Endo-periodontal lesions(EPLs)involve both the periodontium and pulp tissue and have complicated etiologies and pathogenic mechanisms,including unique anatomical and microbiological characteristics and multiple contributing factors.This etiological complexity leads to difficulties in determining patient prognosis,posing great challenges in clinical practice.Furthermore,EPL-affected teeth require multidisciplinary therapy,including periodontal therapy,endodontic therapy and others,but there is still much debate about the appropriate timing of periodontal therapy and root canal therapy.By compiling the most recent findings on the etiology,pathogenesis,clinical characteristics,diagnosis,therapy,and prognosis of EPL-affected teeth,this consensus sought to support clinicians in making the best possible treatment decisions based on both biological and clinical evidence.

Odontogenic maxillary sinusitis(OMS)is a subtype of maxillary sinusitis(MS).It is actually inflammation of the maxillary sinus that secondary to adjacent infectious maxillary dental lesion.Due to the lack of unique clinical features,OMS is difficult to distinguish from other types of rhinosinusitis.Besides,the characteristic infectious pathogeny of OMS makes it is resistant to conventional therapies of rhinosinusitis.Its current diagnosis and treatment are thus facing great difficulties.The multi-disciplinary cooperation between otolaryngologists and dentists is absolutely urgent to settle these questions and to acquire standardized diagnostic and treatment regimen for OMS.However,this disease has actually received little attention and has been underrepresented by relatively low publication volume and quality.Based on systematically reviewed literature and practical experiences of expert members,our consensus focuses on characteristics,symptoms,classification and diagnosis of OMS,and further put forward multi-disciplinary treatment decisions for OMS,as well as the common treatment complications and relative managements.This consensus aims to increase attention to OMS,and optimize the clinical diagnosis and decision-making of OMS,which finally provides evidence-based options for OMS clinical management.
3.Standardized nomenclature of oral microorganisms in Chinese: the 2023 update

Oral microbial community, as an important part of human microbial community, is closely related to oral and general health. Oral microbiological research has become the forefront of international microbiological research. Standardized and unified nomenclature for oral microorganisms in Chinese is of great significance to support the development of oral medicine research. Standardized translation of microbial names is the basis for writing canonical and authoritative professional textbooks and reference books, which helps students to accurately acquire the characteristics and classifications of oral microbes. Unified translation of oral microorganisms is also conducive to academic communication and cooperation, and plays an important role in oral health education and science popularization, which enables oral microbiology knowledge to be accurately disseminated to the public. Therefore, in order to standardize the words in scientific research, funding application, publications, academic exchanges and science popularization within the field of oral medicine, we have fully discussed and revised the Chinese names of oral microorganisms in 2017 edition and ones of newly discovered oral microbes, finally reaching a consensus to form the 2023 edition of Chinese names of oral microorganisms.
4.Critical coronavirus disease 2019 caused by Delta variant: a case report with literature review

Objective:To investigate the curative efficacy and application value of convalescent plasma (CP)in severe and critical coronavirus disease 2019 (COVID-19) caused by Delta variant.Methods:The treatment process and results of CP therapy for a patient with critical COVID-19 caused by Delta variant were reported. The clinical application value of CP for COVID-19 caused by Delta variant was analyzed along with the literature review.Results:Our case was a 50-year-old male, who was imported from abroad and had not been vaccinated against COVID-19. The novel coronavirus nucleic acid test was negative before entry. On the second day after entry, fever occurred, novel coronavirus nucleic acid test was positive. Chest CT images showed bilateral multiple mottling and ground-glass opacity with symptoms of nausea, headache, loss of appetite, diarrhea, but no running nose, nasal obstruction, dyspnea, abnormal smell and taste. The infection rapidly developed from medium to critical. On the basis of standard treatment, Delta variant CP was intravenous dripped on the 10th day of hospital admission (the 6th day after becoming severe). The patient's condition improved rapidly.Conclusion:The curative efficacy evaluation of this patient proved that CP therapy is of great value in the treatment of severe and critical COVID-19.
5.Reflection on the development of Artificial Intelligence in the field of medical scientific research management

Objective:To put forward suggestions for the problems identified in the application of Artificial Intelligence in the field of medical research management.Methods:Literature review were conducted to analyze the concept of Artificial Intelligence and its application in the field of medical scientific research management. The existing problems of medical Artificial Intelligence projects were identified and feasible countermeasures were put forward.Results:A total number of 16 Artificial Intelligence projects were going in the hospital. Some problems were identified during the management process, which including lack of regulatory regulations and professional standards, the communication barriers between the two partners are also prominent, the collection, sharing, and quality control of medical big data need to be standardized and managed ethically.Conclusions:Taking into account of the problems identified in the management of Artificial Intelligence projects in hospitals, scientific research management should put forward some countermeasures, such as improving management system and supervision mechanism, strengthening top-level guidance and improving employment capacity, paying attention to process management and fine business process, focusing on data supervision to promote business format promotion, cultivating talents to help comprehensive development, strengthening ethical awareness and risk management.
6.Successful cardioversion of fetal persistent supraventricular tachycardia with intrauterine intramuscular injection of cedilanid under ultrasound guidance: a case report

We report the intrauterine treatment of a case of fetal persistent supraventricular tachycardia complicated by heart failure. The ultrasound findings at 32 +1 weeks of gestation showed fetal tachycardia of 242 beats/min, fetal generalized edema, ascites, and visible pulsation spectrum in the intra-abdominal segment of the umbilical vein. Fetal cardiac arrhythmia persisted after maternal oral digoxin for 46 hours. In view of the continuous deterioration of the fetal condition with the persistent umbilical vein pulsation spectrum and worsening edema, fetal intramuscular injection of cedilanid under ultrasound guidance was performed, resulting in successful cardioversion. The pregnant woman gave birth to a baby girl by cesarean section due to premature rupture of membranes and oligohydramnios at 34 +6 weeks of gestation, with normal functions of heart and other organs. During a follow-up to 8 months, no obvious abnormalities were found.
7.Establishment and application of clinical discipline evaluation index system in cancer hospital based on Delphi method

Objective:To establish a scientific, directional and operable evaluation index system for clinical disciplines, which can meet the needs of discipline construction and is suitable for the evaluation and selection of various disciplines in oncology hospitals, so as to provide decision-making basis for discipline construction.Methods:Literature analysis and in-depth interview were used to draw up the preliminary evaluation index system. The index frame of subject evaluation was compiled, and the expert inquiry paper was compiled, and the index was added, subtracted and modified by Delphi method through two rounds of expert consultation. The index weight was determined by Analytic hierarchy process. Each index was given a specific scoring standard. The evaluation index system was used to test and analyze 22 clinical medical disciplines in the hospital. The total score of each discipline was calculated by weighted summation method. The validity of evaluation was judged by Spearman grade correlation analysis.Results:The clinical discipline evaluation index system consisted of 4 first-class indexes, 10 second-class indexes and 34 third-class indexes. The weights and scoring criteria of each index were determined. 22 disciplines were evaluated by the index system, which was divided into three grades: excellent, medium and poor.Conclusions:The evaluation index system of clinical discipline constructed in this study has high authoritative coefficient of experts, and the P value is less than 0.01.The evaluation results of 22 disciplines are highly correlated with those of experts, and the P value is less than 0.01. It has strong scientific validity and feasibility, and lays a foundation for the widespread application of clinical evaluation.
8.Salivary mycobiome dysbiosis and its potential impact on bacteriome shifts and host immunity in oral lichen planus.

The biodiversity of the mycobiome, an important component of the oral microbial community, and the roles of fungal-bacterial and fungal-immune system interactions in the pathogenesis of oral lichen planus (OLP) remain largely uncharacterized. In this study, we sequenced the salivary mycobiome and bacteriome associated with OLP. First, we described the dysbiosis of the microbiome in OLP patients, which exhibits lower levels of fungi and higher levels of bacteria. Significantly higher abundances of the fungi Candida and Aspergillus in patients with reticular OLP and of Alternaria and Sclerotiniaceae_unidentified in patients with erosive OLP were observed compared to the healthy controls. Aspergillus was identified as an "OLP-associated" fungus because of its detection at a higher frequency than in the healthy controls. Second, the co-occurrence patterns of the salivary mycobiome-bacteriome demonstrated negative associations between specific fungal and bacterial taxa identified in the healthy controls, which diminished in the reticular OLP group and even became positive in the erosive OLP group. Moreover, the oral cavities of OLP patients were colonized by dysbiotic oral flora with lower ecological network complexity and decreased fungal-Firmicutes and increased fungal-Bacteroidetes sub-networks. Third, several keystone fungal genera (Bovista, Erysiphe, Psathyrella, etc.) demonstrated significant correlations with clinical scores and IL-17 levels. Thus, we established that fungal dysbiosis is associated with the aggravation of OLP. Fungal dysbiosis could alter the salivary bacteriome or may reflect a direct effect of host immunity, which participates in OLP pathogenesis.

9.Research progress of probiotics in the prevention and treatment of oral infectious diseases

Oral infectious diseases include caries, periodontal disease, halitosis, candidiasis albicans and so on. Over the past few decades, probiotics have mainly been studied in the field of the gastrointestinal tract. In recent years, probiotics have begun to be used in the prevention and treatment of various oral diseases and have become a new field in the research of oral disease prevention and control technology. This paper reviews the research progress of probiotics applied in the prevention and treatment of various oral infectious diseases. A review of the literature shows that probiotics can prevent and cure dental caries by inhibiting the growth of Streptococcus mutans and competing with them for nutrition and attachment sites. Probiotics not only inhibit periodontal pathogens and reduce the production of sulfide, they also regulate the body’s immune function to alleviate halitosis and periodontal inflammation. Probiotics can inhibit periodontal caries by inhibiting them. Probiotics can inhibit them mycelial growth of Candida albicans and interfere with its adherence, thus playing a role in the prevention and treatment of oral candidiasis. Current studies have shown that probiotics play an auxiliary role in the treatment of caries, periodontitis, halitosis and oral candidiasis. However, the mechanism of probiotics in the prevention and treatment of oral infectious diseases is still unclear, and the safety of probiotics remains to be further studied. In the future, oral probiotics should be studied with reference to intestinal probiotics to better work to prevent and treat oral diseases.
10.Potential threat of pharyngo-esophageal diverticulum to thermal ablation treatment of thyroid nodule and rapid differential diagnosis through swallow contrast-enhanced ultrasonography

Objective To explore the potential risk of misdiagnosis of pharyngeal esophageal diverticulum( PED) for the treatment of thyroid nodules with thermal ablation and its rapid and effective method of diagnosis . Methods The process of diagnosis and treatment were reviewed on 5 cases of PEDs which had been misdiagnosed as thyroid nodules and recommended for thermal ablation therapy . A comprehensive analysis was carried out for the reasons of misdiagnosis ,the timing and clues of the diagnosis as well as the advantage of swallow contrast-enhanced ultrasonography ( sCEUS ) in achieving rapid and correct diagnosis . Results The lack of recognition and vigilance to the acoustic image of PED were the direct causes of misdiagnosis . Five cases of PED misdiagnosed as thyroid nodules were all discovered by the same chief operator before thermal ablation . They were confirmed through sCEUS and mistreatment by ablation were fortunately avoided ,but all true thyroid nodules requiring treatment were safely ablated . Conclusions PED is easily misdiagnosed as thyroid nodule .Improving the recognition and vigilance of the ultrasonic features of PED ,strictly implementing the reconfirming ultrasound examination by the chief operator ,and promptly conducting sCEUS are sensible measures to quickly correct the diagnosis and prevent w rong ablation of PED .
