OBJECTIVE To explore the effect and mechanism of Zhiqiao gancao decoction (ZQGCD) on pathological angiogenesis of degenerative intervertebral disc. METHODS The rats were randomly divided into sham operation group (normal saline), model group (normal saline), hypoxia inducible factor-1α (HIF-1α) inhibitor (YC-1) group [2 mg/(kg·d), tail vein injection], and ZQGCD low-dose, medium-dose and high-dose groups [3.06, 6.12, 12.24 g/(kg·d)], with 8 rats in each group. Except for sham operation group, lumbar disc degeneration model of rat was constructed in all other groups. After modeling, they were given relevant medicine once a day, for consecutive 3 weeks. After the last medication, pathological changes and angiogenesis of the intervertebral disc tissue in rats were observed; the levels of inflammatory factors [interleukin-1β (IL-1β), IL-6, tumor necrosis factor-α (TNF-α)] and the expressions of angiogenesis-related proteins [HIF-1α, vascular endothelial growth factor (VEGF), VEGF receptor 2 (VEGFR2), angiotensin 1(Ang 1), Ang 2] in the com intervertebral disc tissue in rats were all determined. In cell experiment, the primary nucleus pulposus cells were isolated and cultured from rats, and cellular degeneration was induced using 50 ng/mL TNF-α. The cells were divided into blank control group (10% blank control serum), TNF-α group (10% blank control serum), YC-1 group (10% blank control serum+0.2 mmol/L YC-1), and 5%, 10%, 15% drug-containing serum group (5%, 10%, 15% drug-containing serum). After 24 hours of intervention, the nucleus pulposus cells were co-cultured with HUVEC. The expressions of Collagen Ⅱ, matrix metalloproteinase-3 (MMP-3) in nucleus pulposus cells were detected. HUVEC proliferation, migration and tube forming ability were detected, and the expression levels of the HIF-1α/VEGF/Ang signal axis and angiogenesis- related proteins (add MMP-2, MMP-9) in HUVEC were detected. RESULTS Animal experiments had shown that compared with model group, the positive expression of CD31 in the intervertebral disc tissues of rats in each drug group was down-regulated (P< 0.05), the levels of inflammatory factors and angiogenesis-related proteins were decreased significantly (P<0.05), and the pathological changes in the intervertebral disc were alleviated. Cell experiments had shown that compared with TNF-α group, the expression of Collagen Ⅱ in nucleus pulposus cells of all drug groups was significantly up-regulated (P<0.05), and the expression of MMP-3 was significantly down-regulated (P<0.05); the proliferation, migration and tubulogenesis of HUVEC were significantly weakened (P<0.05). The mRNA and protein expressions of HIF-1α, VEGF, Ang 2 as well as the expression of angiogenesis-related proteins (except for the expression of Ang 2 mRNA and HIF-1α, VEGFR2, Ang 2 protein in 5% drug- containing serum group) were significantly down-regulated (P<0.05). CONCLUSIONS ZQGCD may inhibit the HIF-1α/VEGF/ Ang signal axis to weaken the angiogenic ability of vascular endothelial cells, improve pathological angiogenesis in the intervertebral disc, and delay the degeneration of the intervertebral disc.
3.Ectopic thyroid gland in parotid region: a case report and literature review

Objective:
To explore the clinical characteristics, diagnosis, and treatment of ectopic thyroid gland in the parotid gland area, and to provide clinical ideas for the diagnosis and treatment of ectopic thyroid gland.
Methods:
A case of a normal thyroid gland with ectopic thyroid gland tissue in the parotid gland area in the neck was reported. The male patient was 20 years old. The chief complaint was the discovery of a painless mass gradually increasing under the left earlobe for one month. Clinical examination showed obvious bulging of the tissue under the left earlobe. A strip-shaped mass approximately 3.0 cm long could be palpated. It was soft in texture, with a clear boundary, and located under the skin. The skin was pale red and of normal temperature. The body position movement test was negative. Color Doppler ultrasound of the thyroid gland in the neck showed that the shape and size of the thyroid gland were normal. CT images of the head and neck showed a band-like soft tissue density shadow at the area of the parotid gland behind and below the left earlobe, with a clear boundary. The CT value was approximately 30 HU, and further enhancement yielded no additional findings. The admitting diagnosis was a mass in the left parotid gland area. The tumor was incised using a conventional surgical method for the parotid gland area. During the operation, it was found that the tumor was located under the skin, and the contents were bright-red granulomatous tissue without a capsule and adhesive to the skin tissue. The parotid gland capsule was not involved. After the tumor was completely scraped off, intermittent suturing was performed. The resected tumor was sent for pathological examination. A retrospective analysis of the diagnosis and treatment of this type of case was conducted in combination with a literature review.
Results:
The wound of the patient failed to heal in the first stage after the operation. By applying iodoform gauze for pressurized dressing changed weekly, the wound gradually healed about 2 months later. The postoperative pathological report showed an ectopic thyroid gland in the left parotid gland area. The results of the literature review indicate that ectopic thyroid glands can be partial or complete. In the former, normal thyroid gland tissue exists in the neck, and some thyroid gland tissue appears in other locations, mostly at the base of the tongue and mediastinum. In the latter, the thyroid gland in the neck is absent. Both can present with abnormal thyroid gland function and local compression symptoms, and the symptoms are more obvious in patients with a complete ectopic thyroid gland. Ectopic thyroid glands are mainly diagnosed and differentiated through physical examination and imaging examination. Ectopic thyroid glands occurring subcutaneously in the parotid gland area are extremely rare. Physicians should design personalized treatment plans based on clinical examinations and surgical indications.
Conclusion
A subcutaneous ectopic thyroid gland in the parotid gland area is rare. For ectopic thyroid gland surgery, a reasonable surgical plan should be designed considering the patient's aesthetic needs and prognosis. Puncture biopsy should be performed when necessary to formulate the surgical plan.
4.Long-term survival of surgical versus non-surgical treatment for esophageal squamous cell carcinoma in patients ≥70 years: A retrospective cohort study

Objective To compare the long-term survival of elderly patients with esophageal squamous cell carcinoma (ESCC) treated with surgical versus non-surgical treatment. Methods A retrospective analysis was conducted on the clinical data of elderly patients aged ≥70 years with ESCC who underwent esophagectomy or radiotherapy/chemotherapy at Sichuan Cancer Hospital from January 2009 to September 2017. Patients were divided into a surgical group (S group) and a non-surgical group (NS group) according to the treatment method. The propensity score matching method was used to match the two groups of patients at a ratio of 1∶1, and the survival of the two groups before and after matching was analyzed. Results A total of 726 elderly patients with ESCC were included, including 552 males and 174 females, with 651 patients aged ≥70-80 years and 75 patients aged ≥80-90 years. There were 515 patients in the S group and 211 patients in the NS group. The median follow-up time was 60.8 months, and the median overall survival of the S group was 41.9 months [95%CI (35.2, 48.5)], while that of the NS group was only 24.0 months [95%CI (19.8, 28.3)]. The 1-, 3-, and 5-year overall survival rates of the S group were 84%, 54%, and 40%, respectively, while those of the NS group were 72%, 40%, and 30%, respectively [HR=0.689, 95%CI (0.559, 0.849), P<0.001]. After matching, 138 patients were included in each group, and there was no statistical difference in the overall survival between the two groups [HR=0.871, 95%CI (0.649, 1.167), P=0.352]. Conclusion Compared with conservative treatment, there is no significant difference in the long-term survival of elderly patients aged ≥70 years who undergo esophagectomy for ESCC. Neoadjuvant therapy combined with surgery is still an important choice to potentially improve the survival of elderly patients with ESCC.
5.Investigation and analysis of the charging status and standard of pharmacy intravenous admixture service in China

OBJECTIVE To investigate and analyze the operational costs and current charging policies of pharmacy intravenous admixture service (PIVAS) in China, and provide a reference for promoting high-quality and sustainable development of PIVAS. METHODS Questionnaires were distributed in 30 provinces, autonomous regions, and municipalities across the country through the “Wenjuanxing” platform from May 6th to July 1st, 2022. The operational costs, charging status and suggestions of PIVAS were investigated and analyzed. RESULTS A total of 761 PIVAS participated in the survey nationwide, including 666 tertiary medical institutions, 93 secondary medical institutions, and 2 primary medical institutions. Approximately 60.58% of PIVAS had implemented a charging system that allowed charges. Among them, most PIVAS required inspection and evaluation before charging. The annual operating cost of PIVAS in China was approximately 2 098 100 yuan, with the integrated operating cost comprising 89.36% of the total, while the dispensing cost accounted for only 10.64%. Human costs emerged as the highest annual consumption (74.20%), followed by decoration and facility maintenance costs (4.77%) and equipment acquisition costs (3.44%). Regarding charges for different drugs nationwide, common drugs had an average charge standard of 4.39 yuan per bag while antibacterial drugs averaged 5.01 yuan per bag; hazardous drugs had an average charge of 23.17 yuan per bag, whereas parenteral nutrition solutions averaged 38.75 yuan per bag. However, the recommended average charges of the four drugs mentioned above were 6.71, 9.63, 38.35 and 44.03 yuan per bag, respectively. CONCLUSIONS At present, there is no unified inspection and evaluation standard and charging standard in China. Moreover, the current charging standard is lower than the recommended standard. It is necessary to combine operational costs and develop more reasonable and fair charging standards.
6.Construction of a whole business process supervision and management system: based on management information system of blood banks

【Objective】 To achieve supervision and management of the whole business process of blood center, raise productivity and ensure blood quality by enabling blood center managers comprehensively grasp the key business operation situation of the whole process at anytime and anywhere. 【Methods】 A whole business process supervision and management system was established covering background of preparation, business scope, content of position supervision and management, overall framework design, interface design of management and supervision management, physical database design, program development and online debugging, and was integrated with the blood bank management information system. The display and management were through a mobile APP to record key indicators of business process from blood collection to blood supply timely and comprehensively. Statistical analysis was conducted on total collection volume, total preparation volume and total supply volume, as well as discarding rate of test unqualified and of non-test unqualified (lipemic blood excluded) in 2023 and 2022. 【Results】 We established a mobile APP based on a blood bank management information system for business supervision and management of whole process, and achieved management by phones. After its implementation in 2023, the total collection volume, total preparation volume and total supply volume in 2023 were all higher than those in 2022, with growth rates of 5.88% (13 247/225 454 U), 4.73% (24 156/510 698 U), and 6.70% (34 814/519 914 U), respectively. The discarding rate in 2023 was lower than that in 2022 (0.54%, 2 868/534 854 U) vs (0.60%, 3 047/510 698 U) (P<0.01), and the non-test unqualified discarding rate (lipemic blood excluded) in 2023 was significantly lower than that in 2022(0.12%, 649/534 854 U) vs (0.19%, 991/510 698 U)(P<0.01). 【Conclusion】 The construction of supervision and management system of a whole business process based on blood bank management information system can meet the standardized service needs of managers at anytime and anywhere, continuously raise productivity and the standardization and scientific level of blood bank management, thus ensuring blood supply.
7.Benefit from luteal phase progestin primed ovarian stimulation with clomiphene citrate supplementation in young women with diminished ovarian reserve:a retrospective study

Objective:To compare the pregnancy outcomes of luteal phase and follicular phase progestin-primed ovarian stimulation protocol with clomiphene citrate supplementation(LPPOS+CC and FPPOS+CC)in young women with diminished ovarian reserve(DOR).Methods:A total of 483 women aged≤35 years with DOR,who underwent in vitro fertilization(IVF)/intracytoplasmic sperm injection(ICSI)/embryo transfer(ET)with controlled ovarian stimulation using LPPOS+CC(n=257)or FPPOS+CC(n=226)protocols during June 2018 and December 2021 at the First Affiliated Hospital of Wenzhou Medical University,were included in this retrospective study.The baseline characteristics,superovulation results,laboratory related indicators between the two groups,and the pregnancy outcomes of women who achieved at least one high-quality cleavage-stage embryo or good-morphology blastocyst were compared between the two groups.Results:No statistically significant differences were identified between the groups with respect to age,duration of infertility,proportion of secondary infertility,previous failed cycles,body mass index,anti-Müllerian hormone,antral follicle count,basal luteinizing hormone level,basal progesterone level,number of oocytes retrieved,oocyte maturation rate,high-quality cleavage-stage embryo cycle rate,the percentage of women with profound pituitary suppression,live birth rate and preterm birth rate(all P>0.05).The LH levels on the day of trigger[4.0(2.7,5.3)vs.5.1(3.2,7.2)IU/L],the percentage of women with LH levels of>10 IU/L on the trigger day(3.13%vs.10.67%),and the two pronucleus(2PN)rate of ICSI oocytes(72.16%vs.79.56%)were significantly lower in the LPPOS+CC group than those in the FPPOS+CC group(P<0.05 or P<0.01).The duration of stimulation[11(9,12)vs.9(8,11)d],the consumption of total gonadotropin[2213(1650,2700)vs.2000(1575,2325)IU],the progesterone levels on the day of trigger[1.3(0.8,2.9)vs.0.9(0.6,1.2)ng/mL],the clinical pregnancy rate[61.88%vs.46.84%],and implantation rate[42.20%vs.31.07%]in the LPPOS+CC group were significantly higher than those in the FPPOS+CC group(all P<0.01).Conclusion:Compared to FPPOS+CC,the LPPOS+CC protocol appears to have better pregnancy outcomes for young women with DOR undergoing IVF/ICSI-ET.
8.Sinusoidal alternating electromagnetic field accelerates fracture healing in rats

Objective:To investigate the effect of sinusoidal alternating electromagnetic field(SEMF)on fracture healing and its mechanism.Methods:Femoral fracture model was established using specific pathogen free male Wistar rats.Thirty rats were randomly divided into the control and SEMF groups with 15 rats in each group.The SEMF group was given 50 Hz 1.8 mT for 90 min every day,while the control group was not treated.X-ray examinations were performed every two weeks to determine the formation of bone scabs.Three rats from both groups were sacrificed after 2 and 4 weeks of treatment.Protein was extracted from the fractured femurs,and the expression of type Ⅰ collagen(COL-1),osterix(OSX),Runt-related transcription factor 2(RUNX2),and vascular endothelial growth factor(VEGF)was detected by Western blotting.After 8 weeks,the femur on the operated side was taken for micro-CT scanning to observe fracture healing,angiography to observe blood vessel growth,and organs such as hearts,livers,spleens,lungs,and kidneys were taken for safety evaluation by hematoxylin-eosin staining(HE staining).Results:The bone scab scores of the SEMF group were significantly higher than those of the control group after 2,4,6,and 8 weeks of treatment(all P<0.01).The fracture healing of the SEMF group was better than that of the control group after 8 weeks,and the bone volume scores of the two groups were 0.243±0.012 and 0.186±0.008,respectively(P<0.01);the number of blood vessels in the SEMF group was also more than that of the control group after 8 weeks.Western blotting results showed that the expressions of COL-1,OSX,RUNX2,and VEGF were higher in the SEMF group than those in the control group after 2 and 4 weeks of treatment(all P<0.05).HE staining showed that histopathological results of the examined organs were normal in both groups.Conclusion:SEMF can accelerate fracture healing by promoting the expression of osteogenic factors and vascular proliferation without significant adverse effects.
9.Research advances of myeloid-derived suppressor cells mediated by programmed death-1/programmed death-ligand 1 pathway in sepsis-induced immunosuppression

Sepsis is a life-threatening organ dysfunction due to dysregulation of the body's response to infection. The immunosuppressive phase of sepsis is also an important cause of high morbidity and mortality in patients with advanced sepsis. Myeloid-derived suppressor cell (MDSC), as a heterogeneous population of Gr1 +CD11b + immature cells, play a pivotal role in the immune process of advanced sepsis together with programmed death-1 (PD-1) and programmed death-ligand 1 (PD-L1) with immunosuppressive properties. This review summarized the research progress of PD-1/PD-L1 pathway-mediated mechanism of MDSC in septic immunosuppression in recent years. The mechanism of action of PD-1/PD-L1 pathway in the immunosuppressive stage, the effects of PD-1/PD-L1 pathway on the "dual-signaling" model of MDSC through the signaling pathway and cytokines, as well as the time course of PD-1/PD-L1 pathway mediation in sepsis were described.

The first edition of the behavior change technologies taxonomy v1 (BCTTv1) is the most widely used classification of behavior change technologies, providing a series of shared and standardized terms to identify the effective components of behavioral interventions. This article translated BCTTv1 into Chinese, forming a Chinese version of BCTTv1. BCTTv1 consists of 93 behavior change techniques and they are divided into 16 groups according to a hierarchical structure. Each group contains 3-11 behavior change techniques, and each technique is named, defined, explained and illustrated with examples.Briefly describe the application of behavior change techniques in behavior intervention research, providing reference for promoting the application and promotion of behavior change intervention methods and techniques in China.
