1.Reliability and validity of the Chinese version of Supervision Evaluation and Supervisory Competence Scale

BackgroundClinical supervision is a critical component in the training and professional development of psychological counselors. Scientifically evaluating the effectiveness of clinical supervision is essential, yet reliable and effective tools for such assessments are lacing in China. ObjectiveTo translate Supervision Evaluation and Supervision Competence (SE-SC) Scale into Chinese version and evaluate its reliability and validity in clinical supervision in China, so as to provide a tool for the evaluation of supervisory effectiveness. MethodsThe SE-SC scale was translated, back-translated and culturally adapted, followed by a pilot survey to develop the Chinese version of SE-SC scale. A total of 42 counselors engaged in clinical counseling and receiving supervision at a counseling center in Shanghai from July 2021 to February 2022 were selected as the study participants. Item analysis was conducted to assess item discrimination, with critical ratio method applied to determine which items retention. Hierarchical cluster analysis was performed to compare the structure of Chinese version with the original scale. Criterion-related validity and convergent validity were used to evaluate the validity of the scale, while Cronbach's α coefficient was used to assess its reliability. ResultsChinese version of SE-SC scale consisted of a total of 28 item, including six clusters. Registered supervisors scored significantly higher than internship supervisors on the total score and on clusters three, four, five and six (t=2.536, 2.747, 5.881, 3.718, 6.090, P<0.05). The total and cluster scores of the Chinese version of the SE-SC scale were positively correlated with self-rated supervision helpfulness and overall satisfaction (r=0.492~0.758, 0.412~0.815, P<0.01). The Cronbach's α coefficient for the overall scale was 0.975,with values for the six clusters were 0.938, 0.821, 0.962, 0.871, 0.884 and 0.823, respectively. ConclusionChinese version of SE-SC scale demonstrates good reliability and validity, and it can be considered as a promising assessment tool for evaluating the effectiveness of clinical supervision.
2.Current situation and related factors of AIDS discrimination among junior medical students in Jiangxi Province

Objective:
To explore the current situation and related factors of AIDS discrimination among junior medical students in Jiangxi Province, so as to provide a reference for effective AIDS anti discrimination intervention measures in medical colleges.
Methods:
Using a convenience sampling approach, 2 484 medical students were selected from five universities in Jiangxi Province from July to August 2023. An anonymous survey was conducted using a general information questionnaire, a AIDS knowledge questionnaire, and the Chinese version of Zelaya s AIDS Stigma Scale. Independent sample t-tests and analysis of variance were carried out to analyze the level of AIDS discrimination among medical students with different characteristics. Multiple stepwise regression analysis was performed to identify the related factors of AIDS discrimination.
Results:
The total score of AIDS discrimination among medical students was (2.55±0.67). The dimension with the highest score was fear of contracting the disease (2.89±1.01). The results of the multiple stepwise regression analysis showed that the factors related to AIDS discrimination included gender ( β = -0.17 ), grade ( β =-0.08), being an only child or not ( β =-0.04), whether knowing about AIDS knowledge or not ( β =0.22), willingness to use condoms during sexual activity ( β =0.07), willingness to participate in school sexual health knowledge based activities ( β =0.05) and the perceived importance of selfhealth ( β =0.11) ( P <0.05).
Conclusions
AIDS discrimination is prevalent among junior medical students in Jiangxi Province. Efforts should be undertaken to enhance humanistic education and relevant knowledge dissemination among junior medical students to reduce the level of AIDS discrimination.

Objective:To compare the clinical outcomes between percutaneous Kirschner wire leverage plus plaster fixation and elastic intramedullary nailing in the treatment of radial neck fractures in children.Methods:A retrospective study was conducted to analyze the 60 children with radial neck fracture who had been treated by percutaneous Kirschner wire leverage plus plaster fixation at Department of Pediatric Orthopedics, Children's Hospital of Soochow University from January 2016 to July 2023 (set as an internal fixation-free group). They were 30 males and 30 females (34 left and 26 right sides) with an age of (7.7±3.0) years. At the same time, another cohort of 60 patients were chosen as an intramedullary nailing group who had been treated by percutaneous Kirschner wire leverage plus elastic intramedullary nailing and matched in age and gender with those in the internal fixation-free group. The preoperative fracture angulation, operative time, hospitalization time, fracture angulation on the first postoperative day, fracture angulation at 1 month postoperatively, rate of angulation loss after reduction, Mayo elbow performance score (MEPS) at the last follow-up and complications were compared between the 2 groups.Results:There was no significant difference between the 2 groups in their preoperative general data, showing comparability ( P<0.05). The 120 pediatric patients were followed up for (7.5±3.2) months. The operative time [(27.4±15.0) min] and hospitalization time [(3.4±1.9) d] in the internal fixation-free group were significantly shorter than those in the intramedullary nailing group [(45.4±13.5) min and (4.4±1.3) d] ( P<0.05). The preoperative fracture angulation (50.8°±1.9°), fracture angulation on the first postoperative day (11.3°±1.2°), fracture angulation at 1 month postoperatively (12.1°±1.3°), rate of angulation loss after reduction (2.9%±0.5%), and MEPS at the last follow-up [(90.4±2.0) points] in the internal fixation-free group showed no significant differences from those in the intramedullary nailing group [49.5°±1.7°, 11.1°±1.2°, 13.3°±1.5°, 3.9%±1.4%, and (90.2±2.3) points] ( P>0.05). None of the patients in the internal fixation-free group developed pin-tail irritation sign or premature epiphyseal closure after surgery, whereas 3 patients in the intramedullary nailing group developed pin-tail irritation sign and 2 ones premature epiphyseal closure after surgery, showing a significant difference in the complication rate between the 2 groups [0 (0/60) versus 8.3% (5/60)] ( P<0.05). Conclusions:Percutaneous Kirschner wire leverage plus plaster fixation and close elastic intramedullary nailing can both achieve satisfactory outcomes in the treatment of radial neck fractures in children. However, percutaneous Kirschner wire leverage plus plaster fixation needs shorter operative time and hospitalization time, leads to fewer complications, and requires no reoperation to remove internal fixation.
5.Discovery of proqodine A derivatives with antitumor activity targeting NAD(P)H: quinone oxidoreductase 1 and nicotinamide phosphoribosyltransferase.

NAD(P)H: quinone oxidoreductase 1 (NQO1) is a flavin protease highly expressed in various cancer cells. NQO1 catalyzes a futile redox cycle in substrates, leading to substantial reactive oxygen species (ROS) production. This ROS generation results in extensive DNA damage and elevated poly (ADP-ribose) polymerase 1 (PARP1)-mediated consumption of nicotinamide adenine dinucleotide (NAD+), ultimately causing cell death. Nicotinamide phosphoribosyltransferase (NAMPT), the rate-limiting enzyme in the NAD+ salvage synthesis pathway, emerges as a critical target in cancer therapy. The concurrent inhibition of NQO1 and NAMPT triggers hyperactivation of PARP1 and intensive NAD+ depletion. In this study, we designed, synthesized, and assessed a novel series of proqodine A derivatives targeting both NQO1 and NAMPT. Among these, compound T8 demonstrated potent antitumor properties. Specifically, T8 selectively inhibited the proliferation of MCF-7 cells and induced apoptosis through mechanisms dependent on both NQO1 and NAMPT. This discovery offers a promising new molecular entity for advancing anticancer research.

7.Syndrome Differentiation and Treatment of Children Bronchiolitis Obliterans by Stages from the Perspective of Phlegm,Stasis,Block and Deficiency

It is believed that children bronchiolitis obliterans (BO) is located at the lung and closely related to the spleen and kidney. Phlegm, stasis, block and deficiency are the main pathogenesis. This article promotes staged differentiation and treatment of BO considering the clinical manifestations and pathogenesis characteristics. The attack stage is dominated by phlegm and stasis blocking the lung, for which the method of dissolving phlegm and dispelling stasis, relieving cough and calming panting should be used; Xiaoqinglong Decoction(小青龙汤) and Sanzi Yangqin Decoction (三子养亲汤) with modifications and selfmade Modified Wuhu Decoction (五虎汤加味) are recommended for cold-phlegm blocking the lung syndrome and phlegm heat blocking the lung syndrome, respectively. In sustained stage, the upper excess and lower deficiency together with phlegm-stasis blocking the lung are the pathogenesis, for which the method of dissolving phlegm and dispelling stasis, reopening the block and supplementing deficiency is suggested, and Xiaoqinglong Decotion (小青龙汤) and Duqi pills (都气丸) with modifications can be used. In convalescent stage, the pathogenesis is lung-spleen-kidney depletion with residual pathogen. It suggested to supplement the deficiency and consolidate the root, as well as clear the residual pathogen, for which Baogen NO.1 Decoction (宝根1号方) with modifications can be used.
8.Protective effects and mechanisms of dexmedetomidine on kidney donors after warm/cold ischemia in rats

Objective To investigate the protective effects and mechanisms of dexmedetomidine(Dex)on kidney donors after warm ischemia and cold ischemia in rats.Methods A total of twenty-one male SD rats were randomly allocated into the control group,the model group(0 μmol/L Dex group)and the Dex group(1 μmol/L Dex group),with 7 rats in each group.All the rats were killed by bloodletting,the kidneys were extracted after 30 minutes of warm ischemia,and the kidneys of the control group were directly fixed or preserved at-80 ℃.The kidney donors of the model group and the Dex group were respectively refrigerated in the university of Wisconsin organ preserva-tion solution containing 0 μmol/L Dex and 1 μmol/L Dex at 4 ℃ for 24 hours,to simulate the cold ischemia state of the kidney donors.HE staining was performed to observe the morphological changes and the tissue injury score was conducted.The activity of N-acetyl-β-D-glucosaminase(NAG)was determined by colorimetry,the apoptosis level was detected by TUNEL,the expression level of receptor-interacting protein kinase 3(RIPK3)was detected by Western blot,and the expression level of phosphorylation mixed lineage kinase domain like protein(pMLKL)was determined by immunofluorescence method.Results In the control group,there were no obvious abnormalities in glomerular or renal tubules.In the model group,some renal tubules were dilated and tubular epithelial cells were flattened.Compared with the model group,the damage of renal tubules in the Dex group was alleviated.Compared with the control group,kidney injury score,NAG activity,TUNEL positive cell number,pMLKL and RIPK3 expression levels were increased in the model group and the Dex group(P<0.05).Compared with the model group,kidney injury score,NAG activity,TUNEL positive cell number,and pMLKL expression level were decreased in the Dex group(P<0.05).Conclusion Dex has protective effects on kidney donors after warm ischemia and cold ischemia,and its mechanism is related to reducing apoptosis and inhibiting necroptosis.
9.The Mechanism of Zhuangxuan Yin in the Treatment of Children with H1N1 Pneumonia Through Regulating Gut Microbiota Mediated by p38 MAPK Signaling Pathway

Objective To explore the mechanism of Zhuangxuan Yin in the treatment of children with H1N1 pneumonia through regulating gut microbiota mediated by p38 mitogen-activated protein kinase(p38-MAPK)pathway.Methods A BALB/c mouse model of H1N1 pneumonia was prepared using the H1N1 influenza virus allantoic solution for nasal drop.The model was randomly separated into 5 groups:model group,Zhuangxuan Yin low-,medium-and high-dose groups,and high-dose Zhuangxuan Yin(28.66 g·kg-1)+anisomycin(10 mg·kg-1)group.The control group was infused with sterile physiological saline of equal volume using the same method.After treatment with Zhuangxuan Yin and anisomycin,the lung index of mice in each group was measured,and HE staining was applied to detect the pathological morphology of lung and large intestine tissues.16SrRNA gene sequencing was applied to detect the structural difference of gut microbiota in mice of each group.Enzyme-linked immunosorbent assay(ELASA)was applied to measure the levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-4,IL-6,and IL-1β in bronchoalveolar lavage fluid(BALF)and large intestine tissue of mice in each group.Western Blot was applied to detect the expression of p38-MAPK pathway-related proteins in lung and large intestine tissues of mice in each group.Results Compared with the control group,the lung and large intestine tissues of the model group mice showed obvious pathological damage,the lung index,pathological score of lung and large intestine,ACE index,Shannon index,abundance of class Clostridia,the levels of TNF-α,IL-4,IL-6,and IL-1β in BALF and large intestine tissues,and p-p38-MAPK/p38-MAPK in lung and large intestine tissues increased(P<0.05).The abundance of class Bacteroidales decreased(P<0.05).Compared with the model group,the pathological damage in the lung and large intestine tissues of mice in the Zhuangxuan Yin low-,medium-and high-dose groups were reduced.The lung index,pathological score of lung and large intestine,ACE index,Shannon index,abundance of class Clostridia,the levels of TNF-α,IL-4,IL-6,and IL-1β in BALF and large intestine tissues,and p-p38-MAPK/p38-MAPK in lung and large intestine tissues decreased(P<0.05),the abundance of class Bacteroidales increased(P<0.05),and in a dose-dependent manner(P<0.05).Compared with the high-dose Zhuangxuan Yin group,the pathological damage in the lung and large intestine tissues of mice in the high-dose Zhuangxuan Yin+anisomycin group was aggravated,the lung index,pathological score of lung and large intestine,ACE index,Shannon index,abundance of class Clostridia,the levels of TNF-α,IL-4,IL-6,and IL-1β in BALF and large intestine tissues,and p-p38-MAPK/p38-MAPK in lung and large intestine tissues increased(P<0.05),and the abundance of class Bacteroidales decreased(P<0.05).Conclusion Zhuangxuan Yin can improve the imbalance of intestinal microbiota in H1N1 pneumonia mice by inhibiting p38-MAPK signal activation,thereby inhibiting inflammation and reducing lung and large intestine tissue damage in mice,which may have a therapeutic effect on children with H1N1 pneumonia.
10.Modified Wuhutang Alleviates Airway Inflammation and Down-regulates Expression of Muc5AC, STAT3, NF-κB, and NLRP3 in Lung Tissues of RSV-infected Asthmatic Mice

ObjectiveTo explore effect of modified Wuhutang on airway inflammation and expression of mucin (Muc) 5AC, signal transducer and activator of transcription 3 (STAT3), nuclear factor kappa B (NF-κB), and NOD-like receptor thermal protein domain associated protein 3 (NLRP3) in respiratory syncytial virus (RSV)-infected asthmatic mice. MethodSeventy male BALB/c mice of 6-8 weeks old were randomized into normal control (CON), asthma (ovalbumin, OVA), RSV infection-induced asthma (OVA+RSV), high-, medium-, and low-dose (4.08, 2.04, 1.02 g·kg-1·d-1, respectively) modified Wuhutang, and dexamethasone (Dxms, 0.1 g·kg-1d-1) groups (n=10). The model of asthma was established by sensitization and atomization inhalation with OVA. The RSV infection-induced asthma model was established by three consecutive RSV nasal infusions (1.0 × 106 PFU·mL-1, 50 μL). Wuhutang was administrated by gavage, and Dxms by intraperitoneal injection. The CON group was given the same amount of normal saline by gavage. The mice were anesthetized with 2.5% pentobarbital sodium 24 h after the last administration, and then the lung tissue was stained by hematoxylin-eosin (HE) and Van Gieson (VG) for observation of airway inflammation. The immunohistochemical assay was employed to detect the expression of Muc5AC. Western blot was employed to determine the protein levels of phosphorylated (p)-STAT3, STAT3, p-NF-κB, NF-κB, and NLRP3. ResultCompared with the CON group, the OVA group presented airway inflammatory cell infiltration, tissue hyperemia and edema, and collagen fiber deposition. The OVA+RSV group showed severer airway inflammatory cell infiltration and tissue hyperemia and edema than the OVA group. Compared with the OVA+RSV group, modified Wuhutang alleviated the airway inflammatory cell infiltration, tissue hyperemia and edema, and collagen fiber deposition, and the high-dose group had the best performance. Compared with the CON group, the OVA group and the OVA+RSV group showed increased expression level of Muc5AC (P<0.01). Compared with the OVA+RSV group, modified Wuhutang reduced the expression level of Muc5AC, and the reduction was significant in the high-dose group (P<0.05). Compared with the high-dose modified Wuhutang group, Dxms lowered the expression level of Muc5AC (P<0.05). Compared with the CON group, the OVA and OVA+RSV groups showed up-regulated protein levels of p-STAT3, p-NF-κB, and NLRP3 (P<0.05, P<0.01). Compared with the OVA+RSV group, modified Wuhutang down-regulated the protein levels of p-STAT3, p-NF-κB, and NLRP3 (P<0.01). Compared with the high-dose modified Wuhutang group, the Dxms group showed up-regulated levels of p-STAT3, p-NF-κB proteins (P<0.01). ConclusionModified Wuhutang can reduce airway inflammation and down-regulate the expression of Muc5AC, p-STAT3, p-NF-κB, and NLRP3 in RSV-infected asthmatic mice, which suggests that Wuhutang reduces airway inflammation in RSV-infected asthma by regulating the STAT3/NF-κB signaling pathway.
