1.Analysis of factors influencing the achievement of target vancomycin plasma concentration and construction of a predictive model in patients from high-altitude regions: a single-center retrospective study

OBJECTIVE To analyze the influencing factors for achieving target plasma drug concentration (trough) (abbreviated as “PDC”) of vancomycin in patients from high-altitude regions and establish a predictive model for PDC using single- center data, providing references for rational clinical drug use. METHODS Inpatients with vancomycin (1 g, q12 h) administered intravenously in our hospital from January 2021 to June 2024 were retrospectively included. Demographic data, liver and kidney function and hematological indexes were collected. Spearman correlation analysis was used to evaluate the correlation between vancomycin PDC and each detection index. Univariate analysis was used to evaluate the differences of each index in patients with different PDC, and the effects of different gender, body mass index, age and underlying diseases (hypertension/diabetes) on vancomycin PDC. Based on the results of correlation analysis and univariate analysis, multiple linear stepwise regression analysis was used to obtain the independent predictors of vancomycin PDC and construct the prediction model. RESULTS A total of 141 patients were included, with an overall attainment rate of 46.81% for the target PDC of vancomycin. Correlation analysis showed that the vancomycin PDC was positively correlated with age, blood urea nitrogen, uric acid (UA), serum creatinine (CRE) and β2- microglobulin (β2-MG), and negatively correlated with height, weight, creatinine clearance rate (CCR), glomerular filtration rate (GFR), alanine transaminase (ALT), hemoglobin (HGB), white blood cell count and neutrophils (P<0.05). There were significant differences in age, CRE and other 14 indexes among different PDC groups (P<0.05 or P<0.01). Age and underlying diseases had significant effects on vancomycin PDC (P<0.05 or P<0.01). CCR, direct bilirubin (DBil), β2-MG, UA, HGB and height (standardized coefficients were -0.371, 0.367, 0.169, 0.232, -0.140, -0.132; P<0.05) were independent predictors of vancomycin PDC. The F value of the regression equation was 34.858 (P<0.05), the R2 was 0.610, and the adjusted R2 was 0.592. CONCLUSIONS The vancomycin PDC of patients in high-altitude regions is affected by multiple factors such as renal function, liver function and hematological indexes. CCR, HGB and height could be used to predict vancomycin PDC negatively, while DBil, β2-MG and UA could be used to predict vancomycin PDC positively. The variables of the established prediction model could explain 59.2% of the variation of vancomycin PDC.

The Expert Consensus on Clinical Application of Qinbaohong Zhike Oral Liquid in Treatment of Acute Bronchitis and Acute Attack of Chronic Bronchitis (GS/CACM 337-2023) was released by the China Association of Chinese Medicine on December 13th, 2023. This expert consensus was developed by experts in methodology, pharmacy, and Chinese medicine in strict accordance with the development requirements of the China Association of Chinese Medicine (CACM) and based on the latest medical evidence and the clinical medication experience of well-known experts in the fields of respiratory medicine (pulmonary diseases) and pediatrics. This expert consensus defines the application of Qinbaohong Zhike oral liquid in the treatment of cough and excessive sputum caused by phlegm-heat obstructing lung, acute bronchitis, and acute attack of chronic bronchitis from the aspects of applicable populations, efficacy evaluation, usage, dosage, drug combination, and safety. It is expected to guide the rational drug use in medical and health institutions, give full play to the unique value of Qinbaohong Zhike oral liquid, and vigorously promote the inheritance and innovation of Chinese patent medicines.
3.A modified surgical technique of robot-assisted inferior vena cava thrombectomy for patients with left renal cell carcinoma and tumor emboli: a report of 7 cases eliminating preoperative interventional embolization

Objective: To explore the safety and feasibility of the disconnection of the left renal artery preferentially during robot-assisted inferior vena cava (IVC) thrombectomy for patients with left renal cell carcinoma and tumor emboli. Methods: Clinical data of 7 patients who underwent robot-assisted IVC thrombectomy and radical nephrectomy in the First Affiliated Hospital of Zhengzhou University during Dec.2021 and Oct.2024 were retrospectively analyzed.Thrombectomy was performed first,followed by nephrectomy. The “IVC-first, kidney-last”robotic technique was developed to minimize chances of IVC thrombus. When patients in left lateral decubitus position, the left renal artery was severed from the right side through the inferior vena cava and abdominal aorta. After removal of thrombus from IVC was completed, patients changed to the right lateral position to complete radical left nephrectomy. Results: Imaging examinations revealed that the median diameter of the renal cell carcinomas was 83(46-99) mm; the median length of the inferior vena cava cancerous emboli was 49(2-91) mm.According to the Mayo classification,the cancerous emboli were gradeⅠ in 2 cases,gradeⅡ in 4 cases,and grade Ⅲ in 1 case.All surgeries were successful.The median operation time was 248(201-331) minutes,blood loss 500(200-1000) mL,and 6 cases required intraoperative blood transfusion.The median time for transition into the intensive care unit was 1(1-4) days,and drainage tube removal 6(5-12) days.Serum creatinine increased significantly in 5 cases,4 of which returned to normal after 1 week,but 1 had renal insufficiency (creatinine 166 μmol/L).Chylous fistula occurred in 1 patient,and lower extremity venous thrombosis developed in 3 patients.Pathological examinations indicated 6 cases of renal cell carcinoma and 1 case of MiT family translocation renal cell carcinoma.During the median follow-up of 17(1-35) months,5 cases were tumor-free,while 2 had lung and retroperitoneal metastases.They received targeted therapy of axitinib combined immunotheraphy and lived with tumors. Conclusion: In the left lateral position for left renal cell carcinoma with cancerous emboli,robot-assisted laparoscopic thrombectomy by crossing the inferior vena cava and abdominal aorta and disconnecting the left renal artery first is safe and feasible.
4.Advances in population pharmacokinetics of meropenem in critically ill adult patients

Meropenem (MEM) is one of the important drugs for the treatment of severe infections, but the standard dose is often difficult to achieve an effective therapeutic concentration target. This article reviews the related studies on the population pharmacokinetics of MEM in patients with severe infection. It is found that the apparent volume of distribution (Vd) and clearance rate are the most important factors affecting the dose adjustment, and the factors affecting Vd include serum albumin, age, overall weight, shock status, and chest/abdomen/cerebrospinal fluid drainage. The main factors affecting the clearance rate were renal function, renal replacement therapy treatment mode and combination therapy. For adult patients with severe infections in China, MEM is recommended to be administered in an individualized manner based on glomerular filtration rate, with a dosage range of 500 to 1 500 mg given every 4 to 6 hours, and prolonged infusion is preferred. When the minimum inhibitory concentration (MIC) of the pathogenic bacteria reaches 64 mg/L, therapeutic drug monitoring is required. For therapeutic efficacy, it is essential to ensure that the trough concentration remains above the MIC; to prevent drug resistance, it should be maintained above 4×MIC. Regarding safety, it is recommended that the upper limit of the trough concentration be 32 mg/L, and blood sampling for monitoring can be conducted as early as after 1 to 2 doses of administration.

Debates persist regarding the efficacy and safety of azvudine, particularly its real-world outcomes. This study involved patients aged ≥60 years who were admitted to 25 hospitals in mainland China with confirmed SARS-CoV-2 infection between December 1, 2022, and February 28, 2023. Efficacy outcomes were all-cause mortality during hospitalization, the proportion of patients discharged with recovery, time to nucleic acid-negative conversion (T NANC), time to symptom improvement (T SI), and time of hospital stay (T HS). Safety was also assessed. Among the 5884 participants identified, 1999 received azvudine, and 1999 matched controls were included after exclusion and propensity score matching. Azvudine recipients exhibited lower all-cause mortality compared with controls in the overall population (13.3% vs. 17.1%, RR, 0.78; 95% CI, 0.67-0.90; P = 0.001) and in the severe subgroup (25.7% vs. 33.7%; RR, 0.76; 95% CI, 0.66-0.88; P < 0.001). A higher proportion of patients discharged with recovery, and a shorter T NANC were associated with azvudine recipients, especially in the severe subgroup. The incidence of adverse events in azvudine recipients was comparable to that in the control group (2.3% vs. 1.7%, P = 0.170). In conclusion, azvudine showed efficacy and safety in older patients hospitalized with COVID-19 during the SARS-CoV-2 omicron wave in China.

The primary intravenous anesthetics employed in clinical practice encompass dexmedetomidine (Dex), propofol, ketamine, etomidate, midazolam, and remimazolam. Apart from their established sedative, analgesic, and anxiolytic properties, an increasing body of research has uncovered neuroprotective effects of intravenous anesthetics in various animal and cellular models, as well as in clinical studies. However, there also exists conflicting evidence pointing to the potential neurotoxic effects of these intravenous anesthetics. The role of intravenous anesthetics for neuro on both sides of protection or toxicity has been rarely summarized. Considering the mentioned above, this work aims to offer a comprehensive understanding of the underlying mechanisms involved both in the central nerve system (CNS) and the peripheral nerve system (PNS) and provide valuable insights into the potential safety and risk associated with the clinical use of intravenous anesthetics.

7.Research on the current status of self-medication and pharmaceutical service needs among residents in Xining area

OBJECTIVE To investigate the current status of self-medication behaviors and the demand preferences for pharmaceutical services among residents in Xining, providing a basis for developing pharmaceutical service intervention strategies tailored to regional characteristics. METHODS A self-designed questionnaire, developed based on literature review and revised after expert panel discussion and a pre-survey, was used to conduct anonymous surveys among residents purchasing medicines at 12 retail pharmacies in Xining area from April 2023 to April 2024. Descriptive analysis, Chi-square test, and Spearman correlation analysis were employed to analyze the characteristics and influencing factors of residents’ self-medication behaviors, and pharmaceutical service needs. RESULTS A total of 859 valid questionnaires were collected, with 605 respondents (70.43%) reporting self-medication behaviors. The primary reason for self-medication was mild symptoms (46.94%). The main conditions treated were gastrointestinal discomfort (38.51%) and cold, fever, headache (35.37%). Chinese patent medicines were the most commonly used (58.68%). Drug selection relied mainly on recommendations by pharmacy staff (52.07%), and retail pharmacies were the primary source of medicines (65.95%). Dosage and administration were determined primarily by referring to the drug package insert (67.27%), while a minority relied on personal experience (9.92%). Notably, 20.33% of respondents never read the package insert before medication, with a higher tendency observed among ethnic minorities, farmers, and individuals with chronic diseases (P<0.05). While 65.29% of respondents reported partially understanding the insert content, those aged ≥60 years, ethnic minorities, and individuals with chronic diseases qhsrmyy-28) were more likely to report being completely unable to understand it (P<0.05). Self-medication was “often effective” for 52.73% of respondents, whereas 7.77%“ often experienced adverse drug reactions”; farmers were more prone to poor efficacy or adverse reactions (P<0.05). A majority (72.89%) of respondents residents believed it necessary to learn about self- medication knowledge, and 47.11% preferred to obtain pharmaceutical services through WeChat public accounts or mobile applications (APP). CONCLUSIONS Self-medication is common among residents in Xining area. Issues such as neglecting to read package inserts and relying on personal experience are evident. It is essential to develop science popularization programs suitable for the characteristics of the local population and utilize digital platforms like WeChat public accounts and APP to disseminate self- medication knowledge, thereby enhancing residents’ awareness and capacity for rational medication use.
8.The value of synthetic MRI combined with field of view optimized and constrained undistorted single shot diffusion weighted imaging in differential diagnosis of BI-RADS 4 breast lesions

Objective To evaluate the differential diagnostic value of synthetic magnetic resonance imaging(syMRI)and field of view optimized and constrained undistorted single shot diffusion weighted imaging(FOCUS DWI)in patient with breast imaging reporting and data system(BI-RADS)4 breast lesions.Methods A total of 68 patients who underwent breast MRI and were classified as BI-RADS category 4 were prospectively enrolled.Among them,there were 31 cases of benign lesions(benign group)and 37 cases of malignant lesions(malignant group).All patients underwent T1WI,T2WI,FOCUS DWI,dynamic contrast-enhanced magnetic resonance ima-ging(DCE-MRI)and syMRI.Two radiologists delineated the region of interest(ROI)on quantitative parameters images of syMRI and the apparent diffusion coefficient(ADC)images generated from FOCUS DWI and calculated T1,T2,proton density(PD)and ADC values,respectively.The thresholds and diagnostic efficacy of syMRI and FOCUS DWI parameters were evaluated by the receiver op-erating characteristic(ROC)curve.Logistic regression analysis was used to combine the parameters of the two sequences and evalu-ate the diagnostic efficacy.Results The T2,PD,and ADC values in the malignant group were significantly lower than those in the benign group(P<0.05),but there were no statistically significant difference in T1 values between the groups(P>0.05).T2 and ADC values could be used as important diagnostic indexes in patient with malignant lesions of BI-RADS 4 breast lesions,and the area under the curve(AUC)of T2 and ADC was 0.833 and 0.867,respec-tively.There was no significant difference in AUC of T2 and ADC between benign and malignant groups(Z=0.485,P=0.627).The AUC of T2 value combined with ADC value was 0.910,and the sensitivity and specificity was 90.3%and 89.2%,respectively.Conclusion T2 value of syMRI and ADC value of FOCUS DWI can be used as quantitative diagnostic indicators in patients with BI-RADS 4 breast lesions.The combination of T2 and ADC values can further improve the differential diagnosis efficiency.
9.Research on Construction of Index System for the Quality Evaluation of Multidisciplinary Diagnosis and Treatment Model for Cancer on the Multi Case Coding

Objective It aims to construct a quality evaluation index system for cancer multidisciplinary diagnosis and treatment(MDT)model in China from a full process perspective,providing guidance for practical application and model optimization.Methods Based on the number of MDT publications and practical situations,20 provincial-level hospitals nationwide were selected as typical cases.Rooted theory was used to extract evaluation indicators from the original text of the cases through three-level coding.A cancer MDT quality evaluation index system was constructed under the Input-Process-Output framework.Results Through three-level coding,27 initial categories,8 subcategories,and 3 main categories were sorted out,and a cancer MDT quality evaluation index system was constructed with input,process,and output as the primary evaluation indicators,and top-level design,management system,object resources,meeting preparation,meeting progress,plan implementation,patient outcomes,and hospital outcomes as the secondary evaluation indicators.Conclusion The quality evaluation index system of cancer MDT mode based on the perspective of the entire process can effectively guide practical optimization,but there is still a need for the improvement of supporting policies and information systems to assist in quality evaluation.
10.Distribution characteristics and drug resistance rate of pathogenic bacteria in patients with pulmonary tuberculosis combined with Candida infection on the Tibetan plateau

Objective To investigate the pathogenic distribution characteristics and drug resistance of opportunistic Candida in patients with pulmonary tuberculosis on the Qinghai-Tibet Plateau.Methods 3 012 hospitalized cases of pulmonary tuberculosis at Qinghai Province Fourth People's Hospital from March 1,2020 to December 31,2020 were analyzed,sputum samples were collected,Candida identification was carried by VITEK-32-YBC automatic bacterial analysis system,and the detected Candida was tested for drug sensitivity.Results Among the 3 012 cases of pulmonary tuberculosis in this investigation,283 cases of pulmonary tuberculosis patients with Candida infection,accounting for 9.40%.Among them,Candida albicans was the main type of Candida,accounting for 79.86%of the total.Conclusion The prevalence rate of pulmonary tuberculosis complicated with Candida infection was high in Qinghai-Tibet Plateau.Therefore,the selection of antimicrobial drugs should be based on a comprehensive analysis of the patient's condition,in order to select the best and most effective drugs for treatment.
