbidity and mortality characteristics of asopharyngeal carcinoma among registered residents in Yuyao City of Zhejiang Province from 2012 to 2021

ObjectiveTo analyze the morbidity and mortality characteristics and prevalence trends of asopharyngeal carcinoma among registered residents in Yuyao City of Zhejiang Province from 2012 to 2021, and to provide suggestions for the prevention and treatment of the cancer. MethodsThe incidence and mortality data of asopharyngeal carcinoma in Yuyao City from 2012 to 2021 were collected. The incidence, mortality, standardized rate, cut-off rate, cumulative rate and annual percentage of trend change were calculated. A linear logarithmic model was used to analyze the variation trend. ResultsFrom 2012 to 2021, the crude incidence rate was 6.46/105 in Yuyao, while the Chinese standard rate was 4.78/105 and the world standard rate was 3.68/105, which all increased over the years. The indicators in males were higher than these in females (χ2=85.377, P<0.01). The cumulative incidence rate of ≥65 years old group was the highest (13.63/105). The crude mortality rate was 2.45/105, while the Chinese standardized rate was 1.47/105 and the world standard rate was 1.14/105. The crude mortality rate in males was 3.25/105,which was higher than females(1.68/105)(χ2=20.958, P<0.01). Overall, the cumulative rate was highest reached its peak in ≥75 years old group (10.75/105). The male mortality rate reached its peak in the ≥80-year-old population (12.36/105), and the female mortality rate in ≥75 years old group (11.03/105). ConclusionThe incidence rate of asopharyngeal carcinoma in Yuyao City is generally increasing, but the mortality rate has no obvious variation. The key population for prevention and treatment in Yuyao City is male, middle-aged and elderly.
9.Related factors of recurrence of renal cell carcinoma after robot-assisted partial nephrectomy

【Objective】 To investigate the recurrence of patients with renal cell carcinoma (RCC) after robot-assisted partial nephrectomy (RAPN), and to explore the related factors of recurrence. 【Methods】 Clinical data of 367 RCC patients who underwent RAPN during 2015 and 2020 in our hospital were retrospectively collected. Patients were divided into recurrence group and non-recurrence group. The related factors of recurrence and 5-year cumulative recurrence rate of RCC and clear cell renal cell carcinoma (ccRCC) in the recurrence group were analyzed. 【Results】 The median follow-up was 48 (IQR:38-60) months. Recurrence occurred in 9 patients, with a 5-year cumulative recurrence rate of 97.5%. For RCC patients, univariate analysis showed that tumor size, T-stage, R. E. N. A. L. score, R value, N value, and operation time were significantly associated with postoperative recurrence (P<0.05), while multivariate analysis showed that N value (HR=15.75, 95%CI:2.00-124.17, P=0.009) was an independent related factor. For ccRCC patients, univariate analysis showed that hypertension, tumor size, T-stage, R. E. N. A. L. score, N value, operation time and WHO/ISUP grade were significantly associated with postoperative recurrence (P<0.05), while multivariate analysis showed that WHO/ISUP grade (HR=4.99, 95%CI:1.04-24.01, P=0.045) was an independent related factor. 【Conclusion】 The 5-year cumulative recurrence rate of RCC patients after RAPN is 2.5%. N value and WHO/ISUP grade are the independent related factor of recurrence of RCC and ccRCC, respectively.
10.Influencing factors of conversion from robot-assisted partial nephrectomy to radical nephrectomy

【Objective】 To explore the influencing factors of planned robot-assisted partial nephrectomy (RAPN) converted to radical nephrectomy (RN). 【Methods】 Clinical data and operation records of 488 patients planned for RAPN in our hospital during 2015 and 2020 were retrospectively collected. The patients were divided into converted and unconverted groups, and relevant clinical data of the two groups were compared. The causes and influencing factors for conversion were analyzed. 【Results】 Among the 488 patients, 14 (2.9%) converted to RN. The causes included: ①complicated tumor anatomy and surgical difficulties; ②local advanced renal tumor suspected during operation; ③severe intraoperative hemorrhage. Univariate analysis showed that R. E. N. A. L score (P<0.001), E value (P<0.001), N value (P<0.001), L value (P<0.001), renal hilum position (P<0.001) and T stage (P=0.002) were influencing factors of conversion. 【Conclusion】 Causes for the conversion of RAPN to RN include complicated tumor anatomy, suspected local advanced renal tumor and severe intraoperative hemorrhage. The R. E. N. A. L score, E value, N value, L value, renal hilum position and T stage are influencing factors.
