1.Molecular biological research and molecular homologous modeling of Bw.03 subgroup

[Objective] To study the molecular biological mechanism for a case of ABO blood group B subtype, and perform three-dimensional modeling of the mutant enzyme. [Methods] The ABO phenotype was identified by the tube method and microcolumn gel method; the ABO gene of the proband was detected by sequence-specific primer polymerase chain reaction (PCR-SSP), and the exon 6 and 7 of the ABO gene were sequenced and analyzed. Homologous modeling of Bw.03 glycosyltransferase (GT) was carried out by Modeller and analyzed by PyMOL2.5.0 software. [Results] The weakening B antigen was detected in the proband sample by forward typing, and anti-B antibody was detected by reverse typing. PCR-SSP detection showed B, O gene, and the sequencing results showed c.721 C>T mutation in exon 7 of the B gene, resulting in p. Arg 241 Trp. Compared with the wild type, the structure of Bw.03GT was partially changed, and the intermolecular force analysis showed that the original three hydrogen bonds at 241 position disappeared. [Conclusion] Blood group molecular biology examination is helpful for the accurate identification of ambiguous blood group. Homologous modeling more intuitively shows the key site for the weakening of Bw.03 GT activity. The intermolecular force analysis can explain the root cause of enzyme activity weakening.
2.Diarrhea caused by foodborne Salmonella infection in children aged 0-6 years in Guizhou Province from 2016 to 2023

Objective:
To analyze the epidemilogical and seasonal characteristics of foodborne Salmonella-associated diarrhea among children aged 0-6 years in Guizhou Province from 2016 to 2023, so as to provide a basis for the prevention and control of foodborne diseases.
Methods:
Data were extracted from the Foodborne Disease Survellance System for cases reported between January 1, 2016, and December 31, 2023. The incidence, seasonal characteristics, and peak periods were analyzed by the method of concentration and circular distribution.
Results:
A total of 6 434 cases of diarrhea in children aged 0-6 years were collected, and 455 cases of Salmonella were detected, with a positive detection rate of 7.07%. Salmonella typhimurium was the main serotype causing diarrhea (59.34%). The peak of the disease was from May 3 to September 30, with certain seasonal characteristics. The highest detection rate was found in children aged 1-3 years (8.66%). Among food types, the positive detection rates of Salmonella were relatively high in other foods (17.39%), fruits and their products (10.22%), infant and toddler foods (10.09%), and aquatic animals and their products (9.80%). The processing and packaging methods of food were mainly home-made (9.38%) and bulk food (7.54%).
Conclusions
The detection rate of Salmonella in children aged 0-6 years is high in Guizhou Province, with strong seasonal characteristics. The detection rates of other foods, fruits and their products, infant and toddler foods, and aquatic animals and their products are high. Enhanced pathogen surveillance for susceptible populations and high-risk foods, coupled with public health education during summer/autumn, is recommended.
3.Impact of Wenyang lishui formula on ventricular remodeling in rats with pulmonary hypertension-induced right heart failure

OBJECTIVE To discuss the impact of Wenyang lishui formula on ventricular remodeling in rats with pulmonary hypertension-induced right heart failure (RHF) based on the Hippo/Yes-associated protein (YAP) signaling pathway. METHODS Ten rats were randomly selected as the control group. The remaining 63 rats were given a single intraperitoneal injection of monocrotaline to establish the pulmonary hypertension-induced RHF model. The 50 rats that successfully underwent the model establishment were randomly divided into the RHF group, low-dose group of Wenyang lishui formula (4.25 g/kg), high-dose group of the Wenyang lishui formula (17.00 g/kg), furosemide group (20 mg/kg), and high-dose group of Wenyang lishui formula+ Hippo/YAP signaling pathway activator group (17.00 g/kg of Δ Wenyang lishui formula+16 mg/kg of PY-60), with 10 rats in each group. The rats in each group were given the corresponding drug solution or normal saline by gavage or/andtail vein injection, once a day, for 4 consecutive weeks. During the experiment, the general conditions of the rats in each group were observed; after the last administration, the right ventricular diameter, right atrial diameter, end-diastolic volume, pulmonary artery blood flow acceleration time (PAAT) and its ratio to ejection time (ET) (PAAT/ET), pulmonary artery pressure and its ratio to pulmonary arterial flow velocity (pulmonary artery pressure/velocity) were measured. The plasma levels of brain natriuretic peptide and angiotensin Ⅱ (Ang Ⅱ) were detected. The pathological changes of the right ventricular tissue were observed, and the collagen volume fraction, the phosphorylation levels of the large tumor suppressor 1/2 (LATS1/2) and YAP, and the protein expression of the transcriptional coactivator of PDZ-binding motif (TAZ) were also detected. RESULTS Compared with the RHF group, the rats in Wenyang lishui formula low-dose and high-dose groups showed improved hair color, movement, diet, and mental state. The atrophy of right ventricular myocardial cells, the increase of inflammatory cells, collagen deposition, and hypertrophy of myocardial fibers were significantly alleviated. The right ventricular internal diameter, right atrial internal diameter, end-diastolic volume, pulmonary artery pressure, pulmonary artery pressure/velocity, the plasma levels of brain natriuretic peptide and AngⅡ , collagen volume fraction, the phosphorylation level of YAP and protein expression of TAZ were significantly decreased, while the PAAT, PAAT/ET and the phosphorylation level of LATS1/2 were significantly increased (P<0.05). PY-60 could significantly reverse the improvement effects of high-dose Wenyang lishui formula on the above quantitative indicators (P< 0.05). CONCLUSIONS Wenyang lishui formula can restore the right heart function of pulmonary hypertension-induced RHF rats, reduce their pulmonary artery pressure, alleviate the pathological changes in their cardiac tissues, and the above effects may be related to the activation of Hippo expression and the inhibition of YAP phosphorylation.
4.Shear wave elastography for evaluating hepatic iron overload in children with β-thalassemia major:Correlations with MR T2* value and serum ferritin

Objective To observe the value of shear wave elastography(SWE)for evaluating hepatic iron overload in children with β-thalassemia major(β-TM),as well as the correlations of relative parameters with MR T2*value and serum ferritin.Methods Totally 96 children with β-TM and 100 healthy children(control group)were retrospectively enrolled.Children with β-TM were divided into hematopoietic stem cell transplantation(HSCT)group(n=41)or non-HSCT group(n=55)according to underwent HSCT or not.SWE parameters were compared among groups.Spearman correlation was performed to observe the correlations of liver shear wave velocity with MR T2*value and serum ferritin,as well as Young's modulus with MR T2*value and serum ferritin in children with β-TM.Results Liver shear wave velocity(LSWV)and Young's modulus in HSCT group and non-HSCT group were all higher than those in control group(all P<0.001).No significant difference of LSWV nor Young's modulus was found between HSCT group and non-HSCT group(both P>0.05).SWE parameters of children with β-TM were moderately and negatively correlated with MR T2*value(r=-0.501,P<0.05;r=-0.514,P<0.05),while weakly and positively correlated with serum ferritin(r=0.488,P<0.05;r=0.470,P<0.05).Conclusion SWE was helpful for evaluating hepatic iron overload in children with β-TM,with parameters being negatively correlated with MR T2*value and positively correlated with serum ferritin.
5.Study on the Effect of Moslae Herba Oil on the Percutaneous Absorption of Active Ingredient of Evodia Rutaecarpa in vitro

OBJECTIVE To investigate whether Moslae herba oil has penetration enhancing effect on the active components of Evodia rutaecarpa and its ability to promote transdermal absorption.METHODS In vitro transdermal experiments were performed u-sing the diffusion pool and the isolated skin of rats.Using various components of Evodia rutaecarpa as indicators,HPLC was deter-mined to calculate the umulative penetration amount of each component.Changes in the skin and cuticle layers were observed by HE staining,immunohistochemical staining,and ATR-FTIR.GC-MS was used to measure Moslae herba oil components after administration.RESULTS Moslae herba oil showed the penetration-enhancing effect on various components in Evodia rutaecarpa.Compared with the postive drug nitrogenone,the cumulative penetration amounts of hyperoside,evodiamine and rutae-carpine were greater.In addition,the sesquiterpene composition and thymol in the Moslae herba oil were more likely to be stored in the skin,which could disturb the lipid structure of the cuticle layer and promote the penetration of drugs.CONCLUSION The results provide reference for the selection of transdermal absorption agent.
6.Latent profile analysis of clinical nurses'core competency in palliative care

Objective To investigate the current status of clinical nurses'core competency in palliative care,analyze the potential characteristics of different types of nurses in palliative care,and provide references for formulating intervention plans for nurses'core competence in palliative care.Methods The oncology nurses'palliative care core competence questionnaire was used in a hospital in Xiamen in December 2022 to conduct a questionnaire survey on 205 clinical nurses.Data analysis was performed using SPSS 26.0 and Mplus 8.3.Results The average score of core competence in palliative care for clinical nurses was(125.72±31.93),and two potential categories were identified:The low core competence group and the high core competence group,accounting for 30.7%and 69.3%respectively.The results indicated that whether to participate in palliative care training and whether to discuss death with patients were the influencing factors of the potential profile category of nurses'core competence in palliative care(P<0.05).Conclusion The core competence of clinical nurses in palliative care is at a medium level,and there are two potential profile categories:The low core competence group and the high core competence group.Nursing managers should adopt precise measures to enhance the core competence of palliative care for clinical nurses based on different potential categories of influencing factors.
7.Neutrophil to lymphocyte ratio at admission predicts hemorrhagic transformation after intravenous thrombolysis in patients with acute ischemic stroke

Objective:To investigate the risk factors for hemorrhagic transformation (HT) after intravenous thrombolysis (IVT) in patients with acute ischemic stroke (AIS), and the predictive value of Neutrophil to lymphocyte ratio (NLR).Methods:Consecutive patients with AIS received IVT in Zhengzhou People’s Hospital from January 2021 to December 2022 were retrospectively enrolled. HT was defined as no intracranial hemorrhage was found on the first imaging examination after admission, and new intracranial hemorrhage was found on the imaging examination 24 h after IVT or when symptoms worsened. sHT was defined as HT and the National Institutes of Health Stroke Scale (NIHSS) score increased by ≥4 compared to admission or required surgical treatment such as intubation and decompressive craniectomy. The baseline clinical and laboratory data of the patients were collected, and NLR, lymphocyte to monocyte ratio (LMR), and platelet to neutrophil ratio (PNR) were calculated. Multivariate logistic regression analysis was used to identify the independent predictors of HT and sHT, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of NLR for HT and sHT after IVT. Results:A total of 196 patients were included (age 65.37±13.10 years, 124 males [63.3%]). The median baseline NIHSS score was 4 (interquartile range: 2-10). Twenty patients (10.2%) developed HT, and 12 (6.1%) developed sHT. Univariate analysis showed that there were statistically significant differences in age, baseline NIHSS score, creatinine, NLR, and stroke etiology type between the HT group and the non-HT group (all P<0.05); there were statistically significant differences in age, NLR, PNR, creatinine, baseline NIHSS score, and stroke etiological type between the sHT group and the non-sHT group (all P<0.05). Multivariate logistic regression analysis showed that NLR was an independent predictor of HT (odds ratio [ OR] 1.375, 95% confidence interval [ CI] 1.132-1.670; P=0.001) and sHT ( OR 1.647, 95% CI 1.177-2.304; P=0.004) after IVT. The ROC curve analysis showed that the area under the curve for predicting HT by NLR was 0.683 (95% CI 0.533-0.833; P=0.007), the optimal cutoff value was 5.78, the sensitivity and specificity were 55.0% and 84.1%, respectively. The area under the curve for predicting sHT by NLR was 0.784 (95% CI 0.720-0.839; P=0.001), the optimal cutoff value was 5.94, the sensitivity and specificity were 66.67% and 84.24%, respectively. Conclusions:A higher baseline NLR is associated with an increased risk of HT and sHT after IVT in patients with AIS, and can serve as a biomarker for predicting HT and sHT after IVT.
8.Application status and safety of Levofloxacin in pediatric clinical practice

Objective:To investigate the clinical application status and safety of systemic administration of Levofloxacin.Methods:Hospitalized children treated with Levofloxacin from January 1, 2017 to December 31, 2022 in Beijing Children′s Hospital, Capital Medical University and Baoding Hospital of Beijing Children′s Hospital were retrospectively recruited.Clinical data like the gender, age, weight, discharge diagnosis, drug name, dosage, route of administration, course of treatment, pathogenic microorganisms, and adverse events were collected.Results:A total of 220 children were included.The age of Levofloxacin use ranged from 1 month and 14 days to 16 years, 7 months and 27 days.The use of Levofloxacin in children was mainly distributed in school age (70 cases), preschool age (65 cases), and early childhood (44 cases), accounting for 81.37% of all children.Children treated with Levofloxacin were admitted in a total of 13 departments, mainly included the respiratory department (77 cases), hematology department (49 cases), infectious medicine department (47 cases), pediatric intensive care unit (14 cases), and stem cell transplantation department (9 cases), accounting for 89.08% of all children.The use of Levofloxacin was mainly applied to children with pneumonia (150 cases), systemic disseminated tuberculosis (25 cases), sepsis (20 cases) and blood disease complicated with infection (16 cases). The common pathogens were Mycoplasma pneumoniae (102 cases), Klebsiella pneumoniae (14 cases), Stenotrophomonas maltophilia (14 cases), Acinetobacter baumannii complex (10 cases), Staphylococcus aureus(9 cases), and Pseudomonas aeruginosa (9 cases). The use and dosage of Levofloxacin were concerned, including insufficient single dose(25 cases), overdose(9 cases), incorrect dosing frequency (9 cases). There were 3 cases of adverse drug reaction of Levofloxacin, including 2 cases of gastrointestinal reactions (1 case of diarrhea, a single dose of 15 mg/kg, clinically cured; 1 case of nausea, clinically improved), and 1 case of rash (clinically improved). Serious adverse drug reactions were not reported. Conclusions:Levofloxacin is relatively safe for short- and medium-term use in children.It should be administered according to the correct usage and dosage.During the administration period of Levofloxacin, the gastrointestinal system, rash, liver and kidney functions, blood system, blood sugar, heart system, nervous system and other indicators should be closely concerned.
9.Anti-PD-L1 antibody enhances curative effect of cryoablation via antibody-dependent cell-mediated cytotoxicity mediating PD-L1highCD11b+ cells elimination in hepatocellular carcinoma.

Cryoablation (CRA) and microwave ablation (MWA) are two main local treatments for hepatocellular carcinoma (HCC). However, which one is more curative and suitable for combining with immunotherapy is still controversial. Herein, CRA induced higher tumoral PD-L1 expression and more T cells infiltration, but less PD-L1highCD11b+ myeloid cells infiltration than MWA in HCC. Furthermore, CRA had better curative effect than MWA for anti-PD-L1 combination therapy in mouse models. Mechanistically, anti-PD-L1 antibody facilitated infiltration of CD8+ T cells by enhancing the secretion of CXCL9 from cDC1 cells after CRA therapy. On the other hand, anti-PD-L1 antibody promoted the infiltration of NK cells to eliminate PD-L1highCD11b+ myeloid cells by antibody-dependent cell-mediated cytotoxicity (ADCC) effect after CRA therapy. Both aspects relieved the immunosuppressive microenvironment after CRA therapy. Notably, the wild-type PD-L1 Avelumab (Bavencio), compared to the mutant PD-L1 atezolizumab (Tecentriq), was better at inducing the ADCC effect to target PD-L1highCD11b+ myeloid cells. Collectively, our study uncovered the novel insights that CRA showed superior curative effect than MWA in combining with anti-PD-L1 antibody by strengthening CTL/NK cell immune responses, which provided a strong rationale for combining CRA and PD-L1 blockade in the clinical treatment for HCC.
10.Poisoning caused by Coriaria sinica maxim among children in Guizhou Province during 2015 to 2020

Objective:
To analyze the occurrence and epidemiological characteristics of acute poisoning caused by Coriaria sinica maxdim in Guizhou Province during 2015 to 2020, and to provide scientific basis for prevention of Coriaria sinica maxim poisoning.
Methods:
A total of 176 cases of Coriaria sinica maxim poisoning were reported in Guizhou Province during 2015 to 2020, with all of the 505 affected were children and adolescents under 16 years old. No death was reported. Poisoning caused by Coriaria sinica maxim was most commonly reported in the year of 2020, with 97 reported incidents and 292 poisonings. Poisoning incidents were most freqently reported in the mature stage of Coriariasinica fruit during April to June, with 153 cases and 437 poisoning cases reported in May. The top three areas reporting Coriaria sinica maxim poisoning included Bijie, Anshun and Zunyi City, with the number of reported eventws being 57, 27, and 27, and poisoned children and adolescents of 160, 90, and 73. Most of the affected children were from rural areas. The median incubation period was 2 hours, and the primary clinical symptoms included vomiting( 93.66 %), nausea(58.02%) and abdominal pain(38.42%).
Conclusion
A large number of Coriaria sinica maxim poisoning incidents occur due to unsupervised access to Coriaria sinica maxim among rural children. The publicity and education of children, especially for rural left behind children, should be strengthened to reduce the incidence of Coriaria sinica maxim poisoning.
