1.Fundamental study on the anti-inflammatory activity of the petroleum ether extract from Melastoma dodecandrum based on spectrum-effect relationship

OBJECTIVE To explore the material basis of the anti-inflammatory effect of the petroleum ether extract from Melastoma dodecandrum by establishing its fingerprint and combining it with cellular pharmacodynamics experiments. METHODS HPLC method was adopted; the fingerprints of 20 batches of petroleum ether extract from M. dodecandrum were drawn using The Similarity Evaluation System of TCM Chromatographic Fingerprint (2012 edition); similarity evaluation and common peak identification were carried out. The lipopolysaccharide-induced inflammation model of mice mononuclear macrophages (RAW264.7) was established; the inhibitory rates of nitric oxide (NO) and tumor necrosis factor α (TNF-α) were used as indexes to investigate the anti-inflammatory activity of the petroleum ether extract from M. dodecandrum; grey correlation degree method and partial least square regression analysis were adopted to study the spectrum-effect relationship. Molecular docking was used to validate the binding activity of the anti-inflammatory active ingredients with TNF-α and iNOS protein receptor. RESULTS There were 19 common peaks in the fingerprint of the petroleum ether extract from M. dodecandrum, the similarity of 20 batches of samples ranged from 0.603-0.990, and five components were identified, such as vitexin (peak 5), isovitexin (peak 6), ellagic acid (peak 7), quercetin (peak 9) and luteolin (peak 10). The grey correlation degree between 19 common peaks of the petroleum ether extract from M. dodecandrum and the inhibition rates of NO and TNF-α were all greater than 0.7; peaks 19, 13, 9 (quercetin), 12, 5 (vitexin), 6 (isovitexin), 8, 7 (ellagic acid), 18, 1 were positively correlated with NO inhibition rate, and peaks 8, 10 (luteolin), 13, 15, 3, 19, 17, 7 (ellagic acid), 18, 1 were positively correlated with inhibition rate of TNF-α. The binding energies of vitexin, isovitexin and quercetin with iNOS protein receptor were less than -5.0 kcal/mol. CONCLUSIONS Vitexin, isovitexin and quercetin may be the basis of the anti-inflammatory effect of the petroleum ether extract from M.dodecandrum.
2.Correlation analysis of sleep behavior and overweight and obesity in junior high school students

Objective:
To analyze the association between different sleep behaviors and overweight and obesity of junior high school students in Yangzhou City, and to provide a basis for policies and interventions related to adolescent health management.
Methods:
A total of 1 589 students in grades 7-9 from two middle schools in Yangzhou City were selected using the cluster sampling method and were administered with sleep time, bedtime, social jetlag difference, and sleep habits.
Results:
Totally 64.38% were sleep deprived during the school days, 86.78% went to bed too late, 46.51% had a social jetlag of ≥1 h, and 37.44% took a nap every day(Incluldes holidays and school days). Social jetlag length was statistically different between grades( F =6.97, P < 0.01 ). Girls[(0.95±0.65)h] shown significantly higher social jetlag than the boys[(0.76±0.59)h]( t=6.19, P <0.01). Later bedtime on weekends, later wake up time on weekends and poor sleep behavior were risk factors for overweight and obesity in junior high school students( OR=1.20, 1.14, 1.04, P <0.05).
Conclusion
Junior high school students had less sleep and later bedtimes with the increase of grade, and weekend bedtimes,wake up times and poor sleep behavior were independently associated with the risk of overweight and obesity in junior high school students. Parents and schools should be instructed to pay attention to their sleep health and carry out adolescent sleep health guidance.
3.c-MYC-mediated TRIB3/P62+ aggresomes accumulation triggers paraptosis upon the combination of everolimus and ginsenoside Rh2.

The mammalian target of rapamycin (mTOR) pathway is abnormally activated in lung cancer. However, the anti-lung cancer effect of mTOR inhibitors as monotherapy is modest. Here, we identified that ginsenoside Rh2, an active component of Panax ginseng C. A. Mey., enhanced the anti-cancer effect of the mTOR inhibitor everolimus both in vitro and in vivo. Moreover, ginsenoside Rh2 alleviated the hepatic fat accumulation caused by everolimus in xenograft nude mice models. The combination of everolimus and ginsenoside Rh2 (labeled Eve-Rh2) induced caspase-independent cell death and cytoplasmic vacuolation in lung cancer cells, indicating that Eve-Rh2 prevented tumor progression by triggering paraptosis. Eve-Rh2 up-regulated the expression of c-MYC in cancer cells as well as tumor tissues. The increased c-MYC mediated the accumulation of tribbles homolog 3 (TRIB3)/P62+ aggresomes and consequently triggered paraptosis, bypassing the classical c-MYC/MAX pathway. Our study offers a potential effective and safe strategy for the treatment of lung cancer. Moreover, we have identified a new mechanism of TRIB3/P62+ aggresomes-triggered paraptosis and revealed a unique function of c-MYC.

5.Research advances in acute pancreatitis scoring system

Acute pancreatitis (AP) is a severe disease with an increasing incidence rate in clinical practice. Although most patients have mild pancreatitis, the fatality rate of severe pancreatitis remains at a relatively high level, and therefore, early-stage, simple, and accurate clinical scoring systems are urgently needed to determine the severity of AP, so as to facilitate effective disease management and symptomatic treatment and reduce the fatality rate of patients. At present, a large number of studies have demonstrated that the scoring systems such as Ranson score, APACHE Ⅱ score, BISAP score, CTSI score, and some serological markers have been used to evaluate the severity and prognosis of AP, but all of them have certain limitations. This article reviews the research advances in the existing scoring systems, single serological markers, and related modified scoring systems in recent years. Through a literature review, it is concluded that there is no a single scoring system or a single indicator that can cover the whole process of AP diagnosis and treatment and accurately judge the severity of AP, and therefore, it is necessary to develop a new scoring system or combine various indicators for comprehensive evaluation.
6.Surgical correction of velopharyngeal insufficiency in cleft palate patients with two procedures of reconstruction of radical intravelar veloplasties (IVVs)

Objective To summarize the clinical outcomes of surgical correction of velophary geal insufficiency (VPI) with two procedures of radical intravelar veloplsties,and to discuss the indication selection of surgical technique.Methods 58 cases of various cleft palate were speech evaluated and diagnosed as mild to moderate VPI,who aged from 3 to 28 years (mean age 7.5 years).Group Ⅰ (36 cases) were performed surgery in the way of modified Furlow double-opposing Z-plasty.Group Ⅱ (22 cases) were performed surgery in the way of Sommerald radical IVV.All cases were followed up for 6 months to 1 year after surgery,with speech evaluation and objective examination of nasopharygoscopy and fixed position lateral X ray.The X-ray images of lateral view of velum was read through PACS image system.The effective work length of soft palate and the pharyngeal gap of the velum at rest and function were measured directly through PACS system.The data of each case before and after surgery were compared.Results 34 of 36 cases (94.50%) in modified-Furlow group and 21 of 22 cases (95.5%) in Sommerlad group finally recovered complete velopharyngeal closure.Compared between before and post operation,the effective work length of soft palate in modified Furlow group increased by 9.50±2.35 mm,the length in Sommerlad group increased by 7.50±3.32 mm.The differences were statistically significant by paired T test (P<0.001).None of all cases complained nasal airway obstruction.Conclusions Both types of radical IVVs are highly effective to be recommended for surgical correction of mild to moderate VPI.Strict selection of surgical indication and excellent surgical skill are necessary for good treatment outcomes.
7.Correlation between family cohesion and adaptation and work adaptation in postpartum nurses with second child

Objective To explore the correlation between family cohesion and adaptation and work adaptation in postpartum nurses with second child. Methods There were 155 postpartum nurses with second child collected from 4 hospitals in Chengdu of Sichuan Province between October and December 2017, by convenience sampling. Family Adaptation and Cohesion Evaluation Scale Ⅱ and Questionnaire of Return Adaptation for Postpartum Nurse were used in the investigation. Results The family real cohesion, ideal cohesion and dissatisfaction of coherence were (66.95 ± 8.37), (71.68 ± 6.45), (7.87 ± 5.13) points in this study, which were higher than norms: (63.90 ± 8.00), (68.80 ± 5.30), (6.40 ± 5.00) points, and there were significant differences (t=-3.086,-4.079,-2.403, all P<0.05) . The family real adaptation was (48.86 ± 7.33) points, which was less than norms: (50.90 ± 6.20) points, and there was significant difference (t=2.514, P<0.05). The ideal adaptation had no significant difference with norms (P>0.05). The dissatisfaction of adaptation was (9.36±5.22) points, which was higher than norms: (7.40 ± 5.30) points, and there was significant difference (t=-3.076, P<0.05). There were 24 (15.5%) nurses in extreme type family, 54 (34.8%) in intermediate type and 77 (49.7%) in balance type. The average score of work adaptation was (2.41 ± 0.76) points. The scores of work adaptation in nurses of extreme type family were lowest. Time since working, dissatisfaction of adaptation, dissatisfaction of coherence and hospital level were the influencing factors of work adaptation. Conclusions Nursing managers should face up to the situation of work adaptation in postpartum nurses, humanize their management, pay attention to the emotional state, promptly dispel their bad negative emotions, take measurements to improve the understanding of nurses'families and help them pass the adaptation period of returning to work.
8.Oxygen Uptake Efficiency Slope Predicting the Prognosis in Patients With Idiopathic Pulmonary Arterial Hypertension

Objectives: To explore weather oxygen uptake efficiency slope (OUES) may predict the prognosis in patients with idiopathic pulmonary arterial hypertension (IPAH). Methods: The consecutive newly diagnosed IPAH patients in our hospital from 2010-11 to 2015-06 were prospectively enrolled and regular follow-up study was conducted to record cardiovascular events (death and lung transplantation). Kaplan–Meier curve, uni- and multivariate Cox regression analysis were performed to assess the survival rate in relevant patients. Results: A total of 210 IPAH patients at the mean age of (32±10) years were finished cardiopulmonary exercise test (CPET) and received regular follow-up study including 159 female. There were 31 patients died and 1 received lung transplantation over 41 months follow-up period. OUES was positively related to peak oxygen uptake (VO2)/body weight (r=0.71, P<0.0001). Multivariate analysis demonstrated that OUESI and NT-proBNP could independently predict the prognosis of IPAH patients. The 5-year survival rate in patients with OUESI≤0.52 L/(min?m2) was lower than those with OUESI>0.52 L/(min?m2) (41.9% vs 89.8%), P<0.0001.Conclusion: OUES as a submaximal CPET parameter may well predict the prognosis in IPAH patients.

Objective:To develop a more suitable sterile examination method for artemether injection in order to control its quality better. Methods:The direct inoculation and the membrane filtration was respectively used for the sterile examination of artemether in-jection. Results:In the direct inoculation method, Staphylococcus aureus, Bacillus subtilis and Clostridium perfringens grew well in the test tubes containing artemether injection in 24 h, and Escherichia coli, Aspergillus niger and Candida albicans grew well in 48 h. In the membrane filtration method, Staphylococcus aureus grew well in 24 h in the test group containing artemether injection, and the other 5 strains grew well in 48 h. Conclusion:The positive strains can grow well in the applicability test of both methods. However, due to the simpler operation and higher accuracy of membrane filtration method, it is recommended to be used for the sterile test of artemether injection.
