1.Exploration of the blocking effect of Fab fragment of IgG autoantibody on erythrocyte autoantigens

【Objective】 To use Fab segments of autoantibodies to bind to autoantigens on red blood cells in order to block the binding of intact autoantibodies to the autoantigens. 【Methods】 The pure autoantibodies were obtained from the serum of patients by absorption and elution test, then Fab segments were obtained by hydrolyzing IgG autoantibodies at 37℃ overnight with papain enzyme. The Fab segments were used to bind the corresponding autoantigens specifically, thus blocking the binding of intact autoantibodies to autoantigens. 【Results】 The pure autoantibodies obtained through absorption and elution test were all hydrolyzed by papain to generate Fab fragments. When mixed with intact autoantibodies in their respective elution solutions, these Fab fragments were able to block the binding of intact autoantibodies to red blood cell autoantigens. Among the three samples, there was a mutual blocking effect between sample 1 and sample 2, while no mutual blocking effect was observed between sample 2 and sample 3. 【Conclusion】 Fab segment of autoantibody can eliminate the agglutination of intact autoantibodies and erythrocyte autoantigens, and can be applied to clinical practice to solve clinical problems.
2.Role of Notch 1 signaling and glycolysis in the pathogenic mechanism of adenomyosis

Objective To investigate the expressions of glycolysis-related factors and changes in Notch1 signaling in endometrial tissues of adenomyosis(AM)and Ishikawa cells to explore the pathogenesis of AM.Methods Eutopic endometrial tissues were collected from 8 patients with AM and 8 patients with uterine fibroids matched for clinical characteristics(control group).The expressions of Notch1 signaling proteins and glycolysis-related factors in the collected tissues were detected using qRT-PCR and Western blotting,and the levels of glucose and lactic acid were determined.An Ishikawa cell model with lentivirus-mediated stable Notch1 overexpression was established for assessing cell survival rate with CCK-8 assay,cell migration and invasion abilities with Transwell migration and invasion assays,and glycolytic capacity by determining the extracellular acidification rate.Results Compared with those in the control group,the endometrial tissues in AM group showed significantly increased expression level of carbohydrate antigen 125(CA125),increased mRNA expression levels of Notch1,HK2 and PDHA and protein expressions of Notch1,GLUT1,HK2,PKM and PDHA,lowered glucose level and increased lactate level.The Ishikawa cell models with stable Notch1 overexpression exhibited significantly increased cell survival rate with attenuated cell migration and invasion abilities and decreased glycolysis capacity and reserve.Conclusion The Notch1 signaling pathway participates in the pathogenesis of AM possibly by regulating the proliferation,migration,invasion and glycolysis of endometrial cells.

4.Metabonomics Combined with Prescription Compatibility Theory to Explore the Lipid-lowering Effect of Qige Decoction Compatibility

Objective To explore the lipid-lowering effect of Qige Decoction before and after compatibility through the combination of pharmacodynamics and liver metabolomics,and to provide new research strategies for exploring the scientific notation of traditional Chinese medicine compatibility.Methods According to the pharmacodynamic strategy,three groups of drug administration were set up as Qige Decoction group,Astragali Radix-Puerariae Radix group,and Pericarpium Citri Tangerinae group.Four indices of blood lipids,serum biochemical indicators,and liver morphology and pathology were used to evaluate the intervention effect of Qige Decoction on hyperlipidemic rats.Liver metabolomics technology was used to analyze the effects of Qige Decoction on metabolites before and after compatibility,and multivariate statistical analysis was used to evaluate the differences between groups in terms of differential metabolites and metabolic pathways.Results Compared with the model group,the callback abilities of four indices of blood lipid in the Qige Decoction group were higher than those in Astragali Radix-Puerariae Radix group and Pericarpium Citri Tangerinae group,among which the total cholesterol(TC)and triglyceride(TG)levels in the Qige Decoction group decreased(P<0.05).A total of 86 potential biomarkers were identified by liver metabolomics,with 23,13,and 7 metabolites being significantly different in the Qige Decoction group,Astragali Radix-Puerariae Radix group,and Pericarpium Citri Tangerinae group,respectively(P<0.05).Metabolic pathway analysis of 29 specific biomarkers with significant callback effects showed that they were related to glycerophospholipid metabolism,linoleic acid metabolism,α-linolenic acid metabolism,sphingolipid metabolism,arachidonic acid metabolism,and unsaturated fatty acid biosynthesis.Qige Decoction mainly regulates glycerophospholipid and linoleic acid metabolism,and uniquely acts on sphingolipid metabolism.Conclusion Qige Decoction has more lipid-lowering targets after compatibility,with better lipid-lowering effects than the Astragali Radix-Puerariae Radix group and Pericarpium Citri Tangerinae group.This study provides experimental evidence and research strategies for further revealing the scientific notation of traditional Chinese medicine compatibility.
5.Mechanism of FOXA2 in colorectal cancer and its application in diagnosis and treatment

Forkhead box protein A2 (FOXA2) , characterized by its unique DNA binding domain, plays a key role in transcriptional regulation. FOXA2 transcription factor is highly expressed in colorectal cancer, and binds with corresponding targeted genes to regulate tumor growth and inflammatory response, thus playing the role of oncogenes. In-depth understanding of the role and function of human FOXA2 transcription factor in colorectal cancer will contribute to further development of FOXA2 as a potential therapeutic target and diagnostic and prognostic marker of colorectal cancer.
6.Present situation and reform of symptomatology and clinical history taking teaching with introducing the clinical manifestation of disease

Symptomatology and medical history taking are the focuses of diagnostics study. With the improvement of teaching and learning, students can basically master the standard process of clinical inquiry of chief complaints to family history efficiently. However, due to the lack of systematic learning of clinical diseases, it's difficult for most students to form a targeted, logical and speculative inquiry thinking pattern, so they are unlikely to write a medical history of present illness that reflects their thinking on disease inclusion and exclusion. In this study, we come up with the characteristics and defects of symptomatology teaching at present, and put forward a new idea of symptomatology teaching by introducing clinical manifestations of diseases and examples of inquiry.
7.Effects of mechanical vibration on micoRNA-214-3p and serum interleukin-1β in ovariectomized rats with a fracture

Objective:To observe the effect of mechanical vibration on the expression of microRNA-214-3p and serum interleukin-1β (IL-1β) in the broken ends of fractured bones in ovariectomized rats.Methods:Thirty 3-month-old female Wistar rats were randomly divided into a control group, a model group, and a vibration group, each of 10. An operation of ovariotomy was performed in those of the model and vibration groups to establish osteoporosis model. Five months later a model of mid femur fracture was made with animals in all the groups. Five days after their fracture, the vibration group received 20 minutes of whole-body vibration treatment at 35Hz for 5 days a week, while the control group and the model group received natural rearing without any additional intervention At 2 and 6 weeks after the operation, the Lane-Sandhu X-ray scoring system was used to evaluate the quality of fracture healing, and reverse-transcription polymerase chain reactions and enzyme-linked immunosorbent assays were used to detect miR-214-3p in the fractured bones and the serum levels of IL-1β.Results:At 2 weeks and 6 weeks after the operation the average growth score of the broken ends in the model group was significantly lower than that of the control group, while that of the vibration group was significantly higher than the model group′s average. Compared with the control group at the same time point, the average miR-214-3p content of the model and vibration groups was significantly higher 2 and 6 weeks after the surgery. Compared with the model group, the average level of miR-214-3p of the fractured ends of the vibration group was significantly lower at 6 weeks. Two and six weeks after the surgery, the average IL-1β of the model group was significantly higher than the control group′s average, and that of the vibration group was significantly lower.Conclusion:Mechanical vibration can promote osteoporotic fracture healing by inhibiting the expression of miR-214-3p and reducing the level of IL-1β at the broken ends of fractured bones.
8. Analysis of the significance of cerebral perfusion imaging combined with CT angiography in the diagnosis and treatment plan of patients with ischemic stroke

Objective:
To investigate the significance of cerebral perfusion imaging combined with CT angiography (CTA) in the diagnosis and treatment plan of patients with ischemic stroke.
Methods:
Total of 44 patients with acute cerebral infarction were enrolled in Taizhou First People′s Hospital from January 2018 to September 2018. Cerebral perfusion imaging and CTA examination were performed simultaneously within 12 h after the onset of the disease. The images of perfusion imaging were processed using an image workstation to measure the level of perfusion parameters in the ischemic region of the patient′s brain. The head and neck of the patient were measured by CTA. The angiography was performed, and the cerebral angiography results of all patients were as the gold standard. The clinical diagnosis and diagnostic efficacy of different examination methods in patients with ischemic stroke was observed and compared, and the differences in the parameters of perfusion imaging between different perfusion abnormal areas and normal control areas were compared.
Results:
When patients′ cerebral angiography and single-photon emission computed tomography (SPECT) was as the gold standard for comparison, cerebral blood flow (CBF) and mean transit time (MTT) had the highest accuracy among all parameters of perfusion imaging. The accuracy rate of perfusion imaging+CTA was 93.2%, with a specificity of 100.0% and a sensitivity of 90.1%, which was significantly higher than that of the clinical efficacy of single diagnosis.
Conclusions
Perfusion imaging combined with CTA can provide important supplementary reference information for clinical diagnosis and treatment of patients with ischemic stroke.
9.Application of whole-body magnetic resonance diffusion weighted imaging in evaluating the chemotherapy response for lung cancer

Objective To study the clinical value of whole -body magnetic resonance diffusion weighted imaging(WB-DWI) in evaluating the chemotherapy response for lung cancer,thus to provide evidence for optimizing clinical imaging examination. Methods From October 2017 to May 2018,60 patients with lung cancer confirmed by histopathology in Linfen Central Hospital were selected. The patients underwent DWI examinations before chemotherapy and after two cycles of chemotherapy. The change of tumor size,distant metastasis and apparent diffusion coefficient (ADC) value were compared before and after chemotherapy. The correlation between the change rate of ADC value and the shrinkage rate of tumor size in the effective group was analyzed. Results Of 60 cases,1 case had new cerebral metastases after chemotherapy. There were statistically significant differences in ADC value [(1. 12 ± 0.33) ×10 -3mm2/svs.(1.56±0.40) ×10 -3mm2/s]andtumorsize[(4.63±2.75)cmvs.(2.28±1.45)cm] between before and after chemotherapy in the effective group(t= -3. 954,4. 711,all P<0. 01). There was correlation between the change of ADC value and tumor size(r=0. 34,P<0. 05). Conclusion WB-DWI can not only detect the change of tumor size and distant metastasis quickly and effectively,but also can observe the microscopic changes of tumor cells by measuring ADC value. So it can predict the early therapeutic response of the tumor and make effective evaluation for the staging and chemotherapy response of lung cancer.
10.Significance of ultrasound tissue characteristics of carotid atherosclerotic plaques in predicting cardiocerebrovascular events in patients with type 2 diabetes

Objective To investigate the predictive value of ultrasonic tissue charateristics of carotid atherosclerotic plaques in cardiocerebrovascular events in patients with type 2 diabetes mellitus (T2DM).Methods From January 2003 to December 2015,patients with T2DM treated in Xi'an Gaoxin Hospital were selected.Their carotid atherosclerotic plaques were examined by ultrasound.Intergrated backscatter (IBS) and thickness of carotid plaques of the patients were documented.The cardiocerebrovascular events were folbwed up prospectively.Kaplan-Meier curve,COX proportional risk regression model,and receiver operating characteristic (ROC) curve were used to compare and analyze the predictive values of calibratedIBS (C-IBS),Framingham Risk Score (FRS) and plaque thickness for the risk of cardiocerebrovascular events in patients with T2DM.Results There was significant difference in the risk of cardiovascular events between the low C-IBS group (n=48,C-IBS <17.1 dB) and the high C-IBS group (n=48,C-IBS ≥17.1 dB) (35.4% vs.12.5%;P=0.015).There was no significant difference in the risk of cardiocerebrovascular events between the thin plaque group (n =48,plaque thickness < 1.3 mm) and the thick plaque group (n=48,plaque thickness >1.3 mm) (14.6% vs.33.3%;P=0.070).COX proportional risk regression model analysis showed that C-IBS (hazard ratio,1.505,95% confidence interval 1.196-1.893,P=0.001) and plaque thickness (hazard ratio,4.794,95% confidence interval 1.142-20.130,P =0.032) were the independent risk factors for the risk of cardiocerebrovascular events.ROC curve analysis found that the combination of C-IBS,FRS and plaque thickness could predict the risk of cardiocerebrovascular events in patients with T2DM.Conclusion Ultrasonic tissue characteristics of carotid atherosclerotic plaque (C-IBS and plaque thickness) in combination with FRS may predict cardiocerebrovascular events in patients with T2DM.
