		                        			Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
		                        		
		                        		
		                        		
		                        	
2.Correlation Between Ceftazidime-avibactam Blood Concentration and Efficacy in Critically Ill Patients and Influential Factors

		                        			Objective To evaluate the relationship between ceftazidime-avibactam(CAZ-AVI)blood concentrations and efficacy in critically ill patients and to investigate the factors influencing blood levels.Methods The CAZ-AVI trough concentrations(Cmin)were detected in 29 patients who received CAZ-AVI treatment for at least 48 hours.The clinical materials of the patients were collected together for retrospective analysis.Results The Cmin of ceftazidime(CAZ)and avibactam(AVI)were(50.95±5.17)and(7.52±0.96)mg·L-1 in the effective group and(31.16±7.03)and(5.37±1.32)mg·L-1 in the ineffective group,respectively.The Cmin of CAZ in the effective group was significantly higher than in the ineffective group(P<0.05),and there was no significant difference in AVI Cmin between the two groups(P>0.05).Spearman's correlation analysis showed that CAZ Cmin was positively correlated with clinical efficacy(P<0.05),and no correlation between AVI Cmin and clinical efficacy(P>0.05).The optimal CAZ Cmin threshold was 24.59 mg·L-1.Multiple linear regression analysis showed that age and creatinine clearance was significantly correlated with the Cmin of CAZ,and creatinine clearance was significantly correlated with AVI Cmin(P<0.05).Conclusions The Cmin of CAZ correlates with efficacy,and it may be more beneficial for clinical treatment to keep the concentration of CAZ-AVI always greater than the minimum inhibitory concentration during the dosing interval.The creatinine clearance should be fully considered when optimizing CAZ-AVI dosage in critically ill patients.
		                        		
		                        		
		                        		
		                        	
3.Effect of mirror therapy on upper limb function and cerebral cortex activity in patients with type Ⅰ complex re-gional pain syndrome after stroke:a randomized controlled trial

		                        			Objective To investigate the effect of mirror therapy on upper limb function and cortical activity in patients with type I complex regional pain syndrome(CRPS)after stroke. Methods A total of 72 post-stroke patients with type I CRPS were recruited at the Third Rehabilitation Hospital Affiliat-ed to Shanghai University of Traditional Chinese Medicine from October,2017 to February,2022.They were ran-domly divided into control group(n=36)and mirror therapy group(n=36).The control group received conven-tional rehabilitation training,while the mirror therapy group received mirror therapy in addition.Before treat-ment,as well as at three and six weeks after treatment,they were evaluated using the Visual Analog Scale(VAS)for pain,modified Barthel Index(MBI),edema volume and Brunnstrom stage.Resting-state data were collected for 440 seconds using a 32-channel functional near-infrared spectroscopy(fNIRS)system. Results After treatment,VAS scores significantly improved in each group,showing better after six weeks than after three weeks(P<0.01).The mirror therapy group was better than the control group after six weeks(P<0.05).MBI scores also significantly improved in each group,showing better after six weeks than after three weeks(P<0.001).Edema volume significantly decreased in each group(Z>30.113,P<0.001),while the mirror therapy group was better than the control group after six weeks(Z=-3.347,P=0.001).Edema volume in the mirror therapy group significantly reduced at both three and six weeks(Z<-0.667,P<0.01),with a stronger effect ob-served after six weeks(Z=-0.667,P=0.005).Brunnstrom stages improved significantly in each group(Z>29.714,P<0.001),while the mirror therapy group was better than the control group after six weeks(Z=-2.046,P=0.041).After treatment,the control group showed strong connectivity between right M1 and right primary so-matosensory cortex,while the mirror therapy group demonstrated stronger connectivity between left M1 and right M1,right primary somatosensory cortex,right pre-motor and supplementary motor cortex.Connectivity be-tween left and right primary somatosensory cortex increased in mirror therapy group,as well as the connectivity between left pre-motor-supplementary motor cortex and right M1,right pre-motor-supplementary motor cortex and left primary somatosensory cortex,left M1 and left primary somatosensory cortex,and left primary somato-sensory cortex and right M1(∣t∣>3.402,P<0.01). Conclusion Mirror therapy may relieve pain and edema,and improve upper limb motor function in post-stroke patients with type I CRPS,which may associate with stonger connectivity between sensory regions on the unaffected side and sensory-motor regions on the affected side,promoting sensorimotor cortical reorganization.
		                        		
		                        		
		                        		
		                        	
4.Diagnostic and therapeutic value of endoscopy and mucosal histology for digestive tract diseases in infants

		                        			Objectives:To investigate the clinical value of endoscopy and mucosal histology for digestive tract diseases in infants.Methods:Clinical data of 357 infants who underwent 422 gastrointestinal endoscopies from January 2010 to December 2021 were collected. The indications, endoscopic manifestations, histological features of mucosa and diagnosis were analyzed.Results:A total of 159 gastroscopies and 263 colonoscopies were performed. Diarrhea (185 cases), bloody stool (178 cases) and vomiting (46 cases) were common symptoms. Endoscopy showed manifestations including non-specific inflammatory changes (265 cases), ulcer-like changes (72 cases), and normal mucusa (48 cases). A total of 373 biopsies were performed, including 260 cases of abnormal mucosal histology, 109 cases of normal mucosal histology, and 4 cases of too small biopsy specimens for analysis. Diagnoses were 208 cases of anaphylactic disease, 45 cases of inflammatory bowel disease, 15 cases of variation of structure, 10 cases of intestinal lymphangiectasis, 2 cases of autoimmune enteropathy, and 1 case of celiac disease. There was only 1 colonic perforation complicating endoscopy in terms of endoscopic complication.Conclusions:It is safe and effective to perform endoscopy standardly for digestive tract diseases in infants. 		                        			The vagina contains at least a billion microbial cells,dominated by lactobacilli.Here we perform metagenomic shotgun sequencing on cervical and fecal samples from a cohort of 516 Chinese women of reproductive age,as well as cervical,fecal,and salivary samples from a second cohort of 632 women.Factors such as pregnancy history,delivery history,cesarean section,and breastfeeding were all more important than menstrual cycle in shaping the microbiome,and such information would be necessary before trying to interpret differences between vagino-cervical micro-biome data.Greater proportion of Bifidobacterium breve was seen with older age at sexual debut.The relative abundance of lactobacilli especially Lactobacillus crispatus was negatively associated with pregnancy history.Potential markers for lack of menstrual regularity,heavy flow,dysmenor-rhea,and contraceptives were also identified.Lactobacilli were rare during breastfeeding or post-menopause.Other features such as mood fluctuations and facial speckles could potentially be predicted from the vagino-cervical microbiome.Gut and salivary microbiomes,plasma vitamins,metals,amino acids,and hormones showed associations with the vagino-cervical microbiome.Our results offer an unprecedented glimpse into the microbiota of the female reproductive tract and call for international collaborations to better understand its long-term health impact other than in the settings of infection or pre-term birth.
		                        		
		                        		
		                        		
		                        	
7.Clinical application of washed red blood cells in perioperative patients with liver cancer

		                        			【Objective】 To estimate the application value of washed red blood cells (RBC) in perioperative patients with liver cancer. 【Methods】 86 perioperative patients with liver cancer who met the inclusion/exclusion criteria were divided into observation group (n=42) and control group (n=44). In the observation group, 22 patients were transfused with RBC and 20 with washed RBC. Patients without RBC transfusion worked as the controls. The name of disease, tumor stage, tumor size, Hb before and after blood transfusion, transfusion volume and blood components, adverse reaction to blood transfusion, operation time and blood loss during surgery, systemic infection, tumor recurrence and metastasis, and survival time were recorded. Blood transfusion efficacy, survival time, adverse reaction to blood transfusion, tumor recurrence and metastasis among these groups were compared. 【Results】 Among the non-transfusion group, washed RBC group and RBC group, the Hb(g/L)were 93.9±16.5 vs 80.4±24.5 vs 74.7±26.1, operative time (h) 2.8±0.7 vs 4.3±1.6 vs 3.9±2.0, operative blood loss(mL) 291.0±0.3 vs 388.0±165.8 vs 466.3±198.4 respectively before blood transfusion (all P<0.05). There were no significant differences in the efficacy of blood transfusion and survival time among the three groups (P>0.05). There were significant differences in tumor metastasis (50% vs 43%) and recurrence (50% vs 43.1%) between blood transfusion group and non-blood transfusion group (P<0.05). There was no difference in tumor metastasis (50% vs 48%) and recurrence (50% vs 49%) between the washed RBC group and RBC group (P>0.05). The nosocomial infection rate in washed RBC group (36%) was significantly lower that that in RBC group (88.6%) and non-transfusion group (50%) (P<0.05). 【Conclusion】 Blood transfusion caused by hypoxia may increase tumor metastasis and recurrence in perioperative patients with liver cancer. Transfusion of washed RBC can achieve the curative effect and reduce adverse reactions to blood transfusion, but has no significant impact on the survival time.
		                        		
		                        		
		                        		
		                        	
8.Status quo and influencing factors of quality of life in neurological patients with cognitive disorder

To explore the quality of life in patients with cognitive disorder and to discuss its influencing factors so as to provide a reference for improving the quality of life in patients with cognitive disorder. Methods? Totally 1 398 neurological patients from a neuropsychiatric hospital in Nanjing were screened using cluster sampling and 125 patients with cognitive disorder were selected as subjects between January and December 2017. The subjects received a cross-sectional study with Quality of Life in Alzheimer's Disease (QOL-AD), Montreal Cognitive Assessment Scale (MoCA), Activities of Daily Living Scale (ADL), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) and the general information questionnaire. Variance analysis, Pearson correlation analysis and multivariate linear regression analysis were used for statistical analysis. Results? The morbidity of cognitive disorder in neurological patients from our hospital was 9.3%. Their QOL-AD score was (31.88±6.54). Sex, educational background, nature of work, household income, sleep and bad social relations are factors affecting the quality of life of the subjects (P<0.05). According to multivariate linear regression analysis, MoCA, SAS and SDS were main factors affecting the quality of life of the subjects (P<0.05). Conclusions? Male patients with lower education, physical work and bad social relations are high-risk patients with lower quality of life. Cognitive disorder, anxiety and depression are main risk factors affecting the quality of life in patients with cognitive disorder. Nursing workers should improve and maintain the patients'cognitive ability, enrich their cultural life and social activities, and enhance their mental health, thereby improving their quality of life.
		                        		
		                        		
		                        		
		                        	
9.Analysis of DYSF gene mutations in two pedigrees affected with limb-girdle muscular dystrophy type 2B.

OBJECTIVETo analyze mutations of DYSF gene in two pedigrees affected with limb-girdle muscular dystrophy 2B (LGMD-2B).
METHODSGenomic DNA was extracted from peripheral blood samples of the two probands and unaffected family members. Variant sites were screened by next-generation sequencing using gene panel as well as Sanger sequencing.
RESULTSFour pathogenic mutations of the DYSF gene were detected, which included a de novo mutation and three mutations with uncertain significance. In pedigree 1, the proband carried compound heterozygous mutations of c.1667T to C (p.Leu556Pro) and c.5567T to A (p.Val1856Glu), which were respectively inherited from her mother and father. Proband of pedigree 2 carried compound heterozygous mutations of c.4853A to G (p.Tyr1618Cys) and c.4876G to A (p.Val1612Ile), among which c.4876G to A (p.Val1626Ile) was also found in his father and grandfather, while c.4853A to G (p.Tyr1618Cys) was detected in his mother and grandmother.
CONCLUSIONThe two compound heterozygous mutations of the DYSF gene probably underlie the LGMD2B in the two pedigrees. Next generation sequencing has conferred great advantage for gene diagnosis of hereditary myopathy.
10.Analysis of Bacterial Resistance in a Hospital during 2013 and 2015

		                        			Objective: To analyze the distribution and drug resistance characteristics of clinical separation germ in a hospital from 2013 to 2015 to provide reference and basis for the prevention and control of nosocomial infection and rational use of antibiotics.Methods: The microbial susceptibility of isolated strains was detected using the conventional methods, and the drug sensitivity was analyzed by BioMerieux ATB 1.22.The drug sensitivity was determined according to CLSI 2014 criteria.Results: A total of 18 421 specimens were isolated during 2013 and 2015, and a total of 3 744 strains were isolated with the total positive rate of 20.32%.The separation and identification of pathogenic bacteria at the top 5 were Escherichia coli (967 strains, 44.34%), Bauman Acinetobacter (323 strains, 14.81%), Klebsiella pneumoniae (312 strains, 14.31%), Staphylococcus aureus (297 strains 13.62%) and Pseudomonas aeruginosa (282 strains, 12.92%).Besides the natural resistance of Klebsiella pneumoniae to amoxicillin, the resistance rate of Escherichia coli to piperacillin was over 75%, while the sensitivity rate of Klebsiella pneumoniae to piperacillin and tazobactam was more than 90%.The sensitivity of Acinetobacter baumannii and Pseudomonas aeruginosa to clinical antibiotics was basically below 40%, and the overall resistance level was higher than that of Bauman.MRSA was sensitive to nitrofurantoin, minocycline, quinupristin-Dafoe and leptin glycopeptide antibiotics (such as teicoplanin and vancomycin).Conclusion: The hospital should strengthen the monitoring of bacterial resistance and track the results in a timely manner so as to provide reference for the rational drug use in clinical practice.
