1.Evaluation of short-term and long-term effects of preincision of pancreatic duct sphincter in patients with different types of endoscopic retrograde cholangiopancreatography difficult to intubate

		                        			Objective To investigate the short and long term effects of preincision of pancreatic duct sphincter in different types of patients with difficult intubation by endoscopic retrograde cholangiopancreatography(ERCP).Method A retrospective study was conducted on 100 patients with difficulty in ERCP intubation from January 2019 to June 2020.Patients were divided into a control group(50 cases)and a study group(50 cases)according to different treatment methods.The control group underwent routine ERCP intubation without pancreaticotomy,while the study group underwent preincision of pancreatic duct sphincter on the basis of routine ERCP intubation.Calculate the successful intubation time,successful intubation rate,and length of hospital stay for two groups;Enzyme linked immunosorbent assay was used to detect postoperative C-reactive protein(CRP),interleukin-6(IL-6),and blood amylase levels in two groups of patients;Calculate the incidence of complications related to pre incision surgery in two groups of patients;Follow up the incidence of reflux cholangitis and recurrent pancreatitis in two groups of patients at 3,6,and 12 months after surgery,and evaluate the long-term efficacy of the two groups of patients.Result Compared with the control group,the study group showed a shorter in successful intubation time,and an increase in successful intubation rate(P<0.05).There was no difference in hospital stay between the two groups(P>0.05);Compared with the control group,the study group showed a decrease in CRP,IL-6 and blood amylase levels on the first day after surgery(P<0.05);The postoperative complication rate of patients in the control group was 14.0%,including 4 patients with acute pancreatitis,2 patients with bleeding,and 1 patient with perforation.The postoperative complication rate of patients in the study group was 2.0%,with 1 patient with bleeding.Compared with the control group,the postoperative complication rate of patients in the study group decreased(P<0.05);Compared with the control group,the incidence of reflux cholangitis in the study group decreased at 12 months after surgery,and the incidence of recurrent pancreatitis decreased at 3,6,and 12 months after surgery(P<0.05).Conclusion Preincision of the pancreatic duct sphincter has a mild impact on systemic inflammation in patients with difficult ERCP intubation,with normal blood amylase indicators and no increase in the incidence of postoperative pancreatitis.It can improve the success rate of intubation and has significant long-term efficacy.It is a safe and effective technique which is worth recommending.
		                        		
		                        		
		                        		
		                        	
2.Study on quality standard of Jingtian Zhitong cream

		                        			Objective To revise the determination method in the quality standard of Jingtian Zhitong cream. Methods The total saponins of angelica sinensis, Ligusticum wallichii, Rhizoma corydalis, and Panax notoginseng saponins were qualitatively identified by thin-layer chromatography (TLC). The contents of notoginsenoside R1, ginsenoside Rg1, and ginsenoside Rb1 in the preparation were determined by high performance liquid chromatography (HPLC). Results TLC showed strong specificity and good resolution. The concentration of notoginsenoside R1 showed a good linear relationship in the range of 0.1604 and 2.0050μg (r=0. 999). The concentration of ginsenoside Rg1 showed a good linear relationship in the range of 0.8003 and 10.0035μg (r=1.000). The concentration of ginsenoside Rb1 showed good linearity in the range of 0.6182 and 7.7275μg (r=1.000). The sample recovery rates were 101.43%, 98.75% and 100.95%, respectively. The relative standard deviation (RSD) were 2.56%, 2.71% and 2.75%, respectively. 4.Content Determination of 6 Kinds of Monosaccharide in Astragalus membranaceus with Different Growth Years by Pre-column Derivatization-HPLC

		                        			OBJECTIVE:To analyze and compare th e contents of 6 kinds of monosaccharide in Astragalus membranaceus from different growth years . METHODS :2-4 years old A. membranaceus from three areas were extracted with water extraction and alcohol precipitation ,Sevage deproteinization to obtain A. membranaceus polysaccharide. The samples were firstly hydrolyzed with trifluoroacetic acid (TFA)and then derivatized by 1-phenyl-3-methyl-5-pyrazolone(PMP). HPLC analysis was adopted to determine the contents of 6 kinds of monosaccharide as mannose ,rhamnose,galacturonic acid ,glucose,galactose,arabinose. The   determination was performed on Symmetry C 18 column with phosphate buffer solution (pH 6.8)-acetonitrile(84∶16,V/V)as mobile phase at the flow rate of 1.0 mL/min. The detection wavelength was 245 nm,and column temperature was 35 ℃. The sample size was 20 µL. RESULTS :The contents of mannose ,rhamnose,galacturonic acid ,glucose,galactose and arabinose were 0.50-0.94, 0.76-1.60,3.35-7.86,87.33-275.77,1.95-8.96,2.35-14.04 mg/g,respectively. Total contents of monosaccharide from 2,3,4 years old A. membranaceus were 98.26-139.92,173.81-295.71,122.37-182.41 mg/g,respectively. There was significant difference in the contents of glucose between 3 old years A. membranaceus and 2,4 old years A. membranaceus (162.71-275.77 mg/g vs. 87.33-107.70,111.54-167.26 mg/g,P<0.05). CONCLUSIONS :Above 6 monosaccharides are detected in 2,3,4 years old A. membranaceus,among which the content of glucose is the highest. 5.The application values of the relative length of lesion in differential diagnosis of extrahepatic infiltrating cholangiocarcinoma and cholangitis

		                        			Objective:To evaluate the diagnostic value of relative lesion length in differentiating extrahepatic bile duct infiltrating cholangiocarcinoma with inflammation.Methods:From October 2014 to February 2018, 24 cases of infiltrating extrahepatic cholangiocarcinomas confirmed operatively and pathologically and 23 cases of extrahepatic bile duct inflammation confirmed clinically from the Third People′s Hospital of Datong City were respectively enrolled in this study. Upper abdomen MR and/or CT image data of all patients were respectively reviewed. The extrahepatic duct wall was defined as wall thickening with obvious enhancement. The length of the lesion was measured. L lesion/L duct was referred as the ratio of the lengths of lesion to extrahepatic bile duct (common hepatic duct+common bile duct)was calculated. The difference in the average values of L lesion/L ductbetween the cholangiocarcinomas group and inflammation group was analyzed with t test, and the differential diagnostic efficacy of L lesion/L ductratio was analyzed with receiver operating characteristic curve (ROC) test. Results:Significant difference was found in the length of lesion between the extrahepatic cholangiocarcinoma group [(22.01±1.86) mm] and the cholangitis group [(47.36±2.81) mm] ( P<0.01). The average ratio of L lesion/L ductwere 0.26±0.02 for the cholangiocarcinomas group and 0.54±0.03 for the inflammation group, respectively ( P<0.01). The area under the ROC curve of L lesion/L duct in diagnosis of the infiltrating extrahepatic cholangiocarcinomas was 0.92. With <0.40 as cut-off point, the diagnostic sensitivity and specificity were 87.5% and 82.6%, respectively. Conclusion:The L lesion/L ductmight be taken as an important diagnostic sign in differentiation between infiltrating extrahepatic cholangiocarcinomas and extrahepatic bile duct inflammation.
		                        		
		                        		
		                        		
		                        	
6.Evaluation of intervention effect of different health education on dietary behavior of high school students

		                        			Objective:
		                        			To understand the effects of different health education on dietary behavior of high school students, and to provide support for appropriate and healthy dietary education.
		                        		
		                        			Methods:
		                        			Five high schools were selected in Gulou District, Nanjing. Two classes were randomly selected from each high school for health education intervention. Questionnaire survey was conducted before and after intervention to compare the changes in dietary behaviors.
		                        		
		                        			Results:
		                        			After intervention, the dietary knowledge score for the control group was (9.79±2.28), and (14.66±2.71) for the intervention group, which was statistically significant (P<0.01). For adolescents in the intervention group, no significant differences were found before and after intervention. However, significant differences in dietary knowledge score were found for the control group before and after intervention (P<0.01). Dietary behavior score in the intervention group before intervention was (3.34±1.78) and increased significantly after intervention (5.05±1.79, P<0.01). There was no statistical significance before and after intervention in the control group (P>0.05). After the intervention, dietary knowledge scores increased significantly in the education group, mobile APP intervention group, the lecturebased study group, and the classroom-based study work group after intervention (t=-10.196, -29.222, -5.721, -14.394, P<0.01). Multivariate logistic regression analysis showed that mobile APP intervention group (OR=2.91), dietary knowledge score ≥13 (OR=4.43), age≥17 years (OR=3.72), classroombased study group (OR=2.35) were positively associated with dietary behavior among high school students.
		                        		
		                        			Conclusion
		                        			Oneweek intervention can effectively improve dietary knowledge and dietary behavior among high school students. 7. Effect of intratumor heterogeneity of esophageal squamous cell carcinoma on chemotherapy sensitivity

		                        			To investigate the relationship of heterogeneity of esophageal squamous cell carcinoma (ESCC) and chemotherapy sensitivity.
		                        		
		                        			Methods:
		                        			Five different region specimens isolated from primary tumor(R1~R5)and 1 specimen(R6)isolated from adjacent non-neoplastic tissue from 10 ESCC patients who underwent surgical treatment were cultured in vitro. The inhibitory effect of cisplatin on proliferation of ESCC cells from different regions was determined by methyl thiazolyl tetrazolium (MTT). The cell cycle and apoptosis induced by cisplatin was determined by flow cytometry (FCM) analysis. The mRNA levels of ATP7A and ATP7B were determined by quantitive RT-PCR (qRT-PCR).
		                        		
		                        			Results:
		                        			The result showed that different regions of each specimen exhibited different chemotherapy sensitivity to cisplatin, and the cell survival rates of region R6 of each specimen were higher than other regions from the same specimen. 8.Association of gankyrin protein expression in human colorectal cancer with postoperative prognosis.

OBJECTIVETo investigate the association of gankyrin protein expression in colorectal cancer (CRC) with its prognosis.
METHODSClinical data and resection samples of 100 colorectal cancer patients identified by pathology undergoing resection in our department from June 2008 to June 2009 were collected. The gankyrin expression in CRC tissues and matched adjacent noncancerous tissues collected during the operation of 100 CRC cases was detected by immunohistochemical staining and Western blotting. The associations of gankyrin expression level with overall survival, clinicopathologic features were analyzed by Chi square test, Cox regression analysis, Kaplan-Meier analysis and log rank test.
RESULTSImmunohistochemical staining showed that the positive brown granules were mainly distributed in the cytoplasm, and nuclear immunostaining was observed in tissue samples of 29 cases, of whom 16 cases had distal metastasis [55.2% (16/29)]. The positive rate of gankyrin and the relative gray value of Western blotting in CRC tissues were 67% (67/100) and 0.69±0.23, respectively, which were significantly higher than those of 2 cm adjacent noncancerous tissues [6% (6/100) and 0.31±0.16] and 10 cm adjacent noncancerous tissues [1%(1/100) and 0.16±0.11] (all P<0.001). Patients with positive expression of gankyrin had worse survival than those with negative ones (41.8% vs. 72.7%, P=0.008). The gankyrin expression was associated to lymph node metastasis (P=0.005), tumor stage (P=0.001) and distal metastasis (P=0.002). Cox regression analysis showed that distal metastasis (P=0.004) and high expression of gankyrin (P=0.038) were independent risk factors for poor prognosis of patients with CRC.
CONCLUSIONUp-regulated expression of gankyrin is related to invasion and metastasis of human CRC, and gankyrin may be valuable in predicting prognosis.

9.Clinical significance and distribution characteristics of expression of vascular endothelial growth factor receptor 2 in hemorrhoid mucosa

		                        			Objective To investigate the micro-vessel density and vascular endothelial growth factor receptor 2 (VEGFR2) expression in mucosa and submucosa tissue of the patients with hemorrhoids. Methods Sixty hemorrhoids patients were randomly recruited and involved into experiment group and 20 cases of normal patients involved in control group. RASP, MASP, RSTV, RMTV, Expression of VEGFR2 and microvessel density in two groups were detected by immunohistochemistry. The expression abundance of VEGFR2 mRNA was detected by RT-PCR. Results Excessive angiogenesis was found in hemorrhoids tissue (5.59 ± 0.98) in experiment group as compared with that in control group (3.92 ± 0.81), the difference was statistically significant (P < 0.05). Staining score of hemorrhoid tissue and the control tissue were 5.95 ± 1.01, 2.32 ± 0.72, respectively (P < 0.05). Abundance of VEGFR2 mRNA expression in hemorrhoid tissue was significantly higher than that in control tissue (P < 0.05). Conclusion VEGFR2 plays an important role in angiogenesis during the development of hemorrhoids, which may be closely related to the occurrence of hemorrhoid disease and clinical symptoms.
