1.Effects of Acupuncture at Acupoints along Meridians on the Expressions of Frontal Lobe Associated Protein Kinase in Insomnia Rats

Objective To observe the effects of acupuncture at acupoints along the meridians on the expression of frontal lobe associated protein kinase in pchlorophenylalanine(PCPA)induced insomnia rats;To explore the mechanism of acupuncture in improving insomnia.Methods Totally 60 rats were randomly divided into blank group,model group,sham acupuncture group,acupuncture group and Western medicine group,with 12 rats in each group.An insomnia model rat was induced by intraperitoneal injection of PCPA.The acupuncture group received acupuncture at"Baihui","Shenmen"(bilateral),and"Sanyinjiao"(bilateral),while the sham acupuncture group only fixed the needle at the corresponding acupoints but did not penetrate the skin,while the model group was only fixed for 30 minutes,the Western medicine group was given a solution of estazolam by gavage for 7 consecutive days,the blank group was not treated.The pentobarbital sodium reversal experiment was used to detect the sleep latency and sleep time of rats,ELISA was used to detect the content of melatonin(MT)in plasma,RT-PCR was used to detect the mRNA expressions of calmodulin dependent protein kinase(CaMK)Ⅱ,protein kinase C(PKC)and p38 mitogen activated protein kinase(MAPK)in frontal lobe tissue,and Western blot was used to detect the protein expressions of CaMKⅡ,PKC and p38MAPK in frontal lobe tissue.Results Compared with the blank group,the sleep latency of the model group rats were significantly prolonged and the sleep time were significantly shortened(P<0.01),the content of MT in plasma was significantly reduced(P<0.01),and the mRNA and protein expressions of CaMKⅡ and PKC in frontal lobe tissue were significantly decreased(P<0.01),the mRNA and protein expressions of p38MAPK were significantly increased(P<0.01).Compared with the model group,the sleep latency were significantly shortened and the sleep time were significantly prolonged in the acupuncture group and the Western medicine group(P<0.01),the plasma MT content significantly increased(P<0.01),the mRNA and protein expressions of CaMKⅡ and PKC in frontal lobe tissue significantly increased(P<0.05),and the mRNA and protein expression of p38MAPK significantly decreased(P<0.01).Compared with the sham acupuncture group,the sleep latency shortened and sleep time were significantly prolonged in the acupuncture group and the Western medicine group(P<0.01),plasma MT content significantly increased(P<0.01),PKC mRNA expression in frontal lobe tissue significantly increased(P<0.01),and p38MAPK mRNA expression significantly decreased(P<0.01).There was no statistically difference in various indicators between the acupuncture group and the Western medicine group(P>0.05).Conclusion Acupuncture at acupoints along the meridians can shorten the sleep latency and prolong sleep time,improve sleep status by regulating the expression of associated protein kinase in frontal lobe of insomnia rats.

Objective:To evaluate the efficacy and safety of hypomethylating agents (HMA) in patients with myelodysplastic syndromes (MDS) .Methods:A total of 409 MDS patients from 45 hospitals in Zhejiang province who received at least four consecutive cycles of HMA monotherapy as initial therapy were enrolled to evaluate the efficacy and safety of HMA. Mann-Whitney U or Chi-square tests were used to compare the differences in the clinical data. Logistic regression and Cox regression were used to analyze the factors affecting efficacy and survival. Kaplan-Meier was used for survival analysis. Results:Patients received HMA treatment for a median of 6 cycles (range, 4-25 cycles) . The complete remission (CR) rate was 33.98% and the overall response rate (ORR) was 77.02%. Multivariate analysis revealed that complex karyotype ( P=0.02, OR=0.39, 95% CI 0.18-0.84) was an independent favorable factor for CR rate. TP53 mutation ( P=0.02, OR=0.22, 95% CI 0.06-0.77) was a predictive factor for a higher ORR. The median OS for the HMA-treated patients was 25.67 (95% CI 21.14-30.19) months. HMA response ( P=0.036, HR=0.47, 95% CI 0.23-0.95) was an independent favorable prognostic factor, whereas complex karyotype ( P=0.024, HR=2.14, 95% CI 1.10-4.15) , leukemia transformation ( P<0.001, HR=2.839, 95% CI 1.64-4.92) , and TP53 mutation ( P=0.012, HR=2.19, 95% CI 1.19-4.07) were independent adverse prognostic factors. There was no significant difference in efficacy and survival between the reduced and standard doses of HMA. The CR rate and ORR of MDS patients treated with decitabine and azacitidine were not significantly different. The median OS of patients treated with decitabine was longer compared with that of patients treated with azacitidine (29.53 months vs 20.17 months, P=0.007) . The incidence of bone marrow suppression and pneumonia in the decitabine group was higher compared with that in the azacitidine group. Conclusion:Continuous and regular use of appropriate doses of hypomethylating agents may benefit MDS patients to the greatest extent if it is tolerated.
3.Comparison of esophageal motility and reflux characteristics of patients with endoscopic-negative heartburn based on the Lyon Consensus

【Objective】 To explore the characteristics of esophageal motility and reflux of endoscopic-negative heartburn patients based on the Lyon Consensus Diagnostic Criteria and discuss the differential diagnosis value of the mean nocturnal baseline impedance (MNBI) and the postreflux swallow-induced peristaltic wave (PSPW) index for reflux hypersensitivity (RH) and functional heartburn (FH) patients. 【Methods】 We enrolled 132 patients with heartburn as the main symptom who visited the Gastroenterology Department of our hospital from January 2017 to June 2021, including 24 in the non-erosive reflux disease (NERD) group, 24 in the RH group, and 84 in the FH group. All the patients completed gastroscopy, esophageal high-resolution manometry and 24h-pH impedance monitoring. We analyzed and compared the related indexes of esophageal motility and reflux. The receiver operating characteristic (ROC) curve was used to analyze the esophageal proximal MNBI, distal MNBI, and PSPW indexes for the differential diagnosis of RH and FH. 【Results】 The lower esophageal sphincter (LES) average resting pressure, LES residual pressure and intact relaxation pressure (IRP) in NERD were lower than those in RH and FH (P<0.05). The number of acid reflux, the upright acid exposure time (AET), the supine position AET, the total AET, and the DeMeester score were higher in NERD than in RH and FH (P<0.05). The non-acid reflux, distal MNBI and PSPW indexes were lower in NERD than in RH and FH (P<0.05). The distal MNBI and PSPW indexes were lower in RH than in FH (P<0.05). However, there was no significant difference in proximal MNBI among the three groups (P>0.05). When the proximal MNBI, distal MNBI, and PSPW indexes were used alone respectively to diagnose RH, the area under the ROC curve was 0.480, 0.810, 0.682, respectively, with the sensitivities being 87.5%, 100% and 91.7%, and the specificities being 26.2%, 66.7% and 51.2%, respectively. 【Conclusion】 NERD patients have obvious abnormal esophageal dynamics, mainly manifested as LES relaxation, which further aggravates the pathological acid reflux, while patients with RH and FH are mainly exposed to physiological acid or non-acid reflux. In patients with endoscopic negative heartburn, distal MNBI value can improve the clinical diagnosis rate of RH and help distinguish RH from FH.
4.Clinical characteristics of serious interstitial lung diseases

Objective:To analyze the clinical characteristics and treatment of patients with serious interstitial lung diseases (SILD).Methods:The clinical data of 43 patients with SILD hospitalized in the respiratory intensive care unit of the Characteristic Medical Center of The Chinese People's Armed Police Force from January 2010 to December 2020 were retrospectively reviewed. According to the prognosis, the patients were divided into the death group and non-death group.Results:The included 43 patients include 31 cases of acute exacerbation of idiopathic interstitial pneumonia (AE-IIP), 18 cases of usual interstitial pneumonia (UIP) and 16 cases of nonspecific interstitial pneumonia (NSIP), in which 40% were aggravated due to co-infection and 33 patients were dead. The results showed that there was no significant difference between death and non-death patients in age, gender, smoking, hospitalization time, duration, clinical symptoms and signs, blood T lymphocyte subsets, co-infection, mechanical ventilation and glucocorticoid dose (all P>0.05), and there were significant differences in arterial partial pressure of oxygen to the fraction of inspired oxygen (PaO 2/FiO 2) and arterial partial pressure of carbon dioxide (PaCO 2) (all P<0.05). The PaO 2/FiO 2 level in the dead patients was lower, who often accompanied by type Ⅱ respiratory failure. Conclusions:AE-IIP was more common in patients with SILD, and most of their chest images were consistent with UIP and NSIP. Pulmonary infection is a common cause of acute exacerbation of SILD, and type II respiratory failure in the progress of the disease is a sign of poor prognosis.
5.Hepatitis E virus seroprevalence among blood donors in Liuzhou, China

【Objective】 To investigate the hepatitis E virus(HEV) infection among voluntary blood donors in Liuzhou, Guangxi. 【Methods】 Qualified blood donations in Liuzhou from October to November 2019 were tested for anti-HEV IgG and IgM and.HEV antigen. HEV RNA test was performed on samples positive for HEV antigen and/or anti-HEV IgM. and the test results were analyzed statistically. 【Results】 The seroprevalence rates of HEV antigen and anti-HEV IgG and IgM among 5 751 qualified donations were 0, 26.03% (1497/5 751), and 1.67% (96/5 751), respectively.None of the 91 anti-HEV IgM positive samples was positive for HEV RNA, whereas 60.41% of anti-HEV IgM positive samples were anti-HEV IgG positive. The HEV antibody prevalence varied significantly by gender, age, and nationality (P<0.05). 【Conclusion】 The prevalence of anti-HEV IgG / IgM among blood donors in Liuzhou was higher than that in other domestic regions. Significant increase in HEV antibody prevalence was observed among ethnic minorities, such as Miao and Dong, and age group of more than 45 years.Follow-up studies with larger sample size could be conducted to estimate potential risks of HEV transmission by blood transfusion, which may provide references for selective screening of HEV infection marker among high-endemic population.
6.Current status and complications risk analysis on the utilization of peripheral intravenous catheters in hospitalized children in Class Ⅲ children's specialized hospitals

Objective:To investigate the utilization of peripheral intravenous catheter (PIVC) in hospitalized children in Class Ⅲ specialized children's hospitals, aiming to discover existing issues of PIVC in order to provide reference for clinical nurses and nurse administer to improve the quality on intravenous therapy and to avoid potential risk.Methods:This study uses the field survey method. From July 23rd to 31st, 2018, 25 Class Ⅲ children's specialized hospitals were first sampled in 23 provinces, municipalities, and autonomous regions of the country by the convenient sampling method. The cluster sampling method was then used to select all the pediatric patients with PIVCs admitted to the hospital on the date of investigation. A self-designed 'Cross-sectional Questionnaire for Pediatric Patients with PIVC' was used for survey, and images and data were analyzed. SPSS 17.0 software was used to statistically analyze the data.Results:In this study, 8 635 cases of PICVs were investigated, and the utilization rate of PICVs accounted for 86.9% (8 635/9 942) of all venipuncture tools. Among these cases, 4 665 (54.0%) cases' puncture sites were at the dorsal side of the hand, and 1 234 (14.3%) were at the head. Among the PIVCs, 45.8% (3 954/8 635) were safety PIVCs. And 54.6% (4 712/8 635) of the PIVCs were di (2-ethylhexyl) phthalate (DEHP) free products. The incidence rate of PICV related complications was 7.9% (682/8 635) . Univariate analysis and multivariate logistic regression analysis showed that the PIVC time, IV site, dressing fixation problems, catheter maintenance problems, and patient age were risk factors for PIVC related complications ( P<0.05) . Conclusions:PIVC is the widely used among pediatric patients. The IV site is mainly at the dorsal side of the hand, followed by head and wrist. The selection of material and type of PIVC still require further improvement to ensure the safety of pediatric intravenous therapy. Nursing measures should be taken during the use of PIVC to identify the risk factors at early stage and intervene effectively, which can reduce the occurrence of complications.
7.Effect of acupuncture at points selected from different regions on SCF-kit signaling pathway in diabetic gastroparesis rats

Objective: To observe the effect of acupuncture at points selected from different regions on the positive expression of interstitial cells of Cajal (ICC) and the stem cell factor (SCF) in gastric antrum tissues in diabetic gastroparesis (DGP) rats, and to explore the influence of region-based point selection on the acupoint combination efficacy. Methods: Sixty Sprague-Dawley (SD) rats were randomly divided into a normal group (group A), a model group (group B), a group of Zusanli (ST 36) plus Zhongwan (CV 12) (group C), a group of Zusanli (ST 36) plus Neiguan (PC 6) (group D), and a group of Zusanli (ST 36) plus non-meridian non-acupoint points (group E), based on the random number table (n=12). DGP rat model was established by single intraperitoneal injection of 2% streptozotocin and common diet. After successful modeling, the rats were treated once a day for 4 weeks. Positive ICC and SCF expressions were measured by immunohistochemistry. Results: Compared with group A, the gastrointestinal propulsion rate of group B showed a statistically significant decrease (P<0.05). Compared with group B, the gastrointestinal propulsion rate and the expression of ICC in the gastric antrum were significantly higher in group C, group D and group E, and the between-group differences were statistically significant (P<0.05); the expression of C-kit protein in group C was statistically significantly higher than that in group D and group E (P<0.05). The expression of SCF protein was significantly increased in group C than in group B, and the difference was statistically significant (P<0.05). Conclusion: Acupuncture can improve the symptoms of delayed gastric emptying in DGP model rats, and regulate the expression of ICC and SCF in gastric antrum tissues. The effect of Zusanli (ST 36) plus Zhongwan (CV 12) in the gastric region is superior to that of the Zusanli (ST 36) plus distal Neiguan (PC 6) or non-meridian non-acupoint point, indicating that region-based point selection is the key factor affecting the effect of acupoint combination.
8.Effect of monitoring of transfusion specialist nursing in clinical transfusion risk of former feedback control

Objective To investigate the practice and effect of specialist nursing inspection of intravenous infusion in the quality control of specialist nursing. Methods Intravenous infusion treatment nursing group used feed-forward control theory and methods, taking cross-sectional research methods yearly from 2014 to 2016 and referencing intravenous infusion special quality checklist to fully understand the hospital infusion tool selection situation and quality. Intravenous infusion related treatment system operational procedures was established and improved, the monthly quality control plan was made to do quality nursing of intravenous infusion specialist inspections and timely feedback of each month to various sections of the hospital. Results The rational choice of the hospital infusion tool was more reasonable. The proportion of steel needle in 2016 compared with in 2015 and 2014 was decreased by 8.9%(321/920-212/846), 18.8%(321/920-156/968). Intravenous infusion quality was significantly improved in the catheters′normative fix, nursing staffs′mastery of the skills and related expertise control rate, intravenous infusion treatment-related complications e.g. Phlebitis above grade two in 2016 were reduced 22 cases and 30 cases compared with in 2015 and in 2014, meanwhile the number was 18 and 31 in drug extravasation case. Conclusions Special nursing of intravenous infusion has a good effect on the improvement of nursing quality, and feed-forward control is an effective method for quality control of intravenous infusion therapy.
9.Effects of Shengmai injection on platelet parameters and CD4+CD25+T cells in patients with chronic lymphocytic leukemia

Objective To study the effect of Shengmai injection on the platelet parameters and CD4+CD25+cells in peripheral blood of patients with chronic lymphocytic leukemia,and to explore the optimal regimen for patients with chronic lymphocytic leukemia.Methods One hundred and twenty patients with chronic lymphocytic leukemia were enrolled in this study from January 2012 to December 2014.(D1),vincristine 4mg(d1),prednisone 60 mg/d(d1-5)were given to the CHOP regimen in the control group: cyclophosphamide 600 mg/m2(d1),doxorubicin 25 mg/m2(d12)28d a course of treatment,a total of 4 courses,the observation group of patients in the control group of patients treated on the basis of Shengmai injection,compared the two groups of patients with chemotherapy,platelet parameters and serum CD4+CD25+T cells and Th17 cells.Results After treatment,the PLT,MPV and PDW of the observation group were(215.4± 31.7),(9.5±2.5)and(16.9±2.4),respectively,which were significantly higher than those of the control group.The CD4+CD25+T cells in the observation group were(1.5±0.8)The total effective rate of the control group was 35.0%,the total effective rate was 35.0%,and the total effective rate of the control group was 35.0%.There were significant difference between the control group and the control group(P<0.05),The difference was statistically significant.Conclusion Shengmai injection combined with CHOP regimen in chronic lymphocytic leukemia patients can improve immune function,promote platelet growth,improve platelet clinical parameters,and help improve the efficiency of chemotherapy,compared with CHOP alone Program treatment is better,worthy of clinical application.
10.Effect of decitabine on serum platelet derived growth factor levels in patients with leukemia

Objective To investigate the effect of decitabine on serum platelet derived growth factor levels in patients with leukemia. Methods 120 cases of acute myeloid leukemia patients admitted to our hospital from January 2012 to December 2014 were selected as the study subjects.The control group was given CAG regimen: 1-14 d Seventh days of intravenous infusion of 20 mg/d aclarubicin, first to fourteen days subcutaneous injection of 10 mg/m2 , 1 times per 12 h, 1-13 d cytosine arabinosine days under the subcutaneous injection of granulocyte colony stimulating factor 300μg/d.The observation group was given to the west of the lake 25 mg/d, intravenous infusion, used for 5 days.The levels of platelet derived growth factor PDGF in the two groups were compared and the therapeutic effect.Results Before treatment, differences of PDGF in control group and observation group was no statistical significance.After treatment, PDGF in observation group was lower than control group ( P <0.05).The proportion of primary cells of bone marrow of two groups after treatment was lower than before treatment, and the observation group was more lower.Total effective rate of observation group was 55.0%,which was higher than that in control group(35.0%)(P<0.05).Conclusion Decitabine could reduce the level of of PDGF in patients with leukemia, and improve the total effective rate.
